    Bone marrow suppression is the most serious problem associated with the current chemotherapeutic regimen for testicular cancer. Autologous bone marrow transplantation (ABMT) was performed in combination with the conventional chemotherapeutic regimen in 16 patients. The entire chemotherapeutic regimen was completed in all cases. A comparison of the duration of leukopenia under 2000/μl during each course revealed shortening during the third course when combined with ABMT. None of the short-term side effects of ABMT, including bacterial infection, were beyond acceptable limits, and we confirmed its short-term safety. Fifteen cases were followed after exclusion of one fatality due to postoperative hemorrhage. The cases were followed for a minimum of 38 months and a maximum of 54 months. No recurrence of cancer was noted in any of the cases, and we confirmed its long-term safety.
    ABMT was used in conjunction with high dose chemotherapy in cases failing to respond to the initial conventional regimen. A pronounced therapeutic effect was achieved, and no evidence of disease was obtained when combined with surgery. Future problems include the mode of combining high-dose chemotherapy with ABMT and surgery in such cases of advanced testicular cancer.

    From January, 1975, through December, 1988, 204 patients underwent total gastrectomy for gastric cancer. Assessment of the quality of life in these cases was investigated retrospectively with respect to mode of reconstruction following total gastrectomy. There were 11 patients with Roux-en-Y anastomoses and 30 with jejunal interposition procedure in a short-term follow up group surviving from one to less than five years. And there were 7 patients of the former and 19 of the latter in a long-term follow-up group surviving more than five years. Patients with jejunal interposition in the long-term follow-up group often have a normal hemoglobin and red blood cell count compared with Roux-en-Y anastomosis. Total protein was within a normal range after each of the reconstruction. The patients of the jejunal interposition showed rather good recover in weight to the preoperative level. There was a lower incidence of reflux esophagitis in cases after jejunal interposition. Postoperatively, they also were able to lead their normal lives again. The jejunal interposition proved clinically superior to Roux-en-Y anastomosis.

    Effects of Cisplatin (CDDP), Adriamycin (ADR) and a new derivative of camptothecin (CPT-11) on rats with ENNG-induced gastrointestinal carcinogenesis were studied through the survival rate, incidence and cell kinetic analysis. The 12-month survival rate was better in the groups treated with CDDP, ADR and CPT-11 than in the control. The incidence of carcinogenesis in the CDDP and ADR groups was 38% and 33%, respectively, which were significantly (p<0.05) lower than 88% in the control group. The incidence (86%) of carcinogenesis in the CPT-11 group was not different from that (88%) of the control group. However, the number of tumors per rat and the mean largest diameter and weight of tumors were lower in the CPT-11 group than the other groups, suggesting its higher antitumor effect. In cell kinetic analysis, the rate of S-phase cells of the mucosa without tumor was significantly (p<0.01) lower in the groups treated with CDDP, ADR and CPT-11 than in the control. The anti-tumor drugs showed the suppressive effect of carcinogenesis by acting on the normal epithelium level. Carcinogeneticlesions was also significantly (p<0.01) lower in the groups treated with CDDP, ADR and CPT-11 than in the control. We conclude that CDDP, ADR and CPT-11 have suppressive effects of carcinogenesis and anti-tumor effects on rats with ENNG-induced gastrointestinal carcinogenesis.

    To elucidate the mechanism of anemia in rheumatoid arthritis (RA), iron-related parameters were examined in 26 RA patients, and ferrokinetic and erythrokinetic studies were performed on 9 and 8 of the 26 cases, respectively. As a whole, mild or moderate hypochromic normocytic anemia was observed. Serum iron levels were markedly decreased and the percent transferrin iron saturation was decreased or normal, while the serum concentration of ferritin fell within the normal range in all but one of the cases examined. Ferrokinetic studies revealed a decreased red cell iron turnover rate in 4 out of 9 cases and reduced red cell iron utilization in 2 out of 9 cases. Erythrokinetic studies disclosed that red cell life span was shortened in 5 cases and that total red cell volume was decreased in all 8 cases examined. Hemoglobin production per day was reduced in all 8 cases. In addition, the activity of δ-aminolevulinic acid synthetase was decreased in 2 cases examined. These findings suggest that abnormal iron metabolism and heme synthesis may be involved as causes of the anemia complicating RA.

    The microwave coagulator is an effective tool in endoscopic surgery. Its powerful hemostatic effect is useful in endoscopic polypectomy, small tumor resection and stenotic segmentotomy. Seven patients were successfully treated by endoscopic surgery using a microwave coagulator. Two patients had a gastric or esophageal tumor and anothem five had postanastomotic narrowing or post-radiation stemotic constriction of the gastrointestinal tract. In this report, the practical procedures of endoscopic surgery using a microwave coagulator were demonstrated in three patients. Excellent results were obtained in all of them. Microwave coagulation in endoscopic surgery is useful and effective, and will assume an important place in surgical therapy.

    A 36-year-old female was admitted to our hospital because of cough, fever and pulmonary infiltration. Although the initial symptoms improved following antibiotic treatment, bilateral peripheral facial palsy, weakness of extremities, sensory disturbance of glove and stocking type and dysuria appeared on the 7th hospital day. The cerebrospinal fluid contained 20 cells per 1μl and the protein content of the fluid was 320mg/dl. Because the serum antimycoplasma complement fixation titer was elevated to 1 : 512 and the titer was positive at 1 : 8 for cerebrospinal fluid, the patient was diagnosed as having acute mycoplasma pneumoniae infection complicated with acute demyelinating polyradiculoneuropathy.
    The serum antimyelin antibodies of IgM and IgA classes were positive at the initial phase of the neurological disturbance of this patient, while IgG antibody was demonstrated later.

    Serial examination of the electrocardiograms of a 69-year-old man with autopsy proven dilated cardiomyopathy revealed gradual prolongation of the PR interval and development of Q waves in leads II III and aVF over the post last 3 years. On the vectorcardiogram, the QRS loop was gradually distorted and displaced posteriorly in the horizontal plane during the follow up period. Thallium-201 myocardial scintigram revealed perfusion defect of the entire left ventricle, except the lateral wall, consistent with diffuse myocardial fibrosis at autopsy. We thought that gradual loss of electromotive force of the left ventricle resulting from extensive myocardial fibrosis and progression of the intraventricular conduction disturbance might be responsible for the serial electrocardiographic and vectorcardiographic changes in this case.

    A 24-year-old man was admitted to Gunma University Hospital because of paroxysmal hypertension, tachycardia and struma. Computed tomography revealed a right adrenal tumor and thyroid tumors. Plasma catecholamines and calcitonin were markedly increased. During hypertensive episodes, the ECG exhibited various changes such as ST elevation, peaked or inverted T waves, and sinus tachycardia. After removal of the adrenal tumor, the electrocardiographic abnormalities improved.
