    The goal of this study is to identify preoperative factors that influence prognosis in TACE of hepatocellular carcinoma (HCC). Between April 2014 and April 2021, 106 consecutive patients underwent TACE for HCC. We discovered that albumin, ALBI score, AFP, tumor number, and Up-to-7 were useful factors in univariate analysis; albumin (p = 0.025), AFP (p = 0.000), and tumor number (p = 0.000) were useful factors in multivariate analysis. We compared the relationship between two tumor factors (tumor number and tumor diameter) and selective embolization. The AUC of tumor number was 0.754 and that of tumor diameter was 0.693, with tumor number being higher. It has been proposed that tumor number, rather than tumor diameter, is a better predictor of TACE prognosis. It is considered to be one of the reasons why tumor number has a stronger effect on selective TACE.

    During the chronic hepatitis C follow-up of an 89-year-old woman, an enlarged lymph node around the head of the pancreas was discovered on abdominal ultrasonography, leading to the diagnosis of diffuse large B-cell lymphoma. Hepatotoxicity appeared during systemic chemotherapy including rituximab and persisted after chemotherapy, indicating hepatitis C exacerbation due to hepatitis C virus (HCV) reactivation. Hepatotoxicity improved rapidly after glecaprevir/pibrentasvir treatment, and a sustained virological response 12 was achieved. In chronic hepatitis C patients, as in chronic hepatitis B patients, the possibility of chemotherapy-induced HCV reactivation and hepatitis exacerbation should be kept in mind at all times, and if hepatotoxicity persists, antiviral treatment with direct-acting antivirals should be considered for chronic hepatitis C exacerbation.

    The patient was a man in his 70s who had been attending our hospital's Department of Cardiology for an old myocardial infarction; however, in 20XX, a defibrillator was implanted due to cardiac arrest caused by ventricular fibrillation, and amiodarone was used to prevent arrhythmia. He was referred to our department 1 year and 8 months after taking amiodarone due to liver damage, and an abdominal simple CT examination showed high absorption of the liver. He was diagnosed with drug-induced liver injury due to amiodarone and was advised to discontinue the drug; however, the drug administration was continued because there was no alternative drug. He was diagnosed with cirrhosis four years and one month after starting amiodarone, and he stopped taking it. He died of liver failure six months later. Because amiodarone-induced drug-induced liver injury can quickly progress to cirrhosis, it is important to monitor blood levels of amiodarone and adjust the dose.

    The diagnosis of NASH requires a liver biopsy; however, actual clinical practice is unknown. In this study, we analyzed a large database of medical claims from 2013 to 2017, which included 7, 368, 327 insured people. The number of people newly diagnosed with NASH was 2, 224. Among these, 153 (6.9%) had received liver biopsies in the three months prior to the new NASH diagnosis. The elderly had a higher rate of liver biopsy implementation. Over the last 5 years, there had been no change in the implementation liver biopsies. According to the findings of this study, it is desirable to develop an alternative method for evaluating liver fibrosis instead of a liver biopsy, which is not performed at the clinical site.
