CAD/CAMレジン冠が保険導入されて8年をむかえようとしている．この間，本治療法の向上のために日本補綴歯科学会会員は，基礎研究データおよび臨床エビデンスを蓄積してきた．本稿ではその成果である「現在推奨されている接着技法」，「全臨床アウトカム」をまとめ，それらの最新情報から導かれる「具体的な臨床術式」および「患者説明のあるべき姿」を提案する．CAD/CAMレジン冠の治療を成功させる要素は多岐にわたり，歯科医はこれらを総合的にマネージメントする必要がある．さらに，新規メタルフリー治療として日本からの発信を達成するために刷新すべきドグマや臨産官学民連携について吟味したい．

1. 2, 3-Bisphosphoglycerate phosphatase was partly purified by CM-cellulose column chromatography.
2. The enzyme activity was stable in the 50°C case of preincubation up to 30 min. By the addition of 4 mM 2-phosphoglycolate to the preincubation mixture activity remained perfect up to 30 min in the 55°C case of preincubation.
3. This enzyme was inhibited by low concentrations of p-chloromercuribenzoate and heavy metal ions. These results suggest that this enzyme is an SH-enzyme.
4. The reaction mechanism was of a ping-pong type.
5. Inhibition types of dihydroxyacetone phosphate and glyceraldehyde phosphate were uncompetitive against 2, 3-bisphosphoglycerate and competitive against 2-phosphoglycolate.

2014年に保険導入されてから8年間で，CAD/CAMレジン冠は日本の臨床に着々と普及してきている．この新規補綴装置の質を向上させるために，わが国は「臨産官学民連携」で奮起してきた．臨床研究の成果として4年予後調査では，小臼歯CAD/CAMレジン冠の生存率は96.4％であり，脱離をトラブルと捉えた場合の成功率は77.4％であった．基礎的データの蓄積も進み，マトリックスレジンを被着対象としたレジンプライマーの有用性が明らかとなっている．このように，CAD/CAMレジン冠の臨床と基礎研究から，さらなるコンポジットレジン系材料としての可能性がみえてきている．今後，「日本独自のメタルフリー治療」が確立されるために，歯科理工学会にも強く期待せずにはいられない．

本総説の内容は第78回日本歯科理工学会学術講演会（担当校：岡山大学）における学会主導型シンポジウム「さらなるコンポジットレジン系材料の可能性を探る！」で発表した．

In connection with edentulous prosthesis, it is significant to measure the thickness of the oral mucosa by a non-invasive technique in vivo. Ultrasonic waves of 15 MHz were used in an A-mode pulse reflection method. In order to check the suitability of the ultrasonic method, it was compared with the puncturing method on 10 sites of the maxillary mucosa in 6 dentated subjects.
The results were as follows:
1. The equipment has a small probe to measure the thickness of soft tissue in a limited oral cavity, and the axial resolution is 0.1 mm.
2. The relationship between the mean values obtained by the ultrasonic and puncturing methods showed a high correlative coefficient of 0.94. Also, the standard error obtained from each subject by the ultrasonic method was smaller than that of the puncturing method.
3. The difference in the thickness of the oral mucosa varied in individual cases, but compared with the same site of others, there was a regular tendency: the thinnest sites were in the center of the palate with a range of 1.3 to 1.6mm; the thickest sites were in the rear sides of the palate with a range of 5.5 to 5.9 rnm, the other sites were medium thickness.

　1ステップ接着システムを応用したプライマー処理後に，疎水性の高い新規ボンディング材を用いる次世代2ステップシステムの接着強さにおける長期耐久性を評価検討した．ヒト抜去大臼歯の象牙質を被着面として，新規2ステップシステム（以下BZF，GC，東京），G-プレミオボンド（以下GPB，GC，東京）およびクリアフィルメガボンド2（以下MB2，クラレノリタケデンタル，東京）にて歯面処理し，光重合型コンポジットレジン（クリアフィルAP-X，クラレノリタケデンタル，東京）を積層築盛した．各試料を24時間水中浸漬後，試験体を作製し，1年水中浸漬後に，微小引張接着強さ（以下μTBS）試験を行った．また，24時間ならびに1年水中浸漬後の試験体の界面を透過電子顕微鏡（以下TEM）にて観察した．BZF群およびMB2群はGPB群と比較して有意に高い接着強さを示した（p<0.001）．BZF群の主な破壊様式は凝集破壊であったが，MB2群とGPB群はそれぞれ混合破壊，界面破壊であった．TEM観察の結果，BZF群の安定した接着界面が確認された．以上より，新規ボンディング材は高い接着耐久性能を有することが確認された．

Electrode potentials of non precious alloy crowns in the mouth were measured in a clinical situation using a pH-meter in conjunction with a platinum micro-probe and a saturated calomel electrode. Nine patients who participated in the study were restorated with crowns which were prepared from commercial dental alloys of Co-Cr, Ni-Zn-Cu and Ni-Cr. During a period of three months after the cementation of the crowns the electrode potentials of the crowns were in the range of -0.21 V (SCE) for Co-Cr crowns, -0.10--0.20 V (SCE) for Ni-Zn-Cu crowns and -0.13--0.21 V (SCE) for Ni-Cr crowns and were roughly the same for all non precious alloy crowns examined. The measurements of electrode potential in vitro showed that the potentials of the alloys in 0.1 N sodium chloride solutions were close to the potentials in the mouth. In acid solutions, however, the electrode potentials of the alloys were noble to those in neutral or alkaline solutions.
