    Recently, many mutagenic compounds have been isolated from pyrolysates of amino acids and cooked foods. These mutagens are aromatic heterocyclic amines, and Trp-P-2 (3-amino-1-methyl-5H-pyrido [4, 3-b] indole), MeIQx (2-amino-3, 8-dimethylimidazo [4, 5-f] quinoxaline) and IQ (2-amino-3-methylimidazo [4, 5-] quinoline) are three of them. The purpose of this study was to investigate histologically the changes of epithelial cells in the tongue which was excised twice and treated with Trp-P-2 or MeIQx or IQ. In all animals in Group 1, the tongue showed only hyperkeratosis. In 3 of 5 animals in Group 2, the tongue exhibited mild epithelial dysplasia changes. In 1 of 5 animals in Group 3 and in 2 of 5 animals in Group 4, the tongue showed slight epithelial dysplasia changes. The present investigation suggested that Trp-P-2 or MeIQx or IQ treatment in combination with two excisional wounds could produce the epithelial dysplasia in the tongues of hamsters.

    Many investigators have studied the causes of temporomandibular arthritis (OA) in relation with aging, tooth loss, wearing, and derangement of the disc. However, pathogenesis of OA is not clarified ; especially, there is disagreement between the investigators about whether the posterior tooth loss has an influence on disease or not. Because of the disagreement, there is significant correlation of aging with tooth loss and OA appearance.
    At this time, we compared the number of lost teeth of normal skulls (153 skulls, 356 joints) with OA group (50 skulls, 69 joints) showing over five points of score of condylar surface changes.W e divided the group into a group of 0-39 years old and over 40 years old group for accomplishing to exclude the effects of differences of number of lost teeth by aging.
    As a result, the loss of two molar teeth effect to development of OA in 0-39 years old human. Although, we could not decide whether the loss of molar in the group over 40 years old were causes of OA or results of aging, no missing of molar might be featured by the normal group. We could not state that loss of molar teeth is an indispensable condition of OA in all humans.

    An experimental bone substitute, composed of a chitosan-bonded hydroxypatite paste was evaluated with regard to its osteoconductive properties.
    Tissue responses to subcutaneously embedded paste was studied, in fifty adult female SD rats. In the area around the embedded paste, a cell-rich granulation tissue composed mainly of fibobqlasts and macrophages and a small amount of collagen bundles was observed, however, there was no encapsulation by the fibrous tissue nor was there any sign of necrosis or degeneration elicited by the paste. An eosinophilic osteoid-like non calcified matrix was present in the granulation tissue. Furthermore, no abnormalities were detected macroscopically in any part of the whole body of any of the experimental rats during the course of the experiment.
    Evaluation was made using thirty-seven female adult rabbits. Approxmately O. 5 g of the paste was embedded directly on the surface of the shin bone of each animal. Two types of specimens were used for control, one receiving only removal of the osteomembrane, and the other receiving no treatment. Radiographic examination revealed a bone-like irregular radiopacity appearing in the region of the embedded paste. This was judged histologically as a bone formation, elicited by the paste, composed of many slender trabeculae and containing capillarly-rich stromal granulation tissue. In some instances, irregular chondral tissue was formed. The embedded paste was gradually replaced by cell-rich granulation tissue as the process of bone formation progressed over time.
    In conclusion, these data suggest that the paste has osteoconductive properties, and may therefose prove clinically useful as a bioactive bone substitute.

    Fifty-nine closed lock cases of internal derangement of temporomandibular joint were studied on the correlation between the mandibular tracking data and clinical symptoms consisted of opening degree, visual analogue scale of pain (VAS), pain score, and jaw dysfunction score. Results obtained were as follows ;
    1. Mandibular jaw tracking date in patients with internal derangement of the temporomandidular joint with closed lock correlated with the opening degree.
    2. The degree of the subjective symptoms in jaw dysfunction and pain does not clearly correlate with the range of mandiblar motion.
    3. The decrease in maximal and average mandiaular closing velocity does not correlate with degree of the pain.
    4. The decrease in maximal and average mandibular closing veloity correlates with degree of the subjective symptoms in jaw dysfunction.

  • Part II: Acoustic Characteristics Obtained from Asymptomatic Subjects

    The purpose of this study was to evaluate tne acoustic characteristics of TMJ sounds in asymptomatic subjects. The TMJ sound samples obtained from 28 asymptomatic subjects (28 joints) and 21 subjects with craniomandibular disorders (21 joints) were acoustically analyzed.
    All samples were measured using both time domain analysis and spectral characteristics by one octave band analysis. The results were as follows.
    The maximum amplitude in time domain were significantly lower in asymptomatic subjects than in symptomatic ones.
    The one octave band having the maximum level in asymptomatic subjects showed significant correlation between the opening and closing sounds, whereas that in symptomatic subjects did not show the correlation, The numbers of one octave band within 4 dB of the maximum level for closing sounds was significantly more in the asymptomatic samples than in the symptomatic samples.
    According to these results, it was concluded that the asymptomatic sounds had the lower maximum amplitude in time domain, high correlation in the maximum level in spectral characteristics between the opening and closing sounds, and no resonance in closing sounds when compared to the symptomatic sounds.

    Mucous cyst occurs most frequently on the lower lip, but rarely occurs on the upper lip. Histologically, this lesion is classified as (1) an extravasation cyst, or (2) a retention cyst. The extravasation type is far more common than the retention type.
    We experienced a very rare case of retention cyst of the upper lip in a 62-year-old male. Microscopically, the cyst was lined by pseudostratified ciliated epithelium with goblet cells. Hence, the present cyst seemed to be a result from an obstruction of the duct of a labial salivary gland.

    A case of gustatory sweating following excision of lymphoangioma in the submandibular region, was reported.
    A 45-year-old male had complained of right submandibular swelling. About 10 years ago, he was incised by a otolaryngolist, to relieve swelling in the same area.
    After clinical investigations, the lesion was diagnosed as a benign tumor of the submadibular gland. Under the general anesthsia, the lesion was removed with the submandibular gland. Histopathological examination revealed a picture of lymphoangioma.
    A benign, congenital proliferation of lymphatic tissue is termed lymphoangioma. This tumor usually is discovered in children. But, the case report of this tumor in an abult is rare.
    About 7 months after the operation, the gustatory sweating of the operation area with an approximate that of 2.5×1.5 cm had occurred. The case report of gustatory sweating following submandibular gland excision was unusal. Based on the phenomenon in this case, it was considered that submandibular gland secretory nerve fibers were misdirected to the sweat glands of the covering skin during healing after the surgery.

    The mycological effect and therapeutic usefulness of MJR-1762 were evaluated in 26 patients with oral candidiasis in whom this agent was applied to the oral cavity at a dose of 5 g four times a day. Underlying diseases were found in only 5 cases. Most of the patients were artificial denture wearers. The palate and tongue were the most frequently involved sites of infection. Fungi were also isolated from alveolar and buccal mucosal tissues. Candidiasis was caused by single infection with C. albicans in 11 cases and with C. glabrata in 3. Double infection with C. albicans and C. glabrata was detected in 8. Application of MJR-1762 produced its effect in 24 out of 26 patients. The disappearance of pathogenic fungi was confirmed in all the cases in whom the therapeutic effect was rated as satisfactory. A disagreeable sensation in the palate was complained by 1 patient, but the relation between this side-effct and MJR-1762 was unknown. These results confirmed the usefulness of MJR-1762 in the treatment of oral candidiasis.

    In 1882, Recklinghausen first described multiple neurofibromatosis as a disease charactesized by tumors of the skin and peripheral nerves. Recklinghausen's neurofibromatosis is an autosomal dominant inherited syndrome defined. by the presence of multiple cafe-au-lait spots, benign neurofibroma. Oral involvement is unusual in cases of von Recklinghausen's neurofibromatosis. Although the most obvious lesions occur on the skin, deeper structures also may be affected including the kidney, bladder, larynx, heart and central nervous system. Clinical manifestations are protean, many being clearly related to the local effects of the neurofibroma, but others having a less certain origin.
    We experienced two patients who had been referred to our clinic for exact examination of intraoral swelling. They were both histopathologically diagnosed von Recklinghausen's disease associated with oral lesion.

    We experienced a case of osteomyelitis of mandibular bone in Kimura's disease. The patient received radiation therapy to head and neck area against the same disease.
    Bone tissue which received radiation therapy had developed malnutrition as side effect of radiation, and osteomyelitis was induced by infection from the teeth.
    Even in the benign soft tissue disease like Kimura's disease, especially after radiation therapy, pathologic fracture of bone may happen due to bone damage caused by radiation.

    Fifty-six cases, diagnosed as squamous cell carcinoma of the mandibular gingiva, have been examined radiographically in an attempt to explain the relationship between modes of bone resorption and prognosis.
    We investigated radiographs on the marginal figures and the shapes of bone destruction, entrapments of bone and bone reactions.
    The results obtained were as follows
    1) Of the 56 patients, 47 (83.9%) showed radiographic evidence of bone involvement.
    2) According to the mode of bone resorption, the outlines of the radiolucent lesions were classified into well-defined type (40%), ill-defined tyye (24%) or irregular type (36%) and the shape of bone destruction was classified into well-defined type (44.7%), diffuse (permeative or infiltrative) type (44.7%) or moth-eaten type (10.6%).
    3) The entrapment of bone was detected in 28% of cases with bone involvement.
    4) The incidence of bone reaction was 11%.
    5) Well-defined outline or well-defined shape of bone destruction tended to have little recurrence and had good prognosis.
    6) Carcinomas that had ill-defined outline or moth-eaten appearance of bone destruction tended to recur often.
    7) Metastases after operation often occurred in patients with irregular type of outline or diffuse shape of bony destruction. Especially in irregular type, the incidence of metasasis was 66.6% and survival rate was 26.7%.
    These results suggest that the modes of bone resorptions reflect some of the characteristics of carcinoma and the present classification is helpful for planning the therapy.

    Induration, abcsess formation and trismus are typical symptoms of actinomycotic infection. Recently we seldom, if ever, find a typical case. Therefore the short dosage of antibiotics in its early stage complicates its symptom, leading to some untypical cases. Now we present a case with tumor-like and untypical actinomycotic infection. The patient was a 54-years-old male. He complained to pain free swelling in the left submandidular region. At first we diagnosed him as non-specific infection in submandibular and submental region. Although we administered antibiotics, the swellings increased. Then we supected it to be some kind of malignant tumor. Therefore, we performed surgical incision on his submenton discharging the pus. Final diagnosis was decided as actinomycotic infection by staining the specimen and pathological examination. Our new antibiotic therapy which consisted of dropped intra-venation of Cefapirin and Ampicillin. The following treatment was per os dosage of Ampicillin, for 1.5 months. Finally, we the longstanding symptoms of the patient disappeared and he returned to his former social living till now.

    A total of 80 patients of internal derangement of temporomandibular joint diagnosed by arthrogram, who visited Oral and Maxillofacial Surgery I of Tohoku University over a 4-year period from 1986 to 1990, were clinically and statistically observed.
    1. By sex, the ratio of males to females was 1: 7.9, and the predominance of female patients was significant.
    2. Distribution of age had two peaks in the twenties and thirties.
    3. Among the initial symptoms, temporomandibular joint noise mostly occurred.
    4. The most common chief complaint was temporomandibular pain or difficulty in opening the mouth.
    5. In most patients the period from the onset of temporomandibular joint symptoms to the initial visit to a clinic was more than two years.
    6. As the mouth opening width, 30-39 mm was most common, followed by 20-29 mm.
    7. About half of the patients experienced muscle pain.
    8. Class I and Class II of canine relationship were frequent. And class I of molar relationship were most frequent.
    9. Distribution of overbite had two peaks in 2-4 mm and more than 4 mm. Distribution of over et was the same.
    10. Class I of Wits appaisal was most frequent.
    11. Patients of internal derangement without reduction were compared with those of internal derangement with reduction, in age, the period from the onset of temporomandibular joint symptoms to the initial visit to a clinic, and mouth opening width.

    Angiokeratoma is reported in a relatively low incidence in the field of oral surgery. A case of angiokeratoma of the tongue is presented in this report. An 8-year-old girl consulted our clinic with tumor formation in the tongue dorsum. This lesion was diagnosed clinically as a kind of arigioma and excised under general anesthesia. Histologically, cavernous dilatation of papillary vessels, irregular acanthosis, parakerathosis with elongation of rete ridges and encasing dilated vessels, and so-called “blood cyst” formation were observed. The pathological diagnosis was angiokeratoma. Postoperative course is not eventful.

    This report shows the moving ratio of orthodontic measuring points for the jaw deformity postoperative features prediction system worked out. This system gets three dimensional coordinates of the orthodontic measuring points from roentogenographic cephalograms taken from two directions (posterio-anterior view and lateral view), predicts the postoperative coordinates of these pointd from designed moving distance of dentition and moving ratio of these points, and draws up postoperative features to retouch the preoperative picture cephalograms on the video display monitor. To confirm the data of moving ratio of these points, the preand postoperative roentogenographic cephelograms of 17 patients, who were sagittal splitting method of mandibular ramus under the diagnosis of mandibular prognathism at the Second Department of Oral and Maxillofacial Surgery in the hospital attached to the Faculty of Dentistry, Osaka University. To compare the moving ratio of mandibular measuring points with that of “Ll”, it was shown that the moving ratio of “point B”, “pog”, “Me” which nearly locate the midline of the face, were 1.1-1.2 times as that of “Ll”, but that the moving ratio of “Go” and “L6” were scattered. To compare the moving ratio of the points on the lower third of the face, it was shown that the moving ratio on X coordinates of “Ms” was 1.3 times as that of “Me”, other points were 0.9 times as that of mandibular measuring points. On Y-distance, the distance and the moving ratio was greatlr increased. On Z distance, the moving ratio was 0.6-1.2 times. Postoperative predicted features were drawn up with these results and they simulated postoperative features very well.

    81 malignant tumor cases of the oral and maxillofacial regions were clinicostatistically observed at the Department of Oral Surgery at Dokkyo University Hospital from 1977 to 1988.
    From this study, we obtained the following conclusions:
    1) Of the 81 patients, 51 were male and 30 were female (1.7: 1), The patients ranged in age from 30 to 96 years, and the mean age was 65.4 yeacs.
    2) The primary tumors sited on the gingiva in 35 cases (43.2%), tongue 14 cases (17.3%), oral floor 9 cases (11.1%), buccal mucosa 7 cases (8.6%), maxillary sinus 6 cases (7.4%), hard palate 5 cases (6.2%), lip 4 cases (4.9 %), submaxillary 1 case (1.2%).
    3) Histopatopathologically, 67 cases (82.7%) were diagnosed as squamous cell carcinoma, 7cases (8.6%) as malignant tumors in the salivary gland, 2 cases (2.5%) as malignantlymphoma, 1 case (1.2%) as malignant melanoma and so on.
    4) According to the TNM classification (UICC 1978, JJC 1977), 77 cases of oral squamous cell carcinomas involving lip and maxillary sinus were classified as 13 cases (16.9%) of T1, 27 cases (35.1%) of T2, 22 cases (28.6%) of T3, 15 cases (19.5%) of T4. In the NM classification, cervical lymph nodal metastasis was found in 32 cases (41.6 %) and 3 cases (3.9 %) had distant metastasis to the lung. In stage grouping, 11 cases (14.3 %) were in stage I, 18 cases (23.4%) in II, 26 cases (33.8%) in Ill, 22 cases (28.6 %) in IV.
    5) 21 cases (25.9%) were treated by irradiation (R) alone, 17 cases (21.0 %) by surgery (S) +chemotherapy (C) + (R), 16 cases (19.8%) by (S+R), 11 cases (13.6%) by (C + R), surgery alone, 3 cases (3.7%) by chemotherapy alone and 2 cases (2.5%) by (S+C).
    6) The 5-year survial rate was 43.0%, and tumors (22 primary lesions, 3 cervical metastases and 8 remote metastases) and other diseases were responsible for 11 deaths.

    With the recent increase in the aged population, the number of senile patients requiring oral surgical treatment at our Hospital.is increasing.
    However, in the treatment of such patients, many problems can arise because most of them have some general complications.
    In this present report, we made a clinico-statistical study of senile patients (over sixty five years of age) who required hospital treatment in our department during the past ten years (from 1980 through 1989). The following results were obtained.
    1) Among the total 2839 cases of inpatients treated in our department during the above period, senile patients accounted for 10.2%(295 cases) of them.
    2) The kind of causative diseases of hospitalization and the number of patients with each disese are as follows orofacial infection: 148 cases (50.2%), benign tumor: 38 cases (12.9%), malignant tumor: 37 cases (12.5%), cystic lesion: 20 cases (6.8%), orofacial trauma: 12 cases (4.1%), miscellaneous: 40 cases (13.6%).
    3) Among the 295 dases, 243 cases needed for operation as treatment, but the remainder 52 cases did not.
    4) General complications besides the primary disease were found in 87.4% of senile patients. And they were mostly related to heart and circulatory system (87.4% of them).
    5) The mean period of hospitalization of senile patients was 27 .4 days.

    Gingival overgrowth, histologically characterized by increased connective tissue, has commonly been reported to be associated with diphenylhydantoin. Recently other medications have been reported to cause gingival overgrowth, such as cyclosporin A and nifedipine.
    This paper presents a case of gingival overgrowth induced by nifedipine. A 51-year-old male with a history of angina treated with nifedipine for approximately 4 months noted to have an exess of gingival tissue. Gingival overgrowth was observed mainly on the lower anterior gingiva. The patient was treated to improve his oral hygiene and the drug administration was stopped. After this procedure, gingival overgrowth was reduced remarkably except for the lower anterior region After these treatments, gingivectomy was performed at that site. Histopathological examination showed the proliferation of collagenous bundles and thick epithelial layer with prolonged rect pegs. Seven months later, all symptoms of the gingival overgrowth had disappeared.

    The diagnosis of herpetic infection is usually confirmed by the onset of eruption and lymphoadenopathy with other systemic symptoms.
    To use Acyclovir (ACV) for this disease after the clinical diagnosis of herpes infection, is ineffective. Therefore, ACV is very effective when given too fast. Recently, we experienced four cases of herpes primary infections among the same family with ACV tablets. One of them, a 6-year-old girl, visited our department with a fever.
    As we had treated other families with herpetic gingivostomatitis, she was administered oral ACV at once.
    ACV tablet of 200 mg was administered four times a day for five days.
    The diagnosis of this case was confirmed by viral isolation from the saliva.
    No other clinical symptoms were demonstrated and the fever left.
    We reported a case in which oral ACV has shown to be very effective for primary herpes infection.
