    The investigation of the subpopulation of peripheral blood lymphocytes is important to research the immunological pathogenesis and mechanism of progression in patients with various glomerulonephropathies. We isolated lymphocytes by Metrizoate-Ficoll centrifugation, used neuraminidase treated sheep erythocytes as T indicator cell, and utilized sheep erythrocytes coated IgM antibodies and activated C_3 complements as B indicator cell. With these materials studied 13 normal subjects, clinically stable 17 chronic renal failure patients treated by long term intermittent hemodialysis, 11 chronic glomerulonephritis, 5 SLE nephropathies under steroid treatment, and 4 lipoid nephrosis proven by renal biopsies and prior to steroid therapy. We obtained 45% lymphocytes out of whole peripheral blood lymphocytes and found that isolated cells had 84% lymphocytic purities. Mean percentage values of normal subjects for T and B lymphocytes were 61%・25% respectively. Hemodialysis patients disclosed similar values with 68%・25%. However, real T. B lymphocytes numbers in peripheral blood of hemodialysis patients were a little less than that of normal subjects. T cell percentage decreased and B cell percentage increased in lipoid nephrosis with the increasement of T. B cell numbers in peripheral blood. No conclusive details were obtained from chronic glomerulonephritis and SLE nephropathy by this investigation.

    Ipratropium Bromide, an atropine like anticholinergic agent, was administered to 10 healthy Japanese volunteers by inhalation. The carrier freon gas without the active ingredient was also administered to two volunteers, as the control experiment and their effects were investigated and compared. Dosages smaller than 320 μg indicated no clear pharmacological effects other than the evident bronchodilating activity. The bronchodilator effect was most evident at the dosage of 4 puffs×2 (160 μg) and appeared to be suppressed at the higher dosage. Inhalation of the freon gas without the active ingredient induced dose dependent bronchoconstriction. Since the bronchodilator effect of ipratropium bromide is influenced by the freon gas, a dosage less than 4 puffs×2 is advisable.

    Effects of various intensity of skeletal muscle exercise on venous outflow and plasma osmolality (crystalloid) through muscle were investigated in the 'in situ' hindlimb preparation of the anesthetized rabbits. Input pressure in femoral artery was held constant during a series of experiment. Maximal increase in osmolality during 60 sec exercise by sciatic nerve stimulation was 100.9±0.3% (n=6) at 1 Hz, 109.2±1.5% (n=8) (P<0.01) at 5 Hz and 11.3±2.5 (n=7.9) (P<0.01) at 50 Hz, respectively. At 60 sec muscle exercise, blood flow was found proportional to the increase of osmolality, hematocrit (Hct) and plasma total protein (TP). At 10 min sinusoidal exercise (peak/min), blood flow and Hct increased up to the level of 200% and 110% above control, respectively, but osmolality and TP decreased gradually from the maximal increase level (111% and 118%) at 2-3 min after the beginning of muscle exercise. It appears that vasodilation during a short term muscle exercise is first associated with the change in tissue osmolality, and other metabolic vasodilator substances (unknown) have some hyperemic effect during prolonged skeletal muscle exercise.

    In response to the demand for rapid determination of bacterial antibiotic susceptibility, various attempts have been made. Among them, the hemoglobin reduction method (HR method) by the index of respiratory metabolism of organisms as has been considered of high practical value as criterion for antibiotic susceptibity, speed is its major advantage. By this method, the susceptibility of bacteria can be determined in from two to four hours. It also requires no need for preparing sophisticated instrumentation and technique. Using as specimens 58 strains of organisms isolated from patients in Kyorin University . hospital, the HR method was comparatively investigated against the three: disc concentration method to examine whether it can be put to practical use. Consequently, the studies made with 15 kinds of antibiotics showed considerably high coincidence of 83.6% (against gram-negative rods 79.5%, gram-positive cocci 91.3%) between the HR method and the three disc concentration method. Furthermore, direct determination of antibiotic susceptibility by the HR method were performed, using urine specimens from patients with urinary tract infection. Results by this technique well correlated with those obtained by the routine disc method. In conclusion, the HR method has the advantage in that data can be obtained in such a short time as 2〜4 hours, and it has considerably high correlation with the conventional disc method. If a consideration is given to the relation between the kinds of antibiotic and. the pathogenic organisms, the HR method seems to be useful as a screening procedure which can make the microbial susceptibility test in the clinical laboratory much quicker.

    A 59-year-old woman with long-standing hypertension and rheumatism suddenly suffered the left facial and abducens palsy as well as weakness and numbness of the right limbs. with ipsilateral exaggerated deep tendon reflexes and a feeble extensor plantar response. The left facial paresis disappeared in several days. The other symptoms also have receded gradually by now 8 months after the onset only remain a slight degree of diplopia on the left lateral gaze and of dysesthesia in the right arm and leg. Although no abnormal findings were noted in skull x-ray films, CSF, radionuclide brain scan or CT scan except for EEG which showed sporadic spikes in the centro-parietal region, the clinical course of this patient strongly suggests the pontine lesion to be of vascular, origin. Infarction with concomitant, edema caused by thrombosis of the paramedian artery was suspected from the unique combination of the symptoms and the mode of their disappearance. The classical eponymic syndrome of Millard-Gubler seldom occurs. Actually there have been only 10 case reports in Japan including the present case, in some of which the syndrome was mixed with other various symptoms or observed only transitionally on final massive lesions. Among these 10 cases, the cause of 3 was hemorrhage, 3 infarction, 2 trauma, 1 encephalitis and only 1 tumor, though it has been said that the syndrome results mostly from pontine tumor.

    The way to proper selections of a pulse generator and a lead for an individual case was described. Also methods of implantation of both endocardial and myocardial pacemakers were mentioned. Finally, complications possibly encountered during and after pacemaker implantation and the proper, cares for them were described. In order to obtain maximum therapeutic effect using artificial .pacemakers, very close cares for patients and pacemakers are mandatory after implantation.
