    The purpose of this study was to estimate a clinical significance of esophageal hiatal hernia (HH). Subjects were 2, 845 cases examined for three years in upper GI tract by barium enema. Perhaps 20% of adults aged over 60 have HH and the incidence increased with advancing age. There was preference to female. The ratio of paraesophageal to sliding HH was considerably less than 3 vs 7. In the cases with the longer distance escaped from the diaphragm, clinical symptoms were present in about 50%. By histological examination of diaphragm, the 13 autopsied cases of HH often accompanied with atrophy and hyalin degeneration of muscle fibers more than the 14 autopsied cases of non-HH did. Microscopically, reflux esophagitis was detected in about 50% of HH. We would like to summarize;1) HH was common disorder in the aged, 2) HH escaped long from diaphragm had some significant symptoms, 3) The relaxation of adjacent tissues had probably played an important part in the pathogenesis of HH.

    The serum β2-microglobulin was determined by a radioimmunoassay in 39 healthy subjects and 50 patients with stomach cancer admitted to our hospital for surgical resection. The average value of serum β2-microglobulin in 39 healthy subjects was 1.73±0.52 μg/ml. Even in early stage of stomach cancer the mean value was higher than that of healthy control. Except for early stomach cancer mean serum level of β2-microglobulin increased the progression of stomach cancer, though individual values distributed wide range. Serum levels seemed to be higher in a group of differentiated stomach cancers than that of undifferentiated stomach cancers, and also higher in male patients than in female ones. High postoperation value seemed to be poor prognostic marker especially in Stage IV. No obvious differences was found in the groups either with/without organ involvement or with/without shorter survival (within 12 months death).

    Clinical studies of the diagnosis of duodenal cancer are not reported so much although of course we know, its frequency aeems to be lower than gastric cancer or other gastric diseases.
    However progress of digestive endoscopy has made acurate diagnosis of this disease.
    21 cases of cancer of papilla vateri, 6 cases of primary duodenal cancer, and 13 cases of metastatic duodenal cancer have been investigated clinically.
    Though 80% of cases of cancer of papilla vateri were able to be diagnosed from the characteristic findings of cholangiography by PTC or ERCP that the end of common bile duct showed U shaped obstruction with round filling defect, the other cases were unable to be diagnosed correctly only by X-ray findings.
    On the other hand, all cases of cancer of papilla vateri were shown endoscopically some diagnostic features consisting of polypoid lesion, ulceration, or erosion at the papilla vateri.
    Thus, it may be concluded that degree of invasion of cancer of papilla vateri was presumed by endoscopic findings of papilla vateri.
    On primary duodenal cancer, polypoid lesion (such as IIa) was shown chiefly at the supra-papillary region, while ulcerative lesions (such as Borrman II or III type) were shown at the infra-papillary region. The former was found mostly in the earlier stage than the latter. It may be speculated that the polypoid lesion of primary duodenal cancer had developed to ulcerative lesions according to the growth of cancer.
    Metastatic duodenal cancer showed endoscopic findings of erosion (IIc like), polypoid lesion (ha like) or small ulcer. In addition, one case of cancer of papilla vateri and one very rare case of primary duodenal cancer in the early stage were reported in detail.

    While in a few cases of ulcerative colitis, titers of anti-enterobacterial common antigen antibody were high, there were high titers in many cases of liver diseases, such as liver cirrhosis and fluminant hepatitis.
    Both enterobacterial common antigen and bacterial lipopolysaccharide had activity to induce delayed hypersensitivity against protein antigen in guinea pig, respectively. The activity of lipopolysaccharide was influenced by either its own structure or age of animal, that is, while wild type of lipopolysaccharide showed its activity in relatively matured animal, inability of activation and induction of delayed hypersensitivity was shown in inmatured animal. Re type of lipopolysaccharide, e.g. lipid A+KDO, however, had its activity in younger animal.

    Thin-layer and gas-liquid chromatographic analysis of bile acid were carried out in gallbladder bile obtained from nineteen patients with resection or bypass of the terminal ileum and proximal colon.
    These patients had increased ratio of glycine- to taurine-conjugated bile acid. Free bile acid was detected in 79.0% of the patients. Deoxycholic acid was absent in 9 and decreased in 2 cases out of 12 examined. The percentage of chenodeoxycholic acid was increased.
    Fat malabsorption was found in 66.7% of the cases received fecal 131I-triolein excretion test. Changes of bile acid composition and intestinal flora and rapid intestinal transit time seemed to cause fat malabsorption.

    Changes in various organelle's enzymes of rat intestinal mucosa reared with alcohol diet containing ethanol 36% of total calories for 4 weeks were observed, as follows :
    1) Chronic ethanol administration caused inhibition of Na+-K+-ATPase and alkaline phosphatase activities.
    2) Activity of succinate cytochrome C reductase in mitochondrial fraction increased significantly while that of rotenone insensitive NADH-cytochrome C reductase changed little.
    3) Lysosomal enzymes such as beta-glucuronidase, acid phosphatase were not changed in activity by chronic ethanol treatment.
    4) In microsomal fraction, gamma-glutamyl transpeptidase activity was found to be markedly increased, but no change in activity of glucose-6-phosphatase.
    5) On the other hand, activity of glucose-6-phosphate dehydrogenase in supernatant fraction reduced.
    These findings indicate that ethanol has toxic effects on small intestinal mucosa as well as on liver.

    Within 8 to 15 weeks from onset of hepatic coma, liver biopsies were obtained in 6 patients surviving fulminant hepatitis. Two patients showed typical subacute hepatic necrosis, but the other patients had mild hepatic damage without remarkable necrosis which a considerable proportion of the hepatic parenchyma was morphologically preserved. Although the pathologic features of the disease have been recognized to be basically characterized by acute or subacute hepatic necrosis, there is fulminant hepatitis in part which the histological alterations do not necessary correlate well with the severity of clinical illness. This observation may suggest the results of functional failure of anatomically intact hepatic parenchyma as opposed to morphological destruction of the majority of hepatocytes. Moreover, it appears that patients surviving fulminant hepatitis may rarely develop chronic liver disease.

    The isozymes of serum leucine aminopeptidase (LAP) were separated by polyacrylamide disc gel electrophoresis in 115 subjects consisted of 15 healthy volunteers and 100 patients (acute hepatitis, 2; chronic hepatitis, 30; livercirrhosis, 10; hepatoma, 4; cholelithiasis, 7; choledochal cancer, 3; chronic pancreatitis, 2; pancreatic cancer, 2; peptic ulcer, 8; gastric cancer, 26; rectal cancer, 6.)
    In normal serum only one staining band was obserbed. On the other hand, LAP isozymes in pathological sera were separated into from one to several bands and staining bands in some cases were located at between spacer gel and separating gel.
    The definite abnormal isozymes of LAP were demonstrated in pathological sera. But the abnormality was neither specific in each disorders nor in malignant disorders.
    To determine the source of the LAP isozymes in serum, normal tissue extracts solubilized by bromelain were submitted to polyacrylamide disc gel electrophoresis. Two staining bands were separated in hepatic extracts, 3 bands in gastric and small intestinal extracts respectively, 4 bands in rectal extracts, and 5 bands in pulmonary extracts. Only one staining band coexisted in each tissue and this band had the same electrophoretic mobility as in the normal serum and was sensitive to neuraminidase.
    We could observe that a high molecular weight form of LAP in the sera of patients with obstructive jaundice was separated by gel filtration on Sephadex G-200. After bromelain treatment of this high molecular weight form, LAP activity moved to the y-globulin fraction in which the ordinary serum LAP was eluted. From these results, it is suggested that a high molecular weight form of LAP is present as binding form with small fragment of plasma membrane in the blood stream, and that is solubilized by bromelain treatment and consequently becomes to the same molecular size with the ordinary serum LAP. It is considered that the antigenity of LAP in normal serum is identical that of peak II of LAP in normal liver which is 0.46 of relative mobility.

    Activity of conjugating enzymes of bilirubin, bilirubin UDP-glucuronyl transferase (GT) and bilirubin UDP-xylosyl transferase (XT), was positive in liver and kidney of Wistar strain rat and heterozygous Gunn rat. Ratio of GT activity to XT was about 3:1. No GT and XT activity was measured in human kidney.

    Despite effective releasement of biliary obstruction, jaundice is often prolonged and the mechanism has not been clarified. In order to detect the factors that prolong jaundice, experimental obstructive jaundice was produced in mongrel dogs and liver cell functions before and after releasing the obstruction were studied, by ICG plasma disappearance and transfer rate, Vit. K. loading hepaplastin test, serum Monoamine Oxidase value, general liver function test, and histological examination. The obtained results were as follows :
    1. With biliary obstruction, the serum bilirubin level rapidly elevates and reaches to plateau in about three weeks. Biliary infection added accelerates the elevation and the same tendency is observed in other biochemical examinations.
    2. The intra-cellular functions of the liver, which was detected by D/T ratio of serum bilirubin, ICG plasma disappearance and transfer rate, and Vit. K loading hepaplastin test, proved to be slightly suppressed and showed immediate restoration by releasing the obstruction. On the other hand, the function of hepatic cell membrane was found to be suppressed more strongly and required a longer term to be restored by releasing the obstruction.
    3. Decreasement of the b-value in ICG plasma transfer rate and detorioration of the activity in Vit. K loading hepaplastin test are presumed to be the indicators to show poor prognosis of obstructive jaundice.
    4. With the serum Monoamine Oxidase value, hepatic fibrosis is revealed to progress gradually during the obstructed period and keep progressing for one week after releasing the obstruction.

    Level of serum Ribonuclease activity, which was measured by using poly cytidilic acid (poly (C)) as a substrate, were determined in patients with pancreatic cancer in comparison with healthy persons and patients with various neoplasms and benign diseases. The serum RNase levels in the patients with pancreatic cancer were significantly higher than others. The method for RNase assay is very convienient and requires only a small amount of blood, therefore, is useful for a diagnosis and screening of pancreatic cancer.
    In order to inquire the origin of the elevated serum RNase levels in the patients with pancreatic cancer, RNase contents in tumor tissues were measured and were found 6 times or more higher than those of normal counterpart pancreatic tissues and gastric cancer tissue. Furthermore, major RNase in the serum from patients with pancreatic cancer was confirmed identical to those in pancreatic cancer tissue, normal pancreatic tissue and sera from normal person by column chromatography technique and immunological cross reaction. The results strongly suggest that RNase overproduced in pancreatic cancer tissue was secreted into blood.

    Ascariasis tends to be treated lightly in clinical practice, because it is assumed to be rare in Japan. However recent progress in techniques of gastrointestinal examinations made us possible to detect some cases. The worm shadow could be revealed by X-ray examination, despite negative in the stool. Clinical study mainly on X-ray findings of 9 ascariasis was made and is reported here. The worms were found in the biliary tract in 4 cases (biliary cases), and in the intestinal tract in 5 cases (intestinal cases). The youngest patient with ascaris was 30 years old and the oldest was 74 years old. They visited the hospital with the complaints of abdominal pain, nausea and others. Diagnosis was very difficult because of no past or present evidence of worms or eggs in the vomit or stool, except one case. Biliary cases were diagnosed by ERCP or PTC, and one of them showed mild icterus, which is the characteristic of this disease. Intestinal cases were diagnosed by barium X-ray examination. Ascariasis seems to be increasing with not only the development of the examination techniques but also with the change of the social situation, in which so-called "natural foods" became more popular. Therefore, we must differentiate carefully ascariasis from other diseases in the cases with the complaints concerning the digestive system.
