    Forty-eight patients with chronic arsenical poisoning among inhabitants of a mountaneous village living near an abandoned mine at Toroku, Miyazaki Prefecture, Kyushu, Japan were reported. Epidemiological examinations revealed that these patients had been exposed for a long time to the dust contaminated with arsenicals, while medical examinations revealed involvement not only of the skin, but also of the breathing, smell, hearing and visual organs. Although clinical symptoms vary according to the pathway of entry of arsenicals into the body, we should follow up patients with chronic arsenical poisoning because arsenicals are potentially carcinogenic.

  • —A Report of Two Cases—

    Two cases of hidroacanthoma simplex, which appeared in a 62-year-old man and a 73-year-old woman, are reported. Histologically, intraepidermal tumor masses consisting of similar-sized cells well defined from the surrounding epidermal cells and the histopathologic features showed intraepidermal epithelioma of Jadassohn. On the basis of 12 reported cases and the authors' two cases of hidroacanthoma simplex, clinical and histopathologic findings are discussed and the disease is considered as an intraepidermal type of eccrine poroma.

    Nine groups of scabies infections were reported. Six groups were hospitalized patients, three groups were familial infections. Scabies were treated with Eurax H ointment and sulfur bath. Symptoms took a month to subside after treatment.

    Two cases of tinea capitis, observed in 6-year-old boy and his sister of 4 years of age, are reported. The former case showed “black dot” clinical features. Mycological studies led to the identification of the etiologic agent as Trichophyton violaceum Sabouraud apud Bodin, 1902.

    The patient was a 3-year and 9-month-old girl having erythematous maculae with a tendency to central discoloring and small pustules, both extending diffusely or in fusion on the head, abdomen and limbs, accompanying fever and shaking chills. Histology showed spongiform pustules of Kogoj, parakeratosis, rete ridge elongation, capillary dilatation in the dermal papillary layer. Improvement in symptom was brought about with treatment by oral administration of betamethasone and subcutaneous injection of synthesized ACTH. Upper respiratory inflammation, such as streptococcal infection at her first hospitalization and Gram-negative rods infection at her second one, was inferred as the agent to aggravate eruption. This case was diagnosed as generalized pustular psoriasis von Zumbusch in infant. This, drawing notice lately, has shown an increasing tendency in incidence.

    A case of 24-year-old man with multiple large tuberous xanthomas in several places on the whole body and familial type II b lipidemia was reported. Multiple polypoid tumors were found in the jejunum and ileum by roentgenological examination of gastrointestinal tract. These polypoid tumors were suspected to be xanthomas on the wall of intestinal tract or mesenterium.

    A case of 50-year-old man having tuberous xanthoma, tendinous xanthoma and xanthelasma in association with angina pectoris was reported. Chemical analysis of serum lipid showed hyperbetalipoproteinemia, raised cholesterol and elevated triglyceride level.

    A typical case of morphea-like basal cell epithelioma (MBCE) of 2 years' duration on the epigastrium in a 51-year-old woman was described. A slightly elevated, yellowish and sclerotic oval plaque was noted with partial pigmentation. The margin of the plaque was rather definite. Histopathological change of the skin legion was characterized by the presence of oval, spherical or strand-shaped small nests of atypical basal type cells, surrounded by a dense fibrous stroma. This may be the eleventh reported case of MBCE in Japan.

    Two cases of sarcoidosis in a 55-year-old woman and a 67-year-old woman were reported. Their clinical feature and histological examination showed the plaque type of sarcoidosis. On clinical findings, swelling of lymph node was revealed in the first case, and glaucoma in the second case. The treatment with steroid hormone had good response.

    A sixteen-year-old girl had many hypostatic abcesses on the both extremities and markedly various neurological and psychiatric symptoms in the period of a clinical process of SLE. In spite of temporary improvement of symptoms with administration of large dose of steroid, she was dead of pneumonia following repeated attacks of convulsion and unconsciousness. In autopsy, hypertrophy, fibrosis and hyarinization of the glomerulus was found in the kidney, onion skin lesions in the spleen and hyaline degeneration in the lymph nodes. Fibrous hypertrophy of the arachnoid, various changes of the small vessels, bleeding, softening, degeneration of the nerve cells, swelling of the axis-cylinder, demyelination and increasing of the wasting pigment were demonstrated in pathohistological findings of the brain.

    A case of pigmented basal cell epithelioma, a male of 52-year-old, was treated topically with Picibanil, a streptococcal anticancer preparation. After no changes of tumor were observed by irradiation therapy and treatment with bleomycin or 5-FU ointment, 1 ml of this agent containing 0.25 KE of Picibanil was injected directly into the tumor devided into some small doses. Regression of the tumor was found obviously after three times of injection, and there was grossly an almost complete healing of the tumor following the fifth administration. Histopathological findings revealed the healed lesion consisting solely of a scar and atrophic residual tissue. No abnormal cells were observed. No adverse reactions were noted except for slight degree of pain at the time of injection. The topical use of Picibanil might be available for the tumors with a low degree of cytological atypia or for patients who were reluctant to receive surgical treatment.

    Eighteen patients with chromomycosis in Japan were treated with 5-fluorocytosine (5-FC). They received orally 5-FC in a dose of 4 to 10g daily for 1.5 to 18 months. Eight patients were cured or almost cured. Nine patients were improved, but four of them were relapsed. No response to treatment was observed in one patient. No side or toxic effects were encountered. A 47-years-old male with chromomycosis due to Fonsecaea pedrosoi was reported. He was treated with 5-FC in a dose of 8g daily and improved.

    The ultrathin sectioning of Phialophora dermatitidis in the dermis of the lesions of chromomycosis and the cultured fungi were investigated with ultrathin sectioning and freeze-etching electron-microscopically. The fungi in the lesions have thick cell wall and large vesicles in size and mitochondria. In cytoplasm of almost all the cultured fungi, the small dense granules and the slightly large dense granules (0.3∼0.4μ in diameter) with vesicles endoplasmic reticulum and mitochondria were seen. By freeze-etching, the various sized vesicles, mitochondria and nucleus were clearly seen in the cytoplasm. Cytoplasmic membrane revealed various shape, such as flat and waving.

    Serum lipids were studied in 70 patients with psoriasis. Hypertriglyceridemia was found in 30 patients (43%) and hyperbetalipoproteinemia was found in patients (30%). Hypertriglyceridemia and hyperbetalipoproteinemia of very high levels were found in 11 patients (15.7%). The other lipids (total lipid, total cholesterol, phospholipid, non-esteric fatty acid) were normal levels. Prebetalipoprotein was appeared in 12 patients of 28 patients with psoriasis (43%). Hyperlipidemia with psoriasis was found in patients (55.7%) and the frequency of hyper-lipidemia in 70 patients with psoriasis was equaled to that of healthy controls (Japanese). However the type of hyperlipidemia with psoriasis was mostly Fredrickson's type IV. In 47 patients with psoriasis the concentrations of serum lipids were studied both at admission and at discharge. The results showed that the concentrations at discharge were lower than that at admission, but it's variation was within normal limit. In 39 patients with psoriasis fatty acids were analysed and it showed that palmitic acid level in triglyceride, cholesterol-ester, and phospholipid were significantly higher and linolic acid level was significantly lower than that of normal control. The levels of their components of fatty acid of 17 patients with psoriasis at admission were compared to those at discharge. The results showed that the levels of palmitic acid and linolic acid which were abnormal at admission were neared to normal range at discharge time. The relationships between the involved area of the skin and fatty acid levels were studied. The results showed that the smaller the area of skin lesion, the nearer to normal range the fatty acid levels. From the results, it is imposed that the variation of the fatty acid levels of the patients with psoriasis is well correlated the activity of the skin rash of psoriasis.
