    A case of echinococcosis with a skin manifestation in a 63-year-old man is presented. The patient noticed a small nodule in the right flank region in March 1982. Though treated by surgical excision and successive antitubercular chemotherapy, two subcutaneous nodules occurred in the same area in October 1983. He was admitted to Kyushu University Hospital in November 1983. At the time of surgical excision, we detected the parasite in subcutaneous cysts. X-P, CT and echogram revealed calcificated cystic lesions in the liver and kidney. Parasitological investigations revealed that Echinococcus granulosus was the cause of the multiple cystic lesions of the skin, liver and kidney.

    A 51-year-old woman had a bean-size nodule on the flexor surface of proximal left middle finger. The lesion, which had been presented for two months, was slightly painful to pressure. A local exision was performed. Histologically, there was a papillary and granulomatous proliferation of endothelial cells in the small artery. The proliferating cells were identified as endothelial cells in the electron microscopic study.

    A case of persistent genital herpes and herpetic whitlow associated with malignant thymoma was studied. The lesions lasted for more than 6 months in the presence of high titers of neutralizing antibody to herpes simplex virus (HSV). Suppressed states of cell-mediated immunity of the patient were evidenced by the measurement of lymphocyte proliferative responses to lectins. Two strains of HSV were isolated from each lesions and both were the HSV type 2. Endonuclease analysis of viral DNA revealed that these two isolates were closely related.

    A bean-sized cyst without any subjective symptoms occurred on the scapha of the left auricle of a 52-year-old man. The treatment was a partial excision of the upper wall of the cyst and subsequent pressing of the cyst. Histopathologically, the cyst was present in the auricular cartilage. As there was no epithelial tissue on the inner side of the wall, it was diagnosed as a pseudocyst of the auricle.

    A 53-year-old man with typical pemphigus vulgaris diagnosed clinically and histologically was treated with glucocorticoid and gold sodium thiomalate. After 40 days on continuous gold therapy, pityriasis rosea Gibert-like eruption appeared on the entire skin. The total amount treated was 285mg (190ug/ml in serum). Nine days after discontinuation of the gold therapy, he complained of dyspnea and dry cough. The chest X-ray examination revealed a pneumonitis. Large doses of glucocorticoid were given and the pneumonitis disappeared.

    A 27-year-old man with congenital teeth, complained of subcutaneous nodules on the scalp and trunk, pachyonychia congenita, folliculitis, follicular keratotic papules and hyperidrosis. Histologic findings taken from the subcutaneous nodules showed the steatocystoma in the dermis. Chemical analysis of content in the cysts disclosed three major components, triglyceride (33.9%), wax ester (24.2%) and squalene (15.4%), on the thin layer chromatography. These lipids apparently derived from sebaceous glands.

    A 33-year-old woman with a nodular fasciitis on the flexural aspect of right forearm was treated. The clinical features of 41 persons with nodular fasciitis reported in the literature in Japan were reviewed. The subcutaneous nodules were mainly present on the forearm or thigh, were from tip of the index finger to thumb size, elastic hard, and had a normal skin color. Some patients complained of tenderness of the nodule. There has been no recurrence following excision.

    An 11-year-old boy with an erythematous infiltrated hairloss plaque on the middle of the forehead was treated. Biopsy from the lesion revealed cystic change and a mucoid substance in the pilosebaceous apparatus. Seventy-nine cases reported in literature in Japan were reviewed and clinical features were discussed.

    A boy was seen when he was 4 months old, because of an angioma characteristic of this syndrome and which was present the right preauricular region. Laboratory data showing decreased platelet count and fibrin, increased fibrin-fibrinogen degradation product, and so on were also characteristic. Light microscopy of a biopsied specimen revealed hemangioma tissue mainly composed of immature endothelial lumina with interstitial bleeding. Electron microscopic findings were also characteristic. Oral corticosteroid therapy and electron beam therapy of 9 MeV energy level, divided into three courses, were effective. The local findings and the laboratory data up to the complete cure are described. A comparison with criterion for DIC, was discussed.

    In a 55-year-old man with multiple angiolipomas, 77 nodules appeared, this being the largest number of such nodules in cases reported in Japan. Fifty-eight cases of multiple angiolipomas and 17 of multiple lipomas have been reported. Comparative studies between both diseases, based on the above reported cases and regarding the sex, age, predilection site, pain and angiomatous proliferation etc., revealed no special difference. Thus, these two diseases may be one and the same.

    Comparison was made of results of immunofluorescent staining and virus isolation for detection of herpes simplex virus in 30 clinical specimens, focusing on their relationship to the stage of the disease. Combined results of both methods were positive in all 22 specimens from vesicle or pustule lesions, and in 2 out of 6 specimens from crust lesions. In one of the crust specimens, immunofluorescent staining of the smear was positive, but virus isolation was negative. Two specimens from erosion or ulcer lesions gave combined negative results. The duration of the eruption with positive both results was found to be within 4 eruptive days, negative both results was after 8 eruptive days.

    Birbeck granule (Bg)-like structure (BgS) was observed in interdigitating reticulum cell-like cells which increased in lymph nodes of the DNCB-sensitive mice, the following was evident.
    1. BgS was composed of two layers of limiting membrane and one layer of internal lamina, as in the case of Bg rods. The BgS were thinner than the Bg, and the internal lamina of BgS had no periodicity as observed in Bg.
    2. BgS was often associated with discoid body and rough-surfaced endoplasmic reticulum (r-ER)-like structure showing “reticular pattern”.
    3. BgS was continuous with the r-ER.
    Thus BgS obviously differs from Bg in structure. A remarkable difference between the two is the finding which BgS may be derived of r-ER.

    Immunohistological studies were done on squamous cell carcinoma, basal cell epithelioma, follicular tumor, sweat gland tumor, senile keratosis, Bowen’s disease, Paget’s disease and senile verruca, using antikeratin antibody prepared from nomal human sole skin by the method of PAP and immunofluorescence. Different staining patterns were noted between squamous cell carcinoma and follicular tumor, senile keratosis and Bowen’s disease. In Paget’s disease, a negative reaction for antikeratin ahtibody was noted, while the surrounding keratinocyte showed positive reaction.
