Traumatic laryngeal stenosis are increasing according to addition of recent traffic accidents. This case report was aged 22 years old, male, who had been performed high tracheotomy because of post traumatic dyspnea and had become decanulement. Then he was performed skin graft transplantation in order to inhibite trouble some granulation from the anterior laryngeal wall, and he could leave the hospital one month after the operation. During last 6 years there were 37 cases of traumatic laryngeal stenosis with surgical therapy in the Japanese literatures. The 10 cases were carried out laryngofissure, and three of them were performed arrytenoidectomy. Many kind of molds were used in 7 cases, most of them were detained for onemonth over. There were successful 17 cases which were performed transplantation using skin graft of abdomen or thigh, mucous membrane of oral cavity and vane graft, in 6 of them molds were detained after transplantation, and one case was made laryngeal plasty using his rib for the anterior wall. Transplantation is the best method, I think, due to early come back to the societywhen clicoid injury gives occasion to decanulment.
In the medical treatment upon tinnitus, the effectiveness of pyridoxin hydrochloride I and mesoinositol hexanicotinate II has been already recognized. And now it is recommended to use both substances together for a more effective treatment. Aderazid, commercially available, which contains 10 mg of Pyridoxal phosphate calcium -an active form of I- and 200 mg of II Per tablet, was used for 22 cases complained tinnitus. According to the result from these cases, remarkable effects of Aderazid were obtained in the cases which used the smaller dose during comparatively short period, and in those of early stage after tinnitus had developed. Aderazid is notably advantageous medicament for the treatment, without any side-effects, which is much more effective than other ones without Aderazid which have been done so far.
Ibunac, non-steroid and non-p substance, has long been used for the treatment of inflammatory diseases after for rheumatic diseases. The drug has been shown to be effective for treatment of side effects due to large case of steroid, and also far of inflammatory cases. Ibunac was used for a total of 39 cases. They were 6 cases c acute external otitis media combined, 12 cases c acute pharnygolaryngitis, 6 cases c acute imflammation (cases c temporomandibular joint arthritis and c imflammation of submandibular gtands), 4 cases c chronic otitis: media, 11 cases c chronic pharnyngolaryngitis. Results were summalized as followed: 1) Ibunac has been shown to be effective for acute inflammatory diseases. 2) The effect was particulary prominent in ealy stages of inflammatory process. 3) Striking therapeutic effect was seen in cases c temporomandibular arthritis. 4) Effect was also seen in acute excerbatian of chronic inflammation. 5) This drug is desirable to be replaced by other drugs when the effect is notobtained in 2-3 days in use for chronic inflammatory diseases. 6) No siginificant side effect was seen in any cases.
Among the complications resulting from side injuries during operations for chronicsinusitis, visual impairment is considered not only a medical but also a serious social problem. we have recently experienced a case referred to us for treatment of visual impairment following an operation for sinusitis. This patient, a 51-year-old man, first had a tooth extracted because of swelling of the buccal region and nasal discharge, but the symptoms persisted and he then visited the ENTdepartment of a hospital where the presence of a tumor was suspected on x-ray examination. Anoperation was performed but on the 3rd postoperative day he suddenly complained of visual impairment (blindness). Various treatments were tried without improvement and he was then referred to our hospital for further treatment. Reoperation at our hospital has resulted in some return of his light sense as he can now perceive faintly the light from a window, but no further improvement of vision hasbeen observed. This patient is still under observation.
