The annual the incidence rate of tuberculosis in Tochigi prefecture has been slowed indecreasing or rather been increasing since 1980.
The purpose of the study is to clarify the factors infuluencing the yearly changes of theincidence.
The incidence rate of tuberculosis, positive rate in bacteriological findings, averageduration with active tuberculosis, and etc were observed from 1981 to 1987 by tuberculosiscontrol analytical chart.
The results thus obtained were as follows:
    1. The incidence rate of tuberculosis has increased since 1982 in Tochigi prefecture.
    2. In Tochigi prefecture, the proportion of patients with bacteriological examinationswas lower, and the rate of patients with positive findings was higher than that of Japan.
    3. The incidence rate in the age group of 5-14 years in Tochigi prefecture was higherthan that of Japan.
Younger age group should be given health education of tuberculosis.
    4. While the duration of active tuberculosis had been being shortened until 1984 in Tochigi prefecture, but since then the average duration became as long as that of wholeJapan.
    5. The proportion of infectious patients without medical treatment in Tochigiprefecture had decreased until 1983, but since then it became as high as that of Japan.
Thesurveilance system of tuberculosis should be used to find infectious patients without medical treatment.

To evaluate the usefulness of a new oxymatic conserver, Demand Oxygen Delivery System (DODS), we compared DODS breathing with Standard steady flow (SF) breathingin thirteen subjects with chronic respiratory failure due to mainly tuberculosis sequelae. The value of the DODS (Oxymatic) is that it delivers oxygen only during early inspiration, so as to minimize loss from delivery during expiratory phase. Improvement of SaO2, measured by BIOX3740, and PaO2were observed at rest and on exercise. The oxygenconsumption ratio of the DODS to the SF method was between 0.5 and 0.3, favoring the DODS over the SF method. In some patients DODS hardly ran at rest. But no problemsare observed during exercise. The results indicate the effectiveness of DODS in advancement of quality of life patients with chronic respiratory failure.

To reveal the possibility of differentiating diseases caused by M. tuberculosis and M. intracellulare, simultaneous tuberculin testing by PPDs and PPD-B was carried outamong X-ray suspects of tuberculosis and healthy persons.
PPD-B was prepared by Dr. Tasaka (Department of Bacteriology, Hiroshima University) from M. intracellulare (ATCC13950).
For tuberculin testing, 0.05μg of PPDs from M. tuberculosis (Nihon BCGCo.) and 0.1, μg of PPD-B were used.
The study included61patients with disease caused by M. tuberculosis (TB), 23 patients with that of M. avium complex (MAC) and 40 healthy persons with no roentgenologicalabnormality (H).
Forty healthy persons had been vaccinated with BCG. Statisticalanalysis of the diameter of reaction (redness) in each antigen in each group has been doneby Boxplotting method.
    The results were as follows: 75% upper quartile point, median, 25%lower quartile point/mean S. D.: PPDs in TB (41.8, 30.0, 19.0/32.0-17.7) PPDB in TB (15.0, 10.5, 5.0/10.9 8.1) PPDs in MAC (26.0, 10.0, 7.0/16.4-13.9) PPDB in MAC (20.5, 17.5, 12.5/19.1-11.4) PPDs in H (18.0, 12.0, 6.0/13.5-10.9) PPDB in H (7.0, 2.8, 0.0/4.4-5.4).
Mean of PPDs in TB patients and PPD-B in MAC patients were significantly (P<0.01) larger than those in other groups. Significant differences of reaction by age distribution, previous history of mycobacteriosis or other diseaseswere not observed.
In each patient, larger skin test reaction correlated with the infected organism in 71 (85%) of the 84 cases.
In 61 patients with M. tuberculosis infection, reaction to PPDs waslarger than that of PPD-B in 58, and the reaction to PPD-B was larger than that of PPDs in only one subject, and the reaction to PPDs and PPD-B were smaller than 5mm in 2. In 23 patients with MAC infection, the reaction to PPD-B was larger than that of PPDs in 13, the reaction to PPDs was larger than that of PPD-B in8and the reaction to PPDs and PPD-B were smaller than 5mm in 2.
It is concluded that simultaneous skin test with PPDs and PPD-B is useful in thedifferential diagnosis of mycobacterial disease due to M. tuberculosis and M. aviumcomplex prior to bacteriologic confirmation by culture.

A study to obtain information on the relation between tuberculosis and carcinoma ofthe lung was carried out in 24 hospitals in Chugoku and Shikoku areas during the period from January 1979 to December 1988 using a questionnaire.
As a result of. this survey, 142 cases of coexisting active pulmonary tuberculosis and bronchogenic carcinoma were reported during the period.
The incidence of coexisting caseswas 2.32% of patients with pulmonary tublrculosis and 2.22% of patients with bronchogeniccarcinoma.
The foci of tuberculosis and carcinoma were found more often in the same lobe than indifferent lobes.
The proportions of histological types of bronchogenic carcinoma were asfollows; squamous cell carcinoma 43.9%, adenocarcinoma 36.0%, small cell carcinoma 16.5% and large cell carcinoma 3.6%.
These findings were not singnificantly different from thosein the general population. No significant difference in the proportions of histologicaltypes was found by coexisting lobe.
Some cases indicated that coexisting tuberculosis gave a favorable influence to theprognosis of lung cancer.

Bactericidal activity of ofloxacin against Mycobacterium kansasii was observed inin-vitro experiment. Tested bacteria were suspended to a concentration of one mg wetweight per ml in 10ml of Dubos liquid medium containing no drug or containing 1 or 3kμg/m/l of loxacin and incubated at 37°.
The number of colony-forrming units contained in a 0.02ml-sample of the Dubos liquid medium was counted after incubation for 0, 1, 3 and 7 days. The number of colonies was counted in Ogawa egg medium, to which was inoculateda 0.02ml-sample of dilutions of the medium by a spiral loop that can deliver a 0.02ml-sample by one inoculation. The bactericidal activity of ofloxacin against M. kansasiiappeared after incubation of 3 days or later. This characteristic was different from thebactericidal activity against Mycobacterium tuberculosis, which appeared after incubation of 24 hours.

We reported one case of pulmonary aspergillosis secondary to pulmonaly tuberculosis, which perforated to the pleural space and was complicated with chronic empyema. Weperformed thoracoplasty, cavernoplasty and extraperiosteal detachment. The type andindications of cavernoplasty we use are also described.
