症例は59歳女性で，主訴は額と体幹の皮疹である。50歳頃に額に皮疹が生じて徐々に拡大した。近医皮膚科で尋常性乾癬と診断され，ステロイド軟膏で消褪と再燃を繰り返していたが徐々に拡大するため漢方薬治療を希望して来院した。皮疹は前額部が広範囲で，後頸部，前胸部，上下肢にも認められた。治頭瘡一方加減を処方したところ，約4ヵ月の経過で皮疹がほとんど消失した。前額部の皮疹がわずかに残存したため，21ヵ月後に帰耆建中湯へ転方したところ，やや改善がみられ治療を継続している。乾癬の漢方治療には駆瘀血剤，清熱滋潤作用のある方剤が用いられることが多い。本例では皮疹が頭頸部に集中していたことから，治頭瘡一方加減を処方し著効した。これまで乾癬に対する治頭瘡一方の臨床報告はみられないが，湿性の皮疹が頭部に集中する例では奏功しうると考える。

The definition and terminology of Budo is a frequently discussed topic in the area of the science of Budo. In recent years, at the 2005, 2006 and 2007 congress of the Japanese Academy of Budo, a forum was opened with the title ‘Searching for a modern definition of Budo' and discussion continued along with topics like ‘what is the difference between Budo and competitive sports?', ‘what are foreigners looking for in Budo?'. On the other hand, as Sakai has pointed it out, basic research in the field of Budo must deal with the problem of ‘making it clear what terminology and how it was used in a given era'. He added, ‘as for the future, I hope for an unraveled general picture of what terminology, from what period, which way was in use', claiming the need for a clear understanding of terminology which could help in defining Budo and its cultural implications.

The necessity to clarify Budo terminology is connected to the problem of translation of Budo-related terms in the progress of internationalization. To translate Budo terminology properly, a thorough understanding of all the contents of its meaning is mandatory. We are convinced that an analysis of the terminology having been in use through the history of Budo would create the foundation for this work. Research on the examples of terminology in the history of Budo would greatly help people with different cultural backgrounds to comprehend these expressions, contributing to not to lose the real goals of Budo culture in this rapidly changing world.

Objectives of the research is as below,

1. ‌Concentrating on Kojiki, the oldest historical record in Japanese history, we analyze all occurrences of expressions connected to Budo, exploring the distribution of these expressions in the text.

2. ‌Especially in the case of sword-related terminology, clarification of context and tendencies of usage.

Our research produced the following results:

1. ‌We organized the Budo-related expressions in Kojiki, Japan's oldest book, into three categories: warriors, weapons (and their use) and general terms on warfare. We found that while there were no instances in the general terms category, 85 % of the terms fell into the category 'weapons'.

2. ‌We clarified how terms falling into the categories 'warriors' and 'weapons' are distributed in the three volumes of Kojiki. As an interesting result the fact can be cited that the term 'totsukatsurugi' appears only in the first volume about the age of gods, while the term 'tachi' can be found only in the second and third volumes.

3. ‌From its context it is clear that the term 'totsukatsurugi' was used to describe swords worn by the gods.

4. The term 'tachi' was used in connection with swords that came to earth from heaven.

背景．肺癌の心筋転移は稀である．今回我々は生前に心筋転移を診断し，osimertinibによる治療効果を得られたT790M変異陽性肺腺癌の症例を経験したので報告する．症例．66歳女性．右上葉肺腺癌に対して右上葉切除術を行った．病理検査で混合型腺癌（pT2aN2M0 pStage IIIA），EGFR遺伝子変異陽性と診断した．術後補助化学療法は希望されなかったが，術後4か月で縦隔リンパ節再発を認め，gefitinibによる治療を開始した．治療開始9か月後に新規頭蓋骨転移を認めたため，新規治療導入目的に入院した．入院時に下壁梗塞を疑う心電図変化を認めた．心臓超音波検査とCT検査で左室下壁から心室中隔に及ぶ腫瘤性病変を認め，肺癌の心筋転移と診断した．Osimertinibによる治療を開始し，4か月後には転移巣の縮小とともに心電図変化も消失した．Osimertinibを約9か月間継続し，心筋転移出現から13か月の生存が得られた．結論．急性心筋梗塞様の心電図変化により肺癌の心筋転移の診断を行い，osimertinibによる治療で心筋転移縮小および心電図異常変化の消失を認めた症例を経験した．

Eudistomins (1) have been isolated from colonial tunicates and display antiviral activity. We have synthesized these eudistomins which established the absolute configuration. Reaction of N-hydroxytryptamines (2) and protected L-cysteinals (3) gave optically active nitrones (4) in good yields (Table I). When 4 (R_1=H) was treated with TFA-CH_2Cl_2 at room temperature for a few mins, the tetracyclic compounds (5) were obtained as the major product along with the β-carbolines (6) (Table II). The β-carbolines (6) were obtained as the sole product with high selectivity for 6β when the reaction was carried out at ?78℃. In contrast, 4 (R_1=Me) gave the corresponding tetracycles 5 regardless of the reaction temperature and noβ-carboline was obtained. The tetracyclic compound (R1=H) can be transformed to the correspondingβ-carboline with TFA-CH-2Cl_2. Theβ-carboline (6aβ, 6eβ) was cyclized with NCS-CCl_4 to give the desired oxathiazepine (9b, 4%). As an improved cyclization, the reaction of the sulfoxide (10a) with TsOH gave the oxathiazepine (9a) in 21% yield. The enantiomer of debromoeudistomin L (1a) was obtained by deprotection of 9b. The nitrone obtained from 2a and D-cysteinal gave the natural (-)-1a by the similar reactions. On the other hand, the bromination of the tetracyclic compound (5e) gave the bromo derivative (14) selectively. Ring transformation of 14 with TFA gave theβ-carboline (24α), which was further converted to (-)-eudistomin L (1b) by analogous reactions. Other eudistomins (1e, 1f) have also been synthesized by the similar reaction of D-cysteinals with 6-bromo-, and 6-bromo-5-methoxy-N-hydroxytryptamines, respectively.

腰痛とそれに伴う下肢痛，しびれ感はしばしば難治性で日常生活動作の低下につながるため漢方治療の良い適応となっている。今回我々は，精神神経症状の有無にかかわらず柴胡加竜骨牡蠣湯が奏功した症例を複数経験したので報告する。症例は腰痛を主訴とし，下肢の痺れあるいは疼痛を伴う5例である。そのうち4例には精神神経症状はみられなかった。柴胡加竜骨牡蠣湯の投与によっていずれも2ないし4週間で疼痛の軽減と歩行距離の改善がみられ，鎮痛薬が不要となった。一般に，柴胡加竜骨牡蠣湯は精神神経症状を伴うものが適応とされるが，自覚的な症状がなくとも著効する場合があり，慢性疼痛による潜在性うつ状態を精神神経症状ととらえることで，腰痛緩和の鑑別処方になり得ると考える。

腰痛の慢性化には心理社会的要因などが関わる。 しかし、 腰痛治療に際して心理社会的要因を評価し、 かつ鍼治療と漢方薬治療を併用した報告は我々が渉猟した限りない。 今回、 右殿下肢痛を訴えた慢性腰痛患者の心理的要因を評価し、 鍼治療と柴朴湯を投与した結果、 Quality of Life (QOL） が改善したので報告する。 症例：38歳女性。 主訴 右殿下肢痛。 X-1年8月に主訴を発症。 近医整形外科を受診し腰椎椎間板ヘルニアと診断された。 同年11月にふらつきを自覚し近医精神科を受診、 適応障害と診断されブロマゼパムが処方された。 X年2月に当院整形外科で坐骨神経痛と診断された。 ブロマゼパムの減薬を希望し3月に当院精神科を受診、 4月にうつ病の可能性を指摘された。 患者が腰痛に対する鍼治療を希望し、 同月に当科を受診した。 神経学的所見・整形外科的徒手検査は異常無く、 梅核気・胸脇苦満を認めた。 病態を非特異的腰痛と捉え、 鍼治療を右のL2/3直側、 仙骨外縁部、 梨状筋相当部、 殷門 （BL37）、 委中 （BL40） に実施した。 また、 漢方医学的には気滞と捉え柴朴湯を投与した。 所見・経過：Keele STarT Back Screening Tool はMedium risk、 Roland-Morris Disability Questionnaire (RDQ)は16点、 偏差得点1.22点であった。 2回目の鍼治療の際に、 左右の期門 （LR14）、 内関 （PC6）、 太衝(LR3)と巨闕(CV14)への刺鍼を追加した。 3回目の鍼治療後にRDQ 2点、 偏差得点50.14点に改善した。 考察：鍼治療と柴朴湯による鎮痛機序の賦活や精神症状への作用が推察され、 その結果症状が軽減し、 QOLが改善したと考える。 鍼治療と柴朴湯は、 心理的要因を抱え、 殿下肢痛を有する慢性腰痛患者に対して有用である可能性が示唆された。

局所麻酔下切開排膿適応の感染性表皮嚢腫（IEC）患者125例（男性52例　女性73例）に対して排膿散及湯（HST）内服治療を行い，感染兆候軽快後1年再発のないものを略治とし，略治率，略治までの日数について性別，年齢，罹患部位，抗菌剤併用有無，基礎疾患の有無で後ろ向き研究を行った。結果は全体としては88例（70％）が略治した。性別検討では男女差はなかった。年齢別検討では30—50歳代で脱落が多く，この年代でやや略治率は低い傾向であった。部位別検討では略治率が背部・臀部以外群で63例/78例中（80％）であり有意に高かった。同群の平均内服日数は13.3日だった。抗菌剤併用について，略治率は併用群が高い傾向だったが，平均内服日数は HST 単独群が11.9日で HST 単独群が有意に短かった。基礎疾患群は24例中21例（87.5％）略治し通常群に比べ有意に高かった。平均内服日数に有意差はなかった。

A historical development of the chemistry of cyclic tautomer of tryptophan is reviewed. The cyclic tautomer of tryptophan, pyrrolo [2, 3-b] indole-2-carboxylic acid, was prepared by dissolving N-methoxycarbonyltryptophan ester derivatives in 85% phosphoric acid or trifluoroacetic acid. The cyclic tautomer can be reverted to the indolic form with a dilute acid. The cyclic tautomer is an aniline derivative and the enamine reactivity of the indole ring in tryptophan is protected. The electrophilic substitution and oxidation of these cyclic tautomers opened a new method to prepare 5-substituted and/or 6-substituted tryptophan derivatives such as 5-bromo-, 5-hydroxy, and 6-methoxy-tryptophans. The formation and reactions of cyclic tautomers of diketopiperazines containing tryptophan and 3-indoleacetamide are also discussed. Some indole alkaloids having substituents at the benzene ring such as fumitremorgins, flustramine B, and eudistomines were synthesized by the use of these reactions. Furthermore, enantioselective alkylations of the carbanion at the 2-position of the cyclic tautomer established a new route to optically pure α-substituted tryptophans. The 2, 3-dehydro derivative of the cyclic tautomer is an α, β-unsaturated ester and was found to be a good precursor of optically pure β-substituted tryptophans. The 3a-position of the cyclic tautomer is a benzylic position and subjected to radical reactions to give 3a-substituted-pyrroloindoles.
