2. Distinctions between sine waves and synthesizer tones are difficult in the high-pitched tinnitus compared with low-pitched one.
3. The tones of synthesizers are expressive as pure-tone-like onomatopoeic words.

ジャーナル フリーThis study included 16 ears of congenital ossicular malformations without anomalies of the external meatus or ear drum. The malformation of the incudostapedial joint were recognized in 13 ears. Most of them were characterized by the absence or deformity of the long process of the incus and of the superstructure of the stapes. The fixation of the stapes footplate was recognized in two ears and the remaining one ear was fixation of the malleus and incus in the attic.
It has been reported that stapes anomaly (fixation) may be accompanied by inner ear anomalies. Therefore, we performed small fenestra stapedectomy, which shows less comlication than total stapedectomy, in the cases with stapes footplate fixation.抄録全体を表示PDF形式でダウンロード (1179K) -

ジャーナル フリーTetanus, an infectious disease designated by law is a disease which has long been known. With a progress of chemotherapy and immunotherapy in recent years, the incidence of this disease decreased markedly particularly after the end of the World War II, with the highest incidence of 2168 cases in 1949. Once affected by this disease, however, the patient follows a very serious clinical course, and even today the mortality rate from this disease is still high.
This case is a 25-year-old man, a cook by profession.
On April 9, 1983, he underwent operation for phimosis.
On the 10 th postoperative day he had pharyngeal dysphoria, difficulty in swallowing and in opening the mouth.
Diagnosis of tetanus was made on the basis of clinical symptoms. Treatment using tetanus human immunoglobulin, antispastic agent and muscle relaxant was instituted immediately. Because of dyspnea resulting from general convulsion, tracheostomy was performed on May 2. The patient was able to survive without any sequelae.
Otorhinolaryngologists have few opportunities to encounter tetanus despite the fact that complaints peculiar to the area of otorhinolaryngology such as pharyngeal dysphoria, difficulty in swallowing and hardness of mouth-opening are very often heard of as Stage 1 and Stage 2 symptoms of tetanus.

ジャーナル フリーWe studied the effect of hyposensitization therapy in 26 cases with Japanease Cedar pollinosis by means of questionaires. Air distributions of the pollens throughout the year were studied for the last 7 years.
The number of the pollens were counted on a 10 mm×10 mm size slide glass e very day.
The results were as follows.
1) Clinical symptoms improved more prominently in the order of nasal obstruction, attack of sneezing, and watery rhinorrhea.
2) The rate of overall improvement was 63.9% for subjective complaints ueder this hyposensitization therapy.
3) Skin test threshold and values of total IgE and specific IgE in serum did. not decrease during the treatments but the values of RAST and RIST were recognized to show seasonal changes, and they increased in spring, and decreased in winter.
4) The number of eosinophils in nasal smear tended to decrease in treated cases more than three years. Positive rate of nasal provocation test was decreased only in cates treated for three years.

ジャーナル フリーFibrous dysplasia which is a benign tumor of the bone, is an uncommon tumor arising in the facial and skull bone. Two cases of fibrous dysplasia dysplasia of the maxilla are reported.
Case 1 is a girl aged 8 who complained of a swelling in her left cheek. Case 2 is a woman aged 28 who complained of a swelling and pain in her left cheek. In both cases, the diagnosis of fibrous dysplasia of the maxillary bone was made histologically after exploratory or conservative operation. No recurrence was seen postoperatively. ジャーナル フリーOlfactory neuroblastoma developing primarily in the nasal cavity is rare and shows little specific clinical symptoms. This disease is a difficult disease to be diagnosed at an early stage. About 200 cases have been reported in foreign countries, but only 45 cases have been reported so far as the authors could find in the Japanease literature. Moreover, prognosis is extremely poor as compared with those in Europe and America, which is thought to be attributable to a delay in the definite diagnosis.
The authors report three cases of olfactory neuroblastoma which were detected in the comparatively early stage.
When a tumor is found at the early stage, surgical extirpation of the tumor by means of lateral rhinotomy is thought to be useful, and it is considered that the early diagnosis is essential for a better prognosis.
