    Reduction of cell-cell adhesion and invasion are essential steps in the progression from localized malignancy to metastatic disease. Changes in expression of the cadherin-catenin cell-cell adhesion complex was investigated by immunohistochemical techniques in primary oral squamous cell carcinomas from 34 patients with cervical lymph node metastasis.
    Reduced expression of E-cadherin was found in 82%, and reduced expression of a-catenin was found in 88% of the 34 carcinomas. These findings suggested that down-regulation of a-catenin, which may reflect dysfunction of the cadherin-mediated cell-cell adhesion system, is a parameter of lymph node metastasis in patients with oral squamous cell carcinoma.

  • Especially in Anomaly of Maxillary Incisor Adjacent to Alveolar Cleft

    To clarify the mechanisms of the developmental anomaly of the tooth adjacent to the alveolar cleft, the maxillary incisor close to the cleft, the form of the alveolar cleft and the maxillofacial malformation combined with the cleft in CL/ Fr strain mouse with spontaneous cleft lip and/or alveolus (CL / A) were observed.
    Among 2340 offspring, a total of 515 mice (22.0%) with cleft lip and/or palate (CL/P) were observed. In this study, 11 mice with CL/P survived till weaning (3 weeks). The cleft patterns of survived mice were CL/A.
    In the dry skull observation, the alveolar clefts existed outside of the piriform apertura to the anterior part of the anterior palatine foramen. There were various types of cleft such as the wedge-shaped bone defect outside of the piriform apertura, the narrow cleft like bone suture, and the cleft separating the premaxilla into two parts.
    The side shift of the maxilla and/or premaxilla were observed in all cases. They tended to bend to the non-cleft side (6 out of 7 cases). Incisor anomalies on the cleft side were also observed, for example, tooth width reduction, rough enamel surface, and/or root shortening.
    As a result, the degree of tooth anomaly depended on the type of alveolar cleft.
    In conclusion, it was suggested that the developmental anomaly of maxillary incisor in CL/P mice was related to the cause of the cleft.

    Excellent efficacy of roxithromycin (RXM) in treatment of chronic odontogenic maxillary sinusitis was reported. However, its evaluation was largely depended on changes in clinical symptoms and thus poor in objectivity. Consequently, the efficacy of this long-term therapy was reevaluated by using MRI, which enables qualitative diagnosis, and at the same time, the usefulness of this evaluation method was examined. In addition, adequate administration period of this therapy was also examined by the MRI method. The efficacy rate determined by this method after 3 months of administration was 59.5%, while it was 96.7% when the clinical evaluation method was employed. As for the administration period, 3 months was considered to be standard.

    Basal cell nevus syndrome (BCNS) is an autosomal dominant hereditary disease with various skin lesions and multiple jaw cysts. An 11-year-old male with BCNS followed up for 14 years was reported. The patient was admitted to our hospital with the complaint of reversed occlusion. Clinical examination revealed basal cell nevus of left cheek, temporal and frontal bossings, ocular hyperterorism, and palmar and planter pits. Radiographs revealed multiple cystic lesions of the mandible. Clinical diagnosis was BCNS with jaw deformities, and the cysts were extirpated. Later, the jaw cysts were extirpated 3 times at the ages of 15, 19 and 24 years. Histopathologically, all operated specimens were diagnosed as odontogenic keratocyst. Immunohistochemically, Ki-67 positivity in the lining epithelia of these cysts was higher than that of control odontogenic keratocysts. Careful follow-up is considered necessary because odontogenic keratocyst associated with BCNS tends to have great proliferating potential.

  • Long-term Follow Up Using Roxithromycin

    The clinical efficacy of long-term roxithromycin (RXM) treatment was examined objectively in 8 patients with chronic diffuse sclerosing mandibular osteomyelitis. RXM was administered orally at a dose of 300mg per day for 68 days to 66 months.
    As a result, the results of 7 out of 8 cases (87.5%) were assessed effective, and the symptoms had disappeared in 1 to 12 months. One case assessed poor had several operations and various kinds of antibiotic therapy for more than 11 years before this therapy. In 7 cases assessed effective, the improvement of osteolytic changes on X-ray was observed. However, most of the X-ray findings remained osteosclerosis or the osteosclerosis became more predominant, when this therapy was terminated. It was considered that X-ray findings would take a long time to be normalized. Therefore, the optimum duration of administration which is the time to terminate this therapy should be decided according to the improvement of symptoms along with the disappearance of osteolytic findings on X-ray. Diarrhea and stomach discomfort in 1 case, and liver dysfunction in 1 case were found, however, these side effects were slight.
    The mechanisms of RXM are unclear however, these results indicate that long-term RXM treatment has therapeutic possibilities for diffuse sclerosing mandibular osteomyelitis and should be used as the first choice followed by surgical treatment if necessary.

    Neurilemmoma is an ectodermal tumor originating in Schwann's cells of myelinated nerves and occurs relatively rare in the oral and maxillofacial region. Neurilemmoma, which can originate nerve in the oral floor region, is rare. In this report, a case of neurilemmoma originated lingual nerve in the oral floor was presented.
    The patient, a 78-year-old female was referred to our department because of a swelling in the right oral floor region. The surgical excision of the lesion was performed under general anesthesia. It was confirmed that a branch of the lingual nerve adhered to the mass.
    The removed mass was spherical, 25 × 15 × 15mm in size, and covered with a thin capsule having a smooth, whitish yellow surface and low elasticity. Histopathologically, it was diagnosed as a neurilemmoma featuring mixture type of Antoni A and B. No recurrence was found two and half years after surgery.

    Three cases of aspergillosis of the paranasal sinuses were reported. All cases appeared in the maxillary sinuses, and one case appeared in the ethmoidal sinus, too. As for invasion to the bone, one case was invasive type. In CT, destruction of maxillary tuberosity, frontal, posterior and internal walls of maxillary sinus were seen in the same case. In one case of non-invasive type, calcified body was seen. Aspergillosis was diagnosed in all cases before operation.
    Aspergillosis was diagnosed in two cases by histopathological examinations. The other one was synthetically diagnosed as aspergillosis by clinical and operative findings and detection of Aspergillus DNA by PCR (polymerase chain reaction).

    Hyalinizing clear cell carcinoma (HCCC) is a very rare malignant salivary gland tumor and extremely difficult to diagnose. The case presented here was diagnosed as a rare case of HCCC of the cheek. The characteristics of the pathological findings of HCCC were discussed.
    A 30-year-old Japanese female patient was first seen with swelling of the left cheek without spontaneous pain. When she was 18 years old, the buccal tumor was diagnosed as a mucoepidermoid carcinoma. When she was 24 and 27 years old, the same tumor was diagnosed as an adenoid cystic carcinoma. No metastasis to the lymph nodes of the neck was observed clinically. After chemotherapy and radiotherapy, excision of the cheek tumor was performed. Histopathological examination revealed that this case fulfilled the criteria of HCCC. Light microscopically, the predominant tumor cells were round to polygonal containing clear PAS-positive granules in the cytoplasm. The stroma was composed of hyalinized fibrous connective tissue. Electron microscopically, abundant glycogen granules were observed in the cytoplasm.

    A 59-year-old female with facial multiple miliary osteomas of the face was reported. The osteomas were incidentally found by an occlusal X-ray photograph. Numerous miliary radiopaque nodules were revealed in her facial skin. A biopsy specimen revealed round bone tissue about 1 mm in diameter in the dermis. They are characterized by the presence of concentric mature lamellar bone with osteocytes and central fatty marrow. Except for alkaline phosphatase, all laboratory findings related to bony metabolism were within the normal range.

    Although impaction of deciduous teeth is much rarer than that of permanent teeth, there have been three cases of an impacted deciduous tooth at our clinic.
    1) The age of the three patients (1 male, 2 females) at their first visit ranged from 3 years 8 months to 13 years 10 months. The impacted teeth were a 1st deciduous molar in one case, and a 2nd deciduous molar in the remaining two cases.
    2) Histopathological examination of soft tissue over the impacted tooth revealed that the cause of impaction was hyperplasia of the collagen fiber at the submucosal layer in one case and pericoronal myxofibrous hyperplasia in another case.
    3) The impacted tooth of the first patient was extracted together with the surrounding cystic lesion. The second patient, who was 3 years 8 months old at the first visit, underwent fenestration and traction of the impacted tooth after 2 years of observation. The third patient was over 13 years old and the presence of the subsequent permanent tooth was confirmed radiologically. Therefore, the impacted deciduous tooth was extracted and traction of the permanent tooth was performed without the need for an observation period.
    The condition of the tooth and the patients' age are important factors in determining the optimal time for treatment of an impacted tooth.

    A 47-year-old patient with hyperamylasemia (amylase 25, 075 IU/1) was admitted to our department in November, 1997. Although the oral and maxillofacial regions were examined by panoramic radiography, CT, sialography and 99mTcO4- scintigraphy of salivary glands, there were no pathologic lesions or abnormal findings. The serum level of amylase activity gradually increased to extreme levels (ultimately to 482, 500IU/1), and soon thereafter a subcutaneous mass on the patient's back side was found. Fine-needle aspiration biopsy of the subcutaneous mass revealed it to be plasmacytoma. Furthermore, through isoenzymatic analysis of the serum amylase, salivary type amylaseproducing extramedullary plasmacytoma was finally diagnosed. Following chemotherapy for the extramedullary plasmacytoma, high amylase activity in serum returned to the normal range, corresponding to the reduction of the tumor size.
    Sixteen cases of amylase-producing multiple myeloma including extramedullary plasmacytoma were reviewed and interestingly revealed that all cases showed hyperamylasemia of salivary type amylase. Moreover, cytogenic analysis of myeloma cells done in previous reports revealed structural abnormalities including chromosome 1 positioned near the amylase gene locus.
