    Primary culture cells from three cases of oral malignant melanoma were obtained and morphologically examined.
    The cells, which were round or spindle in shape, were scattered and did not form a sheet like primary culture cells derived from oral mucosa and oral squamous cell carcinoma.
    The electronmicroscopic findings of the cells were diversified from case to case. Desmosomes were not recognized in all the cases. Various kinds of melanosomes were found. Interestingly, in the primary culture cells derived from the case in which the tumour growth was clinically rapid, relatively immature melanosomes and swollen rough endoplasmic reticulum were found.

    Hydroxylapatite is a particulate bone substitute material that has been studied extensively for use in oral and maxillofacial surgery. This material, however, is limited in its application due to its particulate nature. At the time of placement it lacks and cohesive strength, and particles may be misplaced or migrated under externally applied forces. In light of these problems it seemed appropriate to develop a method for containing the particles during a period of time following implantation, until natural healing occurs.
    This paper presents tissue response and absorption of Vicryl®-Schlauch (Polyglactin 910) as containers in mandible of rabbits.
    Results obtained are summarized as follows:
    1. Vicryl®-Schlauch progressed to complete absorption by 13 weeks.
    2. The tendency to elicit tissue reaction was found to be minimal.
    Vicryl®-Schlauch containers for hydroxylapatite implantation were thought to be safe and efficient.

    In order to characterize the role of transferrin receptors (Tf. r.) in the process of 67Ga uptake, we investigated the relationship between 67Ga uptake and the number of Tf. r. in cultured cells (NRK-49F cells, NRK-52E cells). NRK-49F cells derived from fibroblast of normal rat kidney and NRK-52E from epithelial cell of normal rat kidney were cultured in Dulbecco's MEM medium. 67Ga uptake and cell numbers were counted by well type Scintilation counter and by the Coultor counter, respectively. The number of Tf. r. was analized by Scatchard plot. These two kinds of cell lines had two phases of cell proliferation, one was a proliferate phase and the other was a quiescent phase. In a proliferate phase, cells having higher proliferative activity had much more transferrin receptors than that in a quiescent phase. The amount of 67Ga accumulated in these cells in proliferate phase was also much more than that in quiescent phase, although the affinity constant of Tf. r. for transferrin was same value between these two phases. These results suggest that the ability of 67Ga uptake is well correlated with the number of Tf. r. per cell and with the activity of cell proliferation.

  • Part 2: Angular Measurments

    Ninety-six mandibular fossa of Japanese dry skulls were investivated with three-dimensional measuring apparatus. The results were as follows:
    1. On the average, the latero-medial line of the fossa was inclined to inside at an angle of 23.49 degrees from cranial basal view, the angle of formed with the lateral and the medial line of the fossa was about 86.62 degrees.
    2. From lateral view, the angle of lateral line of the fossa was 9.30 degrees, the angle of the medial one was 20.25 degrees.
    3. From lateral view, the angle of line from the deepest point of the fossa to the articular tubercle, that to midpoint of the eminence and that to the medial glenoid tubercle was 38.85, 46.54 and 54.06 degrees respectively.
    4. According to Angle's classification, observation from the lateral view, the angle of line from the deepest point of the fossa to mid point of the eminence was class III, class I and class II in order of steep. On the other hand, the angle of line from the deepest point of the fossa to articular tubercle was class II, class I and class III in order of steep.

  • Implantation into Mandibular Continuity Defects

    This study investigates the tissue response of dogs to mandibular reconstruction of continuity defects using hydroxyapatite blocks. Four dogs were subjected to reconstruction of mandibles with continuity defects using hydroxyapatite blocks.
    Two months after extraction of PM1, PM2, PM3, PM4, M1, M2 and M3 from the mandibles of adult dogs, two stainless-steel bone plates were positioned buccally and lingually. An impression of the area was taken at the same time in order to decide the area of resection and to prepare the porous hydroxyapatite blocks. One month after the first operation, a vertical segment of bone measuring 15 mm was removed from the mandible, and the segmentally resected mandible was reconstructed using the prepared porous hydroxyapatite block and bone plates. The extraoral approach was used for mandibular exposure in both operations.
    Both specimens were retrieved two months after implantation of the porous hydroxyapatite block. Undecalcified sections were prepared for light microscopic observation.
    All wounds healed uneventfully and no implant became infected, exposed or displaced at the time of sacrifice.
    There was good union of implant to the mandible. Bone ingrowth was present throughout most of the implant pores. Additional bones continuous with the mandibular bone could be seen in most regions surrounding the porous hydroxyapatite block.
    These findings suggest the usefulness of implantation of porous hydroxyapatite blocks in the reconstruction of mandibular continuity defects.

    Serine protease modifier was purified in male mouse submandibular gland using bovine pancreatic trypsin coupled Sepharose 4B affinity chromatography, ion exchange chromatography, gel filtration and chromatofocusing with DfB system. Purified modifier gave the single band in non SDS-polyacrylamide gel electrophrosis and a single peak in HPLC for gel filtration and pI was 9.4, though separated in SDS-polyacrylamide gel electrophrosis into two components, 31KD and 16KD which probably indicate the relationship between monomer and dimer. This molecule has inhibitory coefficient at 0.012 μM to epidermal growth factor binding protein. This inhibitor might act participate in the regulatory system of processing biological active peptide including epidermal growth factor in the mouse submandibular gland.

    Recently spontaneous periodontitis has increased in dogs, especially pets. Histological observations were made on 11 Pomeranian dogs (4.5-9.4 years old). Total missing tooth rate was about 37 %(maxilla 43.2 %, mandible 31.4 %) and showed high average in incisors, retromolars and maxillary premolars. There was no missing tooth in canines and mandibular first retromolas. In advanced periodontitis, the pocket epithelium formed complex rete ridges and migrated into the apical region with keratinizing covering epithelium. Numerous epithelial islands were observed in the scar tissue of the tooth missing region.

    : W e investigated the characteristics of osteoblastic and fibroblastic cells (control) and also elucidated the effect of addition of ascorbic acid on the composition of crystals. We used stable ascorbic acid, L-ascorbic acid 2-phosphate (P-Asc), because ascorbic acid is extremely heat labile under tissue culture condition at 37°C. Human periosteal cells (KY-20, YT-48, SN-29), osteosarcoma cells (JOS) and human skin fibroblasts (HSF) were cultured in a Dulbecco's Modification Eagle's Medium supplemented with P-Asc (0, 1.0, 2.0, 3.0, 4.0, 5.0 mM), 2 mM α-glycerophosphate, penicillin/streptomycin (100 unit/ml)_and 10 % fetal calf serum. Proliferation of these cells were significantly stimulated by the presence of PAsc (dose dependence). The cell sheet consisted of epithelial-like and spindle cells, about at day 7 after confluent. These cells produced mainly type I collagen and small amounts of type III collagen determined by sodium dodecyl sulfate polyacrylamide gel electrophoresis (SDS PAGE). Osteocalcin and high alkaline phospatase activity were also detected. Theyhad also the ability to calcify.
    The crystals consisted of calcium and phosphorus, under the condition of P-Asc 0.1-2.0 mM, but they were calcium oxalate with P-Asc 3-5 mM, determined by micro infrared spectroscopy.
    Human skin fibroblastic cells (HSF) had also the similar characteristics. The result indicated the ectopic calcification.

    The present investigation was undertaken to compare changes of the soft tissue frontal view between the bilateral sagittal osteotomies of the rami and the unilateral sagittal osteotomy of the ramus combined with body ostectomy by using the frontal face asymmetric correction index in 26 patients with mandibular protrusion assosiated with lateral deviation.
    The following results were obtained:
    1. Although the frontal view was improved in both osteotomies, the extent of improvement after correction of unilateral sagittal ramus osteotomy with body ostectomy was superior to that of bilateral sagittal ramus osteotomy in all measurements.
    2. As compared to bilateral sagittal ramus osteotomies, unilateral sagittal ramus osteotomy combined with body ostectomy showed significant improvement in upper lip inclination, asymmetric correction indices of the area of the chin, the distance from the submandibular line, and the mandible angle.
    The results suggest that the combination of unilateral sagittal ramus osteotomy and body ostectomy is a better surgical procedure for the treatment of asymmetric face patients.

    Multinucleated giant cells appearing in the mucous cysts of two cases were studied histopathologically, histochemically, immunohistochemically, and electron microscopically with the result that they were found to be made up by the fusion of a number of macrophages. This formation would be related with lipids in the blood. It is considered that multinucleated giant cells had changed their character due to their supernumerary phagocytosis, and already made the function of phagocytosis disappear. We thought that macrophages also were changed, and would be able to particiate with the formation of multinucleated giant cells.

    Blood group ABH and Lewis substances of the tissues from submaxillary and sublingual glands, pleomorphic adenoma and adenoid cystic carcinoma were extracted by PBS and fractionated by 90 % phenol and ethanol precipitation. The distribution of blood group substances in tissues was evaluated by means of immunohistochemical procedure.
    1. From the PBS extracts of tissue homogenate, phenol-insoluble, phenol-soluble and 10% ethanol, and 20 % ethanol precipitate fractions were obtained. The phenol-insoluble fractions were the main properties of the extracts. The differences of the yield between the tissues of adenoid cystic carcinoma were lager than that of submaxillary and sublingual glands, and pleomorphic adenoma.
    2. The preparations from most secretor samples contained large quantities of ABH and Leb substances and those from non-secretors contained Lea substance. The phenol-insoluble fractions showed higher blood group ABH and Lewis activities than those of the phenol-soluble fractions by hemagglutination inhibition tests. The activities of pleomorphic adenoma were similar to those of submaxillary glands and were more active than sublingual glands and adenoid cystic carcinoma. Some preparations of the pleomorphic adenoma and adenoid cystic carcinoma from secretor individuals had weaker blood group Leb activity than Lea activty and some of the adenoid cystic carcinoma showed no ABH and Lewis activities.
    3. In the tissues of the submaxillary and sublingual glands, mucous and duct cells were selectively stained by anti-Lea and anti-Leb antibodies. Serous cells of the same tissues were negative. Moreover the staining reactions to anti-Lea and anti-Leb were demonstrated in duct like tumor cells or hyperkeratic epthelial cells on the pleomorphic adenoma and adenoid cystic carcinoma

    Rabbits received intravenous injections of three kinds of drugs, mithramycin 200 μg/kg, vinblastine 3 mg/kg, and doxorubicin 3 mg/kg. These drugs were administrated once a day for 3 days continuously. The rabbits of the experimental groups were then compared with those of the control. The results were as follows.
    (1) After administration of the drugs, all showed hypocalcification of the dentine, judging from hematoxilin-stained samples and microradiogram findings. They also showed loss of growth and formation, and I concluded that vinblastine inhibited dentine formation and that doxorubicin inhibited growth.
    (2) Both mithramycin and vinblastine significantly decreased serum total calcium and serum ionic calcium compared with the control. Mithramycin was more effective than vinblastine in this respect, but doxorubicin caused no change. None of them had any effect on the serum phosphorous level.
    (3) I studied electronmicroscopic changes in odontoblasts 6 hours after mithramycin was administred and found damage, degeneration and structural collapse. Not only mithramycin but doxorubicin also caused damage, especially in the dental process area and nucleolus. On the other hand, vinblastine remarkably decreased the number of microtubles until I could find fewer in the odontoblasts.
    (4) The serum mithramycin concentration had decreased considerably half an hour after administration. But the concentration of doxorubicin was lower, and disappered more rapidly.
    (5) From these results I assumed that mithramycin and vinblastine caused hypocalcification of dentine because of the damage, and they caused a decrease in serum calcium. Doxorubicin caused hypocalcification of dentine because of the odontblast damage.

    Incidence and clinical observations of the gingival hyperplasia induced by calcium-channel blockers: Nifedipine (NF), Dilitiazem (DT), Nicardipine (NC) and Verapamil (VP), in the treatment of angina pectoris and hypertension were statistically studied in 218 outpatients (males 114, females 104) exclusive of edentulous persons.
    1) The incidences of gingival hyperplasia induced by NF, NC, DT and VP were found in 6.5%, 0%, 2.1% and 0% respectively.
    2) The lesions more often appeared in males than females; the age incidence was observed higher in age group 40 to 60 years than the age group of over 60 years, and diabetes mellitus was complicated in over 37.5% of the patients of gingival hyperplasia.
    3) A good correlation between the patient's periodontal index (Russel's) and the degree of gingival hyperplasia was observed, while OHI-S, daily dosage and duration of treatment were not associated with the hyperplasia.
    4) The stomatitis incidence was 9.3% in NF-, 3.8% in NC-, 16.7% in DT- and 11.1 % in VP-groups.
    It was suspected that such development of gingival hyperplasia induced by the Ca-drugs was exaggerated with chronic gingivitis and diabetes mellitus.

    Odontogenic keratocyst has been reported by many authors with interest of its specific histopathological features and clinical aspects. It is characterized by the presence of a thin, band-like parakeratinized or orthokeratinized stratified squamous epithelium with a prominent basal layer composed of either columnar or cuboidal cells and a connective tissue wall, usually free of inflammation.
    The clinical and histopathologic features of 10 cases of odontogenic keratocysts seen in our department 1977-1990 were investigated. We especially considered about the effect of the irrigational method. In this study, keratinized epithelium was not observed after irrigation of cyst cavity, with irrigational solution contained antibiotics. Recurrences have not been observed in these cases, up to now. It is suggested that the irrigational method is an effective treatment on odontogenic keratocysts, at the point of inhibition of keratinization and growth of cyst.

    Benign lymphoepithelial lesion (BLL) is a salivary gland enlargement characterized histologically by atrophy of glandular parenchyma and lymphocytic infiltration with myoepithelial islands. The term Mikulicz's disease (MD) is often used as a synonym of BLL, but the cause of this disease is unknown. W e encountered two cases of BLL and reviewed 22 cases of BLL or MD in Japanese literature.
    Case 1: 50 year-old-man had a painless nodule at his right submandibular region, but it had not given rise to xerostomia and its sialogram was normal. Grossly, the excised specimen was 36 mm at greatest diameter, yellowish white in color. Microscopically, the tumor was encapsulated and consisted of lymphocytic and epithelial elements, marginal sinus exist in periphery.
    Case 2: 88 year-old-man noted a painless swelling of left hard palate. W e suspected salivary gland tumor and excised. Histologically, the lesion consisted of a lymphoid tissue containing duct epithelium with periductal fibrosis.
    We recognized that case 1 was the reactive hyperplasia of lymphnode involving salivary gland, and case 2 was chronic sialoadenitis based on local inflammatory causes.
    24 cases of BLL or MD reported in the Japanese literature involving our two cases were analysed. 9 cases reported as BLL were solitary lesions and 15 cases of MD were symmetrical multiple lesions. Many multiple lesion cases were accompanied by xerostomia and/or immunological abnormality. But should distinction made between MD and Sjögren's syndrome at present, because chief symptom of MD is enlargement of salivary grands, but the latter is sicca syndrome.

    A rare case of localized type of progressive scleroderma is reported. A 62- year-old woman had complaints of immigrating swelling and sclerosis of the facial skin. Swelling originally occurred on the right mandibular angle region. The swollen region became gradually sclerotic and the skin lost elasticity. This sclerotic change then developed in the upper region, and finally reached around the right orbite. At first visit time, sclerosis and swelling were prominent on the lower palpebra, and impairement of elastic mobility and atrophy of the right side facial skin were remarked. Microscopic observation revealed that collagen bundles in the pars reticularis were somewhat thick and interfasticular spaces narrowed almost to abscence, and that mononuclear cells slightly infiltrated around small blood vessels. Laboratory examinations showed no specific abnormalities. The patient was treated with Predonine, one of the corticosteroids. Thirty mg/d initially was administrated, and the dose was gradually decreased corresponding with the skin condition. In about 3 years, rebound of the lesion occurred twice. Good skin condition without swelling and sclerotic change was finally obtained and maintained with Predonine 5 mg/d. In the course, antibodies and elevation of immunoglobulins which may be usually associated with autoimmune diseases could not be detected. Functions of white blood cells such as NK, LAK activity, lymphocyte blastogenesis and neutrophile O-2 generation were fully normal, however NK activity and O-2 synthesis temporally dropped at the time of high dose administration of Predonine for the rebound.

    We studied a case of painless tumor which occurred on left mental nerve of a 19-year old male patient.
    Under local anesthesia, the lesion was resected as a mass together with mental nerve. There has been no sign of recurrence for 8 years after operation up to now, and the patient is in satisfactory condition.
    Histologically, the lesion was cicatricial fibrous tissue enclosing nerve fiber fascicles and resembled Morton's neuroma.

  • Questionnaire Evaluation of Symptoms Related to TMJ Dysfunction

    The purpose of this study was to identify and analyze the relationship between TMJ sounds and TMJ related symptoms in nonpatient first-year university students.
    The subjects of this study were 180 students who had TMJ sounds detected by 5 dentists out of 2108 first-year university students (average age: 18.4 years old).
    132 students (77 men and 55 women) among the 180 students (106 men and 74 women) answered a questionnaire and more detailed analysis was done.
    The results were as follows:
    1) 68.9% of the students claimed more than one TMJ related symptom.
    2) The students were divided into 5 groups according to occurring side and frequency of TMJ sounds. The frequent bilateral sounds group and occasional bilateral sounds group showed significantly high incidence of TMJ related symptoms compared with others.
    3) 45.4% had noticed TMJ sounds before this investigation and they claimed significantly more frequent TMJ related symptoms.
    4) Most students (88.6%) did not remember the cause of their sounds. Only 15 students (11.4%) reported the causes, 6 of these cases were caused by. spontaneous TMJ pain or trismus, 6 students by an external trauma and 3 students by an intrinsic trauma mixed with an external trauma.
    These results revealed that half of first-year university students who had TMJ sounds had not noticed and suggested that most TMJ sounds could be caused by a spontaneous episode or an intrinsic trauma.

    Malignant small round cell bone tumors are often difficult to definitively distinguish by light microscopy alone. The present case is a rare Ewing's sarcoma that originated from the zygomatic bone. The patient, a 42-years-old man, was admitted with complaints of swelling and spontaneous pain in the left zygomatic region. Radiologic examination revealed the highly destructive zygomatic bone and maxilla involved by tumor filled in infratemporal fossa. Partial maxillectomy was performed after irradiation and chemotherapy treatments. No sign of recurrence has been seen.
    Surgically removed specimen was whitish with gelatinous cut surfaces. Histological examination revealed a non-encapsulated tumor with adjacent periosteal new bone. The small and round tumor cells had a scant and faintly eosinophilic cytoplasm with PAS-positive glycogen granules containing round or oval nucleus having coarse or fine granular chromatins. The tumor cells showed an invasive proliferation forming rosette-like figures. The immunohistochemical analysis showed positivity for vimentin, fibronectin, neurofilament and neuronspecific enolase. Electron microscopic examination confirmed the tumor cells possessed nuclei with finely divided chromatins and cytoplasms with abundant glycogen. Moreover, intercellular connections of desmosomal type were infrequently observed. These results supported the neuroectodermal origin of Ewing's sarcoma.

    For diagnostic imaging of temporomandibular joint (TMJ) disorders, 1.5 Tesla superconducting magnet system was applied in 65 patients with 75 joints. MR imaging was performed with GE-Signa 1.5 Tesla MR imaging system with the body coil used as the transmitter and 3 inch surface coil as the receiver. For detecting T1 weighted imaging, the following parameters were used; TR: 1000, TE: 25, Excitation: 1. Data matrix: 256×128, FOV: 8 cm, Slice thickness: 3 mm. For taking T2 weighted imaging, TR: 2200, TE: 80 was used.
    For anatomical interpretation of TMJ, 3-mm thick images with a 1-mm interspace sagittal multiplane with TR: 1000/TE: 25 sequence was useful, and additional coronal images were beneficial for examining condylar morphology as well as mediolateral disk position. T2weighted imaging obtained the joint effusion suspected of intra-articular inflammation. These findings suggest that MR imaging is useful in the detection and evaluation of TMJ patho-morphology.

    I encountered a 29-year-old man, who had a radiolucent and radiopaque mass in the 5-7 mandible. The mass was diagnosed historogically as cementifying fibroma after reexaminating different diagnoses of cementifying fibroma (at the biopsy's time) or ossifying fibroma (at the enuclation's time). Historical characteristic of the mass is as follows: the size is mostly oval or lump, the property is mostly cement, the cellular component is few, and the connection to tooth root is recongnized.

    Fifty-two subjects with restricted translation of the condyle and degenerative joint disease of the TMJ were operated by arthroscopic lysis and lavage into superior joint compartment of the TMJ. These subjects were diagnosed to 26 joints of anterior disc displacement without reduction and 4 joints of non disc displacement with adhesion and 16 joints of osteoarthrotic changes by pre-operative imaging procedures (Double-contrast arthrotomography or MRI).
    Following surgery, subjects were evaluated for 2 to 29 months by clinical examinations.
    Retrospective analyses were resulted in the followings.
    1) In range of opening motion, mean data of pre-operative conditions were 31.4 mm and these were increased to 40.6 mm post-operatively. (P<0.01, t0=10.41)
    2) Functional pain of the TMJs were decreased from fourty to twenty cases post-operatively. (P<0.05, x2=15.04)
    3) Crepitations of the TMJ were increased from five to thirteen cases post-operatively. (P<0.05, x2=6.12)
    4) Inclusive post-operative results were graded to 19 cases 40%) of excellent, 17 cases (35%) of good, and 12 cases (25%) of poor by B. Sanders standards (1987).

    It is widely recognized that most impacted teeth are located in the region around the mandibular third molar and maxillary canine. There is, however, the rare case in which impaction occurs in the vicinity of mandibular incisor, first molar, or second molar. Here we describe a rare case of multiple impacted maxillary and mandibular second and third molars arising in 25-year-old and 28-year-old brothers. The younger brother had three impacted second molars and three impacted third molars, while the older had four impacted second molars and three impacted third molars. Neither brother had any systemic problems and both were healthy. Because the pattern of impaction in maxillary and mandibular regions was very similar between the brothers, the involvement of heredity in these cases is suggested. Further review of the literature regarding this type of impaction was made. Cases such as the above do not appear to have been reported previously.

    The aim of this study is to clarify the effectivity of conservative therapy for internal derangement with closed lock of the temporomandibular joint (TMJ), and to determine the factors which influence success or failure.
    The subjects were 63 patients diagnosed closed TMJ lock, confirmed in MR imaging, followed by conservative treatment. The patients consisted of 15 males and 48 females, with an average age of 30.4 years. Eight patients were afflicted in both TMJs. Conservative therapy was gradually applied as follows: 1. oral medication of anti-inflammatory drug and muscle relaxant for 2 weeks, 2. mandibular manipulation technique (including pumping manipulation), 3. pivot splint therapy.
    In the results, 35/63 patients (55.6%) were successfully unlocked by the conservative therapy.
    In each therapy, the successfully unlocked patients were 10 by medication, 10 by manipulation technique, and 15 by brace therapy. These factors of age, locking duration and symptom scores by questionnaire in success group were significantly smaller compared to failure group. On the other hand, postero-anterior dimension of the disk in MR imaging, distance and area of lateral excursion in MKG, performed preoperatively in success group were higher than those in failure group.
    In conclusion, to assess patients' pathology by evaluating these factors were considered to be significant for the indication and favorable term of conservative therapy for closed lock TMJ patients.

    The correlation of the shedding of herpes simplx virus type 1 (HSV-1) into saliva and the occurrence of herpetic lesion were studied in 55 healthy individuals and 125 hospitalized patients with antibody to HSV-1. The hospitalized patients consisted of 101 oral surgery patients (34-undergoing oral surgery, 19 oral cancer, and 48 orofacial fracture) and 24-immunocompromised patients in the 1-st inner medicine.
    Specimens were collected regularly for about 2 months from each individual.
    HSV-1 was isolated 10.9% of the healthy individuals, 41.6% of the patients undergoing oral surgery and 54.2% of the immunocompromised patients. The frequency of hospitalized patients was obviously higher than that of healthy individuals.
    The frequencies of herpetic lesion in the healthy individuals, undergoing oral surgery, oral cancer, orofacial fracture and immunocompromised patients were 16.7%, 15.4%, 9.1%, 22.2% and 38.5% respectively. The occurrence of virus lesions of hospitalized patients was not different from that of the healthy individuals, except for immunocompromised patients.
    In the relationship of the occurrence of lesion and the time of the shedding of HSV-1 into saliva, in 5 cases HSV-1 was shed before the lesion occurred, in 4 cases shed just as occurred, and 1 case shed virus after the lesion occured.
    These suggest that HSV-1 is already beginning to reactivate before the herpetic occurred.

    Bleomycin (BLM) and its analog (PEP) are glycopeptide antibiotics used for the treatment of oral cancer. It was reported that efficient breakage of DNA and pulmonary fibrosis caused by BLM were enhanced by the presence of reducing agents, superoxide anion radical (O2-). But no clear evidence was provided, especially for the condition of low concentration BLM. The author reports here on two investigations; 1. Effects of BLM and PEP on O2- production from human polymorphonuclear leukocyte (PMN). 2. Effects of BLM and PEP on free radical generation in cell-free system. For the method to measure free radicals, the author applied CLA-dependent chemiluminescence specific to O2- and ESR spin trapping technique highly sensitive to hydroxyl radical (·OH) As results:
    1. The production of O2- from PMN stimulated by OZ or PMA was remarkably enhanced by the addition of BLM and PEP. The enhancing effect of PEP is stronger than that of BLM. This effect was completely inhibited by the addition of SOD.
    2. Free radicals could be detected from two systems, (1) HPX+XOD→O2- and (2) Fe (II)+H2O2→·OH. Both BLM and PEP increased free radical generation in propotional to the concentration of drugs. The effect of PEP is about two times stronger than that of BLM.
    These results suggested that Fe (II) or Fe (III)-BLM complex in low concentrations reacted with free radicals to produce secondarily O2- or ·OH, which gave damage to the lung tissues.

    A 84-year old male patient with carcinoma was treated by radiotherapy and adoptive immunotherapy (AIT) with lymphokine activated killer (LAK) cells.
    On the first examination the ulcer was recognized in 4+6 region of the gingiva and the floor of the mouth and diagnosed as squamous cell carcinoma histopathologically, but no metastasis was recognized (T3NOMO, Grade II: WHO).
    AIT was overlapped with external and interstitial radiotherapy totaled 94 Gys through the program of the therapies.
    LAK cells were infused interarterically, near the A. facialis and A. lingualis. Primary and secondary infusion of LAK cells was performed, 4 times each with recombinant interleukin 2 (rIL-2; 3.5 × 105u). LAK cells were induced by rIL-2 700μ/ml, and anti CD3 antibody 25 ng/ml. For the primary infusion, collection of LAK cells was performed beginning 17 days after the culture was started. We obtained the LAK cells for, 4 infusions with only one blood-gathering. In the secondary infusion, we collected all LAK cells after 7 culture days, and repeated the infusion 4 times as above.
    Results: We recognized that primary cultured LAK cells showed higher LAK activity compared with secondary cultured cells, but they had the same level in natural killer (NK) activity. Leu 7+/Leu 11+ cells in LAK cells of the secondary culture were more recognized in comparison with the primary culture. Leu 4+/HLA-DR+ cells were recognized in primary culture more than secondary culture.
    Changes of peripheral blood lymphocytes between pre and postinfusion of LAK cells occurred. After primary infusion, both LAK and NK activity showed a tendency to decrease. After secondary infusion, LAK activity was increased, but NK activity was decreased. In addition, the number of Leu 7+/Leu 11+ cells were decreased remarkably, while helper T cells and IL-2 receptor positive cells were increased.
    Pretreatment immunohistochemical examination showed little T cell infiltration. However dominant infiltration of T cells was recognized during LAK therapy. A year later the original lesion had been controlled.

    We carried out clinical and statistical observations and treatment of 106 cases of external dental fistula in the Department of Oral Surgery, Kurmume University during the past 19 years and 9 months (1969, 4-1988, 12).
    Results obtained were as follows:
    1) External dental fistula total was 106 cases. The distribution among 49846 outpatients was 0.21% during 19 years and 9 months.
    2) 106 patients were composed of 57 males and 49 females. As to the age of patients, the 1 st decade and 5 th decades were more frequently seen.
    3) Cheek area (44.7%) was the commonest site of external dental fistula.
    4) The pathogenic teeth were observed more in the mandibular region than in maxillary region, especially frequently in the mandibular molars and canines.
    5) In the treatment of pathogenic teeth, most cases were treated through surgery, but 19 recent cases were treated through endodontics.
