    Disease activity of ulcerative colitis is quantitatively assessed by utilizing multiple regression analysis for purposes of institution of an efficient method of treatment, and an evaluation of therapeutic efficacy. Thirty-two cases with ulcerative colitis encountered during the past five years were collected for this analysis. These cases were divided into three groups; mild, moderate and severe by Truelove and Witts's classification of disease severity, which is interpreted as a dependent variable in this study. Eleven items were selected among physical findings and laboratory data, and these termed as the independent variables. Applying multiple regression analysis with stepwise deletion, factors indispensable for disease activity are selected; i.e. temperature, pulse rate, erythrocyte sedimentatin rate, red blood cell counts, α2-globulin and bowel movements per day. The disease activity is estimated by using a formula of Y=-12.168+0.333X1+0.022X2+0.018X3-0.337X4+0.056X5+0.033X6. By applying this equation to each case, the range of activity indices is to be expressed; mild, less than 1.52, moderate, between 1.52 and 2.44, and severe, more than 2.44.

    In order to elucidate risk factors of colon adenoma, a case-control study by a questionnaire was conducted on 86 pairs of colon adenoma andnon-cancer hospital control.
    There was no definite relation between the blood types and colon adenoma.
    Cigarettes smoking was an increased risk factor for colon adenoma. It is suggested that previous illness of hypertension and colo-rectal cancer in family history were also increased risk factors for colon adenoma.
    Ham and sausage, cooking with oil and black tea were protective factors for colon adenoma. It is suggested that meat and green tea were also protective factors for colon adenoma.

    Proliferation and viability of the cultured human hepatobiliary carcinoma cells under hyperthermic culture conditions were examined using a newly established temperature gradient incubator. JHH-2 hepatoma cells and OZ bile duct carcinoma cells could not proliferate at 40°C and could not survive at 41°C. NOZAWA gallbladder carcinoma cells could proliferate at 41°C but could not survive at 42°C. JHH-2 and OZ were more susceptible to hyperthermia than normal diploid fibroblasts.
    On the observation under phase contrast microscope, the cells became larger and less dense at 41-42°C for 1-2 days. The cell shape were destroyed at 43°C for 1-2 days.

    Aminopyrine breath test (ABT) as en index of hepatic microsomal function was performed on the rats with D-galactosamine (D-Gal)-injured liver before andafter 68% partial hepatectomy. D-Gal 1600mg/kg induced the maximum decrease of ABT score on 2 days after the administration, which returned to normal very incidiously. Following partial hepatectomy ABT scores decreased to the level correlated closely with hepatectomized weight. Howerver in the D-Gal pretreated rats, ABT scores decreased more remarkably than those in normal rats after partial hepatectomy and the decreased ABT scores recovered later. ABT scores measured just before partial hepatectomy reflected a surgical mortality with a significant difference, survivors 53.4±23.0% vs deaths 18.4±12.6% (p<0.01). Hepatic microsomal function measured by ABT reflected the hepatic functional mass in D-Gal injured rats followed by partial hepatectomy and predicted the surgical mortality with a significant difference.

    The new bilirubin fraction -"bilirubin covalently bound to albumin (B-A complex)"-has been considered to exist in the icteric sera. In this study, the B-A complex could be demonstrated in sera of rats with obstructive jaundice, using gel filtration method in 8M Urea. The B-A complex was prepared in vitro, according to Kuenzle's method and partially characterized.
    The equilibrium dialysis and gel filtration studies showed that the B-A complex did not bind specifically to albumin and other organic anion (bilirubin, BSP).
    In addition, this complex had little effect on bilirubin and BSP transport into the isolated hepatocytes.

    We have assessed the diagnostic value of 99mTc-N-pyridoxyl-5-methyltryptophan (99mTc-PMT) hepatobiliary scintigraphy in 28 patients with hepatocellular carcinoma (HCC) who had obvious filling defects on 99mTc-phytate liver scintigram. Uptake of 99mTc-PMT in obvious defects on 99mTc-phytate liver scintigram was observed in 16 out of 28 patients (57.1%). Most of HCC with 99mTc-PMT uptake were histologically well-differenciated. However, well-differenciated HCC did not always take up 99mTc-PMT. No correlation was observed between 99mTc-PMT uptake in the tumor and levels of serum alpha-fetoprotein. Patients with high alpha-fetoprotein level (>3000ng/ml) were only 9 patients (32.1%) in our study. By the combined results of 99mTc-PMT hepatobiliary scintigraphy and alpha-fetoprotein level (>3000ng/ml), diagnostic rate for HCC was markedly elevated up to 75%.
    Therefore, it is considered that 99mTc-PMT hepatobiliary scintigraphy is clinically useful for specific diagnosis of HCC. Furthermore, it provides the information for invasion of HCC into the bile duct.

    An assay for DNA polymerase reaction products using slab gel electrophoresis and autoradiography was compared with the spot hybridization technique for the detection of hepatitis B virus (HBV) DNA in 317 blood samples. The former could identify the nature and size of DNA on electrophoresis, and reduced false positive results due to artifacts.
    Discordant results between the two methods occurred in 36 out of 317 samples; 22 were positive by the spot technique alone, and 14 were positive by the analysis of DNA polymerase reaction products alone. However, the samples positive with the spot test alone showed weak radioactive signals on electrophoresis/autoradiography that were often interpreted as "inconclusive" by blind observations.
    Correlation of hepatitis B e antigen/antibody with HBV DNA was studied in 91 patients with various chronic liver diseases. Discordant results, i.e., presence of the DNA in antibody positive sera, or its absence in the antigen positive sera, were obtained in 15 (19%) cases. Such patients tended to have advanced liver disease with fluctuating serum aminotransferase levels.
    Analysis of DNA polymerase reation products by slab gel electrophoresis and autoradiography is not only sensitive, but is also as specific as the Southern blot technique in the detection of HBV DNA in serum, and may prove useful and practical for clinical purposes.

    Clinical usefulness of serum alcohol dehydrogenase (ADH) activity was studied by experimental animals, comparing to the activities of glutamate pyruvate transaminase (GPT) and glutamate dehydrogenase (GLDH) in serum. ADH was shown to be distributed predominantly in zone 3 of hepatic acinus in rat by immunohistochemical method. Serum ADH activity was markedly increased when bromobenzene was administered, reflecting zone 3 hepatic necrosis. However, ally lalcohol administration resulted in slight increase of serum ADH activity compared with elevation of other enzymes.
    Furthermore, serum ADH activity increased preceding GLDH in early phase after administration of bromobenzene. The results of experimental model with acute alcohol administration or hypoxia induced liver damage also suggested that ADH is a sensitive indicator of hepatic zone 3 damage.

    We studied the clinical effect of a long-term administration of propranolol on the changes of the portal blood flow volume, portal venous pressure and endoscopic findings of esophageal varices in 15 patients with established esophageal varices.
    After a long-term administration of propranolol, the portal blood flow volume and protal venous pressure decreased significantly and the endoscopic improvement of esophageal varices were observed. Besides, no side effects or definite exacerbation of liver function tests were noted. So the results obtained in the present study indicated that a long-term administration of propranololwould be expected to be useful for nonsurgical therapy of esophageal varices for the prevention of rupture.

    The efficacy of adoptively transferred "immune" lymphocytes prepared from a HLA matched patient who had recovered from acute hepatitis B to a patient with chronic active hepatitis type B was reported by Kohler et al. But it is virtually quite difficult to obtain such "immune" lymphocytes. In order to inquire if it is possible to obtain sufficient number of "immune" lymphocytes, we studied the cytotoxic T lymphocyte (CTL) response to hepatitis B vaccination in HBs antibody positive volunteers.
    All of the three vaccinated volunteers showed significant increase in CTL activity. "Immune" lymphocytes prepared from one of the volunteers were transferred to her husband with chronic active hepatitis type B. Adoptive transfer of cellular immunity to her husband was proven with the HBs antigen specific CTL activity. These findings encourage us to try to modify chronic hepatitis B virus specific immunologic procedures.

    The nucleation time (N.T. the time needed until earliest detection of cholesterol monohydrate crystals in bile in vitro) in human bile obtained by needle puncture aspiration at surgery was measured in 21 patients with several types of gallstones and 2 without gallstone (1 gallbladder polyp and 1 bile duct carcinoma). It was compared to the lithogenic index and total protein concentration in bile. N.T. and biliary protein were determined by the methods of Holzbach et al. and Lowry et al. respectively. Biliary lipids were determined by gas-liquid chromatography. N.T. was shorter in patients with cholesterol gallstones than in patients with pigment stones and without gallstone. N.T. allowed more distinct differentiation between cholesterol and other gallstones than the lithogenic index. The total protein concentration in bile showed higher values in patients with short N.T. These results indicated that determination of N.T. is useful in distinction of the type of gallstones and biliary protein is an influential factor associated with N.T.

    This study was designed to elucidate a relationship between pancreatitis and inorganic phosphorus concentration in pancreatic juice. Experimental acute pancreatitis was produced by injection of 0.5% sodium deoxycholate into the dog pancreatic tissue directly through the pancreatic capsule, and exocrine pancreatic function test including measurement of inorganic phosphorus was performed at 3rd, 7th, 14th and 28th day after the injection.
    Acute necrotizing pancreatitis was observed at 3rd and 7th day, and fibrous tissue proliferarion at 14th and 28th day, of which lesions were localized at the point of injection.
    At 3rd day, max, phosphorus concentration and phosphorus output were significantly increased and max. bicarbonate concentration was significantly decreased. These functional alterations were normalized gradually after three days with subsiding of pancreatic inflammation. It was demonstrated that changes of inorganic phosphorus concentration in pancreatic juice was paralleled with histological findings.
    It was suggested that elevation of inorganic phosphorus inpancreatic juice was due to the increased permiability of cell membrane and the destruction of acinar cells induced by pancreatic necrosis and inflammation.
