    In recent years bronchoscopy for children has been accomplished more frequently. For bronchoscopy in children and infants the rigid bronchoscope is still the instrument of choice. The airway resistance is the important factor in causing unphysiologic conditions during the procedures. The present study consists of an experimental evaluation of air-way resistance of new slit-hole type bronchoscope. To cease the occulusion of the side hole (vent) of the standard bronchoscope by mucus, sputum or by the position of the bronchoscope, we made slits between each side hole. In this reports, a newly designed bronchoscope with slit & hole orifice, is analyzed as to the effect of the slit on the air-way resistance. The air-way resistance of the side hole of the standard bronchoscope is higher than that of the slit-hole orifice of the modified bronchoscope. The slit-orifice decreased the air-way resistance, especially in high flow rate (32 L/min -Fig 2) into the bronchoscope. This reports describes the equipments used and the advantages obtained.

    Effects of norepinephrine (NE) and epinephrine (EP) on venous outflow (flow), Hematocrit (Hct) and plasma total protein (TP) were investigated in the "in situ" hindlimb muscle preparation of the anesthetized rabbits. A slight increase in flow and Hct from the lower limb was initially observed immediately after intravenous injection (mean 9 μg/kg) of NE, then it later decreased gradually below the control value. The maximal decrease of Hct was -2.5±0.2% (n=9, p<0.01) in 2 min after the NE injection. Hct returned to the control a few minutes later. We could not obtain a marked decrease of TP in the same set of samples. EP (mean 15.1 μg/kg) also showed some decrease of Hct (-2.1±0.3%, n=8, p<0.01), but no decrease of protein in venous outflow. These data would be explained mainly by such a rheological mechanism as the decrease of microvascular radii caused by vasoconstrictor drugs.

    Trimetaphan was used to control the blood pressure in 38 patients varying in age from 5-81 years under ketamine, pentazocine & diazepam micro drip infusion or NLA. Blood pressure were obtained with the usual Riva-Rocci sphygmomanometer and recorded every five minute during surgery. Systolic blood pressure was reduced with a rate of less than -Δ3mmHg/min by infusing 0.01-0.025% solution of trimetaphan in vactated Ringer's solution. Blood pressure was maintained to a resting level during anesthesia. An inverse relationship was found between patient age and minute dosage of trimetaphan-induction dosage, and also the minute dosage of maintenance. Previously trimetaphan dosage and concentration was adjusted according to recommended dosage raning from 500-1000 μg/ml. But to eliminate excessive hypotension and other dangerous complication, and to attain consistently safe levels of blood pressure in aged patients, infusion of diluted trimetaphan solution (100-200 μg/ml) has been proved to be very useful.

    1. Neurons in the cat fastigial nucleus that project to the upper cervical spinal segments (fastigiospinal neurons) were fired by antidromic stimulation of the contralateral spinal cord. Dye ejection from the recording electrode was used to show that most neurons were in the rostral half of the fastigial nucleus. 2. Fastigiospinal neurons can be excited and/or inhibited by stimulation of forelimb and hindlimb nerves and by stimulation of the vestibular nerve. These inputs converge on many neurons. 3. Antidromic microstimulation was used to trace fastigiospinal axons to the vicinity of motor nuclei in C_2-C_3. 4. The rostral fastigial nucleus was stimulated in preparations with the MLF transected by a wide lesion that impinged on the medial reticular formation in the caudal medulla, to eliminate some potential axon reflexes. Short-latency EPSPs were recorded in some trapezius and biventer-cervicis motoneurons. In many cases there was little or no occlusion between these EPSPs and others evoked by stimulation of the vestibular nerve ipsilateral to the motoneurons. 5. Movement of the stimulating electrode and placement of this electrode lateral to the fastigial nucleus show that the zone from which low threshold EPSPs can be evoked is localized. 6. Latency measurements and lack of temporal facilitation with double shocks suggest that the EPSPs are monosynaptic. The evidence suggests that they are caused by fastigiospinal fibers terminating on motoneurons.

    The occurrence of joint contracture has been often experienced after a long-term plaster fixation. But there has been a few in terms of morphological views. In the present experiment, preparing the plaster fixed rabbits with joint contracture, we have investigated them in the fixed group, discarded group after the fixation and untreated controlled group, by light and electron microscopy. After they were sacrificed, articular cartilage in the lower part of the femur was fixed with aldehyde and osmium dehydrated, embedded, stained with uranium and lead after sectioning, and examined by transmission electron microscopy. The surface of articular cartilage consists of close-arranged fine fibrils and matrix. And there are narrow-long, flattened cells. These cells are arrayed parallel to the surface of joint. Moreover, they are small, and parallel to the surface of joint. Moreover, they are small, their cytoplasm dark, and intracellular organelles a few. In the long-term plaster fixation group, the various fibers in the surface of cartilage are randomly arranged being remarkably rough and can be seen a thick acellular zone. On the other hand, there are no remarkable changes in the surface chondrocyte. The findings here described are the progress of superficial structure and chondrocytes after plaster fixation.

    Numerous papers have been reported in the field of echocardiography, but little is known about the influence of age and sex on echocardiographic patterns in normal adults. In this paper, an attempt was made to correlate echocardiographic measurements to age and sex in 100 normal adults, whose ages ranged from 16 to 84 years. An increase in age showed a slight correlation with dicreased mitral valve diastolic descent rate and mitral valve excursion, but aortic root diameter, left atrial dimension, left ventricular dimension and left ventricular wall thickness were not altered by age and sex. It may be concluded from this study that most of echocardiographic measurements are not influenced by age and sex, which is in sharp contract with pediatric population.

    So-called "Recurrent dislocation" of the patella is a rare disease and some cases have been reported in Japan, although it had been known for many years in Europe and America and a number of cases were reported. In this paper, we report a case of congenital bilateral permanent dislocation of the patella. We made an effort to review extensively the literature on this disease. A nine-year-old girl was admitted to our hospital by complaining gait disturbance. Any congenital and familial disposition for the dislocation had not been found at that time. Since present surgery did not develop any joint movement sequelae such as contractures same operation will be performed on the other side.

    Primary tumor, especially benign tumor, of the ribs is a comparatively rare disease. In this paper we report a case of the multiple fractures suspected of the benign bone tumor of the ribs. The patient, 33-year-old man, complained of the right anterior chest pain of a three months' duration. X-ray examination of the left 4 th, 5 th and the right 4, 5, 6 th ribs revealed thickening of the bone. The right 6 th ribs revealed the pathological fracture line. A biopsy of the right 6 th rib strongly suggested benign chondroma though not conclusive.
