    A total of 166 cases of PBC recorded in the Annuals of the Pathological Autopsy Cases in Japan, from 1984 to 1987, were studied statistically. Its incidence among the total autopsy cases was 0.10%. The mean age with S.D. (range) at autopsy was 57.6±11.1 years (34-85). There was an overall female preponderance of 7.3:1. The majority of cases showed cirrhotic stage with portal hypertension and jaundice. The liver weight ranged from 470g to 2, 400g. The occurrence of HCC was recarded in 7 cases (4.22%). Autoimmune diseases were rarely complicated. Some autopsy cases diagnosed clinically as PBC received a pathalogical diagnosis other than PBC. It is necessary to determine the pathological criteria of PBC in the cirrhotic stage.

    Three normal volunteers and 19 patients with chronic liver disease underwent a new RI examination using 99mTc-diethylenetriaminepentaacetic acid-galactosyl-human serum albumin (99mTc-GSA). Blood retention rates of 99mTc-GSA (%ID) were determined using the heart regression curve and by extrapolation technique following a single intravenous injectian of the ligand (1mg/185MBq). The% ID in the blood 60min after the injection showed significant correlations with serum albumin (p<0.001), retention rate of ICG at 15min (p<0.001), plasma disappearance rate of ICG (p<0.001), prothrombin time (p<0.01), hepaplastin test (p<0.05), and total bilirubin (p<0.005). Increased% IDs were observed in accordance with severities of the liver disease. Our method for the estimation of the blood retention of 99mTc-GSA is considered useful in evaluating the hepatic function.

    Forty five patients with liver diseases received iv indocyanine green (3-4mg/kg). Blood samples were taken in 5 to 30 min interval in 90 to 120 min to determine serum ICG concentration (CP). Vm, Km, VP, b, h were calculated by Stceckel's methad and ELV was determined as a/b (a=Vm/Km). ELV was also determined directly by the computer, adding VL as the 6th parameter of the differential equations led from Stoeckel's model. Both ELV matched well to each other. The fitness of the model to serum ICG curve was excellent in 43 patients when judged by Akaike's Information Criterion. When h was less than 0.04, as it was in all patients studied, a simulatian program gave the calculated hepatic ICG concentrations which were equal to serum concentrations, only when ELV=a/b. The correlations between ELV and serum albumin, hepaplastin test, serum choline esterase, ICG R15, or hepatic glutathione S-transferase activities were statistically significant.

    We developed a measurement system for "in vivo" ultrasonic tissue characterization, and attempted to evaluate the degree of fatty infiltration in the liver. This system consisted of 10MHz A-mode transducer, pulse generator (Shimadzu corp.), A/D converter, personal computer, and oscilloscope. The ultrasonic transducer was placed in contact with the liver surface and frequency-dependent attenuation coefficient (FDA) of the liver could be determined "in vivo" from the ultrasonic signals.
    We applied this system to the rabbits of fatty liver model. FDA correlated significantly with the microscopic degree of fatty infiltration and the content of total lipid and triglyceride in the liver. We also applied this system to the patients with liver diseases during peritoneoscopy. There was a significant relationship between FDA and microscopic degree of fatty infiltration in the liver.
    These results suggested that the fatty infiltration in the liver could be assessed quantitatively by our ultrasonic system.

    61 years old female case of PBC was reported. Serum ALP and IgM were markedly elevated, but AMA was negative in repeated examinations. Liver biopsy showed typical CNSDC and granuloma. Hyper γ-globulinemia, elevated IgG, LE cell phenomenon, positive anti-DNA and other types of autoantibodies, and marked piecemeal necrosis in liver specimen revealed lupoid hepatitis. The patient responded well to UDCA therapy without corticosteroid. These findings made the diagnosis of PBC overlapping with lupoid hepatitis. This case and previously reported cases indicate that PBC overlapping with lupoid heptitis should not be same category as CAH-PBC mixed form.

    A female case of benign recurrent intrahepatic cholestasis with infantile onset was reported. A girl had first experienced jaundice at the age of six months, and suffered three subsequent attacks over the next seven years. Light microscopy revealed that there were bile plugs in the centrilobular areas, but neither significant liver necrosis, bile duct proliferation, nor dilation was apparent. Bile acid composition in the serum, urine, and feces were examined for a period of seven years. The levels of total bile acid in the serum, urine, and feces increased and these mainly consisted of colic acid and chenodeoxycholic acid. And coprostanol was not found in the feces.

    We reported a case of asymptomatic primary biliary cirrhosis (PBC) complicating pancreatolithiasis and latent portosystemic shunt. Tortuous shunt veins, which connect inferior mesenteric vein with left ovarian vein, drain hepatofugal blood flow from splenic vein and the flow from left-sided colon into inferior vena cava. Blood flow in superior mesenteric vein is hepatopetal and no hepatic encephalopathy is observed. Pancreatolithiasis is confirmed in the head of pancreas. PBC in precirrhotic stage often complicate portal hypertension, followed by the formation of portosystemic communications. This is the significant case which has latent portosystemic shunt in precirrhotic stage of PBC and is the first case complicating pancreatolithiasis in PBC.

    A case of cystadenoma of the liver is reported. A 71-year-old woman was followed up for postoperative course after operation for esophageal varices. Examination revealed her to have a monolocular cystic lesion of the liver and high serum levels of CEA, CA19-9 and SPan-1. No abnormal findings suggestive of malignancy were obtained. Cytology of the cystic fluid revealed no malignant cells, but the levels of CEA, CA19-9 and SPan-1 in the cystic fluid were abnormally high. Histological examination of needle biopsy specimens yielded findings compatible with the diagnosis of cystadenoma. Those tumor markers were proved to be localized on epithelial cells immunohistochemically. Operative therapy was withheld in view of her poor functioning liver. Now, 20 months after diagnosis, the patient is well with high serum levels of CEA, CA19-9 and SPan-1.
    Cystadenoma of the liver is rare, only 15 cases having been reported so far in Japan. According to Japanese literature, cystic fluid was positive for CEA and CA19-9 in all cases. Tumor markers in the serum and cystic fluid do not seem to be diagnostic of cystic lesion of the liver. Diagnosis should be made only on the basis of a careful scrutiny of diagnostic images and biopsy findings.

    A 61-year-old asymptomatic female was admitted for the examination of the supra-diaphragmatic mass which had been incidentally detected by US. The lateral chest roentogenography disclosed a well defined intrathoratic mass on the right posterior diaphragma. CT scan, MR image and SPECT revealed a protruded mass from the right lobe of the apparently normal liver through the diaphragma, which was not regarded as a neoplastic mass on hepatic angiography. US guided aspiration biopsy of the mass disclosed normal liver tissues. From these studies we diagnosed the tumor as a herniated liver. MR and SPECT was useful for the diagnosis of the natures of the tumor and US-guided needle biopsy for the definite diagnosis.
