Med., 1985, 147 (4), 317-329-The intermediate filaments (IFs) in human hepatocytes were studied in biopsy specimens from patients with minimal histological changes. They were clearly visualized three-dimensionally by using polyethylene-glycol-embedding method after washing out cellular organelles and other components of cytoskeletons with a solution containing 0.15% Triton X-100 and saponin 0.5mg/ml. We demonstrated that IFs were distributed throughout the cytoplasm in a meshwork fashion, attached to the junctional complex, encircled the bile canalicular lumen, and were directly attached to nucleus. Even after extensive washout of cytoplasmic structures, hepatocytes did not collapse, with the nuclei situated at due places in the cells. These findings may support the view that IFs play a role in nuclear positioning and in keeping the cytoplasmic space in human hepatocytes.

    TANAKA, M. and KIMURA, K. Differential Sensitivity of Leukemic Cells to Growth Inhibition by Deoxyadenosine and Deoxycoformycin. Tohoku J. exp. Med., 1985, 147 (4), 331-341-Both established cell lines and human leukemic cells in circulating blood which were incubated in vitro with 2'-deoxyadenosine (AdR) plus adenosine deaminase inhibitor, 2'-deoxycoformycin (dCF), showed different metabolic responses depending upon the histologic and immunologic types of leukemia. The leukemic T-cell lines in tissue culture were 200-fold more sensitive than B-cell lines to the toxic effect of deoxyadenosine. The increased sensitivity of T-cell lines to AdR plus dCF was associated with the accumulation of deoxyadenosine triphosphate (dATP) in the cells. In established cell lines, an inversecorrelation was observed between ED 50 of AdR plus dCF and the relative increase of dATP levels in the cells after the incubation of the cells with AdR plus dCF. In circulating leukemic cells that had been incubated with AdR and dCF, dATP arose in all groups but the correlation was not found between the sensitivity of AdR and the relative dATP accumulation. The failure to find the correlation in patients's leukemic cells may be attributed to the heterogeneity of the response of the blasts to AdR and dCF.

Med., 1985, 147 (4), 343-348-Vitamin E plays an important role in protecting fat tissue from peroxidation. Platelet survival was compared among vitamin E deficient, control and α-tocopheryl nicotinate supplemented rats. The mean tocopherol level in the sera of the rats fed vitamin E deficient diet for two months was 21.5% of control, and the lipoperoxide level in the sera of these animals had not significantly (p>0.05) increased. Platelet aggregation was not accelerated significantly (p>0.05) after exposure to a concentration of 10μM of ADP used as an aggregating agent. The rate of platelet survival was determined by the 51Cr-labeling technique. The survival rates were all normal in vitamin E deficient, control and α-tocopheryl nicotinate supplemented rats, with no significant difference (p>0.1) among the groups. These results suggest that depletion of vitamine E per se does not shorten the periods of platelet survival.

Med., 1985, 147 (4), 349-356 -A one point dilution enzyme-linked immunosorbent assay (ELISA) procedure suitable for determining immunoglobulin G (IgG) antibody levels to Toxoplasma gondii (T. gondii) in community seroepidemiological surveys is described. A two-fold serial dilution ELISA procedure was first used to determine the IgG titers in 56 and 83 sera earlier screened by the Sabin-Feldman dye test (DT) and the indirect hemagglutination test (IHA), respectively. The regression rate of the results by the DT and ELISA was 0.92. Comparison of the results by the IHA and the two-fold serial dilution ELISA gave regression coefficient of 0.92. Using the absorbance values for the test sera at dilutions of 1: 20, standard curves made by plotting the optical density versus the corresponding dilution factor of a control sera were used to estimate the antibody levels. The regression coefficient of the results by the two-fold serial dilution method and those by the curves for sera with titers of up to 1: 320 was 0.97. The curves could not, however, estimate accurately the antibody level in sera with titers above 1:320. The one point dilution ELISA described is a useful epidemiological tool for the screening of IgG antibody to Toxoplasma gondii in the community. However, larger series are required to confirm our observations.

Med., 1985, 147 (4), 357-364-Chemical analysis of the composition of the thoracic aorta was performed in 19 age- and sex-matched pairs of diabetic and nondiabetic autopsy subjects. Contents of total lipid, cholesterol, triglyceride, phospholipid, free fatty acid and collagen in the aortic wall were greater in cases with advanced atherosclerosis than in those with less advanced atherosclerosis. According to severity of atherosclerosis, the ratio of cholesterol to total lipid in the aortic wall increased, while the ratios of triglyceride, phospholipid and free fatty acid to total lipid decreased. The cholesterol concentration in the aorta increased with dry weight of the aorta per area. The rate of increase was 2.6 times higher in diabetics than in nondiabetics. These results suggest that content of cholesterol in the aortic wall increases markedly according to the advance of atherosclerosis, especially in the diabetics.

Effect of Islet Transplantation on Diabetes Mellitus after Subtotal Pancreatectomy in Rat: A Quantitative Analysis of Islet Tissue. Tohoku J. exp. Med., 1985, 147 (4), 365-377-A reproducible model of diabetic state was produced by resecting more than 90% of the pancreatic tissue in Wistar strain male rats. Five rats with diabetic state after the resection received islet transplantation into the liver through the portal vein. All these five rats showed normoglycemia one week after the injection and it lasted about six months. A morphometricanalysis using 1190 serial sections of the control rat pancreas showed that the islet distribution accorded satisfactorily with Weibull and gamma distribution, when an islet was regarded as a sphere in the three dimentional space. The estimated value of radius and number of the islets was 0.0155mm and 131.2/mm3, respecitively, whereas volume fractions of the islet tissue in the splenic and gastric segments, and parabiliary and duodenal segments were estimated at 1.45 and 0.98%, respectively. The mean half of the largest diameter of the isolated islets under dissecting microscope reached to 0.080mm and 500 isolated islets account for about 14% of volume fraction in a whole pancreas. As the remnant pancreas possibly had approximately five percent of islet volume, the transplanted animal contained about 19% of islet volume as compared to the control animal. It could be concluded that transplantation of 500 islets controlled diabetes in the rat with subtotal pancreatectomy. But the transplanted rats eventually became diabetic about six months after the transplantation possibly because of insufficient volume of implanted islets.

Med., 1985, 147 (4), 379-387 Ligation and section of the main and accessory pancreatic ducts were performed in adult mongrel dogs. Four weeks after the procedure we carried out total pancreatectomy in six duct-ligated dogs, and an attempt was made to prepare the graft of islets from the highly scarred pancreas of the duct-ligated dogs by means of a newly developed collagenase perfusion method. We succeeded to normalize the high fasting glucose level which occurred after the total pancreatectomy in three of six dogs through the autotransplantation of the procured graft. However, insulin secretion was severely depressed in transplanted animals. Two of the three dogs showed high plasma glucose levels 15 and 20 weeks after the transplantation, respectively. Histological examination of the spleen revealed well preserved islets with good immunocytochemical insulin staining in one transplanted dog whose plasma glucose level was controlled for about five months. No pancreatic exocrine tissue was detected in the histological sections of the spleen in the transplanted dogs and in a transplanted control dog.

Effects of Coronary Artery Stenosis on Left Ventricular End-Ejection Pressure-Ejected Volume Relationships in Isolated Perfused Canine Hearts. Tohoku J. exp. Med., 1985, 147 (4), 389-402-We investigated the behavior of the left ventricular end-ejection pressure-ejected volume relation with and without left circumflex coronary stenosis in isolated perfused canine hearts. To change ventricular afterload pressure, the peripheral resistance of the hydraulic model attached to the aortic root of the excised hearts was changed in a stepwise fashion to five different levels. The end-ejection pressure-ejected volume relationship was nearly linear in both preischemic and ischemic states. The slope changes following left circumflex coronary stenosis differed among the hearts used in the present study, and there was no significant difference in average slope (n=10) between the pre-ischemic and ischemic states (i, e., -7.4±1.0 and -8.4±1.2mmHg/ml, respectively). Therefore, the slope of the relation was not useful in judging the presence of depressed ventricular function induced by coronary stenosis. In contrast, the ejected volume axis intercept decreased significantly from 16.5±1.1ml to 13.6±1.1ml (p<0.05) with regional ischemic.

Med., 1985, 147 (4), 403-410-C3H/He mouse embryos in utero were exposed to intensepulsed ultrasound on the 8th day of gestation (VP day=0). Fetal anomaly was observed in mice exposed for 5min to 58.6W/cm2 peak intensity pulsed ultrasound, but not in those of a group exposed to a reduced peak intensity having the same pulse width and repetition frequency. In spite of similar average acoustic intensities, the group exposed to the higher intensity (59.4W/cm2) exhibited fetal anomaly. There was no fetal anomaly in the lower peak intensity group. No increase in fetal anomay was observed after exposure to ultrasound having a smaller pulse width (3μs), even though the peak intensity was 56.3W/cm2. It is suggested that teratogenecity may depend on both the peak intensity and the pulse width of the ultrasound. Critical acoustic intensity on the 8th day of pregnancy in mice is estimated to be 60W/cm2 SATP and 1.2W/cm2 SPTA values.

Endotoxemia in Liver Diseases: Detection by a Quantitative Assay Using Chromogenic Substrate with Perchloric Acid Pretreatment. Tohoku J. exp. Med., 1985, 147 (4), 411-419-With a quantitative blood endotoxin assay using a chromogenic substrate with a perchloric acid pretreatment (PCA-LCT), endotoxemia in various liver diseases was studied. With PCA-LCT, recovery of added endotoxin in human plasma was nearly 90%, as evidenced by an intea- and inter-assay coefficients of variation of 5.7% and 11%, respectively. Because the recovery of endotoxin was not affected in severely icteric plasmas, PCA-LCT proved to be applicable to patients with liver diseases where various degree of jaundice exist. In none of the plasmas from patients with chronic hepatitis, acute hepatitis without hepatic failure or liver cirrhosis without ascites did the endotoxin level exceed the normal range of less than 5pg/ml. With the presence of ascites, however, endotoxemia became detectable, but at low levels and not in all cases. At the stage of hepatic failure complicated with renal failure or disseminated intravascular coagulation, endotoxemia was more frequent and endotoxin concentration greater. It is uncertain, at present, whether endotoxemia itself is deterioratingfactor in hepatic failure or is merely concomitant phenomenon resulting from Kupffer cell failure.

Med., 1985, 147(4), 421-425-Bladder emptying function has been initially evaluated by uroflowmetry and post void residual urine measurement. Post void residual urine volume was commonly measured by a transurethral catheter which was not desirable to some patients. In the present study 7 patents with a complaint of dysuria were subjected to combined uroflowmetry and detection of post void residual urine using sector transabdominal ultrasound scanning. The volume estimation with assumption of bladder as an ellipsoid was not so accurate, nevertheless detection of post void residual urine itself was possible in all cases. Thus, the combined method appears to be very useful for evaluating bladder emptying function.

Tohoku J. exp. Med. 1985, 147 (4), 427-428-A new method was developed to diagnose the depth of cancer invasion using submucosography combined with special charcoal suspension containing mitomycin C, and applied to 8 cases of gastric cancer. Correct diagnosis of the depth of the cancer invasion was obtained in 7 of the 8 cases. As some cases of early gastric cancer can be treated endoscopically, it is important to know the condition of the invasion before any treatment is performed.

Med., 1985, 147 (4), 429-430-Urine levels of neuron-specific enolase were determined in 3 neuroblastoma patients (1 in an advanced state and 2 in remission), 25 control children, 37 control adults and 4 children with hematuria by means of the double-antibody inhibition radioimmunoassay specific to the γ subunit of enolase isozymes. The levels of neuron-specific enolase mean±S.D. ng/creatinine mg in an advanced neuroblastoma patient were elevated (1.25±0.29 before or after treatment and range 1.61-74.2 during treatment) when compared with those of control subjects (0.51±0.26 in children and 0.36±0.17 in adults). The levels in 2 neuroblastoma patients in remission were within normal range. Urine samples with hematuria were not used for the assay.
