    Gastrin-producing tumors frequently develop in the vicinity of the head of pancreas and the duodenum causing hypergastrinemia which results in elevated gastric acid secretion and obstinate peptic ulcer. Here we report a case diagnosed initially as gastroduodenal ulcer based on hypergastrinemia and obstinate peptic ulcer and later as gastrin-producing mucinous cystadenoma of the ovary on the basis of pathologic immunohistochemical findings. The patient, a 50-year-old woman, gravida 2, para 2, visited Department of Internal Medicine in our hospital complaining of upper abdominal pain. She was diagnosed as having gastroduodenal ulcer and treated accordingly, but her conditions alternated between better and worse phases. Abdominal pelvic CT detected a 12-cm tumor-like lesion in the pelvis. Together with a high blood gastrin value (1000 pg/ml), she was suspected of having an ectopic gastrin-producing tumor and referred to us. Simple hysterectomy and bilateral salpingo-oophorectomy were performed, and pathologic examination of tissues excised confirmed the diagnosis of gastrin-producing ovarian tumor of borderline malignancy. Afterward, blood gastrin levels decreased rapidly with an improvement of gastric ulcer as revealed by endoscopic examination of upper digestive tract. [Adv Obstet Gynecol, 58 (1) :1-4, 2006 (H18.2)]
