Axonal branches of single vestibular neurons activated by stimulation of the ampullary nerve of the posterior semicircular canal in the cat were studied by mean of local antidromic stimulation in the trochlear and the oculomotor nucleus. These vestibuloocular neurons were located in the rostral half of the descending vestibular nucleus and the lateral part of the medial vestibular nucleus. The majority of vestibuloocular neurons projecting to the inferior rectus motoneuron pool in the contralateral oculomotor nucleus was activated antidromically from the contralateral trochlear nucleus as well. This suggests that axonal branches of a single vestibular neuron project to both nuclei.

Polygraphic recordings of diurnal sleep and night sleep were obtained after night duty and day duty or off-day, respectively, studies were performed on from 5 hospital nurses working by on three-shift-system and on male 5 guards on alternate-day shift system. Sleep patterns in the above two groups were compared and following results were obtained. 1) In diurnal sleep after night duty, time in bed and total time of sleep were shortened, number of wakenings increased, REM sleep latency shortened, the first REM duration prolonged, and S3 and S4 sleep latency significantly prolonged as compared with night sleep. 2) In diurnal sleep, the appearance of REM sleep in sleep onset ie., the SOREMP (sleep onset REM period) was observed in 3 of 10 subjects. 3) In diurnal sleep, the distribution of sleep stages was clearly different from that in night sleep ; thus in the former, REM sleep amount during the first 120 to 180 mins. after sleep onset was increased, the amount of S4 sleep significantly decreased, and the amount of REM sleep was greater in the early period than in middle period. It can be conjectured from above results that REM sleep may be dependent on the circadian rhythm, and S4 sleep on the length of prior wakefulness. On the other hand, pulse rate showed a higher level in diurnal sleep than in night sleep, and internal desynchronization of sleep stage was visible between the two.

Adrenocortical function in adrenal demedullated (ADMx) and peripherally chemical-sympathectomized ADMx rats was studied by determining adrenal and serum corticosterone (B) levels. Resting levels of B were not changed by adreno-demedullation and peripheral chemical-sympathectomy with 6-hydro-xydopamine. Diurnal variation of B levels was also not changed, suggesting that peripheral adrenergic mechanism would not give any influences on the basal function of hypothalamo-pituitary-adrenocortical axis. Exposure of ADMx rats to stress stimuli, however, resulted in lowered adrenocortical response with less increase in B levels, while peripheral sympathectomy of ADMx rats with 6-hydroxydopamine or guanethidine caused a significant enhancement of adrenocortical response to stress with higher B levels. Adreno-demedullation of rats tended to increase spleen weight, while peripheral sympathectomy with 6-hydroxydopamine of ADMx rats caused a significant increase in spleen weight. Histological changes in the spleen were also described.

The purpose of this paper is to demonstrate the immunological measurement of properdin and its clinical significance. Properdin was measured by single radial immunodiffusion method and relationship between properdin level and CH50, β1A, β1E, β1F and C3A was investigated in 39 samples from the children with acute glomerulonephritis, 32 from vascular purpura and 30 from healthy adults. Reactivity of purified properdin was assayed with immunoelectrophoresis. The range of properdin level in AGN is 0-73.5% of normal human serum and mean value is 43.5±24.3% in acute phase, and 81.56±26.50% in remission respectively. In VP, the range is 28-117% and mean value is 67.08±27.29% in acute phase, 88.40±13.13% in remission. Correlation berween properdin level and each component of complement was studied. High correlation (γ=0.626) was revealed between properdin and β1A in AGN, and no correlation was found in the serum of VP. In 3 of 10 cases of VP with low titer of properdin, decreasing of properdin level was not accompanied with low CH50 but properdin decreased before renal involvement occurred.

Conventional M-mode echocardiography has become an established diagnostic method for noninvasive evaluation of cardiovascular diseases. Now, the advent of two-dimensional real-time echocardiography has opened up a promising way to more extensive investigation in clinical cardiology. In the past few years, there has been a remarkable progress in the clinical application of two-dimensional echocardiography. This technique of echocardiography was performed in about 400 patients at our hospital during the past one year. In this paper, we introduced the general idea of two-dimensional real-time echocardiography and showed a variety of cases.

Convenient micro-turbidimetric methods for urinary protein determination by the use of quaternary ammonium salts-benzethonium chloride and benzalkonium chloride-in alkaline solutions were studied and compared with biuret method, Kingsbury-Clark' method and others. The turbidities produced in these quaternary ammonium salt methods are substantially same for albumin and γ-globulin, and more stable thanthose in sulfosalicylic acid method and trichloroacetic acid method. These methods showed satisfactory sensitivity, reproducibility and recovery. The correlation coefficient between biuret method and these methods were above 0.99, and the results were in substantial agreement with each other. There were however some inconsistent cases. In icteric urines and some of yellow-colored urines, probably attributable to medications, false positive results were observed. In spite of these disadvantages, present methods seemed to be useful as manual methods in routine work, for the simplicity of their procedures, the high and practically same sensitivities for albumin and γ-globulin, and other advantages.

Modified TCA-Ponceau S method for urinary protein determination by addition of surface active agent (Triton X-100) was studied and compared with Pesce-Strande method, the modified method by Sasaki et al. (1979), biuret method and others. This method was independent of temperature and was not appreciably affected by the albumin/γ-globulin ratio. The correlation coefficient between biuret method and this method was 0.998, and showed high sensitivity and satisfactory reproducibility. The procedure is not one-step and not as simple as Kingsbury-Clark method, but the main advantages of this method are its high and approximately same sensitivity for albumin and γ-globulin, and probably its specificity.

It is well known that longitudinal overgrowth occurs after fracture in children. The factors which are considered to affect this phenomenon include age, and site and type of fracture. However, interrelation and difference of clinical significance of these factcrs have not been yet elucidated, In this study, we showed that a statistically significant difference occurs according to cerebral control : namely whether the fracture is on the same side with or opposite site of the dominant hand. A total of 107 cases (consisting of 46 cases of femoral fracture and 61 cases of tibial fracture) were included as the study subjects. Among the cases of femoral fracture, there were 20 cases of dominant side fracture with a mean overgrowth of 7.8mm and 26 cases of non-dominant side fracture with a mean of 12.1mm (p<0.05). For tibial fracture, there were 38 cases of dominant with a mean of 5.7mm overgrowth and 23 cases of non-dominant with a mean of 6.3mm overgrowth (p<0.05). Only in cases of femoral fracture, significant difference at p<0.05 was seen. Similar findings were reported by Meals in 1979. It may be postulated that the leg on the non-dominant side controls the balance at the time of standing up and walking, and this burden will increase the blood flow which may produce overgrowth.

The effect of various displacement, especially angle deformity, persisting after lower leg fracture at the growth plates was studied. Of total cases of lower leg long bone fracture under the age of 15 years significant displacement was seen in 85 cases at the time of follow up. Changes in the amount of epiphyseal growth plates of hip, knee and foot area as a response to these unphysiological change were investigated by means of anterior roentgenogram. Measurements were made with reference to the method described by Shiraishi and wada et al : namely, angles of H, F, T and A were determined, and compared with the chadges occurring in the healthy side. With respect to H. F and T angles, an increasing tendency was noted in proportion to the degree of angle deformity. No change was seen in A angle. Although theoretically the persistence of angle deformity must increase the displacement from knee center on the Mickliz line, we could not see apronounced displacement. Stress of growth plates in great since affected children tend to walk normally in the early phase of recovery. This reaction appears more evident for the latter, with a tendenc of adaptation of Mickliz line. Moreover, it is assumed that normal walking in the extremely early phase will cause a delay in the correction of local angle deformity.

It is generally considered that infection is a fatal complication and recently has increased in acute leukemia. In this paper, we studied relationships between peripheral neutrophil counts (N), antibiotics and infections. According to decreasing of N, incidence of febrile episodes increased overtly. In 78% of febrile episodes, N revealed less than 500/cmm. Forty five of 73 etiologic agents (61.6%) and 6 of 7 bacteremia (85.7%) were gramnegative rods. Infections were mostly detected in the upper-respiratory tract and were observed more frequently in remission induction period than in terminal period. The most effective method was the combination therapy of cephalospolin and aminoglucoside. On the basis of fluctuations of N during treatment of antibiotics, it seemed an increment of N played the leading part in infection.

It is already well established that the joint disease in patients with rheumatoid arthritis (RA) is developed by immune mechanisms. However, there are few reports concerning serum immune complex (IC) levels in patients with RA. Therefore, we tried to examine serum IC levels in 47 patients with RA of various stages in order to clarify the significance of measuring serum IC levels in patients with RA. Serum IC levels were determined by Raji cell immunofluorescence assay which we had developed recently. In some patients, IC levels in synovial fluids were also measured. In 55% in patients with RA serum IC levels were positive and their levels were strongly correlated with the existence of extraarticular symptoms, especially with systemic angitis. On the other hand, the activity of joint disease had more close correlation with the IC levels in synovial fluids than those in serum. In conclusion, serial determination of serum IC levels can contribute to determine the indication of steroid treatment in patients with RA and to control the dosis and duration of steroid treatment of these patients.

Using Littleford and Spector's introducer set, permanent endocardial electrodes were implanted in 11 patients through the subclavian vein puncture. In one patient, J-guide wire curled up to previously inserted catheter electrode for external pacemaking. Then, the guide was difficult to remove from the subclavian vein. First of all, catheter electrode was removed from left brachial vein. J-guide wire was then able to be drawn from the vein. In our knowlege, this is the first reported case of J-guide wire complication upon the subclavian vein puncture and electrode insertion. Its prevention and management of the complication are discussed in detail.

A 32-year-old man developed, during upper respiratory infection, the right otalgia, vertigo and vomitting followed by gait disturbance due to ataxia and hearing loss which reached almost to deafness at one time. Trigeminal neuralgia on the right side appeared, but facial paresis was not noticed at all. The patient also complained of numbness (dysesthesia and paresthesia) in the right upper extremity and posterior neck. The diagnosis was confirmed by herpetic eruptions on the right auricle and posterior neck as well as a significant elevation in serum of CF antibody titer to varicella-zoster virus (250×). Laboratory examinations were performed about one month after the onset when the patient showed mild ataxia and horizontal nystagmus toward the left. CSF revealed slight pleocytosis (35/3). An audiometry disclosed a reduction of hearing of the right ear in the range of high frequency with marked recruitment phenomenon. A caloric test showed no responce in the right ear, while the eye-tracking test and optokinetic pattern were normal. EEG, CT and peripheral nerve conduction velocities revealed no abnormalities. In cephalic herpes zoster, the facial nerve has been considered to be most vulnerable among the cranial nerves, as is shown in Ramsay Hunt syndrome. Cases of herpes zoster infection with auditory and vestibular complications without facial paresis such as this case are rare in the literature.

This 24-years-old patient was delivery at 36 weeks of gestation by cesarian section (normal male infant). Two days after delivery, she was transferred to our hospital complaining of continuous nasal bleeding and fever, (both symptoms being seen for the past 7 days before delivery). Hematologic investigation on admission showed : Hb 8.7g/dl, WBC 16400/cmm (blast cells : 75%, seg : 1%, mono : 4%, lympho : 20%) and Platelets 21000/cmm. The bone marrow was replaced by primitive myeloid cells, the dominant cell type being monocyte. Uric muramidase was remarkably increased. A diagnosis of acute monocytic leukemia was made as result of a bone marrow examination and uric muramidase. Remission induction therapy (DNR, Ara-C. 6-MP, Pred.) and antibiotics therapy (SBPC, GM) were started from the third hospital day. On 7th day of the therapy, operative wound of cesarian section was opened. She developed severe pyrexia associated with infection of cesarian section wound, which eventually broke down. Bacterological examination of cesarian section's pus and blood culture indicated E. coli and Pseudomonus aeruginosa from which septicemia was diagnosed. In spite of intensive supportive therapy, she died 3 weeks later from remission induction therapy. Autopsy findings were follows : 1) Acute monocytic leukemia. 2) Hepatomegaly ; 2480g. 3) Splenomegaly ; 240g. 4) Aspergilosis of the lung, esophagus, stomach and renal pelvis. 5) Rupture of the uterus (at the suture site due to by endometriosis).

The patient, reported here, is a male baby born from the mother diagnosed idiopathic thrombocytopenic purpura (ITP). The baby had petechiae on the face and body, and diagnosed "immune thrombocytopenia" from clinical symptomes and positive anti-platelet antibody. But later we suspected Wiskott-Aldrich syndrome (WAS) and made certain diagnosis by special laboratory examinations, the measurement of antibody titer to polymerized flagellin from Salmonella adelaide, isohemagglutinin, and SK-SD. DNCB and candida skin test. Transfer factor was used for the treatment of cellular immunodeficiency. We reported the case here because WAS is a rate disease, the patient was born from the mother with ITP, and diagnosed by special laboratory examinations.

Recently in the United States there has been a tendency to expand the scope of professional nursing practice. This expansion of nursing practice is carried out by means of the amendment of state nurse practice acts or other measures in order to compensate for the shortage of physicians in some particular areas. In this case, the expansion of practice by professional nurses centers around diagnosis. However, this tendency of expansion has caused some legal problems, e. g., (1) what the relation between the diagnosis by the nurse and that by the physician is, (2) what the content of diagnosis by the nurse should be and (3) how the nurse's negligence is to be judged in a malpractice case occurring in connection with the practice as provided by the new legislations. It is said that social situations in relation to the nursing practice in the United States are by no means favorable to professinal nurses. This thesis surveys the present development of American legal doctrines concerning the above-mentioned three issues whose solutions are now strongly demanded. This will furnish us with helpful information in investigating what the future nursing practice of our country should be.
