    Speech intelligibility test is frequently carried out for diagnosis of articulation disorder and evaluation of treatment there of.
    It was proposed that the test must be carried out objectively. Listening to subjects must be done by many people and listeners must not be used to hearing the subjects. However, this method is insufficient for objective evaluation because of the listener's acoustic and psychologic intermediary.
    Therefore, an attempt was made to evaluate speech intelligibility more objectively with software, which is for speech recognition, for personal computer (PC). The result suggests that PC is useful for evaluation of single-word intelligibility.

    A method to measure bone mineral of mandible has not been established.
    The bone mineral content (BMC) of the mandible with single energy quantitative computed tomography (SEQCT), which was compared with that of the spine, was discussed.
    The subjects were 104 healthy persons (54 males and 50 females, age range: 21-69) and 33 patients of mandibular atrophy (10 males and 23 females, age range: 46-87).
    The BMC changes of the mandible differed according to sex. In males BMC of trabecular bone and cortical bone decreased slightly after 40 and 30 years of age respectively. In females, BMC decreased consistently during menopause. BMC of the spine tended to decrease with aging, especially in females.
    In males having mandibular atrophy, the BMC of trabecular bone of the mandible decreased, and that of cortical bone of the mandible increased with aging. In females having atrophy, the BMC of trabecular bone and cortical bone of the mandible decreased with aging.
    The BMC of the mandible was correlated with the length of the denture-wearing time in males. In females, it appears that the BMC of the mandible participates in estrogen deficiency like the BMC of the spine.
    From the above, measurement of the BMC of the mandible by SEQCT was considered to be very useful for grasping the severity and progressive rate, and other conditions of alveolar ridge atrophy and determining the remedial course.

  • 1. Reflex Responses to Mechanical Stimulation of Tongue Mucosa

    The reflex control from the oral mucosa to the levator veli palatini muscle was investigated in decerebrate cats. Motor unit activities in the muscle were recorded electromyographically and mechanical stimulation was applied to the tongue dorsum.
    The results were as follows:
    1. Excitatory or inhibitory response was elicited in the levator veli palatini muscle by pressure stimulation to the tongue dorsum.
    2. The reflex thresholds from the tongue mucosa were bellow 3g at the tip of tongue, 8g at the body of tongue, and 5g at the root of tongue.
    3. Excitatory reflexes were elicited under low level background activity (BGA). The excitatory effect decreased with increase of BGA and the reflex effect changed from excitation to inhibition. Under high level BGA, the inhibitory effect was increased.
    4. Reversal levels were calculated from BGA-response curves. As stimulation sites were changed from anterior to posterior of the oral cavity, the reversal level tended to increase. This result indicated that the excitatory reflex was easily elicited from the posterior part and the inhibitory reflex was easily elicited from the anterior part of the oral cavity.
    It was suggested that the soft palate movements during mastication, swallowing and speech are ingeniously controlled by integrative reflexes of tongue mucosa stimulation.

    MRI findings of 13 patients with proven salivary gland tumors, including 7 cases of parotid gland tumors, 2 cases of submandibular gland tumors, and 4 cases of minor salivary gland tumors, were reviewed. All examinations were carried out on a 1.5-T MRI system (General Electric, SIGNA Performance plus & SIMAZU, SMT-150X) with spin echo sequence. T1 and T2 weighted images and Gd-DTPA enhanced T1 weighted images were obtained.
    The results were as follows:
    1. All tumors demonstrated low to intermediate signal intensity on T1 weighted images and intermediate to high signal intensity on T2 weighted images. The differences between malignant and benign tumors were not detected by means of the signal intensity of the tumors.
    2. Tumor margins were clear except for 2 cases, including inflamed pleomorphic adenoma of the submandibular gland and adenoid cystic carcinoma of the parotid gland.
    3. Except for the case of adenolymphoma, internal structures of the tumor are inhomogenous, especially on T2 weighted images.
    4. Although the intraparotid facial nerve is not detected directly, whether the tumor is located on superficial or deep lobe could be detected by drawing a line between the lateral surface of the posterior belly of the digastric muscle and the lateral posterior border of the mandible except for one case and using the retromandibular vein except for two cases.

    By using 3T3 cells as a feeder layer, normal submandibular gland cell culture was established. The cultured cells were PAS-positive and immunopositive for anti-amylase antibody. The cells also demonstrated secretion granules under transmission electron microscopy. The cultured cells showed acinic cell-like features morphologically. The effect of FGF-2 on these cultured cells was also examined. FGF-2 significantly enhanced the proliferation of cultured cells from culture day 4. The enhancement effect was peeked at the concentration of 100 ng/ml and reached plateau at higher concentration. In conclusion, by using 3T3 cells as a feeder layer and additing the optimal concentration of FGF-2 (100ng/ml), normal salivary gland cells could be cultured stably within short time.

    Although salivary gland cell culture has been used in many morphological and biochemical experiments, only a few reports have been published concerning its physiological functions. The normal submandibular gland cell culture using 3T3 cells as a feeder layer was established. The physiolosical functions of cultured rat submandibular gland cell were examined. Intracellular Ca2+ mobilization was investigated by giving muscarinic stimulation by calbachol. Intracellular Ca2+ mobilization in cultured submandibular gland cells was less than that of control cells. The reaction was increased as the calbachol concentration was increased up to 3μM. The decrease in number of receptors is considered to cause the reduction in responses.

    Radiotherapy is a useful and convenient therapy for oral cancer. However, there are many side effects such as stomatitis and radionecrosis of jaws. Radionecrosis may cause loosing or infection of biomaterials used for reconstruction of jaws. In this experiment, in vitro investigation was performed to clarify the influence of radiation on initial attachment of osteoblast-like cells to the titanium plate.
    UMR-106 and MC3T3-E1 cells were used as osteoblast-like cells. Cell attachment was evaluated by alkaline phosphatase activity and staining attached cells with crystal violet. The results revealed that initial attachment of osteoblast-like cells to the titanium plate was dose-dependently decreased by radiation and that radiosensitivity of each cell was different respectively. Furthermore, the participation of active oxygen was suggested because of partial recovery of cell attachment by addition of superoxide dismutase and/or an antioxidant such as ascorbic acid.

    The signal transduction of interleukin-2 (IL-2) was examined comparatively in CD3-negative natural killer (NK) cells and CD 16-negative T cells. NK cell-induced killing activity was greater than T cell-induced killing activity of lymphokine (IL-2)-activated killer (LAK) cells, and the killing activity of LAK was decreased to about 50-70 % of the control level by the addition of H-7 and pertussis toxin, and LAK induction was almost completely suppressed by herbimycin A and genistein. Western blot analysis revealed tyrosine phosphorylation of 105 kDa and 110 kDa proteins and 95 kDa and 110 kDa proteins in IL-2-treated NK and T cells, respectively, with rather remarkable phosphorylation in NK cells. This protein phosphorylation was not inhibited by antibody for the α chain of IL-2 receptor but completely inhibited by antibody for the β chain, which also inhibited LAK induction completely. Anti-β chain antibody also suppressed GTP-binding p 21ras (active type p 21ras) in both NK and T cells. In addition, activation of JAK3 by IL-2 was more prominent in NK cells than T cells, however, JAK1, JAK2, STAT 1α, STAT2 and STAT3 were not activated. These results indicate that IL-2 signal is transduced via IL-2Rβ chain, that IL-2 signal flows into the down stream via both ras-dependent and ras-nondependent pathways, and that the superior LAK activity in NK cells depends on their strong tyrosine kinase activity.

    Cleft lip and/or palate is one of the most common birth defects, and genetic factors contribute to the development of this disorder. Previous studies have reported an association between the transforming growth factor alpha (TGFA) gene and nonsyndromic cleft lip and palate in Caucasians. The HOX7 gene is also involved, because it was recently demonstrated that the HOX7 deficient mice exhibit cleft palate. An association study of two genes was carried out. Two alleles of TGFA TagI polymorphism (C1: 3.0 kb, C2: 2.7 kb) were detected using Southern method. The C1C1 and C1C2 genotypes were identified but the C2C2 genotype was not detected. A significant difference was observed in genotype distribution between cleft palate and controls (P=0.040), but not between cleft lip with or without palate and controls. The C1C2 frequency in Japanese controls was higher than those in Caucasians reported previously by three groups. No significant association was found between clefting and each of microsatellitic polymorphism, adjacent to the HOX7 gene. These results suggest that either the TGFA gene itself or an adjacent DNA sequence may contribute to the development of cleft palate in Japanese.

    The masticatory muscle silent period (SP) and jaw jerk reflex (JJR) were investivated on temporal and masseter muscles in a patient with hemifacial sensory paralysis due to localized metastatic brainstem tumor.
    Abnormal involuntary continuous contraction of closing muscles, which must not be found in the normal subjects, was observed on the affected side during examination. JJR of masseter and temporal muscles was absent on the affected side and had normal latency on the unaffected side. The tapping movement SP latency and duration and chin tap SP latency were significantly prolonged for both muscles on the affected side as compared with those of the unaffected side.
    The findings suggested that the SP circuit on the affected side, the afferent inhibitory impulse from the contralateral mechanoreceptor, being mediated by the polysynaptic circuit via the contralateral trigeminal sensory nuclei, probably project on ipsilateral trigeminal motor nuclei in this case.

    Hemophilia A is a blood coagulation disorder due to congenital deficiency of factor VIII. In oral surgery, substitution therapy and local hemostatic management is important. In this report, the clinical and statistical investigation on tooth extraction of patients with hemophilia A was demonstrated.
    Thirty-seven teeth extractions were performed in Nagoya University Hospital. The activity of blood coagulation factor VIII was classified as mild state in 5 cases, moderate state in 2 cases and serious state in 27 cases. Local hemostatic management was used by applying antifibrinolytics and acrylic splints in all cases. In tooth extraction of mild and moderate cases, it was possible to obtain a good hemostatic effect only by giving DDAVP (1-deamino-8-D-arginine vasopressin) before operation. In serious cases, substitution of blood coagulation factor VIII before tooth extraction was essential to obtain a good hemostatic effect.
    Many cases of hemorrhage after operation were observed when the posterior molar teeth were extracted. It can be concluded that it would be difficult to manage local hemostasis by acylic splints in these cases and an investigation on local hemostatic management must be carried out.

    A case of gas producing phlegmon in the deep neck region following periodontitis was reported. A 17-year-old woman who suffered from periodontitis of the left lower third molar underwent root canal treatment on March 2, 1993. Two days later, she had marked swelling of the floor of the mouth and the submandibular region with dyspnea. She was admitted to the Kagoshima Prefectural Ohshima Hospital on March 6.
    Computed tomography of the neck region revealed abundant gas in the soft tissue.
    A diagnosis of gas producing phlegmon of the neck region was made. She was immediately treated with tracheotomy, surgical drainage, and intravenous administration of antibiotics.
    On March 10, hematological examination revealed a bleeding tendency. Intravenous administration of heparin, gabexate mesilate (FOY), and AT Bit was started.
    She recovered uneventfully and was discharged on April 5.
    Early diagnosis, complete surgical debridement with removal of necrotic tissue and administration of antibiotics are thought to be most important in the treatment of gas producing phlegmon.

    A retrospective observation was carried out on the records of 316 tumors with a pathologically proved diagnosis of squamous cell carcinoma of the oral cavity in our clinic from 1976 to 1993. The aim of the study was to clarify clinical features of highly invasive carcinoma, which means mode 4C or 4D by Yamamoto's classification. Eighty cases (26 %) were classified as highly invasive carcinoma ; 49 of these 80 cases (16 %) were mode 4C and 31 cases (10 %) were mode 4D. The incidence of highly invasive carcinoma was low in tumors of upper or lower gingiva when compared with tumors of other locations in the oral cavity. According to the TNM system of UICC set in 1987, the rate of advanced T or N lesion in cases of mode 4C was higher than in cases of the other mode. However, cases of mode 4D among Ti or T2 lesions had the highest rate of N1-3 lesion. On the other hand, clinical stage for highly invasive carcinoma was different accordins to the location of the tumor; there was a tendency that the rate of an advanced stage was higher in tumors of buccal mucosa or gingiva when compared with tumors of the tongue or floor of the mouth. Regarding the growth pattern proposed by Washizu in highly invasive carcinoma, exophytic type was only seen in twelve cases, which belonged to T1 or T2 except for one case.
    These observations suggested that clinical behavior of highly invasive carcinoma might be not only different in mode of invasion (4C VS 4D), but also in location or growth pattern of the tumor.

    Psoriasis is a chronic dermatitis of undetermined etiology. The patient was a 60-year-old male who complained of pus discharge from maxillary frontal gingiva. The patient had been suffered from psoriasis vulgaris for 8 years, and there were many reddish papules in breast, abdomen, limbs and facial skin. Radiological examination showed a lowered maxillary left canine, periapical radiolucency of the maxillary right first premolar and horizontal loss of alveolar bone in maxilla and mandibular bone. After treatment of marginal periodontitis, the maxillary right first premolar and the impacted maxillary left canine were extracted. After these treatments, reddish papules rapidly improved and disappeared completely within 2 weeks after extraction. This clinical course suggests that psoriasis was caused by dental focal infection.

    A case of intramuscular hemangioma which caused trismus was reported.
    A 33-year-old woman was referred to our department on May 25, 1994 for assessment and treatment of trismus. For five years, she had recurrent trismus, which occasionally had been accompanied with pain and usually disappeared in a few hours.
    Clinical examination of TMJ showed no remarkable findings except slight tenderness on the left masseter and sternocleidomastoid muscles. In addition, a firm mass was palpable in the anterior portion of the left masseter muscle. Intraoral examination revealed no abnormal findings except posterior cross bite of left molars. CT and Tl-and T2-weighted MRI suggested the presence of the tumor with a calcified mass in the left masseter muscle. The trismus was considered to have been caused by the intramuscular tumor mass rather than TMJ itself.
    Total resection of the tumor was performed. The postoperative pathological examination showed hemangioma, and analysis of calcified mass was found to be the phlebolith. The clinical symptom disappeared, and there was no sign of recurrence after operation. The mechanism of trisumus caused by intramuscular hemangioma of masseter muscle was discussed.

    The term epulis implies a process localized to the gingiva, and it is used to denote tumor-like masses which have benign character, and almost all the epulises are attributed to inflammation or chronic irritation. Though they are not rare in the field of oral and maxillofacial surgery, there were few clinical reports of epulis. The clinical features in 102 patients of epulis referred to our hospital between 1984 and 1994 were examined.
    1. The 102 patients were classified as epulis fibrosa in 54 patients, epulis granulomatosa in 34 patients, epulis osteoplastica in 13 patients, and epulis hemangiomatosa in 1 patient.
    2. The 102 patients were 40 males and 62 females, and the sex ratio was 1: 1.55. The sex ratio of the patients of epulis fibrosa was 1: 1. 25, that of epulis granulomatosa was 1: 1.83, and that of epulis osteoplastica was 1: 2.25.
    3. Epulis fibrosa and epulis granulomatosa occurred most commonly during the sixth decade of life, but epulis osteoplastica occurred most commonly during the fourth decade of life.
    4. Anterior gingiva, especially of the maxilla, is the most common site of epulis fibrosa and epulis granulomatosa, but epulis osteoplastica arose equally on all sites of gingiva.
    5. There was considerable difference between epulis osteoplastica and other types of epulis in regard to the condition of the surface of tumor and the shape of tumor.
    6. Almost all the cases of recurrence arose on anterior gingiva.

    Mucous membrane pemphigoid (MMP, benign mucous membrane pemphigoid) is a relatively uncommon autoimmune disease which is characterized by vesiculo-bullous lesion of the mucosal surface. The diagnosis of MMP is on the basis of clinical and histopathological findings and immuno histochemical investigation.
    A case of MMP in the oral mucosa of a 68-year-old woman was reported. She visited our hospital with a chief complaint of contact pain of oral mucosa. Oral examination revealed erosive and bullous lesions on the palate and gingivae. Microscopic findings of biopsy specimen revealed subepithelial vacuolation and vesicle formation, and a mild infiltration of chronic inflammatory cells was also observed in the connective tissue. The immunoreactivity of anti-IgG antibody exhibited a linear band in the basement membrane zone.
    Although the oral lesions were resistant to topical application of steroid ointment and local steroid injections, topical steroid therapy with occlusal tray and iontophoresis were effective, resulting in satisfactory prognosis.

    A 32-year-old woman with schwannoma in the right upper neck region was reported. She had noticed the swelling of the right upper neck in November 1991 but did not visit a hospital for treatment because of painless swelling. The patient was referred to our department for swelling on May 12, 1995. The tumor was examined with ultrasonography (US) and magnetic resonance imaging (MRI) and diagnosed as a neurogenic tumor. The tumor was extirpated with the capsule, inclusive of the great auricular nerve under general anesthesia on June 24, 1995. It was spherical in shape, measuring about 22×16×16mm. Histopathological finding was schwannoma (type A of Antoni's pathological classification). Slight paresthesia of auricular region just after the operation was improved about 2 years postoperatively.

    An unusual case of osteochondroma was reported. In this case, a 35-year-old woman complained of recurrent luxation of the temporomandibular joint and asymmetry of the mandible. Xray photograph and CT scan showed an oval, radio-opaque tumor attached to the anterior surface of the right condylar process. Under general anesthesia, condylectomy and reconstruction with a titanium reconstruction plate condyle were performed. The tumor was attached to the anteromedial surface of the condyle, and measured 22×26 millimeter. Histological examination revealed a cartilage-capped overgrowth with cartilagenous ossification.

    Two patients with impacted teeth at the sites of the 2nd and 3rd molars of the maxilla and mandible on the right and left sides were reported.
    A 20-year-old male who came to our department because of delayed eruption at 17, 27, 37, and 47. Although partial eruption of 17 and 27 was observed, 18, 28, 37, 38, 47, and 48 were completely veneered with intact gingiva.
    A 28-year-old male who visited our department with a chief complaint of uneruption at the sites of 17, 18, 27, 28, 37, 38, 47 and 48 on the right and left sides. All these molars were completely veneered with intact gingiva.
    There are only 12 reported cases with impacted molars, including our cases, in Japan. Therefore, cases with multiple impacted teeth are considered to be extremely rare.
