    In clinical and statistical survey on the patients with burns in our clinic from 1963 to 1982, the following results were obtained.
    1) The total number of the patients with burns was 543, that is 0.92% of the total patients seen in this period.
    2) Ratio of the sexes was much the same.
    3) The incidences showed two peaks at the age from newborn to 5 years and from 20 to 29 years.
    4) There was an increased incidence during the months in winter.
    5) Burns related to high temperatures of liquids including boiling water showed the highest incidence, followed by burns related to high temperatures of solids. Burns caused by flames or heated gas were low in incidence.
    6) Legs, hands and thighs were the most frequently involved areas of the body.
    7) Second-degree burns accounted for three quarters of the incidence in the total number of patients.

    Of 48 patients with fresh burns who were hospitalized in the Department of Dermatology, Yamaguchi University Hospital and who were treated with transfusions, 15 cases died (31% mortality). No significant differences were seen between the mortality in the first half decade and that in the last half of these 20 years from 1963 to 1982, and between the mortality in the previous period without ICU and that in the subsequent period with ICU. With regard to the incidence in adults and children, death occurred in those with more than 60% of the whole body surface burned, that is, 82% and 40% respectively. This suggests that the prognosis in adults cases is worse. In the elderly, even when the affected areas accounted for 15% and 30%, the mortality was 80%. With a chemical analysis of the factors influencing the prognosis, we found that acute renal failure and infections of Pseuomonas aeruginosa 48 hour after acquisition of the burn in adults, and respiratory disturbances in cases of children and elderly, were the major problems.

    In clinical and statistical surveys on the patients with photodermatoses seen in our clinic from 1963 to 1982, the following results were obtained.
    1) Patients with photodermatoses numbered 517, that is 0.9% of the total outpatients in this period.
    2) The yearly incidence of the patients tended to decrease after 1975.
    3) The prevalence was most frequent from April to September, especially in July.
    4) There were 141 with drug induced photosensitivity or its suspicion, 31 with polymorphic light eruption, 17 with porphyria, 15 with xeroderma pigmentosum, 11 with photocontact dermatitis, 10 with sunburn, 5 with hydroa vacciniforme, 2 with solar urticaria and 285 which could not be classified.

    There were 146 patients with occupational contact dermatitis hospitalized in our department from 1963 to 1982. Included were 57 with rubber dermatitis, 26 cement dermatitis, 12 agricultural perticides dermatitis, 4 plant dermatitis, 16 chemical burns, 13 dermatitis occurred while working in chemical factories and 18 had dermatitis of other causes. In clinical and statistical surveys the following findings were of interest. 1) Patch test of materials involved in rubber and minor metallic elements in cement revealed high incidences of positive reactions to 2-mercaptobenzothiazole and to Cr6+. 2) One patient with a burn due to phosphoric acid died. 3) Problems related to cross sensitization were evident in contact dermatitis due to 2-amino-4-chlorophenol and in that due to 4-methylsulfonyl-2,6 dinitro N,N dipropyl aniline, both aromatic compounds.

    Clinical and statistical surveys on the 433 patients of cosmetic dermatitis observed at our clinic from Apr. 1963 to Dec. 1982 revealed:
    1) No definite tendency was seen in the incidence of this disease per year among the total numbers of outpatients.
    2) Of all the patients, 423 were women and 10 were men.
    3) Eighty percent of the occurrences were in the second to fourth decades.
    4) The number of positive materials per one case in closed patch testing with cosmetic products increased in the last ten years.
    5) Positive incidences in closed patch testing with make-up cosmetics were lower than the incidences with basic and cleansing cosmetics and hair dressing products.
    Among basic cosmetics, positive incidences of milky lotion, cream and lotion were nearly equal each other, while among make-up cosmetics, positive incidence of make-up foundation was the highest.

    In clinical and statistical surveys on the patients with drug eruptions observed in our clinic from Jan. 1964 to Dec. 1982, the following results were obtained.
    1) The total number of patients with drug eruptions was 1,348, that is 2.2% of the total outpatients seen in this period.
    2) The yearly incidence tended to decrease for the last nine years.
    3) As to age distribution, the incidence of aged patients tended to increase for the last eight years.
    4) Of all the patients, 604 were men and 744 were women with sex ratio 1 (male) to 1.23 (female).
    5) The incidence tended to increase in the exanthematic type, urticaria and lichenoid type but to decrease in the fixed type for the last eight years.
    6) Among the definite cases of drug eruptions, cases due to penicillins, cephalosporins, cinnarizine and thiopronin tended to increase in number, while those due to chloramphenicol, sulfonamides and pyrazolon derivatives tended to decrease for the last eight years.
    7) The cases accompanied with high values of GPT remarkably increased in incidence in the last eight years.
    8) Positive results in the patch test were frequently obtained in cases due to antibiotics and tiopronin.
    9) The positive cases were more frequently seen in the skin window test than in the patch test, and so the former was thought to be more useful than the latter.

    Clinical and statistical surveys on the patients with viral skin diseases observed at our clinic from 1963 to 1982, revealed the following results.
    1) The total patients with viral skin diseases numbered 2,577, that is 4.38% of the total patients seen in this period.
    2) Sex ratio was 1 (male) and 1.3 (female).
    3) There were 1,067 with viral warts, 762 with herpes zoster, 290 with herpes simplex, 225 with molluscum contagiosum, 134 with varicella, 36 with rubeolla and 68 with others.

    In a survey on the patients with mycosis seen at our clinic in the years from 1963 to 1982, the following results were obtained.
    1) The total number of patients with mycosis was 5,545, that is 9.4% of the outpatients seen in this period.
    2) There were 4,613 with dermatophytosis (83.2%), 607 with candidiasis (10.9%), 300 with tinea versicolor (5.4%), 26 with sporotrichosis, one with chromomycosis and one with favus.
    3) Dermatophytosis included 2,093 with tinea pedis, 891 with tinea cruris, 655 with tinea unguium, 622 with tinea corporis, 329 with tinea manus, 11 with kerion celsi, 9 with tinea capitis, 2 with sycosis trichophytica and one with granuloma trichophyticum.
    4) Candidiasis included 201 with erythema blastomycetica infantile, 178 with intertrigo erosiva blastomycetica, 103 with erosio interdigitalis blastomycetica, and others. There has been an increase in each since 1972.
    5) Sporotrichosis included 13 of the lymphatic type and 11 of the fixed type. All patients with a sporotrichosis lived in Yamaguchi Prefecture, 22 lived in the Sanyo districts and 2 in the Sanin districts.

    In a survey on the patients with cutaneous tuberculosis seen at our clinic from 1963 to 1982, the following results were obtained.
    1) The total number of patients with cutaneous tuberculosis was 76, that is 0.12% of the outpatients seen in this period.
    2) There were 11 cases of true cutaneous tuberculosis (14.5%) and 65 of tuberculid (85.5%).
    3) True cutaneous tuberculosis included 4 with lupus vulgaris, 3 with tuberculosis verrucosa cutis, 3 with scrofuloderma and one with primary cutaneous tuberculosis complex.
    4) Tuberculid included 30 with erythema induratum Bazin, 29 with lupus miliaris disseminatus faciei, 3 with phlebitis tuberculosa nodosa, one with papulonecrotic tuberculid and 2 with rosacea-like tuberculid.
    5) There was no remarkable change in the incidence in this 20 years period.

    In a survey on the patients with syphilis seen at our clinic from 1963 to 1982, the following results were obtained.
    1) The total number of patients with syphilis was 304, that is 0.52% of the outpatients seen in this period.
    2) There were 16 with syphilis resens (5.3%) and 288 with sero-reaction, excluding syphilis resens (94.7%).
    3) There were 8 cases in 1965, 1 in 1967, 1 in 1974, 2 in 1978 and no patients with syphilis resens were seen after 1978.

    In a clinical and statistical survey on patients with scleroderma seen in our clinic from 1963 to 1982, the following results were obtained.
    1) The total number of scleroderma patients was 67, that is 0.1% of the total outpatients seen in this period.
    2) There were 22 cases of localized scleroderma (6 males and 16 females) and 45 cases of progressive systemic sclerosis (3 males and 42 females).
    3) Patients with localized scleroderma included 15 with morphea, 1 with scleroderma circumscriptum en bandes, 5 with scleroderma circumscriptum en coup de Sabre and 1 of generalized morphea (positive of antinuclear antibody, anti DNA antibody and RA factor), 14 with a single lesion and 8 others with multiple lesions. Patients with progressive systemic sclerosis included 3 with type 1 of Barnett’s classification, 38 with type 2 and 4 with type 3.
    4) Twenty four patients with progressive systemic sclerosis had Raynaud’s phenomenon as the primary symptom and 6 were associated with autoimmune diseases.

    Clinical and statistical survey on bullous diseases observed at our clinic from Apr. 1963 to Dec. 1982, revealed:
    1) The total number of patients with bullous diseases was 66, that is 0.11% of the total outpatients seen in this period.
    2) Pemphigus comprising four types was seen in 25, dermatitis herpetiformis in 19 and bullous pemphigoid in 23.
    3) There were increases in bullous diseases, especially in pemphigus and bullous pemphigoid after 1977.

    Clinical and statistical observations of psoriasis and parapsoriasis in our clinic from 1963 to 1982 revealed:
    1) The total number of patients with psoriasis was 430, that is 0.73% of the total in this twenty year period. The incidence of psoriasis showed a tendency toward a steady increase.
    2) Sex ratio of psoriatic patients was 2.0 (male): 1 (female).
    3) As for the age of the psoriatic patients at the time of the first examination, the prevalence among both sexes was most frequent in those between 40 and 49.
    4) With regard to the age of the psoriatic patients at onset, the prevalence among both sexes was relatively frequent in those between 20 and 49. In four patients, the onset was within the first year of life.
    5) In 1.8% of psoriatic patients, immediate family members also had psoriasis.
    6) There were 50 patients with parapsoriasis, that is 0.08% of the total outpatients.

    In the statistical observation of xanthomatosis from 1963-1982, the following results were obtained:
    1) The total number of the patients with xanthomatosis including juvenile xanthogranuloma was 82, that is 0.14% of the total patients seen in this period. There was a significant increase in the incidence after 1978.
    2) The total number of the patients with xanthelasma palpebrarum was 47, most of whom were in the 30-59 age group.
    3) The total number of the patients with juvenile xanthogranuloma was 19, all of whom were within 5 years of age, except for 2 cases of the adult type.
    4) In many patients with xanthomatosis, serum lipoprotein abnormalities were apparent.

    In clinical and statistical surveys on the patients with cutaneous amyloidosis observed at our clinic from Apr. 1963-Dec. 1982, the following results were obtained:
    1) The patients with cutaneous amyloidosis numbered 30, that is 0.07% of the total patients in this period.
    2) The yearly incidence to the total number of the patients tended to increase with year.
    3) 43% of the occurrences were in the fourth to fifth decades of life.
    4) PAG electrophoretic analysis of serum lipoproteins revealed that all of 6 with lichen amyloidosus, of 2 with maculo-papular amyloidosis and of one with amyloidosis cutis nodularis atrophicans, and one of 5 with macular amyloidosis were associated with dyslipoproteinemia.

    In clinical and statistical surveys on the patients with phacomatosis observed at our clinic from Apr. 1963 to Dec. 1982, the following results were obtained.
    1) The total number of patients with phacomatosis was 144, that is, 0.24% of the total outpatients seen at the clinic.
    2) The disease showed no definite tendency in annual incidence to the total number of outpatients.
    3) The patients included 92 with Recklinghausen’s disease, 18 with Pringle’s disease, 20 with Klippel-Weber’s disease and 14 with other miscellaneous phacomatosis.
    4) As to the sex distribution, no definite differences were seen in each of the above diseases.

    There were 80 patients with syndrome dermatotumorale seen in our clinic from 1963 to 1982. There were 10 cases of dermatomyositis (accompanied by rectal cancer in 2, lung cancer in 2 and hepatoma and gastric cancer in 1 respectively), 6 of acanthosis nigricans (accompanied by gastric cancer in 3), 22 with dermatitis herpetiformis Duhring (accompanied by cervical carcinoma, gastric cancer and rectal cancer in 1 respectively), 24 of bullous pemphigoid (accompanied by pancreatic carcinoma in 1), 12 of skin metastasis of internal malignancy (mammary cancer in 8, malignant struma, cervical carcinoma, lung cancer and malignancy of unknown origin in 1 respectively) and 6 of other skin diseases (accompanied by gastric cancer in 3, cervical carcinoma, malignant thymoma and prostatic cancer in 1 respectively).

    In clinical and statistical surveys on the patients with cutaneous malignant tumors observed at our clinic from Apr. 1963 to Dec. 1982, the following results were obtained.
    1) The total number of the patients with cutaneous malignant tumors numbered 434, that is, 0.7% of the total number of outpatients seen in this period.
    2) Of all the patients, 229 were men and 205 were women.
    3) There were 120 with squamous cell carcinoma, 128 with basal cell epithelioma, 63 with Bowen’s disease, 29 with malignant melanoma, 24 with malignant lymphoma, 12 with adnexal tumor, 19 with metastatic skin tumor, 5 with cutaneous leukemia, 11 with sarcoma and 2 with other tumors.
    4) Among the total number of outpatients, the yearly incidence since 1972 in each of the tumors, especially in Bowen’s disease and malignant melanoma tended to increase compared with that during the 1960s.
    5) Males outnumbered females in squamous cell carcinoma and malignant lymphoma, while the reverse was true in case of Paget’s disease and malignant melanoma.

    Clinical and statistical surveys on the patients with squamous cell carinoma observed at our clinic from Apr. 1963 to Dec. 1982, revealed the following:
    1) The total of the patients with squamous cell carcinoma numbered 120, that is 0.20% of the total number of outpatients seen in this period.
    2) Of all the patients, 76 were men and 44 were women.
    3) As to the age, the incidence tended to increase in those over 40 and highest peak was noted in the 8th decade.
    4) As to the anatomical sites of tumor, 42 cases, that is 35.0% of the total number of patients had lesions on the face.
    5) The tumors had a pre-cancerous lesions in 60 cases, 32 cases of which were burn scars.
    6) As to the TNM classification, the prognosis became poorer as the tumor enlarged and invasion was deeper.
    7) As to the Broders’ classification, the prognosis became poorer as the histopathological findings of tumors showed less grades in differentiation.

    In statistical surveys on patients with basal cell epithelioma seen in our clinic from Apr. 1963 to Dec. 1982, the following results were obtained.
    1) The patients with basal cell epithelioma numbered 128, that is 0.21% of the total number of the patients.
    2) Sex ratio was 1.1 (male): 1.0 (female).
    3) In age distribution, adults over 40 years old accounted for 94.5% of the total number of patients.
    4) The face, especially lower eye lids, cheeks, nasal area and upper lip were most commonly involved.
    5) With regard to the clinical appearance, nodulo-ulcerative type was predominant (82.6%), followed by the superficial type (14.6%), epithelioma terebrans (1.8%) and morphea-like type (0.9%).
    6) As for the histopathological type, the solid type was predominant (56.0%) followed by the superficial type (14.6%), adenoid type (13.8%), cystic type (7.3%), keratotic type (7.3%) and syringoid type (0.9%).
    7) Therapy for most patients (97.7%) was surgery and there was recurrence only in 3 patients.

    Clinical and statistical survey on the patients with Bowen’s disease observed at our clinic from 1963 to 1982, revealed the following results.
    1) The total patients with Bowen’s disease numbered 63, that is 0.11% of the total patients seen in this period.
    2) Sex ratio was 1.3 (male) and 1 (female).
    3) There were 56 with solitary type (89%) and 7 with multiple type (11%).
    4) Seven patients were associated with cutaneous cancers and six patients had visceral cancers.
    5) In five patients, the possible cause was ingestion of arsenic and in two X-ray irradiation was implicated.

    In clinical and statistical surveys on 36 patients with sweat gland tumors and 22 with Paget’s disease observed at our clinic from Apr. 1963 to Aug. 1983, the following results were obtained.
    1) The total number of patients with sweat gland tumors numbered 36, that is, 0.059% of the total number of outpatients seen in this period and 0.26% of the total number of biopsied specimens obtained.
    2) Sex ratio was 3 (female): 1 (male).
    3) Face and scalp were the most frequently involved.
    4) The patients with Paget’s disease numberd 22, that is, 0.036% of the total number of outpatients.
    5) The ratio of mammary Paget’s disease to extramammary Paget’s disease was 1:1.2.
    6) Patients with mammary Paget’s disease were all female, while extramammary Paget’s disease, sex ratio was 1 (female): 1 (male).
    7) The average age of patients with extramammary Paget’s disease (72 years old) was older than that of patients with mammary Paget’s disease (58 years old).

    In clinical and statistical surveys on malignant melanoma in our clinic from Apr. 1963 to Jun. 1983, the following results were obtained.
    1) The total number of patients with malignant melanoma was 33 and the average incidence was 0.054% of total outpatients in this period.
    2) The incidence of this tumor increased after 1973.
    3) Sex ratio was 1 (male): 2.7 (female).
    4) As to the anatomic sites of primary tumor, the average incidence was 39.4% in the sole of the foot.
    5) In the studies on prognostic factors, the stage grouping, typing of tumor, period from the time of biopsy of the primary tumor to surgical treatment, excisional distance from margin of tumor, regional lymphnode dissection, histopathological characteristics including Clark’s level, Breslow’s thickness of invasion, tumor cell type, degree of lymphocytic infiltration and mitotic ratio except for degree of pleomorphism were positively related to the survival ratio.

    In a clinical and statistical survey on patients with malignant lymphoma treated at our clinic from Apr. 1963 to Dec. 1982, the following results were obtained:
    1) The total number of the patients with malignant lymphoma numbered 25, that is 0.04% of the total outpatients seen in these years.
    2) The 25 comprised 19 with non-Hodgkin lymphoma, 2 with Hodgkin’s disease, 2 with mycosis fungoides and 2 with adult T cell leukemia-lymphoma.
    3) The number tended toward increase in the latter 10 years, owing to the increase in the number of those with non-Hodgkin lymphoma.
    4) There were 17 men and 8 women.
    5) According to the LSG classification, the 17 with non-Hodgkin lymphoma were classified into 1 with small cell type, 5 with medium cell type, 5 with mixed type, 3 with large cell type, 2 with pleomorphic type and 1 with lymphoblastic type.
    6) According to the Ann Arbor staging classification, the 19 with non-Hodgkin lymphoma were classified into 4 in stage I, 3 in stage II, 7 in stage III and 5 in stage IV.
    7) Chemotherapy was prescribed in all, radiation therapy for 6, immunotherapy for 13 and surgical therapy for 1 patient.
    8) The prognosis in the 14 with non-Hodgkin lymphoma seemed to correlate with the manifestation of systemic symptoms and the scored values of parameters concerning the antitumor resistance, but not with the histopathological and staging classification.

    In a statistical survey on inpatients who underwent surgical operations in our clinic from Apr. 1963 to Dec. 1982, the following results were obtained.
    1) The total number of operations was 2,068.
    2) The number of cases involving general anesthesia was 967, that of lumbar and epidural anesthesia was 65, and that of local anesthesia 1,036.
    3) Surgical treatment was frequent in the following diseases: scars (521 cases), malignant tumors (351 cases), benign tumors (263 cases), naevus (236 cases), and radiodermatitis (230 cases).
    4) The operative techniques frequently used were split thickness skin grafts (935 cases), simple excisions (549 cases), indirect distant flaps (149 cases), local flaps (141 cases) and full thickness skin grafts (138 cases).
    5) Split thickness skin grafts were used most frequently for scars, malignant tumors and radiodermatitis and simple excisions for benign tumors and naevus.
    6) The average number of the operations per patient was numerous in the following diseases: radiodermatitis (5.11 times), angiopathy (1.78 times), morbus Paget (1.76 times) and phacomatosis (1.65 times).

    Statistical survey on the diseases seen in our clinic from Apr. 1963 to Dec. 1982, revealed the following results.
    1) Among the total cases, high incidences were seen in eczema-dermatitis, fungal diseases, benign tumors, viral diseases and toxicoderma-drug eruptions.
    2) The incidence of women was higher than that of men in each of the 34 kinds of skin diseases, while it was lower than that of men with erythroderma, bullous diseases, inflammatory keratosis and malignant tumors.
    3) There were increasing tendencies in dermatozoonosis, vascular diseases, erythroderma, bullous diseases, collagen diseases, metabolic diseases, granulomas and pre-cancerous diseases-carcinoma in situ and decreasing tendencies in syphilis and sweating disturbances in the 10 years from 1973 to 1982.

    In a statistical survey on outpatients under fifteen in our clinic for the years from Apr. 1963 to Dec. 1982, the following results were obtained.
    1) The total number of cases among the outpatients under fifteen was 12,955, that is 21.3% of total number of the cases in this period.
    2) In the outpatients under fifteen, there were high incidences in eczema and contact dermatitis, benign tumors, pyoderma, and viral diseases in this decreasing order.
    3) Among the total number of each diseases, high incidences were seen in atopic dermatitis, varicella, acute pyoderma, nonepithelial benign tumors, naevi and phacomatosis in this decreasing order.
    4) In comparison with the number of cases in outpatients over 60, there were high incidences of atopic dermatitis, varicella, naevi, scabies and phacomatosis in this decreasing order, and there were low incidences of PSS, cutaneous amyloidosis, precancerous diseases and carcinoma in situ, malignant tumors, pruritus, radiodermatitis, chronic pyoderma, tuberculid, erythroderma, pustulosis, DLE and dermatomyositis.
    5) Sex ratio was 0.95 (male): 1 (female) in the total number of cases and there were high values in morbus Behçet, erythema nodosum and low values in erythroderma and SLE.

    A statistical survey was done on patients over 60 seen in our clinic from 1963-1982, the following results were obtained:
    1) The total number of the patients over the age of 60 was 7,653, that is 13% of the total patients seen in this period.
    2) Among the total number of each diseases, with a high incidence were Bowen’s disease, keratosis senilis and carcinoma cutis.
    3) Trichophytia showed the highest incidence in patients over 60. The incidence of contact dermatitis, pruritus cutaneus was also high.
    4) In comparsion with patients under 15, carcinoma cutis, tuberculids, erythroderma and pustulosis palmaris et plantaris were high in incidence in patients over 60 years of age.
    5) Phacomatosis, verruca vulgaris and disease of pilosebaceus system were of a high incidence in men. Tuberculids, erythema nodosum and disease of nails were of a high incidence in women.

    In the examination of serum lipid, PAG electrophoresis, analytic ultracentrifugation and measurement of particle sizes of serum lipoproteins before and after the administration of clofibrate (CPIB), the following results were obtained:
    1) Hyperlipidemia became normal in 7 of 7 patients after CPIB treatment.
    2) PAG electrophoresis showed that in one out of 3 patients with type IIb, it changed into BMB and 2 out of 3 into LM, and 2 out of 4 with type IV into BMB and one out of 4 into LM.
    3) Sizes of lipoprotein particles in IDL and LDL of patients were greater than those of healthy individuals. No statistically significant differences were found.

    Immunodiffusion analysis of LDL fractions from patients with type BMB and with LM revealed: each of the fractions showed a precipitation line to the anti-VLDL and the anti-LDL sera, but not to the anti-HDL sera. Thus there seems to be little difference between BMB and LM. The LDL fraction may be much the same as the IDL fraction with regard to composition.

    In analysis of glycolipid patterns of skin specimens from 2 humans without dermatosis, skin lesions from 3 patients with nevus cell nevus and that from one patient with malignant melanoma, using a thin layer chromatographic scanner, the following results were obtained.
    1) All specimens were found to have CMH, CDH, CTH and globoside I for neutral glycolipids and GM3 and unknown spots for gangliosides.
    2) Compared with the glycolipids of the normal human skin, each skin lesion of all patients with nevus cell nevus and malignant melanoma showed a high Rf value of CMH and the amount of CTH was extremely decreased.

    We tried to assess the usefulness of the structuredness of cytoplasmic martrix (SCM) test in in vitro testing of drug eruptions, applying this test to 6 patients with positive results of in vitro tests and 14 healthy controls. In 14 controls examined by SCM test with PHA, polarization percent decreases were from -3.8% to -11.6%, and so the SCM test was considered to be reliable to investigate the sensitization of lymphocytes. Of 6 patients examined by SCM test with implicated drugs, polarization percent decreases were -4.4% in on patient (ampicillin) and -5.4% in the other (amikacin), but more than -1.3% in the other patients. From the above, the SCM test seems to be a useful method of in vitro testing for patients with drug eruptions. Further detailed investigations are underway.

    Specimens from 20 patients with various sweat gland tumors and twelve with related or other diseases for a control study and including Paget’s diseases were stained for carcinoembryonic antigen (CEA) using the peroxidase antiperoxidase technique. In eccrine tumors, CEA was chiefly detected at the wall of luminal structures, while in case of apocrine tumors, evidence was obtained in the portions apart from the luminal structures. Based upon the above, searching for CEA by this technique proved to be useful in order to differentiate eccrine tumors from apocrine tumors and to identify Paget cells.

    By indirect immunofluorescent investigations on the distribution of actin in seborrheic keratosis with specific antiactin antibody, the following results were obtained:
    1. The specific fluorescence was detected in basaloid cells of the 19 cases including all any types of seborrheic keratosis.
    2. It was in 6 of retiform type that the specific fluorescence was detected in squamous cells.
    3. Fluorescences in squamous cells showed a tendency to decrease in grade as they approached the horny layer.
    4. These findings suggest that differences in distribution of actin in squamous cells depends neither upon the keratinization of squamous cells nor the proliferation of basaloid cells.

    Distributions in epidermal tumors of β2-microglobulin, that is the L chain of HLA were investigated, using immunofluorescent and immunoperoxidase techniques, and the following results were obtained.
    1) Normal skin; keratinocytes of the epidermis except for the horny layer, sweat glands, hair follicles and sebaceous gland were positively stained for β2-microglobulin at the cytoplasmic membrane.
    2) Benign tumor; tumor cells of keratoacanthoma and of seborrheic keratosis were positively stained at the cytoplasmic membrane.
    3) Carcinoma in situ; tumor cells in cases of Bowen’s disease were stained positively in some portions but negatively in others.
    4) Malignant tumors; tumor cells of basal cell epithelioma and of squamous cell carcinoma were rarely stained.

    Studies using immunofluorescent and rosette-formation techniques and histopathological examinations on the 12 cases of malignant lymphoma seen in our clinic, 9 of non-Hodgkin lymphoma (NHL), 2 of adult T cell leukemia-lymphoma (ATLL) and one of mycosis fungoides (MF), revealed the following results.
    1. Six of 9 cases of NHL were T cell lymphoma, 4 of which were suspected to be derived from thymic T lymphocytes, 2 were from peripheral T lymphocytes and the other 3 were unclassifiable. Two with ATLL and one of MF were suspected to be derived from peripheral T lymphocytes.
    2. Results of examinations with the rosette-formation technique correlated with those of the above immunohistological examinations.
    3. In the histopathological examinations, epidermotropism was seen in 3 cases of NHL, one of MF and one of unclassifiable lymphoma. A clear zone was seen only in one case of large cell type NHL, convoluted nuclei in one case of NHL, one of MF, 2 of ATLL and 2 of unclassifiable lymphoma. Cleaved unclei were present in 3 with unclassifiable lymphoma, but not in cases of T cell lymphoma.

    To determine histoquantitatively the LSG classification for patients with malignant lymphoma manifesting cutaneous lesions and to make comparative assessment between cutaneous and extracutaneous lesions, histoquantitative analyses on nuclei of tumor cells of 13 patients with non-Hodgkin lymphoma were made using areas and aspect ratios for parameters. An image analyzer computer system was used. The cases were classified into 1 with small cell type, 5 with medium sized cell type, 3 with mixed type, 2 with large cell type, 1 with pleomorphic type and 1 with lymphoblastic type, according to the LSG classification and the biopsy materials were obtained from 11 skin lesions and 5 nodal lesions. The following results were obtained:
    1) Areas of nuclei were large in the small cell type, medium sized cell type, mixed type and large cell type in increasing order.
    2) Aspect ratios of nuclei of tumor cells present in the skin lesions were larger than those in the nodal lesions.

  • 2nd Report: Quantitative Analysis of Basal Cell Epithelioma

    In quantitative analyses on nuclei of tumor cells in each of the 6 histopathological types of basal cell epithelioma, using an image analyzer computer system (Luzex 500), the following results were obtained.
    1) Nuclei of syringoid type tumor cells were analogous in area, aspect ratio and hyperchromatic value to those of normal apocrine duct cells, thereby suggesting that the syringoid type tumor cells may be shaping themselves after apocrine duct cells.
    2) Nuclei of superficial type tumor cells were analogous in area to those of basal cells in normal hair follicles, thereby suggesting that superficial type tumor cells may be shaping themselves after hair follicle cells rather than after epidermal cells.
    3) Nuclei of keratotic type tumor cells were analogous in aspect ratio and hyperchromatic value but not in area to those of basal cells of normal hair follicles.
    4) Similarities were hardly seen (1) in area, aspect ratio and hyperchromatic value between nuclei of adenoid type tumor cells and those of normal eccrine and apocrine gland cells, (2) in area and aspect ratio between nuclei of cystic type tumor cells and those of basal cells of normal sebaceous gland and (3) in area and aspect ratio between nuclei of solid type tumor cells and those of basal cells of normal epidermis and hair follicles.
    5) In a comparative study between T1 and T4, it was shown that nuclei of solid type tumor cells in T4 were larger and more basophilic than those in T1, and nuclei of cystic type tumor cells in T4 were larger and less basophillc than those in T1. This suggests that the parameters of area and hyperchromatic value may indicate the occurrence of tumor invasion.

    We carried out ultrastructural and morphometric studies on 2 cases of verruca senilis of Bloch type, as compared to the normal surface epidermis, follicular infundibulum and isthmus. We measured the maximum length of the attachment plaque of desmosomes and the area of desmosome-tonofibril complexes to analyse development of the tonofibrils. The basaloid cells were similar light microscopically but displayed ultrastructurally different findings in addition to the common features that basaloid cells from each had areas similar to the surface epidermis, follicular infundibulum or isthmus.

    In ultrastructural investigation of a case of bullous pemphigoid, using a transmission electron microscope, the following results were obtained.
    (1) Intercellular spaces of the epidermis were dilated and contained numerous tonofilament-desmosome complexes and migrated lymphocytes.
    (2) Cell debrises packed with filamentous structures were observed in the dilated intercellular spaces of the basal layer of the epidermis, as well as in bulla cavities and upper dermis.
    (3) The formation of bulla occurred beneath the basal lamina of the epidermis as well as at lamina lucida.
    Thus, the degeneration of keratinocytes, especially those in basal layer may participate in the formation of bulla in cases of bullous pemphigoid.

    We treated a 34-year-old man who had numerous pigmented macules disseminated symmetrically on the dorsa of feet, legs, forearms, chest and back. The macules were miliary sized, brownish, irregulary angulated, slightly depressed below the skin surface and tended to form pigmentary network along skin furrows. He had hemangioma simplex on the bilateral lower extremities and buttock, and varix on the legs and descending colon. We diagnosed this case of acropigmentatio reticularis (Kitamura) associated with hemangioma simplex and varix. The possible occurrence, as an expression of phacomatosis, of acropigmentatio reticularis, hemangioma simplex and varix in this case is discussed.

    A 60-year-old Japanese woman was admitted to our hospital, because of lichen planus on the skin, Raynaud’s phenomenon, swollen hands and general arthralgia. Laboratory findings revealed high values of RNP and antinuclear antibodies and normal values of anti-Sm antibody. Histopathological examination of the skin lesion showed liquefaction degeneration and colloid body formation in the epidermis, cleft formation at the D-E junction and band-like infiltrate of small mononuclear round cells in close approximation to the epidermis in the upper dermis. In the direct immunofluorescent study of the skin without lesion, IgG deposits were seen in a speckled pattern on nuclei of epidermal cells, while in the skin lesion, deposits of fibrinogen, IgG, IgA, IgM, C1q and C3 were seen in the colloid body. These findings led to a diagnosis of MCTD accompanied by lichen planus.

    A 31-year-old woman manifested clinical pictures of progressive systemic sclerosis (PSS) and there was substantial laboratory evidence of systemic lupus erythematosus (SLE). Raynaud’s phenomenon, scleroderma, arthralgia and gastro-esophageal hypomotility were present. Pulmonary function test showed a low diffusion capacity. The laboratory examination revealed positive results on anti-nuclear antibody test (homogenous pattern), LE cell preparation test and anti-DNA antibody test, low serum complement level, anemia, leukopenia and proteinuria. Direct immunofluorescence study of the biopsy specimen from the sclerotic skin showed a linear deposition of IgM and C1q at the dermo-epidermal junction. A review is made of the literature on the association of PSS with SLE.

    A 6-year-old girl and her two female relatives had systemic lupus erythematosus (SLE). One of paternal line, was diagnosed as a case of SLE at 13 years of age, and the other, maternal line, occurred in an 8-year-old girl. The patient had a butterfly rash of 4 months duration and later developed renal dysfunction. The immune complex was 32μg/ml, CH50 under 5 mg/dl, anti-nuclear antibody over ×50,000, anti-DNA antibody ×640, LE cell 11.4%, platelet 40,000/mm3, urine protein 600 mg/dl. Urinary sediments contained many casts and red blood cells. After the administration of prednisolone 40 mg/day p.o. for 6 weeks, the erythema disappeared, and the abnormal laboratory data reverted to within normal ranges, except for the anti-nuclear antibody (×1,000) and some casts and red blood cells in the urine sediment. HLA-BW51 antigen was present in all 3 patients.

    We treated an 83-year-old woman with herpetiform pemphigus. Erythema exudativum multiforme-like or dermatitis herpetiformis-like erythemas and bullae developed over her entire body. Histological examination revealed intraepidermal bullae with eosinophilic spongiosis and direct immunofluorescent study revealed deposits of IgG and IgA at intercellular spaces and the epidermo-dermal junction.

    In one family, pemphigus vulgaris occurred in a younger sister and five years later, pemphigus foliaceus occurred in an elder sister. The diagnosis was confirmed clinically and histologically in the younger sister, and additionally immunopathologically in the elder sister. HLA of the elder sister was A2, BW39, BW61 and DR8W6Y. Further studies on families with pemphigus may clarify the genetic aspects.

    In a 68-year-old woman, a tuberous xanthoma developed in the periorbital region. PAG electrophoresis revealed a type III hyperlipoproteinemia. This was a rare case of tuberous xanthoma, with respect to the anatomical location.

    Two cases of black dot ringworm were diagnosed in Yamaguchi Prefecture. One was caused by Trichophyton violaceum in a 17-year-old girl and the other by Trichophyton glabrum in a 57-year-old woman.

    A 67-year-old man, an ex-crew member of ships on foreign routes lived in Shimonoseki. He noted several dull reddish nodules on median surface of right thigh and posterior of left leg one year ago and swelling with pain on right leg 3 months ago. Histopathological examination of the nodules revealed proliferation of endothelium-like cells and fibroblast-like cells with extravasation of red cells and deposition of hemosiderin in the dermis. He had had an anemia of unknown cause nearly simultaneously with the onset of skin lesions and was treated with systemic corticosteroid therapy in a hospital. In our clinical laboratory, we noted a tendency toward pancytopenia in the peripheral blood and hypercellular bone marrow. Systemic corticoid therapy was effective for the swelling with pain in the leg and the thrombocytopenia and radiation therapy was somewhat effective for regression of the nodules. He died of intestinal bleeding due to thrombocytopenia and cachexia. Malignancy was absent in the visceral organs at autopsy. It was considered that the pancytopenia may have been related to lesions of Kaposi’s sarcoma.

    A 14 month-old girl was admitted with a dull reddish macule in the right mandibular region. The lesion was circular, 4×4cm in size and not elevated with teleangiectasia at the surface of macule. The consistency was very hard. The laboratory findings showed atypical lymphocytes in peripheral blood and bone marrow. These atypical lymphocytes were peroxidase negative and contained PAS positive materials. Histological examination of the biopsied specimen revealed atypical lymphocytes diffusely infiltrating into the dermis and subcutaneous fat tissue. A total remission occurred while on vincristine and prednisolone, but she died of meningeal leukemia at about 10 months after being discharged. Acute lymphocytic leukemia with a initial manifestation of skin lesion, and a review of the related Japanese literature are presented.
