Hemorrhagic hypotension in dogs was treated by infusing lactate Ringer solution (LRS) 50 ml/kg after reinfusing the entire shed blood. The renal effects were compared with the non-bleeding group given the same amount of LRS in order to identify the fluid shift during the hemorrhagic period.
The infusion of LRS in the non-bleeding group produced a marked increase in both free water and osmolar clearance. The increase in free water excretion led to elevations of plasma sodium concentration. Since over expansion of ECF volume suppresses ADH secretion as evidenced by lowered osmolar U-P ratios, it follows that changes in the ECF volume are dominant over the effect of osmolarity on ADH release.
Although excretion of free water exceeded that of sodium resulting also in hypertonic trapping in the hemorrhagic dogs, the calculated concentration of sodium which was unexcreted was lower than that in the non-bleeding dogs. Augmented urinary volume despite the reduction of GFR suggests the pos-sible role of the so-called physical factors in the hemorrhagic dogs. The fact that urinary sodium concentration did not differ significantly in both groups, despite the evidence of high activity of aldosterone in the hemorrhagic dogs, indicates a more profound suppression of sodium reabsorption due probably to sodium accumulation in the peritubular interstitial space during the hemorrhagic period. The so-called “third space” therefore actually exists in hemorrhagic shock.

We have treated surgically 7 patients with tracheal injury, 6 patients with tracheal stenosis caused by tracheostomy, and 20 patients with primary or secondary tracheal tumors in the past 20 years. Five of the 20 patients with tracheal tumors had resection of the carina. In 3 of the 5 patients, a new carina was created by anastomosing the trachea with the left main bronchus and the right intermediate trunk. In 4 patients, 9 rings or more the trachea were resected.
All of the 33 patients were treated by primary anastomosis.
Eight of the 33 patients died. In 3 of the death cases, the cause was unrelated to the surgery. Eight patients had complications.

The number of mast cells and their granularities in the cortical inter-stitium of the kidney were estimated in a variety of diseases. Those counts were significantly increased in patients with chronic renal failure, and there was a significant relationship between serum creatinine and the mast cell granulation index. It is supposed that mast cell proliferation is intimately related to the desolation of the kidney cortex.

High risk of lung cancer among chromate workers have been reported by many investigators, but it has been limited to the refining process. A recent report demonstrated that there was a significantly high number of deaths from cancer among chromium platers.
In this report, a cohort study was carried out on platers in Tokyo. Among 952 chromium platers who were followed up from April 1, 1970 to September 30, 1976, 21 deaths were observed, but there was no case of lung cancer.
In spite of the small size of the population investigated, it was discussed that there was not so much risk of cancer, particularly lung cancer, among chromium platers.

Since 1972, many prosthesis for the knee joint have been reported. They are, however, designed only for hinge and rolling functions neglecting the screw-home function. Surgical damage by massive bone resection would not permit secondary operation even if it is required and only osteosynthesis or hinge prosthesis are indicated thereafter. Musculo-tendinous stability being absolutely required on surgery, ligamentous transfer is often accompanyed in those prosthesis.
In view of the above, we deviced a new functional endoprosthesis for the knee joint which satisfies all three functional phases of knee motion and maintains sufficient stability.
The functional endoprosthesis (Keio type) is indicated for osteoarthritis, rheumatoid arthritis and some type of condylar fracture. Operative damage by this endoprosthesis is fairly less and secondly reconstruction using any type of similar prosthesis may still remain the treatment of second choice.
