The growth rate of esophageal cancers was investigated by roentgenographic observation in 11 cases (1 early and 10 advanced cancers) which could be retrospectively studied by previous series of roentgenographic examination of the esophagus. They consisted of 10 males and 1 female and their mean age was 66.6 years. On the basis of the growth patterns (size and morphological changes of the tumors), the doubling time was calculated by the Collin's and Kusama's methods.
The following results were obtained.
1. Roentgenographic observations revealed rapid growth of the tumors in all cases.
2. The mean doubling time of 11 cases was 3.3 months.
3. In 7 advanced cases which had been initially diagnosed in the early stage, the mean doubling time was 3.6 months.
4. In 3 cases diagnosed as advanced cancers by initial roentgenography, the mean doubling time was 2.4 months.
5. The above findings indicated the rapid growth of esophageal cancers in general.

The titers of IgE antibody specific for anisakis larvae were measured in the patients with acute gastric anisakiasis, by radioallergosorbent test with tissue homogenate of Anisakis larvae (type I) as antigen.
In the 76 normal subjects including 27 members of medical staff and 49 junior high school students in Nankoku city, the titers were low (6-26 units), except for 300 units in a 24 y.o. nurse, who was supposed to have experienced a latent anisakiasis or other Nematode infection. Seventeen (85%) of 20 patients with acute gastric anisakiasis showed high titers ranging from 30 to 3, 000 units, although three (33%) of 9 patients with Metagonimiasis yokogawai and one (14%) of 7 patients with allergic diseases (3 bronchial asthma and 4 allergic rhinitis) showed slightly high titers (30-54). But in Nankoku city, 18 (26%) of 70 general inhabitants and 9 (31%) of 29 patients with miscellaneous diseases (peptic ulcer, chronic gastritis, gall stone etc) showed high levels, ranging from 30 to 1000 units. This high percentage of antianisakis IgE antibody in normals and patient cortrols may be caused by high prevalence of this antibody in the general inhabitants of Nankoku City, where overt gastrointestinal anisakiasis is prevalent, but prevalence of other Nematode infections are not ascertained. And these high titers did not change in serial measurements of 1-24 weeks intervals.
In the serial measurements in patients with acute gastric anisakiasis, the titers, which ranged from 18 to 480 units within 2 days after onset, increased rapidly to their maximal levels ranging from 560 to 10, 000 units in the course of 3-5 weeks, and then decreased very gradually. But the titer in one case remained at its moderately high level during the 3rd to 5th year after onset, whose reinfection was not confirmed during her clinical course.
In conclusion, a single or serial measurement of serum anti-anisakis IgE antibody is very useful in the diagnosis of acute gastric anisakiasis.

To elucidate the prevalence of fatty liver in the general population, ultrasound mass survey was carried out in the Ichinohazamacho, a rural community of Miyagi-prefecture, on applicants over 40 years of age. Total number of the subjects were 814 which was 14.5% of the population over 40 years of age in this community. The incidence of fatty liver was 7.1%, although there was some bias because of applicant-subjects. About 80% of fatty liver was suspected to be based on obsesity, while only 3.4% was thought to be result from habitual alcoholism. About 20% of fatty liver had elevated GPT level (>35IU/L) and most of them were below 50IU/L. We consider that the prognosis of this kind of mild fatty livers detected in the ultrasound mass survey is generally good. There have been, however, known a specific type of fatty liver associated with obesity which could progress to the cirrhotic state. Then we will make a follow-up study of detected fatty livers in ultrasound mass survey.

We made a prospective study of the development of hepatocellular carcinoma (HCC) in 160 patients with cirrhosis of the liver from January 1983 to October 1985. During this period, serum α-fetoprotein (AFP) levels were assayed every 2 months and abdominal ultrasonography (US) was done every 3 months. Detection of the carcinoma in the 16 patients found in 1983 or 1984 to have HCC was by US in 75% of the cases and by AFP in 19%. Eight of these patients had small HCC, 2cm or smaller. US was very effective for detection of these lesions. Seropositivity for HBs antigen was a statistically significant risk factor.
To detect small HCC and to minimize the expense of testing, periodic screening using AFP and US is recommended.

Imaging diagnosis by ultrasound (US), X-ray computed tomography and angiography, and clinical findings of portal vein tumor thrombosis (PVTT) in hepatocellular carcinoma (HCC) were studied with respect to location of PVTT in 116 patients. PVTT were followed up by ultrasound in 11 of them and the changes of clinical findings were also studied.
US showed the highest detection rate for PVTT compared with other imaging modalities. Patients with PVTT in the portal trunk had more severe liver dysfunction, and worse prognosis, and more advanced esophageal varices than the ones with PVTT in the peripheral branches of the portal vein. Extention of PVTT was classified into three types from the mode and speed of growth. PVTT with a rapid growth showed poor prognosis. It was also classified into four patterns with regard to the relationship between the initiation of PVTT and its extent from the main tumor. There were cases in which PVTT was produced during a stage when the main tumor was still small.
It was shown that PVTT was closely interrelated with clinical findings and prognosis of HCC, and that diagnosis and therapy of PVTT are clinically important.

Functional activity of the reticuloendothelial system (RES) was studied by using iron-chondroitin sulfate colloid (
59Fe) on the rats with normal and cirrhotic liver which was induced by CCl
4 and on hepatectomized rats of both groups.
Phagocytic index (P.I.) of the cirrhotic group was significantly lower than that of normal group. This decrease was thought mainly due to lessend activity of Kupffer cell. An increased uptake rate of
59Fe in the both spleen and the lung was observed, especially in the lung in cirrhotic group.
P.I. in the normal liver group was significantly decreased after partial hepatectomies, but returned to normal value within 3 days in 30% hepatectomized rats, and 5 days in 70% hepatectomized rats. P.I. in the cirrhotic group was also significantly decreased after 30% hepatectomy but its recovery was prolonged as compared that of normal group.
Postoperative decreased uptake of
59Fe in the liver and increased uptake in the lung was observd in both groups. These changes were most prominent in the rats with massive hepatectomy in normal group and in the rats with partial hepatectomy in cirrhotic group.

Anti Col 2 region of N-terminal propeptide and anti α
1 (III) of type III collagen monoclonal antibodies were established using type III collagen fraction prepared from fetal calf skin as an antigen sourse. These monoclonals had cross reactivities to the respective antigens of fetal and adult human origin, anti Col 2 monoclonal immune stained by an enzyme immunoassay the connective tissues of skin, lung, spleen and liver. The connective tissues of fetal tissue showed stronger staining than those of adult. Pericellular staining along sinusoidal lining with weaker staining of interlobular connective tissue was the typical feature in the normal liver. While it was observed in tissues of hepatocellular carcinoma that carcinoma lesion was stained poorly but the capsel of connective tissue and surrounding tissue without carcinoma was positively stained.

Prolyl hydroxylase (PH) is playing an important role in collagen synthesis. Elevation of serum PH concentration has been reported in liver fibrosis as tissue PH activity increased. The present study attempts to investigate clinical significance of concentration of serum PH in the healthy control and liver diseases.
Subjects studied were 164 normal healthy persons (male 88, female 76) as control, 22 cases of nonspecific reactive hepatitis (NSRH), 13 of acute viral hepatitis (AVH), 15 of chronic inactive hepatitis (CIH), 26 of chronic active hepatitis (CAH), 26 of non-alcoholic liver cirrhosis (LC) and 12 of alcoholic liver diseases (ALD) diagnosed by clinical, biochemical and histological study. Blood was collected at fasting time in the morning and serum was stored at -20°C.
PH was purified from human placenta according to the method of Tuderman et al. Polyclonal antibody to human PH was raised in rabbits, and monoclonal antibody was produced according to the procedure of Oi and Herzenberg. By utilizing polyclonal and monoclonal antibodies a sensitive immunoassay system (EIA, sandwich method) for PH (immunoreactive PH) was established.
The concentrations of serum PH were 45.1±12.6ng/ml (Mean±S.D.) in normal control, 119.7± 39.3 in NSRH, 89.7±22.5 in AVH, 117.9±45.7 in CIH, 170.4±66.9 in CAH, 150.1±60.6 in LC and 161.2±44.8 in ALD. Among liver diseases the concentration was significantly higher in CAH, LC and ALD and lower in AVH than in NSRH. The concentration was well correlated with the grade of fibrosis in CAH and CIH.
These results suggest that the concentration of serum PH is related to the grade and the activity of liver fibrosis and EIA of PH is available for assessment of the grade of liver fibrosis.

The magnetic resonance imaging of gallstones was studied in 34 patients. Diagnosis of the gallstones was possible in 29 patients (85%) using MR imaging, in 97% using ultrasonography, and in 47% using X-ray CT. The gallstones showed as no-signal-areas in all cases, with relatively strong signals being detected from the center of the stones on the spin echo image in six cases. Post-operative studies showed the stones with the central signals to be of the so-called mixed stone type, in which the central area consists of cholesterol with a radiating structure. The central signal was thought to originate in the water in the central cavity of the stone, and is a characteristic indication of the stone's structure.

Serum CA19-9 levels were measured using radioimmunoassay in 45 patients with cholelithiasis and clinical significance of serum CA19-9 was discussed. Results obtained were as follows:
1) Positive rate over cut off level of CA19-9 (>37U/ml) was 27.6% in patients with cholecystolithiasis and 68.8% with choledocholithiasis.
2) Positive rate of serum CA19-9 was high in patients with clinical symptoms such as cholecystitis, cholangitis and obstructive jaundice. The highest titer of CA19-9 was 2350U/ml in a patient with obstructive jaundice.
3) High titer of serum CA19-9 was improved in patients with cholecystolithiasis after subsidence of cholecystitis.
4) There was positive correlation between serum total bilirubin and serum CA19-9 in patients with choledocholithiasis. High titer of serum CA19-9 was normalized in all cases after improvement of obstructive jaundice.
It is suggested by these results that CA19-9 is secreted into bile and this antigen flows back into blood in situation of cholecystitis or obstruction of bile duct.

Clinicopathological studies on 4 cases of mucin-producing adenocarcinoma (including mucionus adenocarcinoma) of the pancreas have revealed some characteristic features, reflecting their pathological characteristics producing abundant amount of mucin. Those lesions were discernible as hyperechoic tumors with irregular margin and small hypoechoic spots by ultrasonogram, and as masses of heterogenous density with ill-defined margin by CT. Tumor stain by angiography was characteristic for this type of pancreatic cancer. Those lesions involving the main pancreatic duct showed the typical findings compatible with the type III of ERCP classification of pancreatic cancer (by Cancer Institute): dilated pancreatic duct with mucin plug resulting in widened orifice of the duodenal papilla. Inspite of their fairly large size at the time of operation, this type of pancreatic cancer may have more favorable prognosis than the ordinary pancreatic cancer.

To evaluate diagnostic significance of secretin test carried out by duodenal aspiration during 60min after a bolus i.v. injection of 100CHR units secretin in chronic pancreatitis, the secretin test was studied in 28 controls and 30 patients with chronic pancreatitis and the results were compaired with those of the corresponding post-secretin 60min period of the conventional pancreozymin secretin test in 50 controls and 43 patients with chronic pancreatitis, since Boots secretin became unavailable.
The mean values, coefficients of variation and lower normal limits in the S test were similar to those of the PS test. The fluid volume, total output of amylase and bicarbonate of the S test did not correlate significanlty with body weight in controls when secretin was given independently of body weight. Therefore, parameters were unnecessary to be expressed as per kg body weight. Frequencies of abnormality in the respective parameter and final diagnosis by the S test was similar to those by the PS test in chronic pancreatitis. The S test is as useful as the conventional test for evaluation of exocrine pancreatic insufficiency in chronic pancreatitis.

Magnetic resonance images of the upper abdomen were obtained from 67 patients with gastrointestinal diseases using combined multidirection and multislice methods. Transverse, coronal and sagittal images were compared with one another and evaluated from the viewpoints of identification of the abdominal organs and clinical usefulness. The clearest image of the liver was visualized on the transverese section. The border of the liver and intestinal gas was clearly delineated on the sagittal scan. The gallbladder was identified as a high-signal area as distinct from the intestines, which appeared as a low signal area, by spin echo sequence on three-directional tomograms. The transverse image of the pancreas was the most useful, followed by the sagittal image. Identification of the pancreas was difficult on the coronal scan. Longitudinal images of the whole of the aorta and the inferior vena cava were as obtained on the coronal and sagittal tomograms as was that of the portal vein on the coronal tomogram.
