    We report pathological features and histometrical analyses of the portal tracts in the liver with extrahepatic portal obstruction in adult. Cavernous transformation was frequently observed in medium-size portal tracts, usually associated with narrowing of the portal vein. The histometrical study showed dilatation of the hepatic artery in extrahepatic portal obstruction, compared with idiopathic portal hypertension and the normal control. It is suggested that the dilatation of the hepatic artery in the medium-size portal tracts may reflect an increased arterial blood flow which serves to compensate for the insufficient portal circulation due to the sclerosed and narrowed portal vein.

    Changes of the plasma membrane during exposure to ultraviolet light were studied in cultured B-16 melanoma cells by the method of electron spin resonance with the spin labelling technique. The lipid peroxide content increased immediately after exposure to UV light and returned to the normal level, 3 hrs post-exposure. Plasma membrane fluidity increased significantly, 3-6 hrs after exposure, and remained at the increased level in the hydrophobic region of the membrane until 18 hrs. There were no significant differences in phospholipid content before and after exposure, except for cardiolipin.

    Subependymal intraventricular hemorrhage (SEH/IVH) has become of a major and important clinical entity in neonates, especially in preterm infants who are born less than 1500 gram of body weight or less than 35 weeks of gestational age. We have developed a miniaturized paper strain gauge transducer, which atraumatically and continuously monitors the intracranial pressure (ICP) via anterior fontanelle. Using this sensor, 22 infants with SEH/IVH and/or ventricular dilatation after IVH were observed for monitoring ICP. ICP measurement in these infants offered useful information to assess the pathophysiological state of the intracranical space with developmental parenchyma.

    Kleeblattschädel syndrome is a rare disease and one type of craniofacial dysostosis. Because of this rarity, only a few reported cases of surgical treatment for the anomaly are available. A case is described involving a 2 month-old girl with the Kleeblattschädel syndrome who was successfully operated on.

    Although abdominal complications caused by ventriculoperitoneal shunts have been reported, it was rare to observe an abdominal shunt tube penetrating into the intestinal tract. Such a case is presented with the literature review.

    A retrospective review of 30 patients with acute cerebral embolism of nonseptic cardiac origin was conducted. Two groups of nine patients received either anticoagulant or thrombolytic therapy, such as urokinase. 12 patients received no drug therapy. Early recurrence of cerebral embolism occurred in one of the 12 patients without drug therapy. A hemorrhagic infarction diagnosed by a CT scan was observed from one day to day 32 in four of nine patients on anticoagulant therapy (44.4%), five patients receiving urokinase (55.6%) and five patients without therapy (41.7%). At the time of identification of the hemorrhage, four patients were receiving either anticoagulant or thrombolytic therapy. The hemorrhage occurred in six of nine patients (66.7%) with rheumatic valve disease. It should be noted that all four patients with rheumatic valve disease who received anticoagulant therapy had hemorrhagic infarctions. Two of the four patients displayed catastrophic outcomes. In contrast, hemorrhagic infarctions occurred in only six of 19 patients (31.6%) with non-valvular atrial fibrillation. It is concluded from the above findings that anticoagulant therapy should be avoided during the first month after a cerebral embolism caused by rheumatic valve disease.

    DMBA-induced adenocarcinomas were transplanted into the neonatal or young rats of the same strain (Wistar). Following subcutaneous injection, tumors exhibited two phases of growth: an inititial avascular phase characterized by slow growth, followed by a later phase of rapid growth in which the tumor became vascularized. The transplanted tumor was more hypervascular than the primary tumor. Histologically, the primary autochthonous tumors and the transplanted counterparts were similar. The most striking result of transplantation was a much more homogeneous solid pattern than occurred in the primary tumor. The second histologic finding was a reduced stromal reaction. Moreover, in the central area of the tumor, the relationship between the vasculature and cell proliferation was observed. The transplanted tumors may provide a useful experimental model for laboratory studies on the chemotherapy of ovarian cancer.

    Brainstem auditory evoked potentials (BAEPs) were recorded in 10 patients who had basilar artery occlusions. Five patients had abnormal BAEPs during the initial recording. Among the remaining patients, three who underwent follow-up study revealed an abnormality within eight days after the onset. An abnormal CT scan preceded the abnormal BAEPs in seven patients. There were five deaths within seven days after onset; three of these patients displayed an abnormal initial BAEP and two patients had a normal initial BAEP. All five patients with abnormal initial BAEPs had severe disorders of consciousness. Abnormal BAEPs during repetitive measurements indicated the severity of the brainstem damage. Therefore, it was concluded that BAEPs are useful both for making an early diagnosis and for determining the prognosis of the basilar artery occlusion, especially when combined with a CT scan.

    A 73-year-old man with a history of ischemic heart disease was diagnosed with an extensive, acute anterior myocardial infarction. Coronary arteriography demonstrated total occlusion of the left main coronary artery, and the right coronary artery was predominant. Selective intracoronary thrombolysis was initiated immediately with a total dose of 480, 000 IU of urokinase and resulted in recanalization to a 99% diameter stenosis of the LMCA. Emergency coronary artery bypass grafting was performed 7 hours after recanalization. An intra-aortic balloon pump, inserted before the operation, was removed on the 8th day after the operation. The post operative course seemed to be good; but the patient expired from acute renal failure and sepsis, 45 days after the operation.
