    The effect of G-CSF on neutropenia due to MVP chemotherapy was assessed in patients with lung cance In the G-CSF + MVP group, neutropenia was milder than in the MVP group. There was a marked decrease in the duration of neutropenia, and higher neutrophil counts were also observed. In addition, the incidence c fever accompanying neutropenia was lower in the G-CSF + MVP group. Thus, G-CSF was found to be use ful to control the toxicity of MVP chemotherapy. On the changing of IL-6, positive correlation was found fo both IL-6 and CRP.

    Interleukin 6(IL-6)is a lymphokine produced by a viriety of cells with pleiotropic functions and is one of the major immunomodulators. The production of IL-6 from human glioma cell in vitro has been reported. However, little is known about the production of IL-6 in vivo. We analysed IL-6 level of the glioma cyst fluid surgically collected. We examined cyst fluids from 4 of low grade glioma(grade I & II ),4 cases of grade III, and 10 grade IV, Although 1 case of low grade glioma showed high level of IL-6(more than 600 pg/ml), remaining 3 cases showed low level(less than 25 pg/ml). Two cases of 4 grade III gliomas showed low level, and remnining 2 cases showed 180 and 111 pg/ml. Five cases of 10 grade IV gliomas showed high level and 3 cases showed 317.6,149,30.7 pg/ml. Remainning 2 cases showed low level. Those results suggest that IL-6 level had a tendency to increase in high grade gliomas.

    Apoptosis or programmed cell death plays a key role in maintenance of the steady state in continuously renewing tissues. However, the apoptosis of head and neck tumor has not been investigated. In this study, we studied morphological change of head and neck tumor-derived cell lines (KB, KBrc, HSC-2,3,4), induced by anti Fas antibody in vitro. Apoptosis, which was determined by light and electoro morphological changes occured during culture for 1 to 2 days. Most ofthe cells underwent apoptosis was greatly enhanced when tumor cells were cultured in the presence of the interferon-γ. However, the antitumor effect of anti Fas antibody on tumor cell were different from cell lines used.

    The changes in the subsets of peripheral blood lymphocytes before and after IFN-αadministration for renal cancer patients was studied. NK cytotoxicity and 2-5AS were also examined.
    We found no change in all the subsets of lymphocytes, NK cytotoxicity and 2-5AS during the day at control period. CD16+CD57-decreased significantly three hours after first administration of IFN-α(P<0.05). CD16+CD57-, CD16+CD57+, CD16+CD56+, CD3+HLADR+, CD4+CD8+ increased significantly after 1 week administration. CD4+CD8-decreased significantly after administration. 2-5AS had a tendency of increase after administration. The results suggest that IFN-αadministration activates NK cells and T lymphocytes.

    In experimental study, hyperglycemia has been found to decrease both tumor blood flow and pH in trans. planted tumors. Both of these factors may further enhanced the response of tumor cells tc hyperthermiainduced cytotoxicity.
    In the present study, the effectiveness of hyperthermia combined with glucose administration has been esti. mated in clinical study.

    The authors have previously reported on an operative mode, termed an ‘extensive transbasal approach’, for lesions in the frontal base, through which an osteotomized supraorbital bar is resected after a bilateral frontal craniotomy, and have widely applied this surgical technique not merely to benign tumors but also to malignant tumors of the frontal base. In radical surgery involving malignant 'tumors of the paranasal sinuses that have invaded the frontal base, it is important to perform an en-bloc resection of such tumors and surrounding healthy structures.
    The authors decribe the case histories of 5 patients with a malignant tumor in the skull base. For their surgical treatment, a combination of the extensive transbasal approach and the transfacial approach was used, and the tumor and its surrounding structures, such as the dura and the orbital content, were resected en bloc. The operative results of these cases were relatively good.
    Since an en-bloc resection that resembles the authors' method has not been found in the Japanese literature, we discuss our technique, provide some representative cases, and comment on the relevant literature.

    20patients with locally advanced and inflammatory breast cencer were treated with pre-operative intraarterial and intraveinal chemotherapy mainly used adriamycin, MMC and 5-Fu. Skin redness was vanished in all cases, and skin ulcer was tended to healing in 7 cases. Tumor size was decreasing in 12 cases, and 6 cases had tumor necrosis. All cases had no severe side effects. These results were suggest that pre-operative chemotherapy is a very useful indication therapy for locally advanced and inflammatory breast cancer in term of excellent local effects.

    Breast-conserving therapy for the early breast cancer has become the alternative therapy to mastectomy in European countries and U. S. A., but not yet commonly employed in our country. From September 1991 to December 1994, 44 women with early breast cancer were treated with radiotherapy after breast-conserving surgery. All of patients have shown a good local control without side effects and satisfactory cosmetic results. We can conclude that the positive aspects of radiotherapy in the management of conservative therapy for early breast cancer are overwhelming.

    A new technique for afterloading intracavity treatment of nasopharyngeal carcinoma with iridium-192sources (Microselectron, Nucletron) is described. From May 1993 to June 1994, six nasopharyngeal carcinoma (NPC) patients received 42-62Gy by megavoltage external radiation therapy to the primary site. At one month after completion of external beams, five patients were treated with 24Gy/ 3 fractions / 3 weeks and one patient was given 40Gy / 5 fractions using the Microselectron. Acute complications were noted in two patients: in one - mucositis and in another - ulceration, but there were no late complications During the followup period (8 to 14 months), five patients have survived (one recurrences) and one patient has died. In conclusion, we found to be able to deliver an extra dose to the nasopharynx while saving the brain stem and other adjacent tissues by remote afterloading applicators.

    Nine patients with malignant biliary obstruction (8 bile duct carcinoma,1 gallbladder cancer with common hepatic duct invasion) were treated with intraluminal irradiation using a high dose remote afterloading 192Iridium brachytherapy machine (microSelectron-HDR) via a percutneous transhepatic or an endoscopic transpapillary route.
    Radiation dose of 10-12Gy at 1cm from the radiation sourse was dilivered within several minutes at each procedure. The mean total dose of radiation was 28.8Gy (range,20 to 36Gy).
    Radiation was effective from the findings of recanalized stricture on cholangiography or disappeared tumor cells on biopsy specimens. The radiation procedure was well tolerated and no procedure-related complications were observed.
    The advantage of endoscopic transpapillary intraluminal brachytherapy should be stressed that it can be perfomed less invasively. The present, preliminary experience suggested that intraluminal irradiation is effective for local control of malignant biliary stricture.

    Two neonatal cases of giant sacrococcygeal teratoma(SCT)were experienced. Case 1: 2780g 31w5d baby boy. Diagnosed at 29w by the routine ultrasonography(US). Bone by caesarian section because of fetal distress. The tumor had already ruptured. After the resuscitation, the resection of the tumor was done. But he died because of DIC. Case 2: 2680g 33w4d baby girl. Diagnosed at 32w by the routine US and magnetic resonance imaging(MRI). She was born by the caesarian section, and underwent the emergency surgery. She was discharged safely.
    Neonatal SCT is not so rare in the teratomas and its incidence is 1/4000-40000 birth. The differential diagnosis to the meningocele is important, but it is relatively easy by the prenatal US and MRI. The complications of the neonatal giant SCT are intrauterine growth retardation, urinary tract obstruction, high-output cardiac failure, rupture, DIC(with/without rupture), etc. So it is nessesary for the cases with such a giant SCT to be diagnosed by noninvasive imaging, to be managed intensively, to be born by the caesarian section preparing for the emergency surgery.
