    Aplastic anemia is characterized by a failure of blood cell production resulting in various degrees of pancytopenia, so patients with the disease show not only anemia but also thrombocytopenia. Dental extraction is one of the most popular surgeries; therefore, it is important to investigate the platelet levels and postoperative hemostatic measures. In this study, a clinico-statistical analysis was carried out on 18 patients with idiopathic aplastic anemia.
    The results were as follows:
    1. It was possible to obtain hemostasis after ordinary dental extractions without platelet transfusion if platelet levels were above 2.5-3×104/mm3.
    2. About 10 units of platelet concentrate were available for hemostasis after dental extraction in patients with platelet levels below 1×104/mm3.
    3. It was concluded that local hemostatic measure is important for these patients.

    Benign pleomorphic adenoma of the minor salivary glands and normal minor salivary glands were examined by immunoelectron microscopy to identify the distribution of cell marker antigens in neoplastic and non-neoplastic cells.
    1. In a normal salivary gland, actin antibody strongly reacted when myoepithelial cells were stained but GFAP antigen did not react in any cells. In the cytoplasm, a positive reaction of actin was seen myofilaments, mitochondoria and endoplasmic reticulum. In pleomorphic adenomas, actin antibody and GFAP antigen strongly reacted when epithelial cells of the outer layer of ductal structures were stained. These cells had irregular nuclei and cytoplasm containing densely packed filaments, often so closely packed that individual filament could not be clearly seen. They were not surrounded by basal lamina.
    2. Vimentin did not react when myoepithelial cells were stained, but it reacted when epithelial cells of the intercalated duct were stained in one case and strongly reacted when fibloblast cells were stained. Most of the spindle-shaped tumor cells in the myxoid area were positive for vimentin, and vimentin reacted when modified myoepithelial cells were stained in one case.
    3. There was a positive reaction of cytokeratin AE 1 /AE 3 when epithelial cells of the duct lining cells were stained. AE 1 /AE 3 was diffusely distributed over the cytoplasm and associated with fila-ments. In pleomorphic adenoma duct lining, there was a positive reaction of AE 1 /AE 3 when epi-thelial cells were stained, and AE 1 /AE 3 reacted when intermediate filaments and mitochondoria were stained. From these results, it was thought that these tumor cells derived from normal duct lining epithelial cells.
    4. Actin, GFAP, and vimentin in one case reacted when modified myoepithelial cells in pleomorphic adenoma were stained. It was thought that modified myoepithelial cells are immunophenotypically different from normal myoepithelial cells.

    This study was conducted to investigate the bone response after placement of implants in the tibiae of ovariectomized rats. Female Wistar rats, aged 12 weeks, were subjected to bilateral ovariectomy or sham surgery. Twenty-one days after surgery, hydroxyapatite-coated implants were placed in the proximal part of the tibia. Then the animals were sacrificed 7, 14, 28, 56, 84, and 168 days after implantation. Undecalcified specimens were prepared and examined by routine microscopy and microradiography. Histomorphometric measurements were obtained with a computer-based image analyzer to quantify the percentage of implant-bone contact and trabecular bone around the implant. In the cortical bone area, only a slight difference was noted between ovariectomized and sham-operated rats in bone formation around implants and implant-bone contact. However, newly-formed bone trabeculae in the medullary cavity of ovariectomized rats tended to decrease more rapidly than those of sham-operated rats. These results suggested that the application of dental implants to postmenopausal patients with increased bone turnover must be carefully managed with respect to decrease in bone trabeculae around the implant.

    Using 4 electromyographic parameters of root mean square (RMS) values, turns per sec., mean amplitude per turn, and mean frequency, surface electromyograms of the masseter muscle were analyzed at various levels of non-fatiguing isometric sustained contraction and at various times during the contraction.
    Nine asymptomatic volunteers participated in this experiment. They were asked to clench at three levels of comparably weak bite force (10N, 30N, and 50N) for 30 sec. Each clenching was divided into three parts (beginning, intermediate, and end), and electric activity for 0.3 sec. was extracted from each part of clenching time. The above 4 parameters were analyzed by two-way analysis of variance (two-way ANOVA) in classifications of the bite force and the clenching period.
    The two-way ANOVA revealed statistically significant differences in the RMS value, the turn per sec. and the mean amplitude per turn, but that of the mean frequency did not. Interactions between the two classifications showed no significant difference in all parameters. Moreover, intrafactorial difference in each classification was compared by Scheffe's multiple comparison. The RMS value, the turns per sec., and the mean amplitude per turn showed significant differences among almost all three levels of bite force and among almost all 3 parts of the clenching period.
    In conclusion, electrical activity of the masseter muscle gradually increased under non-fatiguing clen-ching as the bite force increased or the clenching time was lengthened. Moreover, since it is considered that the turns per sec. reflect firing rates of a motor unit of the underlying muscle and that the mean amplitude per turn reflects a size of the motor units, it was concluded that the turns and the mean amplitude per turn derived from surface electromyography may be useful for evaluation of the underlying muscle function, and that these parameters may directly reflect activity of the motor unit of the underlying muscle.

    Many surgical procedures for macrostomia had been advocated in past literatures, but it is hard to obtain the ideal commissure form. In this paper, 6 cases of unilateral macrostomia treated at our clinic are reported and the availability of the minor triangular flap on vermillion border in commissuroplasty is discussed.
    Five cases underwent new commissuroplasty using the minor triangular flap at vermillion border (Matsuya's procedure) and 1 case without it. In the latter case, deformation of the commissure form caused by postoperative scar contracture was found, though the good form was found during the post-operative period. In all cases using Matsuya's procedure, the good commissure form was made and maintained throughout the growth period. The minor triangular flap on vermillion border was considered to be useful to prevent the influence of postoperative scar contracture on the commissure form

    Thirty-three patients with primary oral and oropharyngeal squamous cell carcinoma were examined by esophago-gastrography and/or esophago-gastroscopy in order to screen simultaneous double cancer and other lesions. A case of oropharyngeal cancer with concurrent thoracic esophageal cancer was found by this screening. The clinical course of this case was presented. Other lesions included gastritis (8 cases), esophagitis (5 cases), atrophic gastritis (3 cases), esophageal leukoplakia (2 cases), and duodenal ulcer (2 cases). These findings suggested that preoperative screening of esophageal and upper gastrointestinal tract is important for designing treatment and estimating the prognosis.

    An 28-year-old female who showed lymphadenopathic toxoplasmosis in the submandibular region visited our clinic. The patient noticed swellings of submandibular lymphnodes 3 months before the initial visit. Physical examination revealed one thumbtip-sized, one forefingertip-sized, and three soybean-sized lymph nodes in the right submandibular region. One azuki bean-sized lymph node was palpable in the left submandibular region. These were elastic hard. No other peripheral lymphnodes were palpable. Submandibular lymph nodes were excised after the clinical diagnosis of malignant lymphoma. Histopathologically, the excised lymphnodes showed typical findings of lymphadenitis due to toxoplasmosis. The serum IgG and IgM against toxoplasma was found to be high. Toxoplasmosis was diagnosed based on these results. The patient was treated with Sulfamethoxazole/Trimethoprim for 2 weeks. There has been no sign of recurrence since the treatment.

    This paper reports a case of gas gangrene in the head, neck, and anterior chest regions accompanied with obstructive pulmonary disease. The patient was a 72-year-old male who was infected after extraction of right lower canine. He entered our hospital January 17, 1994. Marked swelling with redness was seen in the right buccal, cervical, and anterior chest skin. Severe necrosis and pus discharge with a bad smell were observed in buccal and cervical skin.
    Crepitation was evident beneath the affected skin, and computed tomography revealed extention of gas bubbles in the infected regions. He was saved by emergency treatment of tracheotomy for dyspnea and extensive surgical drainage for pus discharge in combination with antibiotics administration. Skin defect areas had spontaneously closed by cicatrical contraction 6 months after treatment.
    The anaerobic environment caused by obstructive pulmonary disease was considered to have induced gas gangrene.

    A rare case of hemophilic pseudotumor occurred in the mandible of 1-year 8-month-old infant.
    The inicial symptom was painless swelling of the mandible.
    The case was controlled by antihemophilic factor A (AHF) during biopsy and fenestration, so peri-and postoperative bleeding was very little.
    Initially, a bone tumor was suspected because there was no hemophiliac in the family history. As a result of clinical examination, moderate hemophilia A was diagnosed.
    After the diagnosis of hemophilic pseudotumor, the mandibular bone was excised and curettage with the same hemorrhagic control was performed.
    There was no evidence of recurrence in the patient 3 years after the operation. This was a rare case of hemophilic pseudotumor located in the mandible and was the youngest case here to fore.

    Lingual osseous choristoma is a rare lesion. A case of osseous choristoma arising in the tongue of a 20-year-old male was presented. The lesion was a hard polypous mass measuring about 8×6×4 mm in the centroposterior region of the tongue and was covered with intact mucosa. The tumor was removed under general anesthesia.
    Histological finding showed a well circumscribed mass of dense lamellar bone with Haversian canals and bone marrow. It was covered with normal squamous epithelium. The excised area of the tongue was free of recurrence for about 6 months.

    Six cases with severely atrophic edentulous alveolar ridges, in which subperiosteal tissue expander (STE) was used before ridge augmentation with hydroxyapatite-tricalciumphosphate compound (HA-TCP) particles, were presented. In many cases, full inflation of the STEs could be achieved immediately after insertion thereof. One week after full inflation of the STEs, implantation of HA-TCP particles was performed. At that time, relatively firm subperiosteal envelopes were observed. Since the envelopes prevented migration of the particles, secondary vestibuloplasty due to loss of depth of the vestibule was not necessary except for one case that reguired removal of denture fibrosis. In all cases, functional and stable ridges were formed. As for complications, one patient had unilateral paresthesia of the lower lip and another had perforation and dehiscence.

    Cancer therapy with high dose rate 192Ir interstitial brachytherapy has been effectively applied to the tongue squamous cell carcinoma of Ti and T2 with NO lesions in the Department of Dentistry and Oral and Maxillofacial Surgery, Hamamatsu University School of Medicine. Based on this favorable result, it was considered possible to apply it to m are advanced cases of tongue carcinoma. In this report, two cases of T4 lesion of tongue carcinoma invading the base of the tongue, in which flexible tubes were inserted into the base of the tongue, were presented.
    Tracheotomy was performed before insertion of the tubes. They were inserted into the base of the tongue during endoscopy. To avoid mucosal damage by buttons, they were not used at the base of the tongue. The tubes were inserted to make a loop at the vallecula of the larynx. Buttons were used at the movable part of the tongue.
    Although the follow-up period and the number of cases were not sufficient, the high dose rate 192Ir interstitial brachytherapy was considered to be useful for treatment of advanced cases of carcinoma of the tongue.

    There are a few papers that reported histological examination of clinically successful osseointegrated implants. In this paper, histological results of two cases involving two implants placed in human irradiated bone were reported.
    For treatment of malignant tumor, case 1 underwent 50 Gy irradiation, and case 2 underwent 60 Gy irradiation before implant placement. Implants were installed in the orbital region in case 1 and to the maxilla in case 2.
    In case 1, the implant was removed 24 months after installation because the patient died due to tumor recurrence. In case 2, the implant was removed 26 months after installation because it was situated at the margin of the tissue to be resected during tumor surgery.
    In each case, new bone formation surrounding the fixtures was observed. The rate of direct contact between bone and fixture surface was 61.3 % in case 1 and 69.0 % in case 2. The rate of inner space occupation of screw thread was 75.8 % in case 1 and 81.2 % in case 2.

    Cole-Engman syndrome is a rare hereditary disorder characterized by the triad of cutaneous reticulated telangiectatic pigmentation of the skin, nail dystrophy and leukoplakia of mucous membranes and involves a variable number of additional features such as aplastic anemia, hyper-hidrosis of the palm and sole, dysphagia, and epiphora. A case of this syndrome in a 9-year-old male was reported. This patient showed the triad, including leukoplakia of the tongue, and other associated anomalies which were hyperhidrosis of the palms and soles and testicular abnormalities. It was thought that long-term follow-up should be conducted, because leukoplakia of the tongue was in a critical condition, malignant change of leukoplakia and aplastic anemia are fatal complications of this syndrome.

    The evaluation of pain in oral and maxillofacial diseases was performed using the Japanese version of the McGill Pain Questionnaire (JMPQ) in order to select the specific expression terms for the diseases.
    These patients were divided into 10 disease groups, including pulpitus and apical periodontitis, pain after exodontia, temporomandibular disorders, inflammation (sinuitis, osteomyelitis, periostitis, phlegmon), lichen planus, trigeminal neuralgia, denture pain, malignant tumor, atypical facial pain, and trigeminal paralysis.
    Twenty-four terms, selected by more than 20 % of the patients in each group, were decided to be the oral and maxillofacial selection of terms, totaling 78 terms.
    Ten logistic regression analyses were performed to clarify the specific terms for the 10 groups respectively. As a result, the specific expression terms for the diseases, of which the relative risk was higher than 2.0, were decided. The terms were as follows: “swelling” for the pulpitus and apical periodontitis group, “piercing” for the pain after exodontia group, “tugging” for the temporomandibular “biribiri (in Japanese)”, “bien (in Japanese)”, “pricking”, “piercing” and “numbing” for the trigeminal neuralgia group, “tender” for the malignant tumor group, “tingling”, “itchy”, “sickening” and “very uncomfortable” for the atypical facial pain group, “piripiri (in Japanese)”, “biribiri (in Japanese)”, “tugging” and “numbing” for the trigeminal paralysis group.
