    The identification of genes responsible for genetic skin diseases was in a rush during the last decade. In this article, I wish to summarize the causative genes of the genetic skin diseases which imvolve the structural components of the keratinocyte and epidermis. Keratin is a component of cytoskeleton of all epithelial cells, and an epidermal keratinocyte has the most welldeveloped keratin cytoskeleton. Mutations of keratin gene were identified in epidermolysis bullosa simplex (K14 and K5) and bullous congenital ichthyosiform erythroderma (K10 and K1). The ultrastructual finding of the aggregated tonofilaments in these diseases suggests that the abnormal keratin is the primary cause. Abnormality of the cornified cell envelope seems to the cause of some congenital ichtyosis including lamellar ichthyosis in which mutations of transglutaminase gene have been idintified.
    In junctional types of epidermolysis bullosa, components of hemidesmosome are defective and in dystrophic types of epidermolysis bullosa, type VII collagen is defective. The results of genetic analysis of both types coincide with the ultrastractural findings.

    The skin is always exposed to a variety of apoptosis inducers such as UV, chemicals and viruses. Recent observations suggest that apoptotic signals triggered by these factors might converge on the caspase cascade via various signalling pathways. The activation of caspases represented by caspase-3 by apoptotic stimuli in turn causes chromosomal DNA degradation and morphological changes in apoptotic cells. This paper discusses how apoptosis signalling triggered by above factors is regulated at a molecular level that may be exploited in the treatment of various skin cancers and inflammation.

    The results of the questionnaire survey conducted on 2, 136 Osaka prefectural high school students (1, 043 males and 1, 093 females) in the Minamikawachi district on October 1996 and on January 1997 to know the incidence rate of food-dependent exercise-induced anaphylaxis are reported. Seventeen students (8 males and 9 females) were estimated to be exercise-induced anaphylactic and 4 students (1 male and 3 females) of them were food-dependent exerciseinduced anaphylactic. The incidence rate was 0.19%. Shrimp, crab, sausage and confectionery were thought to be causative foods.

    The effect of ultraviolet rays on the development of skin cancer is a matter of concern in the society, and it is an important issue to clarify the conditions of development of skin cancer.The cases diagnosed as skin cancer between 1981 and 1990 in Osaka Prefecture were investigated more meticulously by developing the regional cancer registry which is useful for descriptive epidemiology, and BCC 237, SCC 174, MM 36 cases were collected and analyzed in detail.
    The reports of skin cancer cases to the cancer registry program of Osaka Prefecture from each hospital were not so frequently done, and this suggested the necessity of more intensive system of cancer registry. There was no change in the incidence of BCC and SCC on the exposed sites during the first and second halves of the investigation periods, no effect of the strength of the ultraviolet rays on the development of skin cancer has not been observed yet.

    A patient with photosensitivity induced by fleroxacin is reported. The patient developed photosensitivity reaction during a treatment of chronic renal tract infection with fleroxacin. The action spectrum was in UVA range. At 4 days after the discontinuance of fleroxacin administration, photosensitivity improved. The provocation test with oral administration of fleroxacin and UVA irradiation was positive and revealed that the mechanisms involved appeared to be photoallergic reaction. Photopatch tests with fleroxacin and other new quinolones were negative.

    A 49-year-old man suffering from ulcerative colitis since 1987, has been treated by sarazosulfapyridine. In September 1987 he noticed many dark brown macules on the trunk. After replacement of sarazosulfapyridine with mesalazine, redness disappeared. The patch test of sarazosulfapyridine was negative but challenge test with sarazosulfapyridine showed positive reaction. Taken together we diagnosed his disease as multiple fixed drug eruptions.

    A 71-year-old woman with generalized granuloma annulare of non-annular type is reported. One month before the initial visit to us, she noticed slightly pruritic erythematous papules on her right hip which gradually enlarged to the entire body during treatment for diabetes mellitus. The histology of the papule manifested palisading granuloma where histiocytes, lymphocytes, multinucleated giant cells and Langhans giant cells surrounded the degenerated collagen fibers in the upper dermis. Therefore, we diagnosed the patient as generalized granuloma annulare associated with diabetes mellitus. Almost all the skin eruptions disappeared gradually after the skin biopsy from the right thigh without any treatment.

    We describe a case of squamous cell carcinoma (SCC) with malignancy associated hypercalcemia (MAH). A 72-year-old man who had suffered from pyodermia chronica glutealis for 50 years, visited to our hospital, since an ulcerated tumor was found on his right inguinal area. Under the histological diagnosis as SCC, the tumor was excised. Before the operation, the serum levels of Ca showed high, but after that it was normalized temporary. Within 3 weeks it began to increase gradually again. It was co-related with bilateral plural effusion due to the multiple metastasis to the pleura. The serum levels of parathyroid hormone related peptide (PTH-rP) was very high. Since he didn't have any other cause of hypercalcemia, we concluded it as MAH associated with SCC.
    MAH has been often shown in squamous cell carcinoma of the lung, multiple myeloma and adult T cell leukemia. MAH associated with skin squamous cell carcinoma is very rare and we present the case.

    We report a case of penile carcinoma in a man. The lesion had been present for 1 year prior to his visit to our clinic, and developed into a giant tumor. Histopathologically, the tumor was diagnosed as undifferentiated squamous cell carcinoma. His serum SCC antigen level was elevated to 180ng/ml. Radiotherapy and systemic chemotherapy reduced the size of the original tumor. However, satellite lesions developed on the abdominal wall, and accompanied disseminated intravascular coagulation led to his death. At necropsy, widespread metastasis associated with tumor cell embolization were found in the brain and multiple visceral organs.

    Three cases of onychorrhexis treated succesfully with topical steroid are reported.
    The causes of the onychorrhexis are unknown. However, in our three cases, eponychium of affected nails showed roughness and was normalized with the treatment topically applied steroid ointment to eponychium. So it is considered that onychorrhexis is caused by trauma to posterior nail fold.

    The first case was a 62-year-old woman who noticed sharp but irregular demarcated patches with erythema on both lower legs since the age of 57. As these lesions continued to increase in size, she visited our out-patient clinic on Feb.4, 1998. Skin biopsy revealed granulomatous change with disrupted collagen fibers in the dermis. She also had diabetes mellitus. The second case was a 60-year-old woman with progressive and erythematous, atrophic lesions on both lower legs. Biopsy specimen showed degenerative change in collagen fibers, surrounded by histiocytes and giant cells. She did not have diabetes mellitus.

    A 34-year-old male was referred to our department with asymptomatic band-like lesion consisted of closely set many white verrucous hyperkeratotic papules on the left upper arm, which was noticed when he was five years old. Histopathologically, epidermolytic hyperkeratosis was demonstrated. From electronmicroscopical findings, in the granular layer, tonofilaments aggregated and made large bundles, and keratohyalin granules were significantly large. We diagnosed the present case as epidermal nevus with epidermolytic hyperkeratosis.

    The author devised the 40% silver nitrate paste method, an improvement previously reported 40% silver nitrate method (solution method), and achived favorable results in the treatment of MC. This paper is the second report on this new method, and focuses on the practical aspects of using it.
    In this procedure, wheat flour is added to 40% silver nitrate solution to a concentration of 25% and stirred to obtain a paste. The tip of bamboo stick is then used to apply the paste to the top of the MC papule, and it is allowed to dry completely. Since the agent is a paste rather than a solution, it easily adheres to the papule. The mass of MC virus quickly changes into a black crust and falls off in 2-4 weeks (Table 1).
    During the 3-year period between 1995 and 1997, 380 (97.7%) of the 389 patiants who underwent this treatment were cured of MC (Table 2). Complaints of itching or pain were minor, and were made by 65 (16.7%) of the 389 patients, but only 5 of them discontinued treatment because of itching or pain (Table 3).

    The clinical efficacy of the skin care compress containing mugwort extract was evaluated in aid of drugs in 25 cases of atopic dermatitis. On final evaluation, improvement was observed as follows, upper moderate improvement in 20 cases (80%), unchanged in 1 case (4%), worsen in 2 cases (8%) and impossible to conduct clinical trial in 2 cases (8%). Concerning the safety of the compress, this use test showed, safety in 19 cases (76%), unknown in 3 cases, questionable in 1 case (4%) and not safety in 2 cases (8%). Six cases of side effect were observed, 2 cases were stopping the trial at first time use, stopping the trial after 2 weeks use was 1 case, stopping the trial after 3 weeks use was 1 case, 2 remaining cases were used final 4 weeks then dry skin was improvement. From the trial use, skin care compress containing mugwort extract was useful skin care product for atopic dermatitis.

    We evaluated the clinical efficacy and safety of skin care product (NOV EYEGEL®) for a dry and sensitive periocular skin. We tested a total of 45 cases including 31 atopic dermatitis. The results showed that the improvement ratio including moderately to remarkably improved cases was 84.4%, and the usefulness ratio including moderately to very useful cases was 82.2%. We observed mild side effects in five cases with atopic dermatitis. Among them four cases stopped using this product. In three cases the periocular skin lesions were worsened as the other part of AD lesions were aggravated. The one subject developed irritant dermatitis when she applied the product on her erythematous lesions in the periocular skin. The other could continue to use the product when she applied it in the periocular skin except eczematous lesion. Thus the safety ratio including safe to very safe cases was 91.1%. Based on these results we conclude that NOV EYEGEL® is a safe and helpful skin care product for the treatment of a dry and sensitive periocular skin.

    1. In order to study the utility of remicut® on chronic urticaria, 23 patients were enrolledin this study and treated with 4mg/day of emedastine difumarate for 2 weeks. The 22 patients within the subject of effectiveness assessment were analyzed. The results obtained were following; The final global improvement rate was 30.0%(6/20, marked improvement), 40.0%(8/20, moderate improvement), 10.0%(2/20, slight improvement), 15.0%(3/20, no change), 5.0%(1/20, aggravation) and 70.0%(14/20, “moderate” and more), 80.0%(16/20, “slight” and more). No side effect was observed, and the overall safety rate (safe) was 100%. On basis ofthese results, the utility rate were 70.0%(14/20, “useful” and more), 80.0%(16/20, “slight” and more).
    2. The improvement of symptoms and significant decrease in urinary N-methylhistamine (NMH) were observed in patients with high NMH (≥160ng/mgCr, n=13) in the analysis of 18 patients examined the concentration of N-methylhistamine (NMH) after and before the administration of emedastine. However, in the patients with low NMH, there was the improvement of symptoms but not significant reduce of NMH.
