ジャーナル フリーAithought neurinomas of the spinal cord are frequently reported, multiple ones are seldom. As a spinal canal of the lumbar vertebrae is relatively wide, no neurological symptoms will often occur until tumors grow remarkably. Diagnosis will be delayed in many cases. We have recently experienced a case of multiple neurinoma originated from the cauda equina.
A 70-year-old woman had complained of numbness and pain of the left lower extremity for a long time. Her symptoms had slowly gotten worse. She visited our hospital in August 1986. Myelogram and CT-Myelo showed a giant intradural tumor which looked multi-locular.

ジャーナル フリーSpinal intramedullary angiomas are rare in spinal tumors, the clinical diagnosis can only rarely be made with certainty, but it should be suspected in the early stage on the basis of it's special pattern of clinical symptoms when the history was taken. And it is necessary to take the angiographic examination for the exact diagnosis.
An 18 years old female was addmited to our hospital because of paraplegia and decubitus of sacral region. She had a sudden onset of abdominal pain and bladder disturbance about 3 years ago, and had gait disturbance due to weakness of bilateral limbs 9 days later, 3 months later she recovered gradually and ambulated with a stick. Eight months later, she had the 2nd attack and since then she could never walk again. The angiography showed 2 midus of glomus type of angiomas in the 9th thoracic lesion. Laminectomy was performed from Th8 to Th10. With myelotomy the tumor mass was removed, and the pathological finding shows metaplastic bone formation in the tumor. After operation the symptom of lumbar lesion has improved, but there has been no change in the sacral lesion.抄録全体を表示PDF形式でダウンロード (2070K) -

ジャーナル フリーWe experienced one case of intramedullary ependymoma at the 7th cervical region. The patient was a 50 years old female. She had sensory disturbance on the left upper limb, trunk and both lower limbs. She could not walk without support. The tumor was located at the 7th cervical region and we removed it completely with microsurgical technique. ジャーナル フリーPrimary intramedullary spinal glioblastoma multiforme is very rare. Spinal dissemination from intracranial glioblastoma is also uncommon. We present a 13-year-old girl who had difficulty in elevating her right upper extremity and developed spinal symptoms. Metrizamide myelography, computed tomographic metrizamide myelography, and magnetic resonance imaging showed intramedullary mass expanding from medulla oblongata to C-7 vertebral body level. Laminectomy from C-2 to C-7 revealed intramedullary tumor infiltrating the right C-5 spinal nerves. ジャーナル フリーSymptomatic spinal arachnoid cysts are rare. A case of arachnoid cyst who had a severe back pain with minimal neurological involvement was reported. Supine myelography, CTM and MRI seemed to be essential to diagnosis of this cystic lesion. ジャーナル フリーThirty-two patients with spinal neurinomas, admitted to the Kyushu University Hospital during the past 20 years, were retrospectively analysed. The tumors were located in the cervical in 8, in the thoracic in 10, in the thoraco-lumbar in 5, and in the lumbosacral regions in 5 cases. There were four multi-located neurinomas. Age distribution was from 17 to 77 years with the average of 44.5 years. Pain was an initial symptom in 71% of the cases. The length of the case histories varied from 4 months to 5 years with an average of 1.7 years. ジャーナル フリーThe neurilemmoma, or schwannoma, is a solitary, the most common benign tumor arising from peripheral nerve. The tumor characteristically arises from only one location in a peripheral nerve, although several instances have been reported in which the tumors have occurred in multiple locations within the same nerve trunk.
A case of multiple neurilemmomas is reported. There were 2 tumors in the forearm and 1 in the upper arm. All three tumors involved the radial nerve. The tumors were well encapsulated and we could enucleate them easily from parent nerve surgically. Histologically, the neurilemmoma was comfirmed. After operation, the patient showed temporary mild paralysis of the radial nerve which recovered promptly. And now he remains free of additional tumors.

ジャーナル フリーMuscular metastasis of cancer is rare. We had experienced a case of muscular metastasis of gastric adenocarcinoma. The patient was a 68 years old man. The metastatic tumor was located in the left M. vastus lateralis. Its size was 6×5×3cm. The extirpation of the tumor was done in seven months after the total gastrectomy. ジャーナル フリーThree cases of carcinoma developing in the burn scans are reported. Two cases are male and another is female. The ages at onset of cancer ranged from 44-years-old to 73-years-old (average: 58-years-old), and the time interval between burn and onset of cancer ranged from 15 years to 41 years (average: 31 years). The causes of the burn were as follows: the first case by boiling water, the second by bonfire and the the third unknown. All cases had ulcer formation with a large quantity of discharge and bad smell, and pathological diagnosis was squamous cell carcinoma. In all cases, amputation above the site of the lesions followed by chemothrapy was performed and satisfactory results were obtained at the short-time follow-up.抄録全体を表示PDF形式でダウンロード (4750K) -

ジャーナル フリーInfiltrating lipoma is characteristic in its infiltrative growth pattern and its frequency of local recurrence in spite of the benignancy. It involves the intermuscular and intramuscular types.
We evaluated 10 cases with infiltrating lipoma. Five cases were intermuscular and the other 5 were intramuscular type. The age of patients ranged from 1 to 69 years old (mean age: 47.7 years old). The majority of the tumor was sited in the extremities, especially in the upper extremities. No recurrence was seen in all cases. The utility in imaging techniques such as CT or MRI was remarkable for the clarification of localization and even diagnosis of this tumor.
Wide resection must be recommended to prevent the local recurrence, especially in the intramuscular type.抄録全体を表示PDF形式でダウンロード (4357K) -

ジャーナル フリーThree cases of pseudomalignant osseous tumor of soft tissue are described. Histologically the lesion can not be differentiated from myositis ossificans, and sometimes might be misdiagnosed as osteosarcoma because of high cellularity, mitotic activities and osteoid formation. Clinical and laboratory findings are highly suggestive of inflammatory process. Two of the three cases were diagnosed primarily as pseudomalignant osseous tumor of soft tissue, however, the rest of a 5-year-old female was at first clinically as malignant soft tissue tumor, such as Ewing's sarcoma, rhabdomyosarcoma or synovial sarcoma on bases of severe inflammtory condition or immature calcification. The report of histopathologic examination of the biopsy specimen was osteosarcoma. The authors thought, however, that the diagnosis was still in doubt, because the morphological appearances of the cells showed slight nuclear atypia, and then the lesion was left undisturbed. Serial radiographs in following seventeen months showed regression of the lesion.
Careful histological study, supplemented by clinical and rediological data, is necessary in order to arrive at a correct diagnosis and avoid misinterpreting it as a malignant tumor.抄録全体を表示PDF形式でダウンロード (3015K) -

ジャーナル フリーA case of tuberculosis of the pubis, which is uncommon and probably caused by trauma, is reported. The patient was a fifty years old male who noticed pain over the Pubis after striking the groin region. Initially the diagnosis of fracture was made, however, the serial roentgenograms revealed the expansion of the destructive lesion. Eight months after onset, histological examination of the curetted tissue showed tuberculosis. The patient showed resistance to chemotherapy, and, because of his heart complication, we had to minimize the operative damage and could not perform sufficient curettage initially. In the result, the course was prolonged, however, the implant of Streptomycin beads with sufficient curettage was effective for suppression of the disease ultimately. After the inflammation was suppressed, strut bone graft between pubic bones was performed. The patient was asymptomatic ten months after the operation.抄録全体を表示PDF形式でダウンロード (3630K) -

ジャーナル フリーWe report two patients of septic arthritis of the shoulder joint with onset in infant. They have been followed-up for each six and four years. And we evaluated them by roentgenograms and the function of the shoulder joint.
In the present time, roentgenograms showed the deformity of the humeral head and malalignment of gleno-humeral joint in both cases. They showed only slight limitation of range of motion and muscle weakness, and had no disturbance of activity of daily living.

Tibialis anterior was noted in a 75 years old male, He had been bitten by a poisen snake “Mamushi” and bound his right lower leg with the rope for about 200 days when he was ten years old. After that, he had noticed a tumor in the anterior part of his right lower leg. The tumor was growing very slowly for 65 years. When the patient was admitted in July, 1986, the tumor was elastic hard measuring 9×3cm2 in size.
We resected the tumor and its pathological finding was the calcification of M. Tibialis anterior.

ジャーナル フリーWe introduced our treatments for old fractures of facial bones, especially nasal fractures. Late treatment is very difficult for manipulation, thus osteotomy and continuing reduction is the best way for treatment of nasal lateral deformities. When we approach to the osteotomized part, skin incision is usually over the perinasal cavity, but if more strict reduction is required, we choose skin incision on the front of face for the approach. For saddle nose and bone defect of unreduced fracture in the center of the face, we usually use iliac bone graft or silicon rubber for augmentation. Treatment for complex fracture of the face should be done as two stages.
