    Mandibles of 54 rats consisting of males and females of the same number (weighing around 200 to 260 gr.) were measured in 16 dimensions (weight, area and linear dimensions).
    In area and linear dimensions we used projective measurements with a certain constant enlargement of soft X-ray photographs and pictures of both right and left mandibles which were taken under the same conditions.
    Therefore we compared the mandible on the right and left sides in each measurement, for the presence or absence of asymmetry and one-sided dominance.
    As a result, mainly the following two findings were obtained.
    1) The mean values of the dominant and non-dominant sides in each dimension showed significant difference (p <0.01 or <0.001).
    2) Significant difference between the mean values of the right and left sides was not necessarily found in all dimensions but found in only a few dimensions, i.e., the height of angular process was significantly greater on the left side in both male (p <0.05) and female (p <0.001), and the area of the ramus and the length of the condyloid process and the length of the masseter muscle (the masseteric ridge to the posterior end of the angular process) showed significant difference (p<0.05) in only male, they were all greater on the left side.
    From these observation it was suggested that the phenomenon of asymmetry and one-sided dominance found in the height of the angular process and in some other dimensions was caused by the difference of the roughening or hyperostosis of bony area where masticatory muscles were given attachement.

    Purposes: To describe Short TI Inversion Recovery (STIR) signal characteristics of normal structures in the maxillofacial region, and evaluate the usefulness of the STIR imaging in the maxillofacial region compared with conventional T1- and T2-weighted SE images.
    Patients and methods: Signal characteristics of normal structures (parotid glands, submandibular glands, sublingual glands, masseter muscles, lymph nodes, palatine tonsils, laryngeal tonsils, mandibular bone marrow) on STIR images in 80 normal subjects were evaluated. Lesion visibility on STIR images in 40 patients with histopathologically confirmed disorders (16 tumors, 2 malignant tumors, 10 cysts, 12 inflammations) in the maxillofacial region were compared with that on conventional T1- and T2-weighted SE images. Lesion visibility was rated as 1 (poor), 2 (moderate), 3 (good), 4 (excellent).
    Results: On STIR imaging of normal subjects, parotid glands demonstrated intermediate signal intensities (77.5%), submandibular glands demonstrated intermediate to high signal intensities (95%) and sublingual glands demonstrated intermediate to high signal intensities (100%) . Masseter muscles showed intermediate signal intensities (100%) and lymph nodes showed high signal intensities (100%) on STIR images. Palatine tonsils demonstrated intermediate to high signal intensities (81.25%) and the laryngeal tonsils demonstrated intermediate to high signal intensities (88.75%) on STIR images. Mandibular bone marrow demonstrated low signal intensities (83%) on STIR images. STIR imaging is more effective for evaluation in 40 disorders (16 tumors, 2 malignant tumors, 10 cysts 12 inflammations) than Ti-weighted images and T2- weighted images especially in the case of inflammation (p<0.05).
    Conclusion: The use of STIR imaging is effective for diagnosing disorders in the maxillofacial region.

    We investigated the taste synergy between L-theanine and the flavour enhancer, inosine 5′-monophosphate (IMP), by using a human sensory evaluation. When L-theanine was added to IMP, only the umami taste was enhanced. We then investigated this synergistic effect of L-theanine in mice by gustatory nerve recording. We confirmed the synergism between L-theanine and IMP for the umami taste.

    To explain the pain and dysfunction of neck during jaw movement associated with malocclusion, the physiologic relationships between jaw and cervical movement should be understood. However the complicated reflex effects made it difficult to investigate the biomechanical relationship between the head-neck components. For this reason we have created a model for mechanical system dynamic analysis of the normal stomatognathic system to assess the biomechanics of the cervical movement and muscle activity during jaw movement. We have also statistically validated the model and quantitatively verified the model to the human subject by the amounts and conditions of cervical muscle activity. During jaw openingclosing phases the movement of the model was highly correlated to the subject with an identical movement. However low consistency was achieved during intercuspid phase. These findings indicate the establishment of a valid system, which can be used to evaluate the biomechanical relationships between jaw and cervical movement. Moreover the model verification of cervical muscle activity indicate the mechanical action of jaw can be the primary factor to modify cervical muscles, and cervical muscles coordinate to resist changes in head balance during jaw movement to maintains head posture.

    1. The electromyographic discharges of rigid jaw mucles in the decerebrate cat were dominant in the suprahyoid muscle groups and scanty in the jaw closing muscle groups. But the jaw closing muscles were sensitive to stretching and showed strong spastic resistance to even a slight opening of the mouth.
    2. In spite of the marked extensor rigidity of the limbs, jaw rigidity disappeared readily after decerebration at a level caudal to the mesencephalic trigeminal nucleus.
    3. Regular spontaneous unit discharges were recorded from and along the mesencephalic trigeminal nucleus and root of the dog. The background activity in these points was remarkably depressed by elevating the lower jaw and accelerated by depressing the jaw. These responses were obtained by strictly localized punctures in the brain-stem, accompanied by a short latency and slow adaptation as shown in goat by Cooper, Daniel and Whitteridge.
    4. Nearly the same response was recorded in the lateral bulbar reticular formation at points just caudal to the cerebellar peduncles. There were also some localized points in the medial bulbar reticular formation near the hypoglossal nucleus at level rostral to the inferior olive, in which the background activity was inhibited by opening of the mouth.

    Theanine is a unique amino acid present in green tea. It is partly responsible for the taste of green tea and possesses various psychological effects. Theanine has a preferred taste, like umami and sweetness. Therefore, theanine may improve unpleasant tastes such as bitterness and sourness of foods. To investigate new uses of theanine, we investigated its taste-modifying effect by human sensory evaluation. A reduction in sourness was observed with theanine addition. Investigation of the mechanism of taste improvement found that sourness is relatively improved by theanine addition due to the increase in pH and the addition of the umami taste.

    下顎枝矢状分割術後の骨片の分割, 固定状態と骨性治癒との関連を明らかにすることを目的に行った。
    対象は, 当科において下顎枝矢状分割術を施行した下顎前突症例の12例, 24側を対象とし, 術直後と術後1年の3次元CT像により観察した。
    結果は, 1年後の下顎枝前縁・後縁の形態は, 平滑型 (近遠位骨片辺縁が識別不能で滑らかに移行しているもの) は下断面で多く認められた (70.8%) が, 上断面では, 平滑型は少なく (39.6%), 階段型 (近位および遠位骨片のどちらかが突出し階段状となったもの) が多く (43.8%) 認められた。また, 陥凹型 (骨片間に間隙がみられ陥凹した形態のもの) は上下断面ともに前縁に多く認められた。
    下顎枝後縁形態と近遠位骨片の分割状態との関係では, 下断面においては, 下顎枝後縁まで分割されると階段型が, 後縁よりも近位で分割されると平滑型が多い結果であった。
    下顎枝前縁・後縁形態と骨片間距離との関係では, 陥凹型, 階段型, 平滑型の順で骨片間距離が大きかった。また, 陥凹型は骨片間距離の平均が6.1mm以上の症例において認められた。
    遠位骨片の後方突出量と下顎枝後縁形態との関係については, 上断面では後方突出量は階段型, 平滑型, 陥凹型の順で大きく, 下断面では, 階段型, 陥凹型, 平滑型の順で大きくなっていた。また上下断面ともに, 階段型は後方突出量が5.3mm以上の症例で認められた。
    以上の結果より, 骨片の間隙が大きい場合や分割面が下顎枝後縁まで分割され後縁突出量が大きくなる場合には, 術後1年経過しても解剖学的形態でないものが認められた。

    The internal architecture of the masticatory muscles in a murine murid, Rattus rattus, was examined and compared with our previous description of Peromyscus maniculatus, which seems to have primitive jaw muscles for muroid rodents. Remarkable differences were observed mainly in the masseter muscle and corresponding aponeurosis. The anterior and posterior mandibular aponeuroses for insertion of the masseter deep layer are united as a single sheet in Rattus. In Peromyscus, both the origin and insertion of the anterior part of the masseter deep layer lie on the aponeurosis. In Rattus, however, the deep fibers of the anterior part form a muscular attachment to the bone surface at either end. This characteristic suggests that the maximum angle of gape is increased in murine murids, which allows them to ingest various-sized foods.

    The middle meningeal artery and its whole ramifications in the rabbit were studied by the acryl injection method. Simultaneously, the stapedial artery presumed to represent an embryological remnant was investigated in 3 cases of the total of 120 examples observed in relation to the branches of this artery. The middle meningeal artery arose from the maxillary artery just distal to the alar canal in most cases, rarely via common trunks between neighboring vessels, and ran superoposteriorly up to the posterior end of the oval foramen, where it gave rise to twigs to the mandibular nerve, the tympanic tensor and the inferior branch to the trigeminal ganglion. The artery entered the petrotympanic fissure, giving off the superior branch to the ganglion within it and the petrosal branch and the superior tympanic artery within the bone tissue. Rarely, the former was not found but the latter. In the above 3 cases, the stapedialartery of the internal carotid artery was found. The petrosal branch ran posteriorly along the minor petrosal nerve, then the facial nerve up to a position above the vestibular fenestra, when it gave rise to the branch to the promontorium and the stapedial m. branch and anastomosed with the stylomastroid artery. The superior tympanic ran posteromedially along the major petrosal nerve, giving rise to the trigeminal ganglionic branch and a thick meningeal branch, up to the genicular ganglion to supply it. The branch to the promontorium and the superior tympanic may correspond to the stapedial artery represented in an embyological pattern. This artery passed between the stapedial crura and forwards beneath the facial nerve to supply the genicular ganglion, anastomosing with the superior tympanic. The main stream of the middle meningeal artery passed superoposteroanteriorly within the groove for this artery, where it gave rise to the anterior and posterior branches which supplied the dura mater on the petrosal part. The parent artery thus ascended in the parietotemporal suture up to the meeting point between the temporal, occipital and parietal bones, where it gave off the temporal muscular branch which emerged onto the extracranial surface to supply the temporal muscle. The main stream of the artery finally hooked downwards to supply the dura mater on the posterior slope of the petrosa and the parafloccular fossa.

    症例は68歳の女性. 開口障害および左頬部腫脹にて当院口腔外科を受診. 左頬部の筋生検標本にて非乾酪性肉芽腫を認め, 精査のため当科入院となった. MRではT1およびT2強調画像でともに高信号を呈する左咀嚼筋のびまん性腫脹が認められた. 本例は四肢・体幹の骨格筋にも腫瘤形成が認められ, またACE高値であり, TBLBにて非乾酪性肉芽腫を検出したため腫瘤型筋サルコイドーシスと診断した.
    腫瘤型筋サルコイドーシスではほとんどの場合筋肉の機能障害を伴うことはないが, 上肢屈筋群に形成された腫瘤により手指拘縮をきたした症例がいくつか報告されている. 本例は咀嚼筋拘縮により開口障害をきたしたきわめて稀な例ではあるが, 筋拘縮が本症における筋肉の機能障害の要因の1つであることを示唆した症例といえる.

    Gluconate is used as an additive in a wide range of processed foods. In this study, we investigated its utility as a taste-improving substance. To determine whether it has a suppressive effect on bitter taste, sensory evaluations were performed by human subjects. When gluconate was added to a quinine-HCl (QHCl) solution, the taste intensity decreased, but this effect was not observed when it was added to caffeine and to naringin solutions. Then we investigated the mechanism of suppression by performing behavioral and electrophysiological assays on mice. In mice, the addition of gluconate improved the taste preference for and reduced the gustatory nerve response to QHCl. In sum, gluconate had a suppressive effect on the bitter taste of QHCl, which might have been caused by depression of gustatory nerve activity.

    研究の目的は下顎歯肉扁平上皮癌の周囲軟組織進展のCT所見と実際の腫瘍進展範囲と比較し, CTによる腫瘍進展の診断能を明らかにすることである。
    対象は1992年~1999年の間に九大歯学部附属病院で腫瘍切除術がなされ腫瘍進展範囲が肉眼的または病理組織学的に確定した77例である。各症例の術前のCT像を3名の日本歯科放射線学会認定医が観察し, 周囲軟組織への腫瘍進展の有無について「ある」, 「あるかもしれない」, 「わからない」, 「ないかもしれない」, 「ない」の5段階で評価した。
    周囲軟組織への進展部位は, 1) 頤舌筋, 2) 顎舌骨筋, 3) 内舌筋, 4) 内側翼突筋, 5) 咬筋, 6) 唇頬側組織, 7) 広頸筋である。この判定結果を実際の腫瘍進展範囲と比較し, CTによる腫瘍の軟組織進展の正診率を各観察者のROC曲線下の面積Az値の平均値を指標として求めた。結果としてCT像による腫瘍進展の正診率は頤舌筋0.86, 内側翼突筋0.86, 顎舌骨筋0.82, 咬筋0.80, 内舌筋0.76, 唇頬側組織0.71, 広頸筋0.65であった。結論として内舌筋, 唇頬側組織, 広頸筋への進展の判定には視診, 触診を含む臨床所見を参照してCT像を注意深く読影することが必要と考えられた。

    Food contains various taste substances. Among them, umami substances play an important role with regard to the perception of the taste of food, but, few studies have examined the taste characteristics of representative umami substances other than monosodium L-glutamate (MSG). By conducting mouse behavioral studies (the 48-h 2-bottle preference test and the conditioned taste aversion test) and assessing gustatory nerve responses, we investigated the taste characteristics of unique umami substances, including sodium succinate, L-theanine, betaine, and the enantiomer of MSG, D-MSG. Furthermore, we examined the synergy of umami with inosine 5′-monophoshate (IMP). In the case of the mice, sodium succinate had an umami taste and showed strong synergy with IMP. L-Theanine showed synergy with IMP but did not have an umami taste without IMP. In contrast, betaine did not have an umami taste or synergy with IMP. D-MSG might have weak synergy with IMP.

    Hemangioma occurs frequently in the head and neck region. There are, however, relatively few reports of phleboliths occurring with hemangioma.
    This report describes a case of multiple hemangiomas occurring with phleboliths in the oral region. A 45-year-old Japanese man was referred to this clinic with a chief complaint of tumors in the oral cavity. Hemangiomas were found on the bilateral buccal mucosa, tongue, right mandibular gingiva, oral floor and lower lip.
    Surgical resection and Laser treatment were applied, and two phleboliths were extirpated. One of these phleboliths was dissected and its cut surface was examined with an X-ray microanalyser. The cut surface was of a stratiform construction in a concentric configuration. Ca, P, Mg, S, Na, O, C, Cl and Al were detected by point analysis. The concentration distributions of Ca and P were almost equal by line analysis.
    Postoperatively, round and oval radiopaque shadows were seen on a CT scan at the anterior border of the right masseter, the interior border of the internal pterygoid muscle and the left superior submandibular gland.

    Extended surgery for advanced oropharyngeal carcinoma (stage III, stage IV UICC 1987) originating from the lateral wall was performed in 13 patients from 1992 to 1995 using the mandibular swing technique. Histopathologic findings were squamous cell carcinoma in all cases. The primary lesions were T2 in one case, T3 in 6 cases, T4 in 6 cases. The surgical margin of the deep layer was composed of the styloid process, carotid sheath and medial side of the mandibula. En bloc resection encompassing the primary region, dissected neck tissue, internal pterygoid muscle and parapharyngeal tissue was performed. In those cases where the tumor had invaded the mandibula, a marginal or a segmental resection was carried out.
    Reconstruction of the oropharynx was performed with only a forearm flap in 7 cases, and a pectoralis major musculocutaneous flap in 2 cases. Reconstruction with a forearm flap combined with a pectoralis major muscular flap was needed for obliteration of parapharyngeal and submandibular dead space in 3 cases. Reconstruction of mandibula was performed with a fibular osteocutaneous flap in 1 case.
    Postoperative complications, included open rhinolalia in 4 cases, dysphagia in 5 cases, and difficulty opening the mouth in 5 cases. Local recurrence was noted in 2 cases, 13 and 10 months after surgery, respectively. The tumor recurred in the region from the anterior arch of the palate to the retromolar area in both cases. Tumors invading this region should be resected with careful determination of the resected area.

     中咽頭癌側壁型に対する手術治療における切除方法を3つに分け検討を行った。対象は2002年から2007年までに手術加療した25例(男性18例,女性7例),平均年齢は58.8歳であった。術式の選択は,1.口内法,2.Combined法,3.下顎離断法が選択され,それぞれ5例,11例,9例であった。口内法は全例Raw surfaceのままとし,Combined法11例のうち8例は遊離皮弁による再建を行い,3例は一次縫合した。下顎離断法9例は全例遊離皮弁再建を行った。術式の決定には腫瘍の進展範囲,特に舌根方向と内側翼突筋への進展の有無に注意して切除方法を決定する必要がある。

    下顎枝矢状分割術は顎変形症に対して広く用いられているが,術後経過中に後戻りを経験することがある.後戻りの原因の一つとして,術後咬合力などの生体力学的影響が考えられる.そこで,われわれは術後の咬合力や閉口筋筋力が下顎骨や骨接合プレートに及ぼす応力分布の推移について三次元有限要素法を用い経時的に応力解析を行い検討した.すなわち,CT画像データから下顎枝矢状分割術後1か月,3か月,6か月および12か月の下顎骨モデルを作製し,各モデルに咬合力および閉口筋筋力を付与し近位骨片,遠位骨片,骨接合部および骨接合プレートにおける相当応力を調べた.近位および遠位骨片における相当応力は術後1か月から術後12か月にかけて経時的に増大傾向を認めた.特に,各期間において下顎枝前縁部に高い応力の集中を認めた.骨接合部における相当応力は術後1か月から術後6か月にかけて応力の増大を認めたが,術後12か月では減少していた.術後の咬合力および閉口筋筋力により,近位骨片の下顎枝前縁部に応力の集中を認め,術後3か月から6か月の間に骨接合部の骨切り線の上端と下端に引っ張り応力の集中を認めたことから,術後6か月までは遠位骨片に時計回りの回転方向の応力が加わることが生体力学的解析で判明した.以上のことから,下顎枝矢状分割術を施行した際,術後3か月から6か月の間に後戻りの危険性のあることが示唆された.
