    We reviewed our experiences of percutaneous transhepatic portography (PTP) and Doppler ultrasonography (DUS) to disclose the characteristics of portal hemodynamics in 57 patients with fundic gastric varices. The mean portal vein pressure in 33 patients with fundic gastric varices who underwent PTP was 284mmH2O and it was significantly lower than that in patients with high-grade esophageal varices. Main feeders of fundic gastric varices were collaterals near the splenic hilum in 82% of the patients. In 23% of the 57 patients with fundic gastric varices, reversed blood flow was observed in the splenic vein by PTP and/or DUS. Reversed blood flow in the splenic vein appeared more frequently in bleeders than in nonbleeders. Mean blood flow velocity in the feeding vein of fundic gastric varices was measured in 20 patients at splenic hilum by extracorporeal DUS. It was faster in bleeders than in nonbleeders. These findings indicate that high portal vein presure, reversed blood flow in the splenic vein and fast blood flow velocity in the variceal feeder are characterstic hemodynamics associated with bleeding from fundic gastric varices. These findings should be helpful in the strategy of treatment for fundic gastric varices, i.e., indications and methods of the treatment.

    The purpose of this study is to analyse the restriction fragment length polymorphism (RFLP) of pro α2 (I) collagen gene (COL1A2) in Japanese, and to examine its relationship with the pathologic feature and progression of alcoholic liver disease in Japanese patients. We studied 43 normal Japanese, 20 alcoholics with cirrhosis and 22 alcoholics without cirrhosis. Genomic DNA was isolated from the white blood cell, digested with either Eco RI or Msp I, and subjected to Southern blot hybridization with RFLP-specific probes. In the normal Japanese subjects, the E1 (polymorphic site +) and M2 (polymorphic site -) alleles were found more frequently as compared with the previous reports from the Western countries, indicating that there is a difference in genetic countries, indicating that there is a difference in genetic background of collagen gene among races. In contrast, no difference of RFLP pattern could be detected between the two groups of alcoholic subjects, indicating that there is no relationship between COL1A2 RFLP and the progression of alcoholic liver disease in Japanese patients.

    We investigated the influence of warm hepatic ischemia time on the isolation and culture of human hepatocyte from resected liver tissue. Human hepatocytes were isolated by perfusion of a mixed solution of collagenase and dispase, followed by cultivation for 10 days. The metabolic functions of the cultured human hepatocytes were evaluated by the measurement of gluconeogenesis, urea synthesis and DNA contents. The yield of isolated hepatocytes was significantly correlated with the warm ischemia time (0 to 300min) of the liver specimen. The yield of the hepatocytes was significantly higher in the warm ischemia below 120min than that above 180min. However, the viability of isolated hepatocytes was not affected with warm ischemia time of the liver tissue. The metabolic function of the cultured hepatocytes were well preserved for 10days without disturbance of warm ischemia. These results show that the effective isolation and culture of human hepatocytes is possible from the liver fragments surgically obtained up to 120min of warm ischemia.

    Immunohistochemical study was done on fifty cases of the resected hepatocellular carcinomas (HCCs) tissue section with anti-p 53, anti-HSP72 and anti-PCNA rnonoclonal antibody.
    Positive nuclear staining was found in 11/50 (22%) stained with anti-p 53 and in 23/50 (46%) with anti-HSP72 antibody. The tumor sizes of all cases of p 53 and HSP72 overexpession were more than 2cm in diameter. Positive rate of p 53 overexpression was significantly higher in patients with moderately and poorly differentiated type HCCs comparing with well differentiated ones (p<0.05). The vp-positive group showed significantly higher positive rate of p 53 overxpression than that of negative group (p<0.05). HSP72 overexpression was highly positive in patients with moderately and poorly differentiated HCCs or recurrent HCCs. Furthermore, 10 out of 11 cases (90.9%) of p 53 overexpression were significantly correlated with HSP72 averexpression (p<0.01). PCNA LI of HCCs were significantiy higher in patients with liver cirrhosis, high serum AFP level (p<0.05), fc-inf positive growth, massive type and single nodular type with proliferation into surounding area (p<0.01). However, no correlation was found with PCNA LI and p 53 overexpression, or PCNA LI and HSP72 overexpression.
    These results suggest that p 53, HSP72 overexpression and PCNA LI play a crucial role on the evaluatian af biological characteristics of the malignant patentiais in human HCCs.

    A lupoid hepatitis associated with idiopathic thrombocytopenic purpura (ITP) was reported. In this report we focused mainly on the changes in anti-sulfatide autoantibody.
    A 42-year-old female who complained of knee-joint pain and severe general fatigue at the onset. While transaminase levels improved as a result of SNMC administration which had been started before diagnosis, the levels of γ-gl remained high. Prednisolone was administered to counter advancement of thrombocytopenia. Subsequently, the levels of transaminase, γ-gl and platelet count returned to normal levels.
    Upon the marked decreas in the platelet count, anti-thrombocyte and anti-sulfatide IgM antibody became positive. It has been reported that anti-sulfatide is a target antigen in autoimmune hepatitis and ITP. Therefore, it was considered interesting that changes of these autoantibodies could be followed up in a case of autoimmune hepatitis associated with ITP.

    A female, sixty four years in age, with typical autoimmune hepatitis which was diagnosed finally after three-years following-up before manifestation is presented. Three years ago, she was diagnosed to have non-A non-B hepatitis by liver biopsy under peritoneoscopy. Histological diagnosis was chronic active hepatitis. Her liver dysfunction unchanged during three years following-up in outpatient clinic. Then, she was readmitted for the elevated γ-globulin and anti-nuclear antibody titer. Histology showed chronic active hepatitis with bridging necrosis acompanied with occasional plasma cell aggregates. HCV antibody (Abott II) was positive, while HCV RNA was repeatedly negative. Corticosteroid administration was found to be strikingly effective in this case, indicating the presence of autoimmunity as a main cause of this disorder.

    A 53-year-old male with alcoholic liver cirrhosis, who intook heavy alcohol abuse over 30 years, was admitted to our department with complications of jaundice, ascites and edema. The patient had persistent jaundice and hemolytic anemia. On an examination of the peripheral blood smear, approximately 30% of erythrocytes were of spur cell in shape. In analysis of lipids in erythrocyte membranes, free-cholesterol/phospholipid ratio was elevated. Accordingly, the case was accompanied with spur cell anemia. Ecchymosis and endotoxemia appered during admission, for prevention of disseminated intravascular coagulopathy (DIC), administration of Polymyxin B Sulfate and Kanamycin Sulfate improved endotoxemia along with haemorrhagic tendency. Spur cell anemia often induces DIC associated with hemolytic attacks. Its prognosis is poor and its therapeutic modality has not yet been established. Thus it was concluded that the treatment for endotoxemia was useful to prevent DIC in this case.

    A 61-year-old man was admitted to our hospital because of enlarged gastric varices. Cardiofornical varices were seen via endoscopy. Color Doppler ultrasonography (CD) revealed the color flow images of gastric varices and gastro-renal shunt. After balloon-occluded retrograde transvenous obliteration (B-RTO) on gastric varices, size of gastric varices decreased.
    However, the esophageal varices were enlarged requiring endoscopic injection sclerotherapy. Next, we examined hemodynamics of gastric varices prior to and after B-RTO using color Doppler ultrasonography (CD). The blood flow velocity and amount did not change in the portal trunk prior to and after B-RTO.
    New findings were: 1) After B-RTO, blood flow velocity and amount increased in umbilical portion, and that of hilus of the spleen decreased. 2) Paraumbilical vein increased in size after B-RTO. 3) Growth of the left gastric vein and paraesophageal vein was observed with endoscopic color Doppler ultrasonography (ECDUS).
    In conclusion, CD was an effective modality for the evaluation of hemodynamics prior to and after B-RTO.
