JOURNAL FREE ACCESSThe aim of this study was to investigate the effect of candesartan cilexetil (CC) and imidapril hydrochloride (IH) on urinary albumin excretion (UAE) and the urinary fibronectin (U-Fn) levels in diabetic nephropathy. Thirty hypertensive type 2 diabetes patients with microalbuminuria were randomised into two groups. The ACE inhibitor or angiotensin II receptor blocker that all of the patients were receiving was switched to doxazosin mesilate (DOM). After 4 months of DOM therapy, group I (n=15) was treated with IH and group II (n=15) with CC for 4 months. From month 8 to 12 group I received CC, and group II received IH. There were no significant changes in blood pressure, HbA1c, lipid paramenters in either group, exept that HbA1c decreased during CC therapy in group II. CC and IH therapy decreased in UAE and U-Fn levels compared with DOM therapy. The reductions of both UAE and U-Fn levels by CC were significantly greater than by IH. These results suggest that CC is more effective than IH as a treatment for incipient diabetic nephropathy.View full abstractDownload PDF (225K)

JOURNAL FREE ACCESSWe report a case of fulminant type 1 diabetes in an 87-year-old man. The patient was admitted to our hospital because of hyperglycemia (blood glucose level was 712 mg/dl) and found to be in metabolic acidosis based on arterial blood gas analysis. We made a diagnosis of diabetic ketoacidosis, and instricted intensive insulin therapy. His hemoglobin A1c level was 7.5%, and he had never been diagnosed with diabetes. No glutamic acid decarboxylase (GAD) antibody was detected in the serum, and the urinary C-peptide (CPR) level was below the level of detection. Serum CPR was still not detected after intravenous administration of 1 mg of glucagon. These findings were consistent with a diagnosis of fulminant type 1 diabetes mellitus. JOURNAL FREE ACCESSA 53-year-old woman admitted because of sudden fever, disorientation, and generalized seizures. The patient had a long history of sensorineural hearing loss and had been diagnosed with diabetes mellitus at 43 years of age. A 3243 A→G point mutation of mitochondrial DNA was detected at 50 years of age. Insulin therapy was started at that time, and her glycemic control had been good. MRI showed a swollen left temporal lobe with an elevated signal on T 2 weighted images. Intravenous acyclovir was administered for a presumptive diagnosis of herpes simplex encephalitis, although the CSF findings were normal, and no HSV-DNA was detected by PCR. Four months later, the patient again became disoriented and developed seizures. CT showed a new lesion that involved the contralateral (right) temporal lobe. A diagnosis of late-onset MELAS was supported by the elevated serum and CSF lactate levels. It should be noted that MELAS sometimes occurs in adults and that the symptoms may mimic encephalitis.View full abstractDownload PDF (421K)
