ジャーナル フリーLong-term hemodialysis related arthropathies in 10 shoulders of 8 patients were treated surgically. The pathological foci were almost all in the intertubercular sulcus where the long tendon brachii inserts and the rotator cuff attaches to the greater tuberosity of the humerus. Marked proliferation of bursae and multiple cystic bone lesions were found with amyloid deposition observed histologically in all cases.
Surgical treatment such as curettage and bone grafting were quite effective and the clinical symptoms decreased dramatically after surgery. The enthesopathy induced by amyloid deposition is considered to be the main etiological pathology of the hemodialysis related arthropathy in the shoulder.抄録全体を表示PDF形式でダウンロード (1482K) -

ジャーナル フリーWe carried out a pathological examination of the inferior surface of the acromion in cases with rotator cuff tears to clarify the pathogenesis of such tears. Fifteen male patients with an average age of 53.3 years were operated on and all but one had a history of trauma. The lateral portion of the acromion was resected to prevent impingement syndrome and was examined microscopically to determine the degree of degeneration of the inferior surface.
Degeneration of the outer part of the resected acromion was more severe than that of the inner part. The degree of degenertion was siginificantly related with an increase in age but had no relation with the period after trauma or with the size of cuff ruphad no relation with the period after trauma or with the size of cuff rupture.
These results suggested that degeneration of the inferior surface of the acromion among middle aged men may arise with aging, or the cuff dysfunction may pre-exsist before any symptoms become apparent.抄録全体を表示PDF形式でダウンロード (1429K) -

ジャーナル フリーWe operated on 215 cases with rotator cuff tears from July 1986 to 1993. Subjects consisted of 169 males and 46 females, with 163 right shoulder and 64 left shoulder disorders.
Ages ranged from 27 to 81 years with an average of 55.9 years. This paper reports the technique of repair of tears of the rotator cuff. The greatest challenge of repair of rotator cuff injuries is dealing effectively with the torn retracted cuff tendons.

ジャーナル フリーWe have treated a case of tuberculosis of the glenohumeral joint and subacromial bursa with many rice bodies.
A 78-year-old female visited our clinic complaining of continued pain, swelling and limitation of R. O. M in her right shoulder. On admission in March 1992, diagnosis was made by the characteristic X-ray findings of the humeral head, bacterial culture, blood examination and tuberculin test. At operation many rice bodies, necrotic tissue, and synovitis were recognized. Histological findings revealed caseation necrosis surrounded by Langhans giant cells.
Anti-tuberculotic medication was prescribed for 7 months after surgery. At follow-up, good active motion was preserved and there has been no local recurrence.抄録全体を表示PDF形式でダウンロード (3407K) -

ジャーナル フリーWe investigated the frequency of joint disorders among middle school baseball players, analyzing the number of baseball matches, they played and the type of class 1st or 2nd year in two defferent districts. We selected two distrcts that each had either fifty matches (two teams) or twenty-five matches (four teams). 59 players were examined in the group playing 50 matches, and 95 players in the teams playing 25 matches. We also analyzed the groups according to 1st or 2nd year classes and generally, it seems that 2nd year class members suffered from more disorders compared to the 1st year class members. This was especially true for 2nd year class members who belonged to the group which played 50 baseball matches, and the disorders were particularly prebalent in their shoulders. Disorders were also more common in pitchers and catchers playing 50 games, and in the fielders who played less matches.
In conclusion, there was little difference between the two districts in regard to training time, in the degree and site of disorders. However it was clear that there were more shoulder disorders in 2nd year class members who played more matches and also in shoulders and elbows of pitchers.抄録全体を表示PDF形式でダウンロード (305K) -

ジャーナル フリーWe investigated the effectiveness of wedge osteotomy of the radius for treatment of Kienboek's disease.
Radiological assessment showed that the lunate covering ratio (LCR) increased postoperatively according to good clinical results.
According to Tsumura's experimental spring model of the rigid body of the wrist, stress force on the lunate is reduced most by wedge osteotomy. We believe that LCR is one of the most important factors showing a decrease of the shearing force.抄録全体を表示PDF形式でダウンロード (1601K) -

ジャーナル フリーSixty-five hybrid total hip replacements (THRs) in 58 patients with osteoarthritis (OA) and rheumatoid arthritis (RA) were analysed with regard to chinical and radiographical findings more than one year after surgery. The mean follow-up period was 22.2 months. Results are as follows; (1) Thigh pain occurred in 5.5% of OA cases (55 cases) and 10% of RA cases (10 cases). (2) Radiolucent lines were found in zone 1 (21.5%). (3) Bone cement gaps were found in 14.5% of OA cases and 0% of RA cases. (4) No osteolysis was found in any cases.抄録全体を表示PDF形式でダウンロード (1276K) -

ジャーナル フリーTumors of the magnum are rare with many of these lesions having curious presentations due to the complex anatomy of this region there by making their diagnosis difficult.
During the last 12 years, we have encountered 6 cases of foramen magnum tumors, all of which were extramedullary, 4 neurilemomas, one meningioma and one neurofibroma.
All cases were treated with surgical excision. There were no deteriorations in these cases after surgery. From the foramen magnum to C2 level, the cervical canal is wide, so if a space occupying lesion is here, the symptoms are often very slight, although the tumor may be relatively big. It is important to consider this tumor and to make an early diagnosis, MRI is very useful for this, and take appropriate treatment measures before permanent damage results.抄録全体を表示PDF形式でダウンロード (2867K) -
