    The chromosomal gene for cephalosporinase (CSase) production by Enterobacter cloacae GN7471 was cloned into vector plasmid pACYC 184. From the substrate profile and the response to anti-E. cloacae CSase serum, it was shown that the hybrid plasmid (pMS161) carries the chromosomal CSase gene of E. cloacae GN7471. E. cloacae CSase encoded by the cloned plasmid was produced in an inducible manner in both E. cloacae and E. coli, but its expressed specific activity was much lower than that of the original E. cloacae. From the analysis of several hybrid plasmids derived from pMS161, it was deduced that the site of the structural gene of CSase is located in a 1.15-Md fragment and the gene regulating the inducibility of CSase prodution in a 0.3 Md fragment.

    We treated 14 patients with a variety of cervical spine disorders with immobilization by the halo vest. They consisted of four enlargement laminoplasties for a nallow cervical canal, six anterior decompression and fusions for spinal cord injury and OPLL, one odontoid fracture, one hangman's fracture, one Jefferson's fracture and one atlanto-axial dislocation. The average time of halo immobilization was 57 days.
    Eight complications resulted from the use of the ring, i.e., three involving tightness of the head, two pin slippage, one exudation, one headache, one loosened bolt. Complications from the vest included four cases of scapular pain, two of shoulder pain and limitation of shoulder movement, two of exanthema and one of clavicular pain. All complaints were easily reversible, and their occurrence did not influence the stabilization and ultimate fusion.
    The halo vest offers numerous advantages over the traditional skull tong traction and prolonged bed rest and other orthosis instruments. We would like to emphasize that the halo vest is an ideal instrument for both immediate treatment and long-term stabilization of the traumatized cervical spine.

    Five hundred twenty-six Patients with malignant tumors were treated by radiotherapy in the Department of Radiology of Gunma University Hospital from January 1, 1977, to December 31, 1978.
    The follow-up results were as follows.
    1. The overall five-year relative survival rate was 46%.
    2. In 346 patients treated by radical radiotherapy, the five-year survival rate was 66%.
    3. In 159 patients treated to prolong life (palliative irradiation), the one-year, two-year and five-year survival rates were 37%, 16% and 7%, respectively.
    4. Fifteen out of 21 patients irradiated for relief of symptoms, e.g., bleeding, pain and obstruction, were fully relieved.

    The influence of splenectomy and the effects of postoperative administration of BRM (OK-432) upon the NK activity of peripheral blood mono-nuclear cells were investigated. The results obtained with rats were as follows :
    1. Laparotomy as well as splenectomy reduced the NK activity. Splenectomized rats showed reduced NK activity for as long as four weeks after the operation.
    2. The reduced NK activity of splenectomized rats was restored to the level of the preoperative control value by several subcutaneous administrations of OK-432.
    3. Preparation of rats with 1, 2-dimethylhydrazine hydrochloride in advance, which was used as the tumor-bearing model, impaired the effect of OK-432 to improve the NK activity.
    Taking these data together, it is suggested that caution be exercised in performing splenectomy for the purpose of extended lymph node dissection for stomach cancer. But once the spleen is removed, BRM treatment is recommended to compensate for the reduced NK activity of the host.

    1) Tow cases of osteochondritis dissecans of the medial talar dome were reported.
    2) The diagnosis of osteochondritis dissecans of the talar dome usually can be made easily with plain roentgenograms. But lateral tomograms or arthroscopy are helpful in localizing this lesion, and may be required if the lesion can not be seen clearly on the roentgenograms to ascertain the size of the lesion and the margin of the talus.
    3) Patients who continued to have severe pain and limitation of activities after a long term of conservative treatment usually should undergo surgery consisting of removal of the fragment and curettage of the base of the lesion.
