At the present moment, it is believed that the irritation of the vagus nerve causes hypersecretion of hyperacid fluid in the empty stomach at night and the regurgitation of the duodenal fluid is believed only to neutralize, buffer and dilute in order to decrease the acidity. The autor examined the gastric secretion of 94 cases of non ulcer (Cancer not included) and ulcer cases aspirating the fluid continually for 12 hours during the night.
    The findings are as follows:
    1. acid more than 12 mEg non ulcer 16%, gastric ulcer 70%, pyloric and duodenal ulcer 90%. There is a definite difference seen.
    2. Regurgitation of the duodenal fluid into the stomach in cases of ulcers is related to the dysfunction of the pylorus which is caused by the organic and functional changes of the pylorus.
    3. There is always a coloring of the gastric fluid by the regurgitation of the duodenal fluid and the rising of the acid courve agree. Also in experiments in cases of stomach fistulas the duodenal fluid causes tne increase of gastric secretion.
    4. When there is a flow back of the duodenal fluid into the stomach not only is there the regular amount of secretion but the fluid from the duodenum and more secretion of the gastric phase caused by the irritation from the duodenal fluid during the night.
    5. If the neutral Cl in the gastric fluid can be called the quotient of the regurgitated duodenal fluid in the stomach, it can be calculated.
The amount of the duodenal fluid in the nocturnal gastric secretion in cases of ulcers was analysed.
The increased scidity caused by irritation is much more than that which was neutralized and buffered.
    6. Insulintests and nocturnal gastric secretion were compared on the same patient. We found that the increase in the nocturnal gastric secretion and the increase in secretion caused by insulin are not related. Although the amount of acid in the nocturnal gastric secretion may be low, we found a group which we thought to be a pre ulcerous state who had an abnormal amont of gastric acid caused by insulin.
