    A 38-year-old female was diagnosed as alcoholic liver cirrhosis in 1996. In 1999, ultrasound (US) examination of her liver revealed multiple hypoechoic nodules, 10-20 mm in diameter. US-guided biopsy of S8 nodule was performed, and the lesion was histologically diagnosed as hyperplastic nodule. Follow-up biopsy in 2002 gave the same pathological diagnosis as before, although the lesion showed a change of contrast-enhanced appearance in computed tomography (CT). This nodule gradually decreased in size and finally disappeared in 2005. S4 nodule, which was newly visualized with diameter of 5 mm in 2000, gradually increased in size. In 2004, the lesion reached 20 mm in diameter, and was diagnosed as hyperplastic nodule by targeted biopsy. A year later, the lesion showed an increase in size up to 30 mm in diameter and a distinctive change of enhanced configurations in CT. Follow-up biopsy revealed similar histopathological findings to those of the previous one.

    We report an unusual case of metastasis of sarcomatous HCC to the heart. The patient was a 74-year-old man who was found to have HCC in July 2002. He had undergone transcatheter arterial embolization 3 times from July 2002 to June 2003. In February 2004, he began to complain of general fatigue and backache. Abdominal CT revealed a hypovascular liver tumor measuring 6cm in diameter in segment IV of the liver with an accumulation of lipiodol in the center of the tumor. Sixteen days after admission, he died suddenly of unidentified cause and an autopsy was performed. Several nodules were scattered through segement IV, II and III. Hepatic tumor involving segment IV with massive invasion into the diaphragm was observed. The sarcomatoid cells had metastasized to the lung, bone, bilateral adrenal glands and myocardium. Histologically, the hepatic tumor and all of metastatic lesions consisted of spindle-shaped sarcomatoid cells. Immunohistochemical study showed that the sarcomatoid cells were positively stained for cytokeratin and vimentin.

    We studied the timing of withdrawal of lamivudine administration for HBe Ag-negative chronic patients. The amount of serum HBV DNA was measured using the Amplicor HBV monitor (AM) and the Real-time Detection direct (RTD-direct). All values became less than 2.6 log copies/ml according to the AM method after administration. Five of 16 patients were found to have no HBV according to the RTD-direct (less than 1.0 log copies/ml). Thus, withdrawal of administration was attempted for these five patients. After the withdrawal, the viral load and ALT levels had increased in all patients. These results showed the difficulty of withdrawing lamivudine administration from HBe Ag-negative chronic patients, even though they have very low viral load in serum by the treatment.
