    An immunological method for human hemoglobin assay was established, using which contents of fetal hemoglobin in the blood of hepatocellular carcinoma patients as well as in the extract of hehatocellular carcinoma tissue were measured. With the hemolysate, 4 out of 7 hepatocellular carcinoma cases showed slight but definite increase, 2 out of 10 in the serum and 6 out of 8 in the tissue extract. No such excess of fetal hemoglobin was found in cirrhosis or metastatic hepatic cancer cases as far as examined. Immunofluorescent study revealed that the mode of fetal hemoglobin distribution in the red blood cell in cases of hepatocellular carcinoma was quite similar to that in other instances of fetal hemoglobin increase, such as leukemia, aplastic anemia, etc. Immunochemical analysis of fetal hemoglobin appearing in cases of hepatocellular carcinoma again revealed quite the same character as the one existing in the normal cord blood. Discussions are made on the rationality in considering fetal hemoglobin as one of carcinoembryonic proteins on the one hand, on the similarity and difference between fetal hemoglobin and alpha-fetoprotein on the other.

    The results obtained by injection studies on normal vascular patterns of the liver and their architectural changes in various hepatic malignancies were described. Multiferous histopathological and microangiographical characteristics of primary focus of hepatocellular carcinoma (HCC) were pointed out. For the accurate determination of grade of malignancy of HCC should be based on the predominant type of differentiation. The reason of a low incidence of distant metastasis of HCC was discussed.

    Patients with hepatocellular carcinoma developing remote extrahepatic metastases and portal hypertension seem to be encountered clinically more often in recent years, presumably due to prolonged survival attributable to early diagnosis and chemotherapy. The cause of portal hypertension was investigated from the pathological point of view in 225 autopsy livers bearing hepatocellular carcinoma. It was found that the contributing factors are : 1) compression of small hepatic vein branches by regenerative nodulues of the accompanying cirrhosis, 2) increased arteries supplying the tumor and marked increase of arterial blood flowing into the liver, 3) abnormal communications between the hepatic artery and portal vein through tumor thrombi, and 4) marked disturbances in venous blood flow in the liver due to tumor thrombi and dilated arterio-portal anastomoses through plexuses of capillaries around the bile ducts.

    A classification of the patients with hepatocellular carcinoma based on clinical manifestations and disease course is proposed. It is primarily based on the Berman's classification (1951) with additional three more types, namely, “frank”, “febrile”, “acute abdominal”, “metastatic”, “occult”, “cirrhotic”, “cholestatic”, and “hepatitic” types. Certain clinical characteristics are shown to be related to liver pathology. Such classification is of practical value in assessing prognosis and determining the therapeutic measure. Beside these clinical types, certain features of tumor pathology are shown to be closely related to clinical manifestations, such as encapsulated carcinoma, clear cell carcinoma, sclerosing carcinoma and intravascular tumor growth.

  • ALTERATION OF PARKINSONIAN TREMOR BY L-DOPA AND SURGICAL TREATMENT

    Parkinsonian tremor was analysed by accelerometers and electromyogram. Measurement was made during rest, posturing, finger-finger test and finger-nose test of the upper limbs and parkinsonian tremor in different status was characterized by pattern of tremor provocation by above procedures. L-Dopa treatment did not alter parkinsonian tremor markedly, whereas thalamotomy markedly reduced the tremor, especially its resting component. Postoperative residual tremulous movement was dominated by postural and dysmetric components. This residual tremulous movement was correlated to the intrathalamic therapeutic lesions and discussion was made as to the mechanism of tremogenesis.

    The 4-pentenoic acid, one of short-chain fatty acids, produced fatty changes in rat liver. Abnormal mitochondrial changes had been seen prior to the appearance of fatty droplets by electronmicroscope. Mitochondria were swollen with decreased density of matrix. Dense-bodies were disappeared. Although there had been prominent mitochondrial changes, hepatic ATP concentrations in rats were not decreased. These results suggest that antecedent mitochondrial changes may play a primary role in the pathogenesis of fatty liver induced by 4-pentenoic acid, although hepatic ATP changes in concentration did not accompany the mitochondrial damage.

    The adrenal medulla of the adult cats fixed with various fixative methods were examined by electron microscopy. In addition to the two kinds of well-known typical secretory granules, there existed colloidal bodies usually several times as large as the typical secretory granules in diameter both in the noradrenaline-storing cell and adrenaline-storing cell. They were round or elliptic, sometimes irregular in shape, containing the fine, homogeneous f locculent material with a lower density than that of the typical secretory granules. The mechanism of releasing their contents into the intercellular space was taken into consideration. Moreover, the ability of cell division was also confirmed in the adrenal medullary cells of the adult cat.

    From January 1971 to December 1977 left transdiaphragmatic approach and cervical esophagogastrostomy with gastric tube in one stage without surgical positional change was performed on 53 patients with esophageal cancer of the middle or lower thoracic esophagus. The esophagogastrostomy with end to side anastomosis was constructed in the left neck in 50 patients. There were no instance of postoperative anastomotic leakage or stenosis. Four of these patients died of pulmonary complications or cardiac insufficiency within one month of operation. The operation with this approach has several advantages as follows ; The change of surgical posture was not necessary during the operation. The second merit is a decrease on the incidence of postoperative pulmonary complications depending upon this technic without upper median abdominal incision. Moreover, this approach has another advantage on the dissection of upper mediastinal metastatic lymph nodes such as Botallo's node. 1) 53 cases of the resectable esophageal cancer from 1971 through 1977 in our department, were treated with good prognosis. 2) Mostly, we interposed the plastic gastric tube along the retromediastinal route with few complications. 3) The esophagogastrostomy with end to end or end to side anastomosis individually, according to the general conditions of patients. 4) Early tube feeding by means of a plastic gastric tube showed the ideal physiological quality and provided well improved nutrition. 5) The good adequate blood supply of the plastic gastric tube was found not to cause the ischemic change and the necrosis of its stump. 6) The left transthoracic transdiaphragmatic operative approach diminished the postoperative stress and pulmonary complications. 7) This operation satisfied the radical dissection of lymph nodes. 8) The second operation for closure of fistula is harmless and simple.

    Vibration disease results from the usage of vibrating tools such as chain saws or pneumatic hammers for a long period. One of the principal manifestations of this disease is Raynaud's phenomenon in the fingers. In order to examine peripheral circulatory functions, a thermography was applied to the patients with vibration disease. The distribution of skin temperature in digits was clearly demonstrated by a thermography. By combination with auditory stimuli, a thermography showed dynamic changes of skin temperature in time. The recovery time course of skin temperature once reduced by auditory stimuli was related to the activity level of the autonomic nerve. However, this relation was not observed in cold water immersion test which was used as one of the peripheral circulatory function tests. These results suggest that a thermography combined with auditory stimuli is useful to detect not only the distribution of skin temperature but also the activity level of the autonomic nerve.

    The stromal surface of the mammary gland myoepithelial cells of rats under normal and experimental conditions, which were treated with HCl-collagenase method introduced by Evan et al. (1976), have been studied by scanning electron microscopy. Treatment of the mammary gland with HCl and collagenase removed almost completely the extracellular materials such as collagens and the basal lamina and revealed the myoepithelial cells enclosing the glandular alveolus. Myoepithelial cells consisting of an elongated cell body and long, bifurcating processes grasped the glandular alveolus in the form of a delicate network. With the application of oxytocin, the myoepithelial cells were involved in the morphological changes suggestive of contraction. They had a row of prominent folds arranged perpendicularly to the long axis of the cell body and its processes.
