    A new technique employed 99mTc-MAA for the study of esophageal dysfunction and its clinical implication were evaluated in the patients with achalasia, progressive systemic sclerosis, reflux esophagitis and 10 normal controls. To investigate esophageal emptying and gastroesophageal reflux, a homogeneous bolus of 99mTc-MAA in 15m1 of water was swallowed in the upright and supine positions under the collimeter of a gamma camera linked to nuclear medicine data analyser (Shimazu Scinti Pack 1200).
    This radionuclide transit studies made a quantitative evaluation of the esophageal dysfunction possible in all cases. Comparing the conventional esophageal function test procedures, this test is a safe, noninvasive and more physiological and sensitive in detecting abnormal esophagealemptying and gastroesophageal reflux.

    To investigate the mechanism of acute stress ulcer, mucosal blood flow, oxygen tension and energy metabolism has been studied in the rat stomach during stress loading of restraint and water immersion.
    The gastric corporal mucosal blood flow remarkably decreased soon after water immersion, yet antral one did not show a corresponding decrease. Also, gastric mucosal oxygen tension in the corpus moderately decreased by stress, yet antral one did not changed. The energy metabolism in corporal mucosa tended to anerobic glycolysis with hypoxia caused by the decline of blood flow, and the levels of high energy phosphates consequently decreased from beginning to 2 hours after stress.
    In consideration of the decrease of energy production and the rise of its demand by the accelaration of acid output during stress loading, the existence of gastric mucosal energy deficit may be clear. From the dada presented herein, we considered that the flagility of defence factors such as the decrease of mucosal blood flow, hypoxia and energy deficit was strongly concerned in the etiology of stress ulceration which thus occues.

    A prospective clinical study was performed to evaluate the preventive effect of cimetidine on acute gastroduodenal lesions and G-I bleeding following head and thermal injury.
    Acute gastroduodenal lesions developed in twenty three (79.3%) of 29 patients in the control group but only eight (25.8) of 31 patients in cimetidine-treated group (p<0.001). Three of 31 patients who recieved cimetidine prophylactically, had G-I bleeding. On the contrary, fifteen of 29 patients in control group bled (p<0.005).
    Cimetidine decreased the secretory volume of gastric juice and gastric acidity. A significant reduction of acid output was seen from 1 to 4 hours after intravenous administration. In 6 out 8 patients, cimetidine elevated gastric pH level above 4. In addition, cimetidine prevented reduction in gastric mucosal blood flow.
    In conclusion, these results indicate that cimetidine has a protective effect against the development of acute gastroduodenal lesions following head and thermal injury.

  • Reference to the Relationship between Radiological and Pathological Features

    Comparative radiological and pathological studies were performed using 35 resected cases of carcinoma of the duodenal papilla. These cases were radiologically classified into types I, II and III by the duodenal features and types A, B and C by the features of the bile ducts.
    Cancerous masses were localized in the duodenal papilla in 11 of 16 (68.8%) with type I features and in all cases type A. These classification tended to correlate to the degree of infiltration of cancer cells. By the careful radiologic examinations on the duodenum together with those on the bile duct, the cancer infiltration could be detected more easily.
    Fore the early diagnosis of carcinoma of the duodenal papilla, it is important to to detect type I-A. Therefore, radiologists should make efforts to demonstrate fine detailed structures at the orifice of the duodenal papilla and terminal portion of the bile ducts.

    Patients diagnosed as hepatocellular carcinoma in the First Department of Internal Medicine, Okayama University Medical School between 1965 and 1981 were divided into Group I (all negative in H BsAg, anti-H Bs and anti-H Bc, 15 cases), Group II(anti-H Bs and/or anti-H Bc positive, 35 cases) and Group III (HBsAg positive, 55 cases). The mean age of Group I was 60 years compared with 51 years for Group III. The male to female ratio of 4 in Group I was different from 6.9 of Group III. There were no significant difference in the past history of alcohol intake and blood transfusion among each group. Familial clustering of liver diseases in Group I was 13.3%, although Group III showed high figure (44.4%). Of 15 patients in Group I, two had history of acute hepatitis about 25 years ago, when epidemic hepatitis was prevailed often in Okayama Prefecture. In the study of geographical distribution the rate of Group I was slightly high in the east area of Okayama Prefecture, whereas more than 50% of patients were belonged to Group III in Bingo District of Hiroshima Prefecture and in Bityu District of Okayama Prefecture.

    To investigate a function of peripheral blood monocytes from patients with various liver diseases, we separated monocytes from peripheral blood of patients and estimated their responsiveness to lipopolysaccharide (LPS) stimulation by measuring 3H-glucosamine uptake into the cells.
    The resultes obtained were summarized as follows:
    1) Peripheral blood monocytes from patients with acute hepatitis were stimulated markedly by LPS as compared with normal controls and their responsive capacities were changed in accordance with clinical course.
    2) The responsiveness of monocytes from patients with chronic active hepatitis to LPS was varied case by case ranging from a considerably high to relatively low responses.
    3) Monocytes from patients with liver cirrhosis or hepatoma showed a significantly low response to LPS comparing with normal individuals and patients with acute or chronic hepatitis. These results might suggest that the function of monocytes or macrophages is related to the pathological condition of liver diseas.

  • (3) A Transient Increase in Serum GPT Activity During the First Two Weeks After Caloric Restriction

    The effects on serum GPT activity of treatment of obesity by reducing diets were studied in 9 male obese subjects showing more than 15% over ideal body weight (bw).
    1) The mode of changes in serum GPT activity after caloric restriction was divided into two groups. The one showed a transient increase in serum GPT activity during the first two weeks after caloric restriction (a transiently increased group), the other a gradual decrease in serum GPT activity (a gradually decreased group).
    2) The energy intake was significantly less in the transiently increased group (10.2±1.4kcal/day/percent ideal bw*) than in the gradually decreased group (13.8±1.8kcal/day/percent ideal bw).
    3) The mean weight loss during 4 weeks after caloric restriction was significantly more in the transiently increased group (0.38±0.04%/day as percent ideal bw, 210±40g/day as actual bw) than in the gradually decreased group (0.16±0.05% /day, 100±27g/day, respectively).
    This shows that obese patients develop a transient elevation of serum GPT activity during the first two weeks of rigid dieting.
    *Percent ideal bw was calculated according to the followings; a patient's bw(kg)/ideal bw(kg)×100.

  • Development of the Radioimmunoassay of Lactoferrin and Its Cliniral Application

    A specific radioimmunoassay (RIA) was developed for measuring lactoferrin (LF) in aspirate by using the pancreozymin-secretin (PS) test. The method utilized the double antibody RIA technique using radioiodinated LF purified from human colostrum, rabbit antibody to LF, and sheep antibody to rabbit IgG. The minimal sensitivity of this RIA method was 2ng•er milliliter. The mean concentration of LE was significantly higher in chronic pancreatitis than in the control, about thirty five times higher at the pancreozymin phase, thirty two times higher at the secretin phase in chronic calcifying pancreatitis, thirteen times higher at the pancreozymin phase, and five times higher at the secretin phase in non-calcifying pancretitis.
    When abnormal level of LF was shown as 476ng/ml=(Mean+2SD)×2 in the control group, abnormality was observed in 6 of 10 patients with calcifying pancreatitis (60%) and in 4 of 11 patients with non-calcifying pancreatitis (36%), but not in 8 patients with pancreatic cancer.
    The measurement of LF in aspirate by PS test using the RIA method can be an effective diagnostic method in chronic pancreatitis.
