The medical history proves that clinical research has been a powerful weapon for disease control even when the etiology of the disease is unknown, as John Snow found the causal relationship between drinking water and cholera infection, and Kanehiro Takagi between nourishment and beriberi.

Medical science progresses along with the development of new medical technology such as new drugs and medical devices. In the history of hypertension, a great cohort study in Framingham in the USA has enhanced enormous scientific knowledge about the causal relationship between hypertension and cardiovascular diseases. The new sphygmomanometers and a lot of antihypertensive agents contributed to validation of the hypothesis generated from the Framingham study and development of hypertension practice guidelines. The modern data analyzing techniques like multivariable analysis were also produced in the struggle of the statisticians working for that study.

Every research method has strengths and weaknesses, even a randomized controlled trial is no exception. The important point is to use a method well suited to answer a question, whether it is randomized or not. Clinical research methods have been developed for solving several medical issues and will be developed with advances in medical science in the future. For example, “N–of–1 trial” method has rapidly become popular in these years, and CONSORT extension for reporting N–of–1 trials was published in 2015. We should contribute to development not only of new medical technologies but also of clinical research methods.

A role of imported steamships and their main engines at the beginning of modern industry in Japan is scrutinized and examined in this paper.
Before the first arrival of steamship to Japan in the year 1853, some of Japanese artisans had endeavoured to make up a side-lever engine only in dependence on a translation of a Dutch textbook. They successfully built up a steamship and it operated well in 1854, but these trials were suspended and many western steamships were imported, because it was so difficult to make up complicated marine engines such as an oscillating engine without machine-tools and related industry. In addition, there was no time to prepare for the foreign pressure. But the importation of steamships played a great part for the progress of Japanese marine engineering.
At that time, in Europe the marine engine had been in the era of innovation, for example, the spread of oscillating engine or double-cylinder engine etc., and also the change from paddle wheel to screw in propeller. Steamships equipped new-type engines were imported successively, and Japan could keep up with the times. In 1862, excellent engineers went abroad for study and joined in construction of an ordered ship in the Netherlands, thus they could learn in practice. Using the imported ships, Japanese engineers grew accustomed to operation and repair of new machines. There was a rather good understanding of the necessity of dockyards for repair and such shipyards made a contribution to the development of marine engine in Japan.

Kanehiro Takaki, the founder of The Jikei University School of Medicine suggested that a nutritional factor was important for preventing beri-beri, which was a common disease in the Meiji era in Japan and Southeast Asia. He improved the rations fed to crews of the Imperial Japanese Navy to include wheat and meat. The rations he devised effectively prevented beri-beri. Some 30 years later, vitamin B1 was discovered, and a deficiency of vitamin B1 was found to be the cause of beri-beri. Takaki believed that nutrition and exercise were important for keeping our bodies fit. He often gave lectures on how people could keep fit to prevent diseases. Thus, his activities are considered to be the beginning of preventive medicine in Japan. The contributions of Takaki to the physical fitness of the Japanese people have been continued by the graduates of The Jikei University School of Medicine. Some of the graduates became professors of The Jikei University School of Medicine and Tokyo University of Education (now, Tsukuba University). Thus, both universities have the common basis and tradition for research and education in the fields of physical fitness and sports medicine, and have collaborated with each other in these fields. In this article, we provide a brief overview of the history of the development of research regarding physical fitness and sports medicine in Japan. We discuss the contribution of various persons including our graduates, to the health and physical fitness of the Japanese people.
