    埼玉大学では,一般入学試験(個別学力検査等)の合格発表を大学ホームページで公開している.しかし,合格発表時に大学ホームページに閲覧者が殺到し,高負荷のためにWebサーバが機能不全に陥るといった現象が発生し,大学ホームページでの合格発表を閲覧できない状況となっていた.特に平成26年度前期日程の合格発表では影響が顕著であった.後期日程の合格発表では,複数のWebサーバを組み合わせる形で大学ホームページを構築しWebサーバプログラムに対してチューニング等の対策を行い,閲覧者にスムーズな閲覧環境を提供した.本論文は,後期日程合格発表時の大学ホームページへのアクセス集中に対する対策の報告である.この対策を実施する際に,平成26年度前期日程の合格発表時においては,Webサーバへのアクセス量などの情報等が重要であったが十分に得ることができなかった.そこで,限られた期間内で可能な限りに負荷に耐えうる大学ホームページのシステムを構築した.

    The effect of intra-aortic balloon pumping onthe coronary blood flow in hypothermia was studied in mongrel dogs.
    IABP increased the coronary blood flow by 7% with 300 msec of
    deflate
    time in 3790, while it increased by 48 to 50% with 300 msec of
    deflate
    time in hypothermia.
    These results were drived from a relative increase of the coronary blood flow in hypothermia.
    It was concluded in our experiment that IABP was more effective in hypothermia than in normothermia.

    Purpose: We experimentally assessed the pitfalls of distal balloon protection systems to learn technique tips for safe procedures.
    Materials and methods: Silicone carotid artery models were connected to a circulatory system to simulate carotid bifurcation. A distal balloon protection device, PercuSurge GuardWire Plus (PSGWP, Medtronic Vascular) was navigated to the internal carotid artery (ICA) and inflated to occlude ICA flow temporarily. A debris aspiration catheter (Thrombuster, Kaneka Medics) was navigated just proximal to the PSGWP coaxially to introduce and diffuse particulate debris (200-500 micro meter in diameter) in the ICA stump. After debris in the stump was aspirated, the PSGWP was deflated. We recorded all the processes of our simulation experiments on a digital video and observed the movements of debris during these experiments.
    Exp 1) We simulated the movements of debris in the ICA stump when the PSGWP balloon was gradually deflated to produce a gap between the balloon and vessel wall, simulating accidental movement of the PSGWP balloon during the procedure. Exp 2) To assess the optimal placement of the tip of aspiration catheter, the debris in the ICA stump was aspirated from 3 different sites (from just below the GuardWire balloon, from 5 cm below it, and at the ICA orifice). Exp 3) To assess the capacity of the aspiration catheters (7F, 6F Thrombusters (Kaneka Medics), 5.4F Export catheter (Medtronic Vascular), we compared the time to aspirate 20 cc of normal saline.
    Results: Exp 1) When the gap appeared between PSGWP balloon and silicone tube, simulated debris began to concentrate just below the balloon. Then, some debris migrated distally from the gap, and other debris crowded in the gap so that it was impossible to aspirate and migrated. Exp 2) Debris aspiration was most effective from immediately below the PSGWP, and the aspiration ability declined as the distance between the balloon and aspiration catheter became longer. Exp 3) The aspiration time for 20 cc of normal saline is 8.4+/-0.2 sec, 12.8+/-0.5 sec, 13.3+/-0.1 sec. in case of 7F Thrombuster, 5.4F Export catheter, 6F Thrombuster, respectively.
    Conclusion: Our simulation studies show that when the PSGWP was moved accidentally during CAS procedures, or when the aspiration catheter was not delivered all the way to the PSGWP balloon, distal embolization might still occur, even when care is taken. So to protect the distal embolism, the PSGWP balloon should be fully dilated and fixed to the arterial wall during procedures, and the aspiration catheter should be navigated just proximal to the PSGWP balloon.

    In order to adjudge and evaluate the limit of control ability of SMEC IABP system, which ECG R-wave trigger system is applid to set the deflation start and end, so the inflation duration is set automatically, using a mocked up circuits, in-vitro tests were run. Under conditions of heart rate 170〜190/min, defaltion start R-wave trigger, driving with helium gas, the timing setting ability of the control system was perfect. IABP was employed for the failing heart with atrial fibrillation after cardiac surgery of severe valvular disease. In all cases, positive left ventricle assist effects were obtained.

    We describe a case of difficult deflation of the upper right lobe caused by the existence of a tracheobronchial anomaly during the thoracoscopic esophagectomy. A 70-year-old man with a history of myocardial infarction and dysphagia was diagnosed with thoracic esophageal squamous cell carcinoma. Endoscopy revealed a type 2 tumor in the lower esophagus and a superficial lesion in the middle esophagus and computed tomography showed no evidence of metastasis. We performed 2 courses of neo-adjuvant chemotherapy followed by thoracoscopic esophagectomy. During surgery, one-lung ventilation using right bronchus balloon occlusion was performed. However, the upper lobe of the right lung did not

    deflate
    , and upper mediastinal dissection was difficult. Intraoperative bronchoscopy revealed a right tracheal bronchus arising from just under the bifurcation. We reviewed the preoperative 3-dimensional computed tomography, which showed the right tracheal bronchus causing intraoperative incomplete deflation of the upper lobe. We recommend investigating this anomaly with 3-dimensional computed tomography before thoracoscopic esophagectomy.

    ポンプ作用を持っバルーンとして, 従来のバルーンの末梢側に弁用小バルーンを持つIABPを考案した。このバルーン弁は本体バルーン

    時に弁として働き, 末梢側からの逆流を防ぎ, 中枢側の圧を下げ, より後負荷軽減効果を高める作用を持つ。このバルーン弁を持つことによりIABPは, 大動脈弁を流入弁,バルーン弁を流出弁, 胸部大動脈をケーシングとする一つの容積式ポンプと見なすことができる。このバルーン弁は本体バルーンとは完全に独立して制御できるように駆動ケーブルは二連重管になっている。本体バルーンは容量20cc, 弁用バルーンは容量は6ccである。駆動装置は一台で二つのアクチュエーターを持ち二つのバルーンを独立して駆動でき, 同期させた上, 自由にタイミングを設定できる(日本ゼオン社製)。成犬を用いた動物実験ではmain balloonを作動させることにより収縮期圧は160mmHgから150mmHgに低下し, 拡張期圧は125mmHgから175mmHgに増大した。さらにvalve balloonをmain balloonより100msec遅れて

    We reported about two cases of hemorrhagic shock for open pelvic fracture patients, who were successfully treated with intra aortic balloon occlusion (IABO). In the first case, a 69-year-old man was run over by a train and transferred to our emergency and critical care center with an open wound around anus. His blood pressure was 130/55-mmHg and pulse was 90/min. AO classification of the pelvic fracture was 61-C1. The second case was of an 18-year-old man who suffered injuries from a motor vehicle accident. On arrival, an open wound around the perineal area was examined. The patient’s blood pressure was 72/43-mmHg and pulse 110/min. AO classification of the pelvic fracture was 61-C2. Because neither patient responded to fluid administration and emergency transfusion and status of shock continued, a catheter for intra-aortic balloon occlusion was inserted. This procedure stabilized the patients’ blood pressure and operations were performed. In both cases descending aorta was partially and intermittently shut off. Total interruption time of descending aorta for case 1 and case 2 was 10 and 20 minutes, respectively. IABO is a secure method for arresting bleeding below the abdomen and effective for emergency treatment of open pelvic fractures with hemorrhagic shock.

    腹部外傷による腹腔内出血に対し,開腹に先立つ下行大動脈遮断として大動脈閉塞バルーン(intra-aortic balloon occlusion;IABO)を使用し有効であった3例を報告する.症例1は18歳の男性.交通外傷による脾損傷・腎損傷に対し術前IABOを挿入し循環動態を安定させ,開腹止血・脾摘・腎摘を行った.症例2は46歳の男性.自傷による腹部刺創に対する開腹術前に予防的にIABOを挿入した.開腹すると肝左葉に刺創を認め,IABOにより出血を制御しつつ良好な視野で縫合止血を行い得た.症例3は40歳の男性.交通外傷による腸間膜損傷に対し術前IABOを挿入し循環動態を安定させ,開腹止血を行った.術後,一過性に腎機能悪化を認めたが回復した.IABOは術前の循環動態を安定させるとともに,術中の出血制御と良好な視野確保・止血に寄与し有効であるが,虚血による合併症に注意が必要である.

    We report 2 cases of stent-in-stenting for the plaque protrusion after stent deployment.
    In 1 case, a 69-year-old male presented right hemispheric TIA. He had received radiation therapy for middle laryngeal cancer 10 years previously. Diagnostic cerebral angiography demonstrated bilateral severe cervical carotid artery stenosis. The patient was treated with bilateral carotid artery stenting (CAS). After his left CAS, angiography showed satisfactory results, but intravascular ultrasound (IVUS) imaging revealed a plaque protrusion through the struts of the stent. Six days after the procedure, protruded plaque grew bigger and moved rapidly. An additional stent was deployed in the first stent to seal it. The patient's clinical course was uneventful.
    In the second case, a 58-year-old male with unstable angina had multiple major cerebral artery stenoses (bilateral carotid arteries, left subclavian artery). Before CABG, he received subclavian stenting. Although subclavian angiography showed satisfactory results, IVUS imaging revealed mobile plaque protruding through the struts of the stent. An additional stent was deployed in the first stent (stent-in-stenting) to seal it. The patient had no neurological sequelae.

    Membranes surrounding cells and organelles are mainly composed of phospholipids. In order to study the physical characteristics of cell membranes, cell-sized liposomes (giant vesicles or GVs) composed of phospholipids are used as a model system. Since the lipid bilayer membrane is extremely soft, it can easily deform into various shapes when the membrane is in excess to the vesicle volume. Here we evaporated the solvent (water) in the suspending medium to gradually concentrate the solution. As GUVs

    due to the osmotic action, characteristic deformations of GVs, such as internal budding and amoeba-like shapes, were observed depending on the concentrations of MgCl2.

    Plasma diagnostic data analysis often requires the original raw data as they are, in other words, at the same frame rate and resolution of the CCD camera sensor. As a non-interlace VGA camera typically generates over 70 MB/s video stream, usual frame grabber cards apply the lossy compression encoder, such as mpeg-1/-2 or mpeg-4, to drastically lessen the bit rate. In this study, a new approach, which makes it possible to acquire and store such the wideband video stream without any quality reduction, has been successfully achieved. Simultaneously, the real-time video streaming is even possible at the original frame rate. For minimising the exclusive access time in every data storing, it has adopted the directory structure to hold every frame files separately, instead of one long consecutive file. The popular ‘zip’ archive method improves the portability of data files, however, the JPEG-LS image compression is applied inside by replacing its intrinsic

    An improved cellular automaton model based on Yanagisawa & Tomoeda , Nishinari (2007) was established, where both Static Floor Field and collision effect were considered. Several model parameters were carefully determined by going through a turning process based on experimental data provided by other previous studies. Both results by simulation based on the model and analytical approach derived from the so-called Mean-Field Approximation proved that the outflow rate from an evacuation exit, usually estimated by the so-called flow coefficient, can be improved by putting appropriate obstacles in front of the exit.     The deflation of an airspring deteriorates the riding comfort of railway vehicles due to an increased bending vibration of the car body. This paper describes the results of a calculation analysis for the car body-bending vibration with deflated airsprings excited at the car body. The results are summed up as follows. The bending vibration characteristics are affected to a large degree by the damping force acting to the primary suspension (axle spring). In case of the bogie structure in which the frictional force of high damping acts on the axle spring, the natural frequency of car bodybending increases when its airsprings

    . The natural frequency decreases in the bogie structure which has a low damping force such as the usual oil dampers for the axle spring. Unless the oil damper fails, the acceleration amplitude of the latter structure is less than the former one under the deflation of airsprings.

    Treating patients with head and neck tumor occasionally requires permanent occlusion of the internal carotid artery (ICA). To assess whether a patient will tolerate this occlusion, we studied stump pressure, EEG and angiogram under direct clipping of the common carotid artery during surgery for severely invasive hypopharyngeal carcinoma. In other cases, we tried balloon test occlusion and measured cerebral blood flow (CBF) with positron emission tomography (PET) using C15O2. This direct clipping test can be performed with the usual surgical instruments and provides significant information on carotid ligation. However, we recognized that the information was not sufficient for a clear assessment even if the stump pressure and EEG were within the normal range. Balloon occlusion test and PET were performed in two cases. The first case showed temporary hemiplegia after a slight decrease in stump pressure, while the second case did not show any change despite a severe decrease in stump pressure. The second case showed an increase of CBF on PET. Stump pressure does not necessarily reflect changes in CBF, and PET should be used for more reliable assessment.
