  • A CASE REPORT OF CYTOMEGALIC INCLUSION DISEASE

    A case of congenital cytomegalic inclusion disease is described. The main manifestations were encephalomeningitis, mental retardation, neonatal hepati-tis, splenomegaly, and prolonged albuminuria. The diagnosis was established by cytomegaloviruria, high titers of complement fixation antibody to cytome-galovirus persistent during infancy, and a cytomegalia with intranuclear inclusion body in urine.
    The hypothesis that albuminuria of long duration, and prolonged abnor-mality of the cerebrospinal fluid can presumably be ascribed to congenital cytomegalovirus infection is discussed.

    Radiation myelopathy has developed in 9 cases among irradiated patients as a side effect following radiotherapy for malignant tumors. The dose to the spinal cord was 5, 000-10, 000 rads, the over all treatment time 42-71 days. Several causative factors of the radiation myelopathy were picked up.
    The degree of radiation influences in the spinal cord seem to be time-dose dependent, as reported by Boden and Pallis. A recommended tolerance dose to spinal cord was over 4, 000 rads and over 5, 000 rads according to Tsuya's and Friedman's reports respectively and an NSD* of 1, 300 rets and 1, 500 rets was recommended by Maier and Phillips respectively. From the data presented it is apparent that degree of occurrence of radiation myelopathy depends greatly on fractionation and irradiation dose. When the patients are treated in more than 21 fractions at a daily dose of 200 rads and the dose to the spinal cord irradiated is more than 5, 000 rads, it can be considered to be the critial level for radiation myelopathy.

    Five cases of rheumatoid arthritis (RA) of hips were presented on this paper, which were treated surgically and gave special problems. Surgical indications and treatments of RA in the hip joints are discussed here in details. Surgery should be determined considering the atrophy of bone and muscle, bleeding tendancy and general conditions because RA is the systemic disease.
    Generally speaking femoral head prosthesis has a tendancy to think into the ilium or pelvis and is rarely indicated.
    Cup arthroplasty is indicated in the relatively young patients when the disease is unilateral and other joints are preserved. Resection-Angulation osteotomy is indicated in the inactive elderly patients when the function of other joints are preserved well. But rare cases fit to these indications mentioned above.
    Total hip replacement has the wider indications than other procedures. However the history of this operation is too short and many problems have not been solved yet. Therefore we have to be very cautious especially as to its indications and treatments.

    A 68-year-old man was admitted with a palpable liver and a defect in scintigram. Selective celiac arteriography showed a rounded hypervascular tumor with the diameter of 13.5 cm situated in the bifurcation of the hepatic artery. Laparotomy revealed that the liver was normal in appearance and tumor was not palpable. However, needle and wedge biopsy examination dis-closed hepatoma deep in the liver with hepatic fibrosis.
    Inspection and palpation at operation are not always reliable guidelines and open biopsy should be utilized for the identification of liver cancer and concomitant diseases and to determine the resectional line at the time of hepatectomy.
