    Recipient vessels were histologically examined in 30 patients who had undergone extensive tumor resection necessitating microvascular tissue reconstruction. Blood vessels chosen for microsurgical anastomosis were exclusively examined histologically in this study. Past illnesses considered to be risk factors included diabetes and hypertension. Fifty-six percent of the patients undergoing microvascular surgery had vessel abnormalities. The frequency of dysplasia was higher in the arteries than in the veins. This study revealed that most patients undergoing microsurgery in the head and neck region demonstrate preexisting damage to vessels, which generally hinders anastomosis. Although the purpose of this study was to try to identify fully the interrelationships between the extent of dysplasia, past medical history, preoperative therapy, risk factors, and factors that cause free-graft failure, preexisting changes in the recipient and graft vessels may cause technical difficulties and must be regarded as additional factors contributing to graft failure.

    Secondary oral cancer arises independently following exposure to common carcinogens such as alcohol and cigarettes by a process that has been called “field cancerization.” Therefore, it is thought to be important to treat patients with secondary cancer properly or prevent occurence of secondary cancer. An animal model of multiple cancer could be useful for investigating chemoprevention and experimental research. We previously reported that multiple tumors were observed in DMBA-induced hamster cheek pouch carcinogenesis. To clarify the genetic origins of these tumors, the differences in the positions of mutations in the p53 gene in multiple tumors were analyzed. Seven of 10 multiple SCC cases showed abnormally shifted bands in at least one of their nodules upon SSCP analysis of the p53 gene. Of these 7 cases, 2 showed different mutations in different nodules. Mutation in one of the nodules was observed in the remaining 5 cases. The results suggested that each tumor of multiple nodules arises from multicentric origin. It was suggested that this experimental model is useful for analyzing the process of field cancerization in multiple oral cancers.

  • External Application of Aspirin Ointment

    A new oral ointment containing aspirin was developed, and the analgesic effect of the ointment was assessed by a new method for measurement of tongue membrane pain using Jaw-opening reflex (JOR) of rats. A double-blind, vehicle-controlled study to evaluate tolerance and effectiveness of the topically-applied aspirin ointment was performed.
    JOR was elicited by conditioning electric stimulation through wire electrodes inserted into the tip of the tongue in rats. An additional noxious pinch of the tongue enhanced JOR markedly. After external application of aspirin ointment to the surface of the tongue, the pinch-enhanced JOR was inhibited but JOR elicited by electric stimulation was not. An identically applied ointment containing lidocaine hydrochloride inhibited not only the enhancement but also JOR. Unlike lidocaine ointment, aspirin ointment produced selective inhibition of neuronal responses to algesia derived by nociceptive stimulation without blocking nerve conduction. It was concluded that these results strongly support the method presented in this study as a new algesimetry model using the oral structure.
    Forty patients with painful oral mucous ulcers were randomly assigned to the aspirin and placebo groups. Group differences of most criteria for efficacy were statistically significant, and no side effects were reported. The total dosage of aspirin in paste amounted to less than 30 mg per day, which was much lower than that which had been generally administererd. It was concluded that this ointment would be a potent analgesic treatment for painful oral diseases such as aphthous ulcers whose etiological origin is unknown.

    The purpose of this study was to investigate the neurophysiological function of jaw closing muscles in patients with mandibular prognathism by using a polysynaptic jaw reflex electromyographically. Silent period (SP) was evoked in 5 normal subjects (C group: 5 males) and 5 patients with mandibular prognathism without skeletal asymmetry (P group: 5 males) during maximum clenching by tapping the center of the chin with a reflex hammer (20times), in which a microswitch was embedded to indicate the time of its contact with the chin. The latency and duration of SP in bilateral temporal and masseter muscles were analyzed electromyographically.
    The results were as follows:
    1. Both the latency and duration of SP in temporal and masseter muscles did not differ between habitual and nonhabitual chewing sides in both the C and P groups.
    2. The latency of SP in temporal and masseter muscles in the P group were not different as compared with the C group.
    3. The duration of SP in the P group showed significant prolongation as compared with the C group. In the P group, the duration of SP in temporal muscle was significantly prolonged as compared with that in masseter muscle, indicating the characteristic findings of mandibular prognathism.
    These results suggested that SP of jaw closing muscle, as a polysynaptic reflex, in patients with mandibular prognathism might have a depressive factor in the clenched state, especially in temporal muscle.

    The accuracy of differential diagnosis for malignancy was compared between 67Ga scintigraphy and sialoscintigraphy using 99mTcO4-, and both scintigraphies in salivary gland tumors were combined.
    The study cases were 34 salivary gland tumor patients who underwent both 67Ga and 99mTcO4-scintigraphies; salivary gland tumors were confirmed histopathologically. The patients were 23 males and 11 females, ranging in age from 16 to 79 years (mean age: 54. 1 years). There were 17 patients with parotid gland tumors and 17 patients with submandibular gland tumors (20 benign tumors, 14 malignant tumors).
    Four grades were used to categorize the 67Ga accumulation in the salivary gland with the reference uptake: negative(no accumulation), weak(< opposite gland), medium(<liver of the whole body scan and >nasal cavity), and severe (>liver).
    By using 99mTcO4- scintigraphy, the salivary gland function was classified into three different types according to stimulatory selection ratio which was expressed as the ratio of pre-to post-stimulation counts on the salivary gland, namely, normal type, lower functional type, and no functional type. Then the normal type was judged as normal function, and the lower and the no functional type were judged as abnormal function.
    In 67Ga scintigraphy, the positive ratio of the malignant tumors was 85.7 %, and the positive ratio of the benign tumors was 65.0%. All tumors showing severe accumulation of 67Ga citrate were malignant.
    In salivary gland function, 78.6% of malignant tumors were of abnormal function type. On the other hand, 90.0% of benign tumors were of normal function type. There was a significant difference between malignant tumor and benign tumor in salivary gland function(p<0.001).
    In the differential diagnosis of salivary gland tumors, the accuracy or 67Ga was 0.56 and the accuracy of 99mTcO4- was 0.85. However, the accuracy of the combined studies of 67Ga and 99mTcO4- scintigraphies was 0.88.
    The results suggested that combined studies on 67Ga. and 99mTcO4- scintigraphies are useful in distinguishing malignant salivary gland tumor from benign salivary gland tumor.

    This study was conducted to investigate histologically and histomorphometrically the tissue reaction around hydroxylapatite (HA) implants that underwent irradiation in 3 different periods in the course of bone healing after implantation.
    The cylindrical high-density HA implants were implanted in 48 Japanese white rabbit mandibles. A single 15 Gy dose was applied to the mandible 5, 14, or 28 days after implantation. The rabbits were sacrificed 7, 14, 28, and 90 days after irradiation. Nonirradiated rabbits were used as controls. CMR, labelling with tetracycline and calcein, and non-decalcified specimens stained with toluidine blue were used for histological analyses and histomorphometric measurements.
    The results were as follows:
    1. In the rabbits irradiated 5 days after implantation, the HA-bone contact was observed later than that in the controls and the bone-implant contact surface ratio was lower than that in the controls at examination because necrosis of the newly-formed bone occurred just after irradiation.
    2. HA-bone contact of the rabbits irradiated 14 and 28 days after implantation was similar to that of the controls.
    3. Bone remodeling was suppressed in rabbits of each group sacrificed at 90 day after irradiation.
    The results suggested that a short interval between implantation and irradiation causes direct contact between HA implant and bone and a long lapse of time before irradiation hardly affects the bone-implant contact, but delays bone remodeling. Therefore, it is necessary to prevent overloading the HA implants irradiated after implantation and pay utmost attention to conditions around the bone-implant contact.

    Basal cell nevus syndrome is a dominant chromosomal hereditary disease whose predominant symptoms include multiple basal cell tumors of the skin, multiple cysts of the jaw and ectopic calcification. The multiple cysts of the jaw often provide an opportunity to diagnose this syndrome. Since jaw cysts tend to recur in many places even after enucleation, correct diagnosis and adequate treatment of these cysts by oral surgeon are important. Over the last 17 years, seven patients suffering from this syndrome have been examined at our clinic. These patients as well as the treatment methods used are reported.
    There were five male and two female patients, and three of the seven patients were related. In all patients, typical faces, multiple jaw cysts, bridged sella turcica and calcified falx cerebri were found. However, cutaneous symptoms such as palmer pit or nevus were found in only three patients. The generation area is with in the area of the tooth germ. It was suggested that cyst generation was related to tooth eruption.
    The multiple jaw cysts were treated by enucleation in many cases. However, in younger patients with extremely large cysts, marsupialization was performed to carry out secondary enucleation. Small cysts were closed, but large ones were left open. Further, after completely removing the bone from the enucleation cavities, particulate cancellous bone and marrow (PCBM) graft was performed. So far, there has been no recurrence. Since keratocysts are likely to recur and to have the symptoms of basal cell carcinoma, although the frequency is low in Japan, a long-term follow-up study is important.

    The value of mouthguards for preventing injury among athletes is well-known, but only a small percentage of athletes wear them. The main reason for limited use is poor fit, because the majority of athletes who wear mouthguards use over-the-counter products. Custom-made mouthguards have advantages over ready-made ones in that they are more comfortable and fit better. In this study, stabilometric testing was done to investigate the effect of custom-made mouthguards on athletic performance. The subjects were 13 rugby players. All were tested under the following 4 sets of conditions: mouthguard (-) occlusion (-), mouthguard (+) occlusion (-), mouthguard (-) occlusion (+), and mouthguard (+) occlusion (+). The two-dimensional amplitude of the center of gravity, the area moved (Sg), and the distance moved (Dg) were calculated under each set of conditions. Sg and Dg were both significantly lower under mouthguard (+) occlusion (+) condition. In other words, occlusion was adequate and stable when wearing a custom-made mouthguard, and as a result the guard has no adverse effect on maintenance of equilibrium or somatic sensation.

    To examine a problem of prevalence of hepatitis C virus (HCV) infection in dental and oral surgery clinic in a regional general hospital, HCV antibody positive patients were examined.
    The blood analysis was performed for 525 of 1786 patients who vistied the dental and oral surgery clinic of Saitama Medical Center. Among the 1786 patients, 1562 were outpatients, and blood analysis was performed for 301 of the 1562 patients. There other 224 patients were inpatients admitted to other clinics of our hospital.
    The incidence of patients who were positive for HCV antibody was 5.6% of the outpatients with blood analysis and 12.9% of inpatients with blood analysis. The abnormal rate of serum GOT and GPT were found in 64.7% and 70.6% of outpatients with blood analysis respectively. On the other hand, abnormal rate of serum GOT and GPT of inpatients with blood analysis were 31.0% and 34.5% respectively. It seemed that liver functions of outpatients with blood analysis were more severe than those of inpatients. The most frequent routes of infection were blood transfusion (28.3%) but 39.2% of the routes were unknown even though careful examination was performed. 52.9% of the HCV antibody positive cases of the outpatients with blood analysis were revealed at the first examination. HCV was detected in 90 % of the HCV antibody positive outpatients by HCV-RNA qualitative analysis. It was suggested that most of HCV positive patients are HCV carriers.

    Recurrent behavior after primary infection with herpes simplex virus type 1 (HSV-1) in two patients was reported. The patients were 11 and 24-year-old females.
    The excretion of HSV-1 into saliva, the presence of recurrent herpetic lesion and the change of neutralizing, IgM and IgG antibodies to HSV-1 were observed for 6 months in the child and 12 months in the adult.
    Saliva was collected in both cases everyday, and serum was obtained every month.
    In the child, HSV-1 was isolated assymptomatically from saliva two times, 2 and 5 months after onset.
    The neutralizing antibodies attained a maximum level after 2 months. IgM and IgG antibodies showed a peak after 1 month and 2 months, respectively, which dropped to relatively low levels thereafter.
    In the adult, HSV-1 was isolated from saliva five times symptomatically and two times assymptomatically.
    The neutralizing antibodies attained a maximum level after 6 months. IgM and IgG antibodies showed a peak after 2 and 4 months, respectively.
    These levels rose at the time of appearance of a herpetic lesion.
    These results suggested that recurrent behavior of a child is different from that of an adult.

    Mental nerve paresthesia is a complication with the highest incidence in SSRO (sagittal splitting ramus osteotomy). However, the mechanism of paresthesia has not been elucidated. The relationship between operation and paresthesia was investigated by monitoring TSEP (Trigeminal Somatosensory Evoked Potentials) intraoperatively. Bilateral inferior alveolar nerves (16) of 8 patients who underwent SSRO were evaluated.
    TSEP was monitored by stimulating the lower lip before incision and after soft tissue dissection, splitting and osteosynthesis. TSEP waves were recorded as the amplitude of an upward wave in short latency, and intraoperative monitoring of TSEP under general anesthesia was possible. Ten waves disappeared after splitting. On the other hand, the amplitude did not change or changed only a little after soft tissue dissection and osteosynthesis. Nine of 10 nerves with disappearing waves developed paresthesia the next day and 5 of 6 nerves with remaining waves did not develop paresthesia. Therefore, it was suggested that disappearance of the TSEP wave was associated with paresthesia of the lower lip.
    TSEP monitoring was considered to be very useful in the evaluation of the inferior alveolar nerve function, elucidating paresthesia caused by SSRO.

    A case-control study was performed to evaluate the loss of teeth as a potential risk factor for dementia. Dementia subjects were 36 patients (mean age: 79.3±8.3years) with dementia of the Alzheimer-type (DAT) and 39 patients (79.5 ± 6.3) with vascular dementia (VD). A diagnosis of DAT and VD was made according to the NINCDS-ADRDA criteria by the same neurologist. Age-matched controls (77.9±6.4) were 78 residents without dementia in a nursing home. To assess the extent of dementia, dementia subjects were evaluated for duration of dementia onset by Hasegawa's Dementia Scale Revised version (HDS-R), Gottfries Brane Steen Scale (GBS), Activities of Daily Living (ADL), and Cella Media Index (CMI).
    There were no statistical differences between DAT and VD in duration of dementia onset by HDS-R, GBS, ADL, and CMI. There were fewer teeth in the DAT group than the VD group. A significant dose-response relation between the risk of dementia and the number of teeth was noted in DAT, but not in VD. The results suggested that the loss of teeth may be one of the risk factors for DAT.

    Many studies have been conducted to determine facial morphologic changes caused by orthognathic surgery. However, most of the studies were based on two-dimensional measurement of frontal or lateral cephalograms. There is only a few reports about three-dimensional analysis.
    The purpose of this study was to analyze the facial morphologic changes in orthognathic surgery using a laser scanning system (3D-VMR, UNISN, Osaka). The device provides us with 20, 000 to 40, 000 data on the shape of the face during a single measurement session which takes approximately 4.3 seconds.
    The data obtained from the device is processed by a personal computer to compare the facial surfaces before and after surgery. Three-dimensional shape analysis software (3D-SPHINX, Medic Engineering, Kyoto) is used to analyze the facial surface morphology. The two sets of data are superimposed by matching a 70mm square area around the soft tissue nasion which is not changed by surgery.
    Fourteen patients with mandibular prognathism were measured before and after surgery.
    In the case that the mandible was moved backward, the rate of soft-tissue movement to the backward movement of the mandible was 0.71. In the case of facial asymmetry, the location and amount of soft tissue change caused by surgery varied due to the direction of the movement of mandibular segments. There has been no significant relapse for 12 months since surgery.

    Angiomyoma is a benign tumor of the smooth muscle originating in the blood vessels, and is most commonly observed in the subcutaneous tissue of the lower extremities of middle-age females. However, it is rare in the oral regions. A case of angiomyoma arising in the upper lip was reported.
    A 62-year-old male was referred to our department complaining of a painless mass in the right side of the upper lip. The tumor was movable, and elastic hard, and of about 10 mm in diameter. The tumor was excised under local anesthesia. Microscopically, the tumor consisted of smooth muscle-like tumor cells, which proliferated around small vessels. The histopathological diagnosis was angiomyoma. The postoperative course was uneventful for 10 months.
