  • III. 歯肉組織中のβ-Glucuronidase 活性値

    The β-Glucuronidase(β-G) activities in gingival tissue from patients with periodontal disease were measured by Fishman's method, and compared with clinical findings (the depth of periodontal pocket and the degree of alveolar bone loss by X-ray photos) and histopathological findings (Listgerten-Ellegaard's classification)
    The following results were obtained;
    1. The β-G activities in gingival tissue were shown to have marked correlation with the depth of periodontal pocket (P<0.01), but no significant correlation was obtained with the degree of alveolar bone loss.
    2. In the group of patients that were examined on the basis of clinical findings and X-ray photos, the β-G activities in gingival tissue had a tendency to increase with the progression of periodontal disease.
    3. As the histopathological findings, the β-G activities were markedly with the degree of inflammatory cellular infilitration.
    It is concluded from these results that the β-G activities in gingival tissue are closely related to the process of periodontal disease and the degree of inflammatory cellular infilitration.

    The total lipids extracted with chloroform: methanol (2:1 v/v) from human gingiva of periodontal disease.
    The lipids pattern were investigated by the use thin-layer chromatography over Silica gel sintered rod with hydrogen flame ionization detector.
    Typical chromatograms obtained by this method are shown.
    Total lipids have been identified as phospholipids, cholesterol, triglyceride and cholesterol ester.
    The main components of phospholipids in human gingiva of periodontal disease were sphingomyelin, phosphatidyl choline and phosphatidyl ethanol amine.
    Chemical and biochemical data by this apparatus are considered to be useful for analysis of the these tissue lipids.

    The histamine contents in whole saliva and serum collected from patients with periodontal disease were measured by both Anton-Sayre's and Håkanson-Rönnberg's methods.
    Comparative study of both methods was performed to find out the better way of estimating histamine.
    The following results were obtained:
    1. Compared with Anton-Sayre's method which was able to measure histamine in the range of concentrations from 0.05 to 10.0μg/ml, Håkanson-Rönnberg's method was much sensitive to detect from 2 to 1, 000μg/ml.
    2. The recovery of histamine was 70.0% by Anton-Sayre's method and 92.2% by Håkanson-Rönnberg's method.
    3. Both methods were capable of measuring of histamine contents in whole saliva and serum of all subjects ewamined.
    4. The histamine cotents in whole saliva was distinctly higher in patients with periodontal disease than clinically health subjects.
    5. There was no difference in the serum histamine contents between clinically healthy subjects and periodontal disease patients.

    It has been reported that the physical character of the diet influences on the periodontium, and soft food has a tendency to cause gingivitis more than hard food. On the other hand, sucrose has an important role for plaque formation, and so it seems logical to consider that sucrose may also have an influence to cause gingivitis, but very few study has performed to clarify the relationship betwees sucrose and gingivitis.
    The aim of the present study was to investigate the effect of the soft food containing sucrose on the gingiva and to establish an experimental method to cause gingivitis quickly with food control.
    One adult male Macaca irus monkey was used and the schedule of this experiment was as follows:
    1. Preliminaly period (12 days, Hard food, Scaling and Brushing on every 2 days)
    2. Control period (30 days, Soft food without sucrose)
    3. Preliminaly period (12 days, Hard food, Scaling and Brushing on every 2 days)
    4. Experimental period (30 days, Soft food with 20% sucrose)
    The result of this study indicated that the gingival inflammation during the experimental period was developed more severly and more fast than that of the control period.

    In vitro study was made of sealing effects of periodontal dressing materials consisting of a eugenol group (Surgical pack [SHOWA], Neodyne pack, Periodontal pack) and a eugenolless group (Cello pack, Peripac, COE-pak).
    Glass tubes containing experimental materials were put in methylene blue solution (1.0%) at a definite temperature.
    The results obtained were as follows:
    1. COE-pak showed good results in methylene blue solution test, followed by Cello pack and Surgical pack, Neodyne pack, Periodontal pack, Peripac in descending order.
    2. COE-pak showed good results in sealing ability against water, followed by Cello pack, Surgical pack, Neodyne pack, Periodontal pack and Peripac in descending order.
    3. COE-pak showed good results, but not Peripac.
    4. Sixty minutes after methylene blue penetration test, Periodontal pack showed good results.
    5. All in five cases of Periodontal pack and two cases of COE-pak showed a tendency to expansion.
    6. Eugenolless group (Cello pack, COE-pak) was generally showed a better sealing ability than by a eugenol group.

    In vitro studies were made of physico-chemical properties effects of periodontal dressing materials consisting of a eugenol group (Surgical pack [SHOWA], Neodyne pack, Periodontal pack) and a eugenolless group (Cello pack, Peripac, COE-pak).
    The examinations included:
    1. Absorption test, 2. Solubility test, 3. Consistency test, 4. Pigmentation test, 5. Temperature test and 6. pH.
    The results obtained were as follows:
    1. Absorption test:
    Peripac showed a marked absorption; Periodontal pack, COE-pak, Neodyne pack and Cello pack showed a similarly less degree of absorption.
    Surgical pack was in between.
    2. Solubility on water showed similar to absorption test.
    3. Consistency test:
    Fifteen minutes after this experiment consistency was obtained in the following order; Periodontal pack, Neodyne pack, Cello pack, Surgical pack and COE-pak.
    No measurments were possible for Peripac after one minute.
    4. Pigmentation test:
    Nonpigmentat ion was obtained in the following order;
    Neodyne pack, Cello pack, COE-pak, Periodontal pack, Surgical pack and Peripac.
    5. Temperature test:
    Temperature of experimental materials showed 35.6-36.8°C, which did not seen to give a bad influence on a wound healing process.
    6. Change of pH:
    a) COE-pak was on the alkaline side.
    b) Surgical pack and Neodyne pack shifted to alkalinity from acidity (pH experiment No. 1), the other method were showed of alkalinity.
    c) Periodontal pack, Cello pack and Peripac were either acidic or neutral.

    Localization of Actinomyces naeslundii in the chronically inflamed human gingiva was studied by immunohistochemical procedures.
    Immunofluorescence was detected on the surface of crevicular epithelium and the tissue adjacent to epithelium.
    Especially in a inflammatory connective tissue, Immunofluorescence suggesting phagocytosis by histiocytes round infiltrating lymphocytes was detected. While there was no immunofluorescence in the clinically healthy gingiva.

    This study was made of the strain of gingiva on physiological property of free gingival surface according to the Pias 49 Water Pik.
    The dog was subject to this experiment and the strain gauge M-101 (Shinkoh Co.) was adhered to the free gingival surface of maxillary cuspid with alpha cyano acrylate and parallel to the long axis of tooth. The tip of the handpiece was 10mm at the distance and the directions were in vertical and forty five degree against the gingival surface. The strain of gingival caused by stream of water pressure on each dial number setting on the Water Pik were record.
    As the result, the free gingival surface was subject to a tensile strain on each dial number of the Water Pik. The degree of strain were determined by the force of water pressure but this trend appeare more clearly on the dial number three. In the direction of forty five degree the spike of gingival strain showed more than in the vertical direction.

    Severity of gingival and periodontal disease was evaluated by counting the numbers of leukocytes in mouth-rinsing saline and measuring the salivary occult blood with urine test kits.
    The results were as follows:
    1. Salivary leukocytes counts were correlated with the degrees of periodontal inflammation, especially with Dunning's X-ray bone scores and the degrees of tooth mobility. There were more salivary leukocytes in adult patients with complaints of viscous saliva and bad taste than in adults without periodontal complaints or in children.
    2. The levels of salivary occult blood were correlated with clinical gingival bleeding scores, but were not correlated with other clinical scores. Except for gingival bleeding scores, salivary occult blood levels showed greater correlation coefficient with gingival redness scores than with other clinical scores. Salivary occult blood levels had little correlation with salivary leukocytes counts.
    3. Salivary occult blood levels and salivary leukocytes counts showed statistically significant decrease after scaling of salivary donors.
    4. The measurement of salivary leukocytes counts by estimation of peroxidase activity in saliva, failed because of the lack of correlation between these parameters.
    It was suggested that salivary leukocytes counts and occult blood levels were available for indices of periodontitis and gingivitis respectively.

    A new periodontal probe—Electro-Perio Probe—has been devised by J. Morita Co. for the measurement of periodontal depth. This new device features automatical digital display and printout of the pocket depth by simply inserting the probe tip into periodontal pocket. As a natural consequence, fast, easy and unpre-cendented accurate measurement is feasible and the error in measurements can be minimized compared with any other periodontal probe.

    Chronic desquamative gingivitis is caused by systemic or local factors, especially it is influence with the role of estrogen, vitamin and thyroid horman and other with plaque, calculs and some local irritation.
    The patient is a 47-year-old female, who had married and had received ovariotomy at seven years ago. She called in our hospital at 5 years ago with gingival pain as chief complaint. Had 2|2 extracted because of excessive movility and had restoration setted on 543/5|145. On clinical finding, the marginal gingiva was redness, the gingival epithelium was teared spontaneously and accompained with pain and hemorrage.
    The above symptome showed a markedly improvement according to plaque control and defective coronal restorations were removed, were induced by muco-gingival surgery or occlusal adjustment and at same time on topical administration of triamsynolon and acetonoido, the gingiva showed an efficacious effect, but it was relupsed at short intervals due to discontinuance of treatment. At present, it is observed good sign obviously on gingiva by the effect of barb medicin.
