    Purpose : We evaluated the efficecy of Burow's solution, first produced by Burow in the 19th century and reported by Terayama et al. in 2003 to be effective in Japan against refractory otitis externa and otitis media.
    Materials and Methods : Subjects were 57 patients (64 ears) -25 men (27 ears) and 32 women (37 ears) ranging in age from 10 to 94 years. Their diseases included chronic perforative otitis media (30 ears), chronic myringitis (19 ears), and chronic otitis externa (15 ears). Patients were treated with Burow's solution instilled into the ear for about 10 minutes or a pledget soaked in this solution inserted into the ear. We evaluated effects on the volume of ear discharge and bacteria based on a four-grade system of “complete response, ” “remission, ” “no change, ” and “aggravation”. Subjects were divided into two groups-those with granulation and those with eardrum perforation-and evaluated based on gender surgical history, and presence or absence of staphylococcus, and compared for significant differences in treatment efficacy.
    Results : Complete response was seen in 9, remission in 11, no change in 9, and aggravation in 1 of 30 ears in patients with chronic perforative otitis media ; in 10, 4, 2, and 3 of 19 ears in those with chronic myringitis ; and in 13, 1, 1, and none of 15 ears in those with otitis externa. Statistical analysis of treatment efficacy among the three disease groups (chronic perforative otitis media, chronic myringitis, and chronic otitis externa) using Kruskal-Wallis test showed a significant difference (p = 0.01). Bacterial culture examinations detected bacteria and fungi in 61 ears, the most common being methicillin-susceptible staphylococcus aureus (MSSA) in 19 ears (29.7%). Statistical analysis using Mann-Whitney's U test showed p of 0.61 for gender, 0.17 for surgery, 0.02 for ear drum perforation, 0.005 for granulation, and 0.72 for staphylococcus-showing a significant difference in treatment efficacy in cases of ear drum perforation and those of granulation.
    Conclusions : Burow's solution is less effective in patients with ear drum perforation and with granulation, and therefore should not be used chronically in such patients.

    We report a case of cerebellar infarction after infra-arterial infusion chemotherapy. A 74-year-old man complained of discomfort in his throat. A biopsy of the right palatum molle showed a moderately differentiated squamous cell carcinoma (T2N0M0) on the lateral pharyngeal wall of the mesopharynx. The administration of cisplatin through the femoral artery caused hypertension, numbness in the arms, and heavy sweating. The onset of headache and double vision occurred on day 1. Disorientation and dysarthria continued from day 2 onwards. A CT scan revealed a right cerebellar infarction caused by the occlusion of the vertebral artery. After spinal drainage, incomplete left-side paralysis persisted. Stenosis of the common carotid artery should be ruled out using an electrogram prior to intra-arterial infusion procedures to prevent cerebral accidents.

    Auditory neuropathy is a sensorineural disorder characterized by absent or abnormal auditory brainstem evoked potentials and normal cochlear outer hair cell function. A variety of processes is thought to be involved in its pathophysiology and their influence on hearing may be different. We present here the management of a case of unilateral auditory neuropathy with reference to a review of the literature. The diagnosis was made through a neonatal hearing screening programme based on automated auditory brainstem response testing. The auditory brainstem response was normal for the left ear and absent for the right ear. OAEs were present for both ears. Audiometric testing indicated normal pure-tone thresholds across the frequency range for the left ear and a profound loss for the right ear at the age of six years. These findings were consistent with normal outer hair cell function bilaterally and presence of auditory neuropathy in the right ear.

    This is a report on cedar pollen dispersion in the city of Ojiya in Niigata Prefecture from 2002 to 2007. We found that the total amount of cedar pollen dispersed in this region was related to the number of daylight hours during the months between late July to early/mid-August of the previous year. It was also ascertained that in this region, if the maximum daily temperature exceeded 10°C for the first time that year before the cedar pollen had dispersed, then the cedar pollen began dispersing 1) either on that day or in the two days before or after that day, or 2) approximately two weeks or more after that day. We believe that the occurrence of 1) or 2) roughly depends on the accumulated value of the maximum temperature from January 1 to the day when the maximum temperature exceeded 10°C. Therefore, we believe that establishing the day on which the maximum temperature exceeds 10°C for the first time in the year as the day on which treatment regimens must be begun for patients with cedar pollen allergy would be useful for both cedar pollinosis patients and medical therapists in this region.

    Otitis media with effusion is a common disease and is the most common cause of hearing impairment among children. It is important to treat this condition in children to provide them a better quality of life. “Ear popper®” is a new device distributed in the US for non-surgical treatment of middle ear effusion. The device allows easy insufflation of the eustachian tube. We used the “Ear popper®” for the treatment of middle ear effusion in 18 patients. The patients ranged in age from 2 to 9 years old, and were composed of 8 males and 10 females. Successful insufflation without any problems was achieved in 17 out of the 18 patients at our hospital. This was confirmed by visual observation of the tympanic membrane, impedance audiometry and pure-tone audiometry. The new device is portable and easy to use. Although in this clinical trial, the use of this device was restricted only to doctors at the hospital, we are currently attempting to introduce the use of this device at home by the patients by themselves, just as in the U.S.A. We consider that the home treatment of persistent otitis media with effusion in children with “Ear popper®” is highly efficacious.
