    We qualitatively evaluated dynamic enhanced MR images of normal tongues of 26 patients without oral malignancy, inflammatory diseases or systemic dis eases. The selected slices were not affected by appar entartifacts including motion and susceptibility, and the tongue shape was delineated as symmetrical on coronal images, which were obtained using a T1 weighted spin echo pulse sequence (TR/TE=200/20). Slices at the incisor and molar levels were evaluated.
    Structures that could be identified on each pre contrastimage could also be identified on the post contrastdynamic enhanced image. However, identifi cationof the intrinsic tongue musculature was impossible on the images that were composed of symmetrical, relatively high signal areas surrounded by a low signal area. Both areas were gradually but apparently enhanced. The sublingual space was easily identified at the molar level, as it was rapidly enhanced and symmetrically delineated on each image, however, it was difficult to determine at the incisor level. Further, the lingual septum could also be identified in almost all images at the molar level, and showed no enhancement pattern, whereas, the mucosal surface of the dorsum tongue was rapidly enhanced, and identified on each image.

    Fourteen patients with subcutaneous emphysema treated in our department during 17 years were clinically studied. All but one were referred to us by other dentists because of complications. They con sistedof five male and nine female patients. Their ages ranged from 16 to 67 years and 62% of them were in their thirties. The emphysema resulted during sectioning of the mandibular third molar by using an air-turbine handpiece in four patients and from extraction of other teeth in four patients. The use of compressed air during dental procedures was the cause in four patients and the complication was associated with endodontic treatment and implanta tionof a dental implant in the other two patients. A diffuse swelling involving the eyelid, cheek, mandibu larregion and neck accompanied by crepitation on palpation of the swollen area was noted in all patients.Difficulty in opening the eyelid, headache, nausea and mediastinal emphysema were found in four, one, one and one patients, respectively. Antibiotics were given to all patients to prevent infection. Except four one patient with prolonged swelling due to pericoronitis and another patient without adequate information, the swelling with subcutaneous crepitus tended to subside within a week and secondary complications were encountered in no patients.

    We have reported that a lipoteichoic acid-related molecule (OK-PSA) isolated from OK-432, a penicillin killedstreptococcal preparation, is an effective inducer of Thl-type cytokines, and elicits anti-tumor activity in tumor-bearing mice. Dendritic cells (DCs) are potent antigen-presenting cells that promote immune responses against tumor cells. It is important to induce matu rationof DCs to enhance their anti-tumor activity. In the current study, we examined the effect of OK-PSA in the maturation of DCs. OK-PSA treatment of immature DCs induced from human periph eralblood monocytes increased the surface expression of major histocompatibility complex class II, CD80, CD83 and CD86 most effectively among the other stimuli such as OK-432, lipopolysaccharide and tumor necrosis factor-α. Further, OK-PSA-activated DCs enhanced the proliferation of allogeneic T cells and induced allospecific cytotoxic T lymphocytes (CTL) as well as interferon-γ-producing cells far better than the original OK-432. Maturation of DCs induced by OK-PSA was almost completely inhibited by the addition of the antibody against toll-like receptor (TLR) 4, a member of the toll-like receptor family that has been implicated in bacterial component inducedcell signaling. OK-PSA also matured DCs derived from oral cancer patients that were express ingboth TLR4 and MD-2 genes, while OK-PSA stimulated DCs derived from patients in which the expression of MD-2 mRNA was only faint, did not induce CTL activity. These findings indicate that OK-PSA induces maturation of human DCs in vitro mediated by TLR4/MD-2 signaling. OK-PSA may be a potent adjuvant for DC-based immunotherapy for patients with malignant diseases.

    A clinical and statistical study was performed in 103 children less than 15 years old who had maxillary impacted supernumerary teeth as diagnosed by X-ray films. The male to female ratio was ca. 3: 1 and the average age was 8.0 years. The number of maxillary impacted supernumerary teeth was one, the impacted site was the median part, the direction of the dental crown was of inverted teeth, and the form of tooth crown was a monocuspid type in the majority. While diastema was observed as an affected event in the adjacent teeth, the direction of approach was mostly from the palatal side. In the case of multiple impactedsupernumerary teeth, the form of tooth crown was mostly monocuspid type+monocuspid type, though the direction of the dental crown was of inverted teeth.

    Allergic reaction induced by local anesthetics, antibiotics, or antiphlogistics is not common in the field of oral and maxillofacial surgery.
    A 29-year-old woman with perioral and eyelid edema presented in August 2000 about 1 hour after orally taking 60 mg loxoprofen sodium (Loxonin (R), Sankyo Co., Ltd., Tokyo, Japan) for pain of the lower right third molar. The skin lesions were diagnosed as an allergic reaction originated from loxoprofen sodium used to relieve pain from dental focal infection of the lower right third molar.
    An intracutaneous reaction test and provocative test revealed intense positivity. The histopathological diagnosis was superficial perivascular dermatitis consistent with drug-induced dermatitis. From these results together with her anamnesis and clinical history, we finally diagnosed the condition as allergic reaction caused by loxoprofen sodium.

    Mucous retention cysts are rare in the buccal mucosa. On close examination, most unicystic lesions measure less than 15mm in diameter.
    A 59-year-old woman visited the Department of Oral and Maxillofacial Surgery, Kanazawa University Hospital complaining of a swelling on the left buccal mucosa for 13 years. The lesion was a 25×25 mm tumor-like mass covered with normal mucosa and was excised with the surrounding minor salivary glands. No evidence of recurrence was observed one year following excision.

    A case of synovial chondromatosis of the temporomandibular joint with extension to temporal bone is reported. A 68-year-old man complained of discomfort and swelling in the left temporomandibular joint area. The preoperative diagnosis of the histol ogywas chondrosarcoma. MRI (T2-weighted) showed a high signal intensity lesion in the left temporoman dibularjoint area. We performed the removal of the tumor with condylectomy and craniotomy in coopera tionwith a neurosurgeon. The pathological diagnosis was synovial chondromatosis with permeation to bone.
