Long Evans Cinnamon (LEC) rat is a mutant inbred strain, which spontaneously develops hepatitis and hepatoma with abnormal copper accumulation. Flow cyto-metric (FCM) analysis of the nuclear DNA contents and ploidy distribution of the LEC rat liver cells and hepatoma were performed.
As acute hepatitis progressed in LEC rat, polyploidization occurred as higher frequency of octoploid nuclei. When it recovered from hepatitis, tetraploid and octoploid cells decreased and diploid population exclusively existed. Massive necrosis of the degenerated hepatocytes resulted exclusively diploid change.
As the progression to chronic hepatitis predominant diploid nuclei and increase of the octoploid nuclei were continued during the carcinogenic stage. These changes were strongly correlated with accumulation of copper in the liver tissue. Continuous necrosis and copper results hepatocarcinogenesis in LEC rat.

We measured serum concentration of PIVKA-II determined by Abidin-Biotin complex method in hepatocellular carcinoma (HCC) smaller than 30mm in diameter and liver cirrhosis (LC). The level could be measured 0.004 AU/ml. Of 148 patients with HCC, 66 (44.6%) had positive PfVKA-II level, and of 53 with LC, 10 (18.9%) had positive level. With the combination assay of PIVKA-II and AFP (>100 ng/ml), 88 (59.5%) HCCs had abnormal level. According to histological differentiation, the positive rate of PIVKA-II of well, moderately and poorly differetiated HCC were 29.2%, 57.9%, 71.4%, respectively. In retorspective study, we could detect PIVKA-II two or three months before. PIVKA-II determined by Abidin-Biotin complex method is useful for early diagnosis of small HCC.

It is considered that retrograde portal flow is an evidence for the existence of arterio-portal (A-P) shunt. Fifteen patients with hepatocellular carcinoma (HCC), in whom retrograde portal flow was detected by color Doppler flow imaging, were studied. The correspondence between existence of A-P shunt on angiogram and retrograde portal flow identified by spectral analysis with pulsed Doppler was evaluated. A-P shunt was observed in all 4 cases with pulsatile retrograde portal flow. Out of 11 cases with continuous retrograde portal flow, A-P shunt was observed in 5 cases (45%). In 11 cases with continuous retrograde portal flow, case with A-P shunt showed Mean ± SD of maximum velocity of 10.6 ± 5.3cm/sec, which is significantly higher than that of in cases without A-P shunt. A-P shunt was observed in all cases with maximum velocity of retrograde portal flow more than 10cm/sec. We concluded that pulsatile retrograde portal flow or continuous retrograde portal flow faster than 10cm/sec in velocity is diagnostic of the existence of A-P shunt relating to HCC.

Mutations of p53 gene which controls normal cell growth have been reported in many malignancies. By standard immunohistochemical techniques, these altered gene products are detected in the nuclei of the tumor cells. In this study, we made an immunohistochemical analysis for the expression of p53 protein in 30 cases of hepatocellular carcinomas (HCCs). p53 protein was absent in all four well-differentiated HCCs and detected in two out of 16 (12.5%) moderately, five out of 10 (50.0%) poorly differentiated HCCs. Furthermore, the expression of p53 protein was significantly correlated with the increased cell proliferative activity as detected by the Ki-67 staining. Thus, our data suggest that the p53 gene mutation in hepatocellular carcinoma occurs in the tumor with low cellular differentiation and high proliferative activity.

The present study was performed to investigate sequential changes in bile flow and biliary lipid excretion during liver regeneration after partial hepatectomy (PH) in the rat. Experiments were carried out 1, 3, 7 and 14 days after PH. When sodium taurocholate (TCA) was infused at a rate of 0.5μmol/min/100g b.wt. i.v., bile flow at 1, 3 and 7 days after PH was significantly higher than in controls; especially at the 3 day time point an increase to about 160% of the control level was observed. Biliary excretion of phospholipids and alkaline phosphatase at 3 and 7 days after PH demonstrated paticular augmentation compared with control values. Moreover, the transport maximum of TCA at 1, 3 and 7 days after PH showed a significant increase. Using an isolated liver perfusion system, TCA was administered at rates of 0 to 0.33μmol/min/g liver to groups of non-treated controls and hepatectomized rats 3 days after the operation. Bile flow, biliary phospholipid output and the transport maximum of TCA in the operated group were significantly higher than those observed for control livers, mirroring the results of the in vivo experiments. We therefore conclude that acceleration of bile flow and bile acid excretion into bile after TCA loading of partially hepatectomized rats is dependent on sufficient resupply of phospholipid to the bile canalicular membrane.

Twenty-five nodules of focal nodular hyperplasia (FNH) from 23 patients were studied clinicopathologically. Male to female ratio was 1:0.53, and the mean age was about 49 years old. Among 6 cases in whom ultrasound-guided fine-needle aspiration biopsy was performed, 4 cases were misdiagnosed as "well-differentiated hepatocellular carcinoma", and only 2 cases were properly diagnosed as "FNH". "Central scar" (stellate fibrosis), which is one of the characteristic feature of FNH was seen even in 3 minute nodules out of 5 less than 1 cm in diameter. FNH nodules without a distinct central scar accounted for about 30% of all the nodules. There were 3 nodules with capsule formation, 4 nodules with liver cell dysplasia and 14 nodules with fatty change. Chronic hepatitis was associated in two cases. Hemangioma-like lesion surrounded by zonal hyperplasia of hepatocyte in the vicinity of FNH nodule seen in one case suggested the possibility of focal vascular abnormality as the pathogenesis of FNH.

The amino acid metabolism dramatically altered in the two periods after transplantations of the whole liver and pancreas with duodenum (Group-I), the whole liver alone (Group-II), and the whole pancreas with duodenum (Group-III). Within 6 hours after resuming blood supply, serum amino acid levels except for BCAA significantly changed compared with preoperative levels, in Group-I and -II, respectively. However in 5 or more postoperative days, when the rejection became apparent, the levels in Group-I showed the levels in between Group-II and -III. A significant reverse correlation was recognized, between AAA and AKBR, and between BCAA and IRI in the serum, and a right correlation between femoral arteriovenous difference of BCAA and serum IRI.
Therefore, the amino acid metabolism after the transplantations characteristically changed according with the viability of graft in the early period, as well as the degree of rejection in the late period. Moreover, insulin played an important role in BCAA metabolism after the transplantations.

Three cases of Vibrio vulnificus infection were presented. All were suffered from liver cirrhosis as underlining disease and died. Forty six cases in Japan including ours were reviewed about clinical features and the relationship with pre-existing liver diseases. Thirty eight primary septicemias and 2 wound infections were included, and infectious routes were not confirmed in 6 patients. There were 40 men and 6 women (age range, 33-73: mean ± SEM, 57.4±1.1). All cases occurred from June to October, and 44 cases located in Western Japan. 93.5% patients had chronic liver diseases especially liver cirrhosis and 41.9% of them were either patients of alcoholic liver disease or alcohol abusers. Gastrointestinal symptoms developed in 67.4% patients and cutaneous lesion in 87.0%. Mortality rate was 65.2%. Liver cirrhosis patients could be favorite hosts for this bacterial infection because of existence of gastrointestinal bleeding, arteriovenous shunt, insufficiency of intrahepatic reticuloendothelial system and excess of serum iron, etc.

41-year-old female, who had been pointed out abnormalities of liver function tests since 15 years ago, was admitted to our hospital for further examination of slight jaundice. Laboratory findings showed normal except for unconjugated hyperbilirubinemia (T-Bil: 3.7, ID-Bil: 3.0mg/dl) which significantly elevated after low calorie diet for 3 days. Moreover, a marked retention of plasma ICG was observed (R15:65%), whereas plasma BSP retention rate remained normal. Since her serum antiHCV and HCV-RNA were positive and liver biopsy specimen showed chronic inactive hepatitis, she was diagnosed as type C chronic hepatitis accompanied by inherited disorder of hepatic transport of unconjugated bilirubin and ICG. However, a possibility that type C hepatitis had influence on the abnormalities of hepatic dye metabolisms including bilirubin and ICG cannot be excluded.

Usually 15-30 years are required for the occurrence of hepatocellular carcinoma in patients with Ctype hepatitis who received blood transfusion. We treated a patient with C type hepatitis in whom hepatocellular carcinoma occurred early (4 years 3 months) after she received blood transfusion. The case is a 73-year-old female who took cholecystectomy for cholelithiasis at 69 years old. Liver function tests were normal. The liver surface observed at the operation was smooth and histology of the liver appeared normal. She received blood transfusion for bleeding after the operation. Acute hepatitis with jaundice occurred one and half month later. Hepatitis continued and the diagnosis of cirrhosis was made 4 years and 3 months later. At the same time, three small nodules of 1cm in diameter were detected by ultrasonography, which were histologically diagnosed as hepatocellular carcinoma by fine needle biopsy. Early transition to cirrhosis due to active hepatitis is considered for the early occurrence of hepatocellular carcinoma after blood transfusion.

The patient was a 34-year-old male who experienced right hypochondrial pain and was referred to our hospital because of a liver tumor. His liver was palpated 4 finger breadths. RBC was 544 ×104/mm
3 and Ht 55.7%. Serum erythropoietin showed 550 mIU/ml. The imaging diagnosis revealed a giant tumor and multiple lesions with a diameter of 10 to 20 mm around the tumor. The erythropoietin value increased after admission and reached a peak of 7100 mIU/ml. He passed away due to liver failure. The histological findings showed moderately differenciated hepatocellular carcinoma and erythropoietin was confirmed immunohistologically in the cancer region. In patients with hepatocellular carcinoma, erythropoietin is sometimes seen, but little is known concerning the disease state. A review of the literature revealed that there have been no reports of cases of hepatocellular carcinoma with erythropoietin production confirmed immunohistochemically, and this case appeared to be valuable.

Three cases of autoimmune hepatitis (AIH) presenting with acute hepatic failure (AHF) were reported. All of them were middle-aged female with initial symptoms of lassitude, nausea and jaundice. Typical clinical features included elevated γ-globulin (2.8-3.6g/dl), with high IgG (2438-4454mg/dl) and positive ANA (×160-×40960) and LE test. All patients died despite of corticosteroid treatment with artificial liver support.
13 Japanese cases with AIH with similar features have been reported to date. High mortality of these patients (12/13: 93%) suggests that the management of AIH with AHF merits reconsideration.

In 1980, a 64-year-old female was first admitted to our hospital after pointing out of liver injury in a mass survey. A mild elevation of serum ductal enzymes were observed on the laboratory data. And histological findings of the liver revealed fibrous expansion and inflammatoric change of portal area. However, the common bile duct appeared to be normal on drip infusion cholangiography and normal colonogram was obtained. After 7 years she admitted for liver examination again. Ductal enzymes had remarkably elevated, and ERCP demonstrated typical beaded appearance in the whole biliary tree. In addition, barium enema examination showed a loss of haustral marking and the presence of small ulcers of the colon. In the present case, the lesion extended from the intrahepatic bile duct to the extrahepatic bile duct, and intrahepatic PSC preceded UC. In 1988, she first showed jaundice and then ursodeoxycholic acid (UDCA) was administered. Although UDCA improved the elevation of ductal enzymes, recurrent jaundice has been sustained. No significant change was shown in cholangiogram by the examination of follow-up ERCP. Now she is suffering from decompensated liver cirrhosis due to PSC.
