  • In Order to Analyze the Growth Pattern of Gastric Cancer Accompanying Peptic Ulcer

    A histological study on natural history of fibrosis found in peptic ulcer of the stomach was made upon initial type ulcers (34 round ulcers) and recurrent type ulcers (51 ulcers recurring adjacent to the ancient ulcer scar and 6 linear ulcer scars), as a preliminary work for analyzing growth pattern of the gastric cancer with peptic ulcer in itself. To evaluate the expanse of fibrosis, ratio of F-sm (area of submucosal fibrosis) and F-ss (area of subserosal fibrosis), i.e., F-sm/F-ss was used as a parameter. Average value of F-sm/F-ss of recurrent ulcers was significantly smaller than that of round ulcers (p<0.001). This is because preexistent fusion of two muscle layers in ancient ulcer scar has a blocking effect on the expansion of recurrent submucosal fibrosis. Average value of F-sm/F-ss of ancient linear ulcer scar did not show significant difference from that of ancient round ulcer scars. So it suggests that recurrent fibrosis is also absorbable and gradually decreases in amount as time passes as well as that of initial round ulcer. We tried to identify the site of recurrent ulcer histologically, by using F-classification which is the morphological and chronological classification of fibrosis previously reported, and F-classification was available only for the identification of a recurrent ulcer which had occurred within a year after onset.

    Various types of vagotomy were undergone in 22 cases with peptic ulcer. Gastric secretion test and 75g oral glucose tolerance test were performed before and after vagotomy in each case. The results were obtained as follows.
    1) Before vagotomy, positive correlation between integrated incremental response of GIP and maximum acid output was shown significantly.
    2) In selective vagotomy with pyloroplasty (SV+P) group, GIP levels after vagotomy tended to be higher than preoperative levels.
    3) In selective vagotomy with anterectomy (SV+A) group, a discrepancy between GIP and insulin (IRI) secretion was found after vagotomy, which suggested the existence of the other insulinotropic hormones.
    4) GIP secretion of SV+P group tended to be higher than SV+A group, indicating the GIP secretion might be affected by the drainage procedures.
    5) GIP secretion of SV+P group tended to be higher than TV+P group, which suggested that vagal nerve, especially celiac branch played a role in GIP secretion.

    The effects of dietary apple fiber on serum lipids and bile acid levels, fecal bile acid and fatty acid excretion, and fecal mass were investigated in 7 patients with diabetes mellitus.
    The apple fiber 5-15g once or three times daily was given to the patients for 6 months and the effects were compared with those before the administration of the fiber.
    After 6 months administration of the fiber, an average serum cholesterol level was reduced to 8.5%, but serum triglyceride and fecal mass were increased to 37.3% and 20.8% respectively.
    Daily fecal bile acid excretion was also increased from 221.3 to 318.5mg (43.9%, p<0.05). Among the bile acid components, proportion of chenodeoxycholic acid was remarkably increased (8-17%, p<0.01).
    An average stool frequency was changed from 0.85 to 1.33 per day, but neither fat malabsorption nor changes of serum bile acid concentration were seen.
    We concluded that the apple fiber may have a weak effect similar to that of cholestyramine or lactulose.

    Radioimmunoassay was used to study the unique pattern of distribution in a series of gut hormones in mucosal tissues of the canine gastrointestinal tract and pancreas. Gastrin was localized predominantly in the antrum, whereas high contents of secretin and motilin were detected in the proximal small intestin with a gradual decreas in the lower intestin. Glucagon immunoreactivity (GL) was localized predominantly in the gastric fundus and body, whereas glucagon like immunoreactivity (GLI) was evident mostly in the ileum. Pancreatic polypeptide (PP) was almostly confimed to the pancreas, but a small amount of PP was detected in the stomach. Vasoactive intestinal polypeptide (VIP) and substance P were widely distributed and with maximal contents in the colon. On Gel chromatography of the tissue extracts, motilin, substance P and PP showed a single peak corresponding to each peptide hormones, while secretin, GI, GLI, and VIP demonstrated multiformal elution pattern demonstrating their size heterogeneity in the mucosal tissue.

    To clarify the physiological feature of the ileo-cecal region, motility of the ileo-cecal region in conscious dogs was examined by using a strain gauge transducer. The enteric autonomic nervous system in the ileo-cecal region was also examined to clarify the relationship between this system and motility.
    When the propagating mechanism of contractile wave from the small intestine to the large intestine was examined, it was found that the propagation of the contractile wave was inhibited on the border of the ileo-colic sphincter.
    On the other hand, the cholinergic fibers distributed in the ileo-cecal region were distributed just like in the adjacent region, the abundant reticulated adrenergic fibers were observed in the peripheral region of the fasciculous of fibers in the smooth muscle of the ileo-colic sphincter.
    It was suggested that this specific autonomic nervous system was closely related to inhibiting mechanism of propagation of contractile wave in the ileo-colic sphincter.

    The changes in activity of intestinal alkaline phosphatase, one of glycoenzymes located in the brush border of intestine, was studied in cholera toxin-induced secretory diarrhea.
    After the administration of cholera toxin into the duodenum of rats, an increase in alkaline phosphatase activity was observed in both intestinal lumen and intestinal lymph. However, the fact that the activity of alkaline phosphatase in the brush border of villi was not significantly altered after the administration of cholera toxin suggested that the production of alkaline phosphatase increased to maintain mucosal alkaline phosphatase level. The administration of chlorpromazine suppressed secretory diarrhea induced by cholera toxin. Somatostatin treatment also inhibited cholera toxin-induced diarrhea. And both drugs prevented the increase in alkaline phosphatase activity produced by cholera toxin in intestinal lumen and intestinal lymph.
    These results suggest that the changes in alkaline phosphatase activity after cholera toxin administration are closely related to the secretory processes of rat small intestine. On the other hand, L-phenylalanine, a stereospecific inhibitor of intestinal alkaline phosphatase, also suppressed cholera toxin-induced secretory diarrhea in rat small intestine. These observations support the idea that intestinal alkaline phosphatase may play an important role in cholera toxin-induced intestinal secretion.

    When peritoneal exudate macrophages were activated with the lipopolysaccharide prepared from a gram-negative bacteria and cocultured with the isolated liver cells in vitro, macrophage-mediated hepatocytotoxicity was induced. Although this hepatocytotoxicity was not inducible by pretreatment of macrophages with lower doses of lipopolysaccharide than 1μg/ml, a significant hepatocytotoxicity was demonstrated by culturing the isolated liver cells with macrophages pretreated with low doses LPS and estrogen. This increases hepatocytotoxicity by estrogen was shown to be diminished by sinultaneous addition of testosterone with estrogen. The pretreatment of macrophages with estrogen alone did not induced the macrophage-mediated hepatocytotoxicity.
    These results suggested that sex hormones might exert their regulatory effects on the induction of macrophage-mediated hepatocytotoxicity.

    Thirty eight patients with hepatocellular carcinoma complicating liver cirrhosis were subjected to the transcatheter arterial embolization (TAE) therapy. Gelfoam sponge was used as embolic materials. These patients were separated into two groups, i.e. hepatic failure group, who died within one month after TAE, and long survival group, who survived for more than six months after TAE. Twelve liver function tests and six blood laboratory examinations were selected and compared each other between these two groups firstly, and then the indication of TAE was evaluated by means of regression analysis. As a results, no significant differences were seen in six blood laboratory findings, but the serum bilirubin, ICG, albumin levels and the pattern of OGTT showed significant differences in twelve liver function data. It was clarified that the indication of TAE in hepatocellular carcinoma with liver cirrhosis is mainly affected by the serum bilirubin, ICG, gamma globulin levels and the pattern of OGTT.

    A single usual dose of 200mg of cimetidine was infused intravenously to nine patients with chronic liver disease. The effect of cimetidine on the systemic hemodynamics, i.e., arterial pressure, heart rate and cardiac output and hepatic hemodynamics, i.e., hepatic blood flow, portal venous flow and portal venous pressure, were evaluated. The systemic hemodynamics were not changed significantly by cimetidine. Hepatic blood flow estimated by the extraction of indocyanine green from the liver, and portal venous flow assessed by the ultrasonic pulsed doppler method were not changed significantly. Portal venous pressure measured directly and/or estimated by the gradient between wedged and free hepatic venous pressure was unchanged. Thus, it seems that cimetidine can be used in patients with chronic liver disease without worrying about a significant decrease of hepatic and portal venous flow. It follows that cimetidine will have no prophylactic effect on variceal bleeding.

    The liver resistivity was determined in 23 rats with chronic liver damage induced by CCl4 and 61 patients with chronic liver disease.
    In rats, the values were 1.95±0.23 (×103ohm cm) in Fibrosis group, 1.74±0.17 in LC(I) group, 1.39 ±0.15 in LC(II) group and 2.24±0.22 in Control group, with significant difference (p<0.01) between the Control and every CCl4-treated group.
    In patients, they were 1.88±0.24 in CIH group, 1.66±0.23 in CAH group, 1.27±0.19 in LC group and 2.24±0.30 in Control group, with significant difference (p<0.01) or significant tendency of difference (p<0.05) between the Control and each group. A significant correlation was found between the liver resistivity and the blood flow index of the liver, Ch-E, γ-gl and ICG(R15) in liver function tests or prothrombin time in patients.
    The decrease in liver resistivity detected in rats with chronic liver damage or patients with chronic liver disease seemed to be connected with degeneration of liver tissue or advanced liver fibrosis.

    The ultrasonograms of the space occupying lesion of the liver were examined clinically and analyzed in 273 cases.
    A comparative study between the ultrasonograms in water bath and the histopathological findings of 34 specimens taken from patient recognized as having the space occupying lesions of the liver was undertaken.
    The ultrasonograms in 273 cases were classified into the 6 types (Type I-VI), which were furthermore divided to the 12 types including subtypes.
    The more coagulation necrosis was seen, the higher echo level was displayed, and the colliquative necrosis was anechoic. The massive fatty metamorphosis showed the same echo level as the liver, while the massive fibrosis gave low echo level. The peripheral sonolucent layer corresponded to the capsule-like fibrous tissue (2mm or more) without secondary degeneration, or to the combination of the tumor cell without necrosis and the capsule-like fibrous tissue in the case of the hepatocellular carcinoma.
    The layer corresponded to the tumor cell without necrosis, which is sometimes accompanied with fibrosis in the case of the metastatic tumor.
    The carvernous space showed the higher echo level than the liver and the hyaline degeneration showed the lower echo level in the cases of the high echo-level type of the carvernous hemangioma.

    Percutaneous aspiration biopsy of the liver using a heparinized fine 22-gauge needle was performed under ultrasonic guidance in 14 patients with benign hepatic disease and 26 patients with hepatic malignancy. When a heparinized needle and syringe were used, it was possible to centrifuge the aspirated materials obtained and make good smears containing abundant tumor cells. Heparinization also made it possible to recover small tissue fragments from the needle and make ultrathin sections for histological examination by a routine procedure for electron microscopy.
    In this way, materials suitable for cytological examination could be obtained from all patients with hepatic malignancy, and a correct diagnosis was made cytologically 24 (92.3%) of 26 patients with malignant hepatic neoplasm. Histological specimens were obtained from 22 (84.6%) of 26 patient with hepatic malignancy. A correct diagnosis was made in 20 (90.0%) of 22 patients with hepatic neoplasm from which histological materials were obtained. From a combination of histological and cytological results, the overall diagnostic rate for hepatic malignancy increased to 96.2%. These was no relation between the location and size or echo pattern of the tumors and the overall accuracy. By this method, a correct diagnosis could be made in a patient with a small hepatocellular carcinoma of 0.8cm in longest diameter. Therefore, this procedure is a reliable method for diagnosis of hepatic malignancy.

    To determine how to treat silent gallstones in the aged, 196 autopsied cases (mean age, 79 years) with gallstones were studied retrospectively for the frequency of silent gallstone and the complication of gallbladder carcinoma. In 60 patients (mean age, 76 years) with silent gallstones, the natural course, change in the gallstones and visualization of gallbladder on cholecystogram were followed for over 3 years.
    Some 109 cases (56%) of the 196 autopsied cases were silent gallstones. The frequency of gallbladder carcinoma with silent gallstone was 0.9%, which was lower than with symptomatic gallstone (10.3%).
    In the follow-up 60 patients, gallstones disappeared in 12 cases (20%) and grew in 5 cases (8%). In approximately 30% of the patients, the visualization of the gallbladder on cholecystogram became to worse compared with that at the initial evaluation. Complications including mild abdominal pain and liver dysfunction deveoped in only 5 patients (8%), and none showed carcinoma of the gallbladder.
    These results indicate that elderly patient with silent gallstones do not generally develop complications from the gallstones. Thus, silent gallstones in the aged can be followed by ultrasonography and cholecystography at regular intervals.

    The biochemical properties of the macroamylases found in three patients were exermined. The class of heavy chain of the amylase-linked immunoglobulin was proved to be alpha in all the cases, and the type of light chain was determined to be lambda in two cases, kappa in the other. The amylase-immunoglobulin complexes were completely dissociated at pH 5.0 or by Con-A affinity chromatography. The zymograms of dissociated amylase were normal. When free immunoglobulins released by acidification were mixed with purified salivary or pancreatic amylase, the immunoglobulins recombined with both amylases in two cases, but only with pancreatic amylase in one case. In maltopentaose hydrolysis, the Km values of these macroamylases were slightly smaller than those of normal serum, salivary, and pancreatic amylase.
