    Serum hHGF levels were markedly elevated in patients with fulminant hepatic failure (FHF) including fulminant hepatitis, late onset hepatic failure (LOHF) and acute-on-chronic form of FHF. The majority of these patients showed hHGF values greater than 1.0ng/ml. Serum hHGF levels in patients with subacute form of fulminant hepatitis and LOHF before the appearance of hepatic encephalopathy were significantly higher when compared to those in patients with subacute hepatitis not showing hepatic encephalopathy.
    Maximum serum hHGF levels during the clinical course of FHF were significantly higher in non-survivors than in survivors. In particular, no one whose maximum hHGF level was greater than 7.0ng/ml survived. These results suggest that serum hHGF levels are useful for the prediction of both the development of hepatic encephalopathy and prognosis of the patients with severe hepatitis.

    Ultrasonically guided percutaneous acetic acid (15 to 40%) injection into tumor was performed in 48 patients with hepatocellular carcinoma(s) less than 3cm in diameter. After therapy the size of tumors decreased in all patients and 6 months later main tumor became undetectable by Ultrasound examination in 23 patients. Rebiopsy of main tumors was performed after therapy in 19 patients. Coagulation necrosis and fibrosis but no viable tumor cells were seen in all specimens. Two hepatocellular carcinomas obtained by partial resection at 6 and 20 months after therapy showed complete necrosis. Serum alpha-feto-protein level was reduced significantly after therapy in 4 patients with its high level before therapy. One-year, 2-year, 3-year and 4 year survival rate after therapy was 91%, 84%, 81%, and 61%, respectively. No significant side effects was seen. Thus, percutaneous acetic acid injection will be an effective therapy for small hepatocellular carcinoma(s).

  • Study with cultured human hepatoma cells and primary rat hepatocyte cultures

    We have previously reported that administration of dibutyryl cyclic AMP (DBcAMP) has an antitumor effect on hepatocellular carcinoma (HCC) and also assist in recovery from liver dysfunction in several cases with HCC and its tumor thrombosis in portal vein. In the present report, growth inhibition of HCC and protection against cell injury of hepatocytes by DBcAMP were studied with the human hepatoma cell line PLC/PRF/5 and primary rat hepatocyte cultures.
    DBcAMP addition made the doubling time of cell number of PLC/PRF/5+57%, and+123% longer at 100μM, and 1mM, respectively. Alfa-fetoprotein and PIVKA-II releases in the medium were reduced by DBcAMP addition, but LDH release in the medium as an indicator of cytotoxicity did not increase. In primary culture of rat hepatocytes, DBcAMP addition inhibited DNA synthesis. D-galactosamineinduced cell injury of rat hepatocytes was protected by DBcAMP addition. On the other hand, mitomycin C-induced cell death of PLC/PRF/5 was not only inhibited but also enhanced by DBcAMP addition.
    These results are consistent with the previous clinical observations.

    It is well known that the obstruction of biliary tract induces the hepatic fibrosis. Recently, the concerning of fat-storing cells (FSCs) in the experimental secondary biliary fibrosis has been reported, but it is not clear for the role of FSCs. Therefore, I used three groups of rats, first is 5 days following bile duct ligation (BDL) rats group, second is 10 days following BDL rats group, last group is sham operation rats group (control), with vitamin A. FSCs were isolated from those rats by enzymatically and cultured. The liver of BDL exhibited a moderate increased in pericellular fibrosis compared to those of control rats. In the BDL liver, desmin-positive FSCs were located in pericellular fibrosis area coincided with positive of collagen. The type of collagen of pericellular fibers were type I, III and IV, type I was stained dominantly. 3H-thymidine incorporation into the BDL FSCs in culture was 2.18 and 1.78 times greater than that of control FSCs at 5 and 10 days respectively. 3H-proline incorporation into the BDL cultured FSCs was greater than control FSCs in collagen and non-collagen formation. These results suggest that the proliferative response and the collagen protein formation by FSCs may play a pathogenetic role in secondary biliary fibrosis of the liver.

  • A study of five autopsy cases with pneumoconiosis

    The effect of inspired dust on extrapulmonary organs in cases with pneumoconiosis has not been clarified. In the present study we have investigated the histological distribution of black pigment, which is thought to be identical to inspired dust, in various internal organs of five autopsy cases pathologically diagnosed as pneumoconiosis.
    However we histologically recognized black pigment in the liver and spleen from all five cases, we identified it neither in the skin nor gastrointestinal tract. In the liver, black pigment was recognized mainly within interlobular lymphaticus, perivenous lymphaticus, capsular lymphaticus or adjacent stroma. And we could find a few black pigment in sinusoidal non-parenchymal cell as well. In electron microscopic study, we found three patterns of the association between ultrastructural morphology and its element composition. Though immunohistochemistry revealed that cells containing particles of black pigment were positive for lysozyme, α1-antitrypsin and α1-antichymotripsin, these immun- reactivities were weaker than those of cells without black pigment. It may be though that reticulonedothelial cells with black pigment are reduced in their enzyme activities such as lysozyme, suggesting that cellular immunity is impaired in patients with pneumoconiosis.

    Eight young adult patient (15-23 yr) were studied. They were the patients who required hospitalization among patients having suffered from measles at the young adult age and visited our hospital in 1988 and thereafter. All of the patients showed a slight elevation of GOT, GPT and LDH but no jaundice. Serum GPT level increased with development of rash, reached maximum 6 to 7 days after their appearance, and then returned gradually to the normal level. Serum LDH increased at the time of appearance of rash, reached maximum 4 to 5 days after their appearance (i.e. 2 to 3 days earlier than the peak of GPT), and returned to normal about 2 weeks after onset of rash. Serum Al-P decreased transiently in 4 of the 8 patients. In the recovery stage, liver biopsy was madein 6 of the 8 patients. Histological examination showed slight necrosis of liver cells in all patients but no significant changes in portal area.

    A 21-year-old girl student having positive HBsAg and normal serum levels of GPT since 18 years old showed an acute onset of hepatitis with nausea and vomiting, followed by elevations of serum transaminases and bilirubin. Serum α-fetoprotein levels were relatively high, reaching 1, 237ng/ml, at nearly maximum rises in GOT of 175 KU and in GPT of 137 KU. Histology showed chronic active hepatitis with positive AFP stain among injured hepatocytes remaining in necrotic areas. After IFN treatment, she was seroconverted from HBeAg (+) to HBeAb (+), although transaminase and AFP levels remained slightly elevated, and development of cirrhosis was confirmed peritoneoscopically after 2 years of follow-up, suggesting that AFP elevation is an ominous sign representing severe liver injury rather than a simple hepatocyte regeneration after liver cell necrosis.

    A 78-year-old male was admitted to our hospital because of right hypochondralgia due to cholecystitis. On admission, plain X-Ps revealed radio-opaque shadows in the right shoulder, liver, spleen, and abdominal lymph nodes, suggesting the deposition of Thorotrast which he was injected 56 years ago. Liver tumors were detected by abdominal ultrasonography, CT and MRI. After admission he suffered from sepsis and DIC, and died of sudden respiratory failure on 27th hospital day. An autopsy revealed liver angiosarcoma with pulmonary bleeding from the lung metastasis, which seemed responsible for his sudden respiratory failure, and thyroid cancer.
