脳梗塞は発症後に寝たきりになるなど, その予後は芳しくなく, 未病を治すことが重要な疾患である。
〓血証と脳梗塞 (MRI所見) , 起立性高血圧との関連について検討した。麻痺などの神経症状がまだ発症しておらず, 頭痛・肩こり・吐気・めまい・意欲の低下などの不定の症状を有した患者で, 脳のMRIでlacunae型脳梗塞が見出された患者 (32例) を対象に, 起立試験, 〓血スコアなどを検討した。対象者では健常者に比し, 起立試験では起立性高血圧反応 (立位後の交感神経反応の亢進, 特に心臓交感神経の亢進) を示し, 〓血スコアは重度〓血病態であった。特に上半身に現れる兆候に陽性項目が多かった。脳梗塞が鎮静化すると, 起立性高血圧反応は消失し, 〓血スコアも正常化した。未病を治すことは最も重要な医療上の問題である。〓血という概念は, まさに未病診断にとり有効な方法である。今回, 脳梗塞の未病を観察したが, 〓血を日常診療に取り入れ, バイタルサイン並みに扱うことを提唱したい。

1. The activity against 31 strains of Staphylococcus aureus, separated from pyoderma lesions, was studied. Six strains were cross-resistant against lincomycin and clindamycin. Clindamycin has stronger activity against the lincomycin-sensitive Staphylococcus strains.
2. The blood level in healthy human males after the oral administration of 300 mg reached the peak (2. 2∼1. 1 mcgiml) at 1 hour.
3. Clinical use of clindamycin was tried in 15 cases of pyoderma. Three hundreds mg of clindamycin wers administered 3 times daily. Several cases complained of slight stomach trouble. Complaint of diarrhea was absent.

A 56 year-old male, who had received angiography by thorotrast about 30 years ago, was admitted to our hospital because of anemia on Nov. 12, '69.
His peripheral blood pictures and sternal marrow features showed the status of the preleukemic stage such as leukemoid reaction until the manifestation of erythroleukemia on Jan. 22, '70.
The specific droplet shadows caused by the deposition of thorotrast could be observed in the x-ray film of the upper abdomen.
He was suffering from severe anemia, epistaxis, hematemesis, flux, hematuria, and infection of lung since the end of March, '70 and at last died of the severe bleeding diathesis on May. 7. '70.
The findings of autopsy were as follows:
1) Thorotrast granules or mass were observed in Glisson sheath, interstitium, and around the central venous vessles of the liver.
2) Especially, the histological findings of the spleen and the bone marrow suggested the causal relation between the atypical proliferation of erythroleukemic cells and the deposition of thorotrast.
3) The tissue of portal lymphnode showed the deposition of thorotrast and slight atrophy.
4) Wide-spread bleeding, sideroblastosis, and necrosis could be seen in the tissue of liver, spleen, lung, digestive tract, and kidney.
5) Fungus disease of lung and adrenocortex atrophy were proved.
It has been reported that most of the malignant tumors induced by thorotrast were epithelial tumors such as cholangioma or liver cartinoma and non-epithelial tumors induced by thorotrast were very rere in Japan.
But in Europe and America, non-epithelial tumors such as liver sarcoma, Hodgkin's sarcoma, leukemia etc. were reported to occupy about 40% of all malignant tumors induced by thorotrast.
This case report of the acute erythroleukemia induced by thorotrast is considered the first or second case in Japan.

【目的】SLEなどの自己免疫疾患では、自己反応性T細胞の出現や自己抗体の産生など免疫系の異常な賦活化が認められる。スフィンゴミエリン（SM)、スフィンゴ糖脂質、コレステロールからなるリピッドラフトは免疫シナプス形成やTCRを介したシグナル伝達に重要な機能を有している。昨年度の本学会で、SLE患者末梢リンパ球におけるラフトの発現増強について報告した。今回我々は、ラフトの主要構成脂質であるSMのTCR凝集やシグナル伝達における機能について解析した。【方法と結果】_丸1_Jurkat 細胞にSM合成酵素遺伝子(SMS1)-siRNAを遺伝子導入し、SM knock down細胞株(SMkd)を樹立した。_丸2_抗CD3抗体刺激によるTCR凝集能は、SMkdで有意に低下していた。_丸3_活性化分子であるCD69の発現量はSMkd細胞株で著明に低下した。_丸4_ショ糖密度超遠心法で検討したところ、抗CD3抗体刺激によるTCRのラフトへの移行がSMkd細胞株で著明に低下していた。_丸5_抗CD3抗体刺激によるZAP-70、LATのチロシンリン酸化はSMkd細胞株で有意に低下していた。【結語】TCRの凝集、Ｔ細胞活性化シグナル伝達にリピッドラフトおよび細胞膜SMが重要であることを明らかにした。SLE患者でみられた免疫異常がリピッドラフトの発現異常によりもたらされている可能性がある。

注意集中効果に関するほとんどの研究では，情動的刺激として，交通事故や外科手術，殺傷場面などの情動的シチュエーションを使用している。これらの状況においては，実験参加者は情動を喚起させる原因となる詳細な特徴というよりはむしろ，血に注意を向けていると考えられるかもしれない。このことから血には周囲に脅威を伝えたりあるいは助けや保護を求める役割があることから，注意をひきつけると仮説をたて検証した。実験では，16名の実験参加者に対しワイシャツについた血またはバラ（胸花）のイメージを3×3の配列で呈示した。実験参加者には，呈示された刺激画像中に異なる刺激画像が含まれるかについて判断させた。その結果，いくつものバラの中に1枚だけ呈示された血は，血の中に1枚だけ呈示されたバラと比べ，エラーが少なく，より正確に検出されることが明らかになった。この実験結果は，血が注意を引きつけやすいことを示している。

We report a case of severe epistaxis resulting in an unfortunate clinical course. A 35-year-old man seen for bilateral nasal obstruction, a top-heavy feeling, and general fatigue was found to have bilateral swollen nasal mucosae and a completely obstructed nasal airway. Sinonasal X-ray showed the pansinuses to be occupied. Nasal treatment caused such immediate excessive bilateral epistaxis. Disturbance of consciousness occurred. The bleeding could not be arrested completely even under general anesthesia. Astriction was done using anterior packing and posterior balloon tamponade. Perioperative blood examination showed thrombocytopenia, disseminated intravascular coagulation (DIC), electrolyte abnormality, hepatic dysfunction, and overwhelming acidosis, requiring that the man be managed in the intensive care unit (ICU), where he also suffered hemophagocytic syndrome and multiple organ failure (MOF). The diagnosis from bone-marrow biopsy and flow cytometry was aggressive natural killer (NK) cell leukemia (ANKL). Despite chemotherapy with steroids, etoposide, and L-asparaginase, he died of hemorrhagic pneumonia one month after the final diagnosis. Given such nasal symptoms with a worsening general condition due to latent blood dyscrasia, blood examinations should be done even before attempting nasal treatment.

An experimental study concerning the diagnostic evaluation of symptoms due to lead poisoning was made with rabbits which were exposed to lead oxide dusts of different concentrations. Symptoms such as stippling, polychromasia, reticulocytosis, hemoglobin decrease and urinary coproporphyrin increase were estimated and also analysis of serum protein fraction was carried out by the paper electrophoretic method. A chamber for exposure which was equipped with the Wright's dust feed mechanism was tpecially designed for the rabbits allowing to inhale lead dusts. The experiment lasted over a period of one and half years from autumn 1957 so spring 1959. Maximum length of exposure was limited to 4 hours each day, and to 3 days each week. During the course of the experiment, cessation of lead inhalation for 2 month was inserted in order to observe how soon these symptoms disappear after the cessation of exposure. Principal results obtained were as follows. 1) The urinary coproporphyrin level and the stippled cell counts were increased before anemia developed. Besides, it was noted that the increase of stippled cells was not shown until four to five months after the urinary porphyrin became positive. The decrease of hemoglobin amount occured immediately after the definite rise of the stippled cell counts. From these facts, it would appear doubtful that the increased excretion of urinary coprophrin is merely due to the blocking by lead of entrance of iron into the protoporphyrin ring. The increased urinary coproporphyrin in the earlier stage of lead absorption must be due to other mechanism than this hypothesis which explains the decreased hemoglobin amount. 2) Urinary coproporphyrin, stippled cell counts and reduction of hemoglobin were increased parallel with the lead amount inhaled. However, the reticulocyte counts, polychromasia as well as changes of the serum protein' namely the elevation of α₂ and γ-globulin levels and the decrease of the albumin level had no relation with the degree of lead absorption, as far as the experimental animals were concerned. 3) Reduced hemoglobin and increased stippled cells by lead exposure returned rapidly to the normal value after the cessation of inhalation. On the other hand, urinary coproporphyrin delayed to become normal, in spite of the prompt recovery of those symptoms mentioned above.

Some examinations were conducted to find factors which give changes of the specific gravity of the standard copper sulfate solution. (1) So far more accurate and precise values could be assured by a certain device of specific gravity balance with the accuracy of a standard deviation of 0.000014. (2) By improving the preparing method of the standard copper sulfate solution as recommended by R.A.Phillips, mass production of the solution was made feasible within the variation of standard deviation of 0.00002 and with only a slight mean error of 0.000007. (3) The standard copper sulfate solution prepared by this method could be well reserved for 12 months after preparation without significant change of its appearance as well as its specific gravity even at room temperatures. (4) Cutting off ampules containing solution and sealing the end by ordinary rubber caps tightened by rubber bands, the value of specific gravity did not change for a fortnight. This is true even with solutions to which blood drops have been added. (5) If more than 50 drops of blood were dripped into the solution of 100 ml., the value of the specifc gravity decreased dawn to the figure of being effected to 0.0005.

同一個体のウシから得た4種のIgGを, 3種のタンパク分解酵素 (ペプシン, キモシン, パパイン) を用いて分解する際の感受性の違いを, 初乳IgGと血清IgG, さらにIgGのサブクラス (IgG 1とIgG 2) について比較した. ホルスタイン牛3頭より採取した初乳ホエーおよび血清より, 33%飽和硫酸アンモニウムによる塩析, 次いでDEAE-トヨパール, CM-トヨパールを用いたイオン交換クロマトグラフィーにより, IgG 1とIgG2をそれぞれ精製した. 各IgGと酵素とを一定の重量比になるよう混合し, 37℃ に保ち反応させ, 経時的にTSK-gel G 3000 SWを用いた高速ゲル瀕過クロマトグラフィーを行ない, その溶出ピーク高の変化から分解の程度を算出した. ペプシンとキモシンによる分解では, 一部例外もあるがIgG1の方がIgG 2よりも分解されやすかった. また, 特にIgG1において初乳由来の方が血液由来のものよりも分解されやすく, その順はIgG 1 (乳)>IgG 1 ()>IgG 2 (), IgG 2 (乳) であった. 一方, パパインではIgG 1 (乳)>IgG 2 ()>IgG 1 ()>IgG 2 (乳) の順であった.

2011 年3 月11 日に起きた東日本大震災により東北地方は甚大な被害を受けた。それ以降，災害に対して積極的に取り組む医療機関が増えてきている。横浜労災病院は神奈川県にある災害拠点病院であり，薬剤部としても災害に対する取り組みを実践している。内容は，（1）院内災害訓練への参加，（2）薬学教育実務実習（以下，実習と略す）時の災害医療に関する講義，（3）患者を対象とした災害関連勉強会，（4）東日本大震災時，DMAT（Disaster Medical Assistant Team）業務調整員としての参加及び院外診療チームへの参加である。学生や患者への教育，災害時の医薬品の供給管理等，災害医療に関して薬剤師の関われる分野は多岐にわたる。特に実習での災害に関する講義は，災害医療を学ぶだけでなく，日常の薬剤師業務の重要性を再認識できる場としても非常に有益である。
