背景・目的：終末期患者における血液培養（血培）検査の臨床的意義は十分に検討されていない．緩和ケアチーム介入患者における血培検査の実施状況と，死亡前30日以内の陽性率，抗菌薬使用および予後との関連を検討した．

対象と方法：対象は2023年10月から2025年9月までに血培検査が実施された当院入院患者．介入の有無で陽性率を比較し，死亡前30日以内の症例について陽性率，抗菌薬使用，生存期間を解析した．

結　果：血培検査は18,222セット実施され，陽性率は介入群14.2％，非介入群13.0％で有意差はなかった．死亡前30日以内の陽性率は23.9％であった．抗菌薬は78％に投与され，陰性群では中止が多く，生存期間に有意差はなかった．

結　語：終末期患者でも血培陽性例は一定割合存在し，陰性結果は抗菌薬中止と関連する可能性が示唆された．血培検査は終末期医療における抗菌薬適正使用や治療方針決定に寄与し得る．

　高血圧症を含む生活習慣病予防やフレイル状態を防止するために運動習慣の確立と食生活の改善は必須である．しかしながら，一定の啓発活動無くしては運動習慣を広く国民に定着させることは不可能である．2025年6月から2025年12月まで地方新聞紙面での啓発文書掲載と，その内容に連動させた実地講演会が月1回程度行なわれ，その一環として軽登山が行われた．内容は初心者向けの軽登山推奨と食習慣改善提案である．実地講習会には13人が参加した．実際に運動中に血圧を測定することで，運動の血圧に及ぼす影響などを理解してもらうことができた．1回の講習会で本格的な効果をもたらすことは難しいが，血圧管理をしながら安全に運動習慣を確立するには有効と考えられた．

Background & Aims: Cold sensitivity (hiesho) is predominantly observed in women and is associated with impaired peripheral circulation and reduced quality of life. This study examined the effects of auricular massage as a self-care practice in women with sensitivity to cold.

Methods: The participants were 20 women with cold sensitivity. Auricular massage did not require stimulation of specific auricular points; instead, participants massaged the entire ear. They performed a self-administered auricular massage for approximately 5 minutes while watching an instructional video. Skin and core temperatures, skin blood flow, blood pressure, pulse rate, subjective sensation of warmth, and Profile of Mood States 2 (POMS 2) scores were measured before and after the intervention. The study was approved by the Gunma University Ethics Committee for Medical Research Involving Human Subjects (HS2023-049).

Results: Post-auricular massage, skin temperature significantly increased (p＝.002), and participants reported greater warmth in their hands and ears. Negative mood states assessed using POMS 2 also improved significantly (p＝.012).

Conclusion: Comprehensive auricular stimulation through self-administered massage may improve physiological and psychological stress responses in women with cold sensitivity. This noninvasive, simple, and convenient technique can be performed independently and may serve as a self-care nursing intervention in clinical settings.

　症例は40歳女性，21歳の頃より当院にて不定期に子宮頸がん検診をしており，34歳の時，市の子宮がん検診集団検診にて上皮内病変陰性（NILM: Negative for Intraepithelial Lesion or Malignancy），ヒューマンパピローマウイルス（HPV: human papilloma virus）16型単独陽性を指摘され当院にてフォローアップ．HPV16型陽性があったため途中細胞診が上皮内病変陰性を繰り返したが継続して細胞診をフォローアップを実施．指摘から約6年後の40歳の時に高度扁平上皮内病変（HSIL: High-grade Squamous Intraepithelial Lesion）へ異形度の上昇を認めた．細胞診が正常であってもHPV16型陽性であれば要観察の対象にすべきだと思われる症例であった．

　　Pancreaticoduodenectomy (PD) in the presence of celiac axis stenosis (CAS) caused by median arcuate ligament (MAL) compression carries a substantial risk of hepatic ischemia when complex hepatic arterial variations coexist. An 80-year-old man with markedly elevated CA19-9 (4,237 U/mL) was found to have a distal bile duct tumor. Three-dimensional CT angiography demonstrated approximately 60% CAS (type B) and a rare large-caliber posterior right hepatic artery (posterior RHA) arising entirely from the gastroduodenal artery (GDA) and forming a short common channel with the posterior superior pancreaticoduodenal artery. Subtotal stomach-preserving PD was performed via an arterial-first approach. Two-level intraoperative color Doppler ultrasonography during staged GDA clamping established that the posterior RHA was the sole supply to the right posterior lobe and that preserved forward flow was hemodynamically adequate, enabling PD without MAL release. Histopathology confirmed pT1N0M0, Stage I distal bile duct adenocarcinoma (R0). Postoperative liver enzyme elevation resolved by day 7; the patient was discharged on day 28 and remains recurrence-free at three years. By utilizing preoperative vascular mapping and two-level intraoperative Doppler assessment, we successfully performed PD without requiring MAL release. A posterior RHA forming a common channel with the posterior superior pancreaticoduodenal artery simultaneously constitutes a devascularization hazard and a critical collateral conduit; its identification and preservation are essential.

目　的：2010年以降，本邦では梅毒感染の再拡大が続いている．一方，20世紀の多くの期間においては一般大衆雑誌が医学・医療情報の重要な発信媒体であったと考えられる．本研究では，1905年に梅毒トレポネーマが病原体として確認された以降の時代を背景として一般大衆雑誌に掲載された梅毒関連記事を収集・分析し，その内容の変遷と各時代における役割を検討した．

方　法：国立国会図書館サーチ，昭和館デジタルアーカイブ，雑誌オンライン書店（Fujisan.co.jp），図書館所蔵資料および市場で入手可能な一般大衆雑誌を用いて，1910年以降の梅毒関連記事121記事を収集した．記事主題を10項目に分類し，記事への医療従事者の関与の有無を調査した．また，記事内容と梅毒流行状況および社会的背景との関連を検討した．

結　果：梅毒関連記事は1912～1935年に41記事，1947～2001年に63記事，2016～2024年に17記事を認めた．一方，1936～1946年の軍事体制期および感染沈静期後半から感染再拡大初期に相当する2002～2015年に記事は認められなかった．1912～1935年には医学総論，先天梅毒・妊娠梅毒，進行梅毒に関する記事が多く，医学総論記事とともにその多くに医療関係者の監修または協力を伴っていた．1947～2001年には歴史・読み物・文芸記事が最多で，次に感染拡大への警鐘記事が多かった．2016～2024年には感染拡大への警鐘を主題とする記事が大半を占めた．

考　察：一般大衆雑誌は時代ごとに異なる形で梅毒に関する情報を社会へ伝達してきた．戦前から戦後の流行期には医学知識の普及や公衆衛生上の啓発が中心であったのに対し，近年では感染再拡大への警鐘が中心となっていた．インターネットを中心とする情報環境の変化に伴い，一般大衆雑誌における梅毒関連記事の内容と役割は大きく変化してきたことが示唆された．
